1.Case analysis of occupational disease diagnosis appraisal in Shanghai from 2012 to 2024

Background Occupational disease diagnosis appraisal is an appeal procedure initiated when a party disagrees with the occupational disease diagnosis conclusion. It is a legal procedure to safeguard the health rights of employees and the legitimate rights and interests of employers. Objective To analyze the data of occupational disease diagnosis appraisal in Shanghai, identify existing problems, and provide suggestions for improving. Methods Statistical analysis was conducted on basic situation, characteristics of workers and employers, disease classification, and consistency between diagnosis and appraisal conclusions of all occupational disease diagnosis appraisal cases from 2012 to 2024. All diagnosis conclusions were named in accordance with the Classified Catalogue of Occupational Diseases included employers were classified according to the Measures for the Statistical Classification of Large, Medium, Small and Micro Enterprises" and the Notice on Adjusting the Provisions for Classifying Enterprise Registration Types; industry classification followed the Industrial Classification for National Economic Activities (GB/T 4754-2017). Results From 2012 to 2024, a total of 260 cases of occupational diseases diagnosis appraisal were closed. The main diagnosis were 138 cases of ear, nose, throat, and oral diseases (53.1%), chemical poisoning (18.8%), and pneumoconiosis and other respiratory diseases (16.2%). The proportion of ear, nose, throat, and oral diseases gradually increased (2022 excluded). The incidence of annual municipal and provincial cases decreased by 76.3% and 85.0% respectively (2024 vs 2012). The consistency rate of conclusions between appraisal and diagnosis gradually increased. The total consistency rates between final appraisals and diagnostic conclusions were 81.5%, and 79.7%, 87.8%, 100.0%, 85.0% for ear, nose, throat, and oral diseases, chemical poisoning, and pneumoconiosis and other respiratory system diseases, respectively. Specifically, the consistency rates were 80.1% and 80.0% for noise-induced deafness and chronic benzene poisoning. In terms of inconsistency reasons, "inconsistent interpretation of standards and excessive discretion in standard application" accounted for 43.8%, followed by "failure to correctly apply standards" (31.3%) and and "discrepancies in occupational exposure history recognition" (15.6%). Conclusion The revisions and improvements of the legal system for occupational disease diagnosis appraisal have played a positive role in improving the consistency of conclusions between appraisal and diagnosis of occupational diseases in Shanghai. It is suggested to further strengthening the publicity and training of occupational disease diagnosis standards to improve the quality of occupational disease diagnosis.
2.Ultrasound deep learning model for diagnosis and classification of cystocele

Objective To explore the value of ultrasound deep learning(DL)model for diagnosis and classification of cystocele.Methods Totally 696 female patients who underwent pelvic floor ultrasound were retrospectively collected and divided into model development dataset(n=576)and test set(n=120).The former included 432 cases of cystocele and 144 cases of non-cystocele,while the latter included 90 cases of cystocele and 30 cases of non-cystocele.Patients in model development dataset were randomly divided into training set(n=460,including 345 cases of cystocele and 115 cases of non-cystocele)and validation set(n=116,including 87 cases of cystocele and 29 cases of non-cystocele)at the ratio of 8∶2.DL model was trained and established using Vision Transformer architecture based on pelvic floor ultrasound data in training and validation sets for diagnosis and classification of cystocele(non-or Green Ⅰ,Ⅱ and Ⅲ type).Taken diagnostic results of senior ultrasound physicians as standard,the diagnostic efficacy of DL model was evaluated,and its diagnostic efficacy and efficiency were compared with those of 2 junior ultrasound physicians.Results The macro average precision,F1 score,area under the curve(AUC)and overall accuracy of DL model for diagnosis and classification of cystocele in validation set was 90.84%,89.28%,0.97 and 89.66%,respectively,while in test set was 80.85%,79.92%,0.92 and 80.00%,respectively.The overall diagnostic accuracy of 2 junior ultrasound physicians for diagnosis and classification of cystocele in test set was 70.00%(84/120)and 68.33%(82/120),respectively,both lower than that of DL model(P=0.023,0.011).The diagnostic time of DL model was 0.098 s for each case,of junior ultrasound physicians was 46(36,56)s for each case,the former had better diagnostic efficacy(P<0.001).Conclusion Ultrasound DL model could be used for diagnosis and classification of cystocele.
3.Analysis of the status and related factors of hyperopic reserve among non-myopia children aged 5-12 years in Guangdong Province

Objective:
To explore the current status of hyperopic reserve and its related factors among non-myopia preschool and primary school students aged 5 to 12 years in Guangdong Province, so as to provide a basis for formulating intervention strategies for the pre myopia stage of children.
Methods:
From October to December 2023, by using stratified cluster random sampling method, a survey on hyperopic reserve among preschool children and primary school students in Guangdong were conducted. And a total of 10 567 children from the senior class of kindergarten to the sixth grade of primary school who completed autorefraction measurements with and without cycloplegia and the questionnaire survey were included in the study. The prevalence characteristics of low hyperopic reserve among non-myopia children were analyzed, and multivariable Logistic regression was used to analyze the related factors.
Results:
The prevalence rate of low hyperopic reserve among 8 790 non-myopia children was 62.4%. The average spherical equivalent (SE) for children aged 5 to 12 years was 0.88 (0.25, 1.25)D, decreasing from 1.13 ( 0.75 , 1.50)D in senior kindergarten to -1.00 (-2.50, 0.38)D in sixth grade, with the difference was statistically significant ( H=2 475.3, P <0.01). Multivariable Logistic regression analysis, after adjusting for confounders including gender, urban and rural, and grade, revealed that parental myopia was a risk factor for low hyperopic reserve in the preschool stage (one parent with myopia: OR=1.62, 95%CI =1.35-1.93; both parents with myopia: OR=2.05, 95%CI = 1.66 -2.55); in the lower primary school stage, parental myopia (one parent with myopia: OR=1.46, 95%CI =1.27-1.68; both parents with myopia: OR=1.58, 95%CI =1.33-1.89), frequently or always reading or using electronic screens while lying down or on one s stomach ( OR=1.43, 95%CI =1.13-1.81), and never or occasionally maintaining a viewing distance of over 3 meters when watching TV/playing video games ( OR=1.34, 95%CI =1.04-1.72) were risk factors; in the higher primary school stage, failing to take a break every hour during near work ( OR=1.79, 95%CI =1.16-2.75) was a risk factor (all P <0.05).
Conclusions
The emmetropization of children aged 5-12 years in Guangdong Province is accelerated, and non-myopia children generally exhibit insufficient hyperopic reserve. The contributing factors for insufficient hyperopia reserve in non-myopia children vary across different educational stages, necessitating targeted precision interventions.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.
5.Analysis and reflection on the current situation of patent transformation in district and county level tertiary comprehensive hospitals: taking Beijing Luhe Hospital affiliated to Capital Medical University as an example

Objective:This study aims to explore the current situation of scientific and technological achi evements transformation in district and county-level medical institutions, by analyzing 102 successfully transformed patents from a distric level III comprehensive hospital.Methods:Based on the hospital′s scientific and technological transformation registration data, this study comprehensively utilized patent databases and enterprise inquiry platforms, employing EXCEL and SPSS AU software for analysis.Results:Utility model patents predominate, primarily transformed through patent (application) right transfers, the average contract amount was 42 690 yuan and a relatively long transformation cycle. Female inventors and those under 50 years old account for a high proportion, with nurses being the most common first inventors of patents. Collaborating enterprises were mainly local hospitals, startups, as well as science and technology promotion and application service enterprises.Conclusions:District and county-level medical institutions face many challenges in transforming technological achievements. However, by deeply exploring clinical needs, developing differentiated training and support strategies, and focusing on key partner enterprises, it is possible to promote high-quality and sustainable technological achievement transformation.

9.Design and evaluation of a novel arcuate multi-channel rectal endoluminal applicator based on pelvic MRI images

Objective:To design and evaluate a novel arcuate multi-channel rectal endoluminal applicator to enhance dose coverage of tumors in the upper and middle rectum and reduce pressure on the rectal wall.Methods:Pelvic MRI images of 200 Chinese cases without rectal lesions in the Peking University Third Hospital from July 2022 to August 2022 were retrospectively analyzed. Based on the image data, a rectal model with general characteristics of the population and two novel hard and soft rectal endoluminal applicators were designed and fabricated. The following properties of the conventional applicators and two new applicators were compared: deformation to the model rectal wall, maximum pressure, stable pressure, D 90%, D 100%, V 100%, V 150% and V 200% of the GTV, and D 2 cm3, D 1 cm3, and D 0.1 cm3 of the organs at risk (OAR). ANOVA or Kruskal-Wallis H-test was used to compare the differences among three applicators, and Dunnett's multiple comparison test was used for pairwise comparisons. Results:The novel hard and soft rectal endoluminal applicators caused less deformation of the model rectal wall. The maximum pressure on the rectal wall was (0.606 ± 0.182) kPa and (0.481 ± 0.229) kPa for the hard arcuate applicator and soft arcuate applicator, respectively, and the stable pressure was (0.207 ± 0.137) kPa and (0.055 ± 0.097) kPa, respectively, which were significantly smaller than those of the conventional applicator ( P <0.001, <0.001; P =0.024, <0.001), and the degree of reduction was at or near 50%. Under the premise of ensuring target dose, the D 2 cm3, D 1 cm3, and D 0.1 cm3 of OAR in the treatment plan designed with the novel applicator were significantly reduced compared to the cylindrical applicator (all P<0.001). Conclusion:The novel arcuate multi-channel rectal endoluminal applicator can significantly reduce rectal wall pressure and deformation, while also reducing the dose to OAR without compromising target dose coverage, offering certain therapeutic advantages.
10.Discussion on promoting patent conversion through standardized management in district tertiary hospitals

Objective:This study aimed to analyze the specific measures and practical effects of standardizing the management of patent conversion work in a certain district tertiary hospital, aiming to provide valuable references and guidance for improving patent conversion management for other hospitals of the same level.Methods:This study selected a series of standardized management measures taken by the hospital from July 2022 to December 2023 as the research object, including but not limited to the approval process before patent application, the combing and optimization of the approval process of patent conversion application, and the formulation and implementation of the tax reduction system and other policies.Results:The smooth implementation of various standardized management patent transformation measures in the hospital effectively improved the efficiency of patent quality control and patent conversion. Specifically, it was reflected in the shortening of the ″application conversion contract signing″ period, as well as a significant increase in the total number of patent implementation conversions and the total amount of conversion contracts signed.Conclusions:By standardizing the management of hospital patent conversion, not only can the efficiency and success rate of patent conversion be effectively improved, but also can effectively promote the development of hospital innovation and conversion work.
