1.Research on the Formation Mechanism and Intervention Strategy of Doctors' Defensive Medicinerom the Perspective of Multi-Dimensional Driving

Defensive medicine is a complex clinical decision-making phenomenon caused by the change of medical system and the generalization of medical risk.Based on the multi-dimensional driving perspective decon-structs defensive medicine into three types:quality improvement behaviorassurance behavior and avoidance behaviorintegrates event system theory,affective event theory,social cognition theory,self-worth orientation the-oryand builds a multi-dimensional driving chain of"environment-event-emotion-cognitive-attitudinal behavior".Systematically deconstruct the formation mechanism of defensive medicine.The multi-dimensional driving analysis shows that the intensitytime and space of the event jointly trigger the behavior adjustment;Emotional factors af-fect decision-making directly or indirectly through attitude mediating;Environmentcognition and behavior interacted with each other to form a cycle.Behavior is differentiated by attitudinal orientation.On this basis collaborative gover-nance strategies such as optimizing dispute handling mechanismcreating doctor-patient trust atmosphereand im-proving value incentive system are proposed to provide theoretical support and practical path for improving doc-tor-patient relationship and medical quality.
2.Research on Construction of Negative Defensive Medicine Priority Governance Strategies Based on the WSR Methodology

Objective Based on the WSR methodology,a three-dimensional integrated perspective of Wuli,Shi-li,and Renli dimensions was used to sort out the logical relationships and architectures among negative defensive medical governance strategies,forming the priority governance strategies for improving negative defensive medical care.Methods Based on the literature research method and the expert consultation method,the governance strate-gy and the conceptual connotation of WSR are classified into items,and the role relationship in the complex sys-tem of passive defensive medical governance is clarified through the WSR methodology.Results A collection of 40 negative defensive medicine governance strategies has been developed,comprising 8 strategies in the Wuli dimen-sion,23 strategies in the Shili dimension,and 9 strategies in the Renli dimension.Conclusion The WSR methodology provides a three-dimensional systemic perspective for governance strategies against negative defen-sive medicine.Addressing this issue requires comprehensive and systematic governance strategies.

Objective:Based on value orientation,it aimed to scientifically define the concept and connotation of accessibility to novel anticancer drugs,in order to deeply understand the nature and current status of the accessibility issues of novel anticancer drugs,and to provide a reference for the formulation and optimization of policies related to novel anticancer drugs.Methods:Data was collected through literature review and expert interviews,and the concept of drug accessibility was defined using the atomic diagram method.Results:The core images include"affordability","availability","high quality"and"patients".The concept of accessibility to novel anticancer drugs is defined as"the process of ensuring the sustainable supply,equitable access,affordability,and rational use of high-quality anticancer drugs to safeguard the realization of patient benefit goals."The connotation of the value orientation in policies on the accessibility of novel anticancer drugs is profoundly reflected in the multi-dimensional value-driven approach to ensure the ultimate benefit of patients,which includes quality,sustainability,equity,affordability,and rational use.Conclusion:The proposal of the concept and connotation of accessibility provides a theoretical basis for a deep understanding of the accessibility of novel anticancer drugs and offers valuable references for subsequent policy-making and practical operations.

In modern society,the diversity of risk causes,the complexity of risk trans-mission paths and the cascading effect of risk consequences make the uncertainty of the run risk of medical and health system increase sharply,and risk governance has become a rising topic in the academic circle.However,the traditional risk management system is riddled with difficulties,which has been difficult to meet the needs of modern governance,and resilient governance provides a new perspective for improving the governance capacity of the medical and health system run risk,and promotes risk management to"turn brittle into tough".How to empower the technical advantages of resilience governance to manage the run risk of the healthcare system,break the obstruction of the traditional governance mechanism,and explore its internal mechanism and driving mechanism are the key issues to realize the innovation of resilient governance and the modernization of governance capacity.

Objective:To define the concept of public value on digital supervision of medical insurance funds and to interpret its basic characteristics.Methods:Based on defining the concept of"public value"using the atomic atlas method,the formal logic definition method is used to define the"public value on digital supervision of medical insurance funds".Results:Among 47 concepts related to"public value",public,citizen,collective,and preference are the core elements;negotiation,utility,and expectation are the quasi-core elements;government,outcome,and common are the important elements;and integration,construction,and appeal are the non-important elements.The"public value on digital supervision of medical insurance fund"can be defined as"in the practice of digital supervision of the health insurance fund,citizens' value demands and expectations,and the degree of satisfaction of these demands and expectations,based on their knowledge and basic value judgments of the health insurance system and digital technology,and formed through a wide range of value-constructing mechanisms,such as social communication and political consultation."Conclusion:The public value on digital supervision of medical insurance funds has the characteristics of people-centeredness,the construction of multi-subject negotiation,and the integration of multiple value dimensions.These features together constitute the rich connotation of the public value of digital supervision of medical insurance funds,which provides a certain reference for promoting the modernization and governance of healthcare insurance and the construction of digital government.

10.Research on the Construction of a Theoretical Analysis Framework for Responding to Medical Surges in the Context of Complex Networks

Objective To define the conceptual connotation and characteristics of medical surges,and to construct a theoretical analysis framework for responding to medical surges based on complex networks.Methods By systematically reviewing relevant domestic and international research and practices,and from the perspective of complex networks and interdisciplinary integration,it analyzed the concept connotation,characteristics and comprehensive analytical framework of medical surges.Results The concept and connotation of medical surges were defined,and six major characteristics were summarized:sudden and surging congestion,overload spillover,stage recurrence,complex network and multi-dimensional collaboration,dynamic nonlinear multi-path evolution,and cascading amplification.A theoretical analysis framework was developed,encompassing four dimensions("risk and vulnerability coupling-complex surge scenario characterization-complex risk network chain drive-network dynamic collaboration capacity assessment and construction"),multiple hierarchical levels("macro-level healthcare system-meso-level hospital department functional systems-micro-level resource elements system"),and multiple scales("Wuli-Shili-Renli").Conclusion The proposed conceptual connotation,characteristics,and multi-dimensional,multi-level,and multi-scale theoretical analysis framework provide new perspectives and theoretical support for advancing the theoretical system of medical surges,guiding healthcare institutions to respond scientifically to surge impacts,and enhancing system resilience and network dynamic collaboration capacity.
