1.Thumb Reconstruction Using a Modified Masquelet Technique Following Crush Injury: A Case Report

Traumatic crush injury of the thumb is devastating and often
resulted in poor functional outcome. Various reconstruction
options available according to surgical expertise. Masquelet
technique is well-established in the long bones of lower
limb. Only a handful of cases reported for thumb, especially
in Asia region. We described a case of crush injury of right
thumb following a trauma. The bony defect was initially
filled with antibiotic spacer and subsequent reconstruction
with tricortical iliac graft, a modified Masquelet two-stage
technique. It is a versatile tool which provide satisfactory
functional outcome and hand appearance.
2.Asymptomatic lymphatic filariasis in an elderly patient from Bako, Sarawak: A case report and public health implications

Lymphatic filariasis (LF) remains a significant public health challenge in many tropical regions where the
disease is endemic. In Malaysia, LF is found in small pockets across the country. Asymptomatic carriers
play a critical role in transmission but are often undetected. This report details an investigation of an
asymptomatic filariasis reported by local health authorities involving an 83-year-old female patient
residing in the Bako area, Sarawak. Despite being immobile due to a stroke, routine screening identified
an infection with Brugia malayi through microscopy and a rapid diagnostic test. Interestingly, the patient
exhibited no acute or chronic symptoms typically associated with filariasis. Contact tracing among
her family members revealed that her son was also infected. Both patients received treatment with
diethylcarbamazine (DEC) at a dosage of 6 mg/kg, along with albendazole 400 mg and ivermectin 12
mg. Preventive measures included health education, entomological studies, and the implementation of
a ‘Test & Treat Filariasis’ program in the village. By documenting both the index case and a secondary
asymptomatic case within the same household, the study provides a strong example of how routine
screening and contact tracing can identify hidden sources of infection. This adds significant value to
LF elimination strategies and emphasizes the importance of community-level surveillance programs.
Coordinated efforts by health authorities, including contact tracing, environmental assessments, and
targeted treatment, are essential for controlling the spread of LF and safeguarding public health.
3.Outcomes and management of Peyronie's disease with combined treatment of collagenase clostridium histolyticum, vacuum erection device, and tadalafil.

Peyronie's disease (PD) is a connective tissue disorder characterized by abnormal collagen deposition in the tunica albuginea, leading to penile curvature, pain, and erectile dysfunction. This study aimed to evaluate the outcomes of a combined treatment protocol incorporating collagenase clostridium histolyticum (CCH), vacuum erection device, and tadalafil. A retrospective analysis was conducted on 99 male patients with PD treated at the Department of Urology, Hamad Medical Corporation (Doha, Qatar) between January 2018 and January 2020. Patients received 4-8 CCH injections alongside vacuum therapy and daily tadalafil (5 mg). The baseline mean penile curvature of 49.0° improved by an average of 21.4% post-treatment. Erectile function scores also increased significantly, with a mean improvement of 2.3 points on the International Index of Erectile Function. Minor complications were observed in 15 patients, while 13 were dissatisfied with treatment, with six opting for surgery. The modified protocol demonstrated significant improvements in penile curvature and erectile function with minimal complications, offering a safe, cost-effective alternative to traditional intensive treatments.

5.Sensitivity and Specificity of an All-in-one Cartridge-based Dengue Real-time Reverse Transcription-PCR for Point-of-care Detection and Serotyping of Dengue Virus in Samples from Indonesian Patients

Background:
Dengue is a systemic, viral, mosquito-borne infection that continues to be a major public health issue in endemic regions in tropical and subtropical climates. Accurate tests for rapid diagnosis in point-of-care settings are important to reduce the fatality rates of severe dengue. We evaluated the diagnostic accuracy of the Standard M10 DENV 1-4 system (SD Biosensor, Gyeonggi, Korea), which is a cartridge-based, automated system that integrates nucleic acid extraction, reverse transcription-PCR (RT-PCR) amplification, and detection of dengue virus (DENV) serotypes.
Methods:
This was a retrospective diagnostic evaluation study. The index test, Standard M10 DENV 1-4, was evaluated using 320 dengue-positive and 279 dengue-negative archived samples. The reference tests were a combination of Centers for Disease Control and Prevention (CDC) DENV 1-4 real-time RT-PCR, dengue NS1 antigen and IgM antibody detection, and DENV whole-genome sequencing.
Results:
The overall sensitivity and specificity of Standard M10 DENV 1-4 were 94% and 100%, respectively. By serotype, the highest sensitivity was 100% for DENV-1, and the lowest was 82% for DENV-4. The overall between the CDC RT-PCR dengue serotyping method and the Standard M10 DENV 1-4 was 95%. Standard M10 DENV 1-4 RT-PCR had comparable sensitivity and specificity to CDC DENV RT-PCR.
Conclusions
Based on its commensurate performance to an established RT-PCR method combined with additional benefits of convenient storage and transport, easy use, and rapid processing, the Standard M10 DENV 1-4 system has potential for DENV detection and serotyping in point-of-care settings.
6.Relationship between bone quality and shrinkage in maxillary sinus augmentation using synthetic alloplast versus xenograft

Purpose:
This clinical study evaluated differences in bone quality within the maxillary sinus when either alloplast or xenograft material was applied.
Methods:
Twenty participants requiring lateral sinus approach augmentation for delayed implant placement were divided into 2 groups. The first group received a silica-calcium phosphate composite (SCPC) alloplast, while the second received a bovine bone xenograft (Bio-Oss). Cone-beam computed tomography (CBCT) was used to measure changes in bone height and area immediately after surgery and 5 months after augmentation. Picrosirius red staining of bone biopsies, obtained during implant placement, was analyzed under polarized microscopy. A MATLAB algorithm was used for analysis following image acquisition.
Results:
CBCT measurements revealed a statistically significant reduction in linear bone height among xenograft recipients (P≤0.001). This finding indicated greater resorption compared to the alloplast group, which exhibited no significant difference in bone height at 5 months following sinus augmentation. Furthermore, 40% of xenograft recipients displayed an absence of mineralized tissue formation, indicating a limited osteoconductive effect compared to the alloplast group, in which mineralized bone tissue was present in all samples.Polarized light microscopy and color intensity measurements of picrosirius red-stained bone biopsies revealed a significantly higher ratio of mineralized collagen I to collagen III in the newly formed bone for alloplast compared to xenograft recipients. Trichrome staining demonstrated collagen I mineralization with the presence of osteoblasts and osteocytes, indicating new bone formation. The newly formed bone in the alloplast group exhibited markers of maturation, including the formation of reversal lines, Haversian systems, and blood vessels. Resorption of the SCPC alloplast-grafted granules was also observed.
Conclusions
The increased formation and maturation of new bone appear responsible for the preservation of bone height in alloplast group recipients.
7.Analysis of waste management effect on the climate related disease in Larangan Village, Sidoarjo

The repercussions of climate change have profound implications for human health, leading to the emergence of climate-related diseases. Recognizing this, the Ministry of Environment and Forestry in Indonesia has established the Climate Village Program to tackle and mitigate the impact of climate change. Despite the implementation of this program, Larangan Village in Candi Sub-district, Sidoarjo Regency, continues to struggle with cases of climate-related diseases. This research aims to assess the impact of waste management on the prevalence of climate-related diseases in Larangan Village. Using a quantitative, analytical, observational approach with a cross-sectional design, the study concentrates on two neighborhoods within Larangan Village. The population consists of 330 families in the 2nd neighborhood (Non-Climate Village Program) and 344 families in the 9th neighborhood (Climate Village Program), with a sample size of 109. The variables under investigation encompass waste management and climate-related diseases. Data collection involves conducting interviews and distributing questionnaires among community members in both neighborhoods. The collected data undergo analysis using Multigroup Analysis SEM PLS with a significance level of 0.05. The findings reveal a significant negative correlation between waste management and climate-related diseases, suggesting that enhanced waste management is linked to a reduced incidence of such diseases in the community.

Lysosomal dysfunction plays an important role in neurodegenerative diseases, including Parkinson’s disease (PD) and possibly Parkinson-plus syndromes such as progressive supranuclear palsy (PSP). This role is exemplified by the involvement of variants in the GBA1 gene, which results in a deficiency of the lysosomal enzyme glucocerebrosidase and is the most frequently identified genetic factor underlying PD worldwide. Pathogenic variants in the SMPD1 gene are a recessive cause of Niemann–Pick disease types A and B. Here, we provide the first report on an association between a loss-of-function variant in the SMPD1 gene present in a heterozygous state (p.Pro332Arg/p.P332R, which is known to result in reduced lysosomal acid sphingomyelinase activity), with PSP-Richardson syndrome in three unrelated patients of Chinese ancestry.
9.Flexor Hallucis Longus Transfer And V-Y Plasty: An Effective Treatment Modality for Chronic Achilles Rupture - A Case Series

Introduction: To assess outcomes of FHL transfer and V-Y
plasty for chronic Achilles rupture due to insertional Achilles
tendinopathy.
Materials and methods: A case series of 12 patients was
conducted between 1st January 2017 and 31st December
2018. The patients had short flexor hallucis longus tendon
transfer with gastrocnemius lengthening by V-Y plasty for
Achilles tendon rupture. Patients were allowed full weight
bearing at six weeks post-operatively, and were followed up
at three months and six months post-operatively, when the
range of motion of the ankle was examined, and the outcome
was assessed using the EFAS score.
Results: Of the 12 patients in the study, the majority were
males; the mean age was 50.6±8.96 years. A significant
improvement in dorsiflexion and plantarflexion was noted at
the six-month follow-up compared to the three-month
follow-up (P=<0.001 for both). When compared to the
normal side, dorsiflexion and plantarflexion of the affected
ankle were significantly less at three months but were
comparable at six months post-operatively. A significant
improvement was noted in the mean EFAS score at the sixmonth follow-up (25.5±5.71) compared to three months
(18.6±0.90) post-surgery (P=0.001). Males were also noted
to have significantly higher EFAS scores at their six-month
follow-up than females (P=0.022). In contrast, a negative
correlation was noted between the European Foot and Ankle
Society (EFAS) score at the final follow-up and age
(P=0.011).
Conclusion: FHL tendon transfer with V-Y plasty in chronic
Achilles rupture due to insertional Achilles tendinopathy is
an effective procedure resulting in the restoration of the
ankle range of motion and improvement in functional scores.
10.Variation of Proconvertase 1 and Resistin Gene as Risk Factor for Type II Diabetes Mellitus in Obesity Papua Population

Introduction: Risk factors for type 2 diabetes mellitus (T2DM) include obesity and some genetic factors. Obesity
involves mild chronic inflammation that predisposes cells to insulin resistance. Two genes that influence obesity
and insulin resistance are Proconvertase-1 (PC-1) and resistin (RETN). PC-1 affects the activation of hormones that
regulate satiety and hunger. Resistin is one of the inflammatory factors that influence the occurrence of insulin resistance. This study aimed to determine the influence of polymorphism in the PC-1 gene rs1044498 (C>A) and resistin
gene RETN + 299 G>A rs3745367 on the risk of diabetes in obese Papua population. Methods: This study involved
58 obese people with T2DM and 58 obese people without DM. We examined the characteristics of blood pressure,
lipid profile and insulin resistance by HOMA-IR. The genes examined were PC-1 rs1044498 (C>A) and RETN+ 299
G>A rs3745367 by the PCR-RFLP method. The relationship of gene variations with biochemical parameters was
determined with analysis of variance. The results were considered significantly different if P < 0.05. Results: In this
study, parameters of diastolic blood pressure, triglycerides and insulin resistance were higher while high density
lipoprotein (HDL) levels were lower and significantly different in the obese with T2DM group compared to the obese
only group. The carrier of the A allele in the PC-1 gene rs1044498 was higher in the obese group than the obese with
T2DM but not significantly different in biochemical parameters. Carrier of the AA genotype in the RETN gene + 299
G>A rs3745367 had higher triglycerides and HOMA-IR and lower HDL levels significantly different (P<0.05) than
other genotypes in the obesity with T2DM group. Conclusion: PC-1 rs1044498 gene was a risk factor for obesity but
not for T2DM, while RETN gene rs3745367 was a risk factor for dyslipidemia and diabetes in obese people in the
Papua population.
