Objective: To analyze the serological characteristics of anti-Mur antibodies and investigate the distribution frequency of the Mur antigen among voluntary blood donors in Shiyan, thereby providing a basis for guiding clinical transfusion and establishing a Mur blood type database. Methods: ABO blood grouping of donors and patients was performed using an automated blood typing analyzer and the gel card method, respectively. Unexpected antibody screening and identification were performed using the saline, tube anti-human globulin, and polybrene methods. The specificity of anti-Mur antibodies was confirmed using Fisher's exact probability test. Plasma treated with 2-mercaptoethanol was used to distinguish IgM and IgG antibodies. IgM and IgG anti-Mur titers were determined by the saline tube method and the anti-human globulin tube method, respectively, at 4℃, room temperature, and 37℃. A total of 1 659 donor red blood cell samples were initially screened for the Mur antigen phenotype using three samples of human-derived anti-Mur plasma by the micro-tube method. Donors who tested positive for Mur antigen were further tested by the direct antiglobulin test (DAT); those with negative results were confirmed for Mur antigen by the gel card and polybrene methods. Results: Three blood samples were identified to contain mixed IgG and IgM anti-Mur antibodies. The titers of both IgM and IgG anti-Mur antibodies were highest at 4℃, intermediate at room temperature, and lowest at 37℃. The positive frequency of the Mur antigen among voluntary blood donors in Shiyan was 1.99% (33/1 659). Conclusion: anti-Mur antibodies were detected in both blood donors and patients in our region. The Mur antigen shows a certain distribution frequency among voluntary blood donors in Shiyan. Screening for the Mur blood type and establishing a corresponding database could enhance transfusion safety.

Objective Constructing a transitional care program for critically ill children in the ICU and exploring the effects of its application on the relocation stress and caregiving abilities of parents of children to improve their caregiving experience. Methods Convenience sampling method was used to select 150 children and their families transferred from PICU to the general ward of a tertiary pediatric hospital in Shanghai from January to September 2023 as research subjects.Subjects enrolled from June to September 2023 were selected as an experimental group, and were applied with the ICU transitional care program for critically ill children. Those enrolled from January to April 2023 were selected as a control group, and received routine ICU transitional care.Family Relocation Stress Scale and Family Caregiver Task Inventory were used to evaluate children's families at 24h before,24h after, and 72h after children's transition from ICU.The incidence of unplanned readmission to ICU within 48h and adverse events were compared between 2 groups of children. Results Eventually 74 pairs in the experimental group and 75 pairs in the control group completed the study. The Family Caregiver Task Inventory scores in the experimental group were lower than that in the control group(P=0.046).Generalized estimation equation showed that, there were significant differences on the intervention effect, the time effect, and the interaction effect of the Family Relocation Stress Scale scores in 2 groups(P<0.05).Conclusion The ICU transitional care program for critically ill children constructed in this study can effectively improve the level of migration stress and caregiving ability of parents of critically ill children, and optimize the quality of transitional care for critically ill children.

Hip fractures in the elderly are characterized by high incidence, high disability, and high mortality rates. Early, scientifically, and safe weight-bearing exercises are crucial for promoting functional recovery and reducing complications. Intelligent intervention is revolutionizing the field of rehabilitation through real-time monitoring, data-driven decision-making, and remote management, demonstrating particular potential in optimizing weight-bearing exercises. This paper systematically reviews the current application status of various types of intelligent interventions, such as real-time monitoring and feedback systems, intelligent rehabilitation assistance, and virtual reality technology in weight-bearing exercises for elderly hip fracture patients. This paper also analyzes their advantages in developing personalized rehabilitation plans, enabling precise monitoring and feedback, and preventing fall risks and explores challenges currently faced by intelligent interventions, such as device technical maturity, patient acceptance, economic costs, and data security. The purpose of this paper is to guide future technological optimization, clinical research, and policy support, thereby advancing the application of intelligent interventions in postoperative rehabilitation.

Objective·To gain a comprehensive understanding of the current clinical status of assessment and prevention of cutaneous graft-versus-host disease(GVHD)after haematopoietic stem cell transplantation in children, construct review indicators, analyze obstacles and facilitators in the process of conducting the study in light of clinical reality, and formulate strategies for change. Methods·Utilizing the JBI Evidence-Based Health Care Model as a theoretical guiding framework, the clinical problem was clarified. An evidence-based practice team was established, and a systematic literature search was conducted. The evidence was then evaluated and summarized.Additionally, review indicators were constructed, and review methods were clarified in relation to the evidence.Using the Ottawa Model of Research Use,the three dimensions of evidence change, potential adopters, and practice environment were analyzed for barriers and facilitators, and corresponding strategies were developed. Results·A total of 24 pieces of best evidence were included in the study, comprising five dimensions:skin assessment, assessment of protection, management of adverse skin reactions, medication guidance and follow-up and screening.Twenty-two review indicators were constructed on this basis,of which 14 had an implementation rate of<60%and 10 had an implementation rate of 0%;8 had an implementation rate of>60%and 6 had an implementation rate of 100.00%.The main obstacles were the lack of relevant training and the absence of a rational management mechanism. The main facilitating factors were reliable sources of evidence and a high level of cooperation from potential stakeholders.Considering the results of the analysis of the implementation rate of the indicators under review and the obstacles, alternative measures were formulated. Conclusion·There is a large gap between the evidence and clinical practice for the assessment and prevention of cutaneous GVHD after paediatric haematopoietic stem cell transplantation.Clinical change should be effectively implemented based on barriers and facilitators.

Objective:To explore the mediating effects of parental perceptions of child vulnerability (PPCV) and child anxiety in the relationship between parental social support and the quality of life in children with epilepsy. Methods:This was a cross-sectional study. Totally 315 children with epilepsy and their 315 parents who visited the neurologic outpatient clinic for video EEG monitoring from September 2021 to April 2022 were selected by convenience sampling and investigated with General Information Questionnaire, Child Vulnerability Scale (CVS), Social Support Rating Scale (SSRS), PROMIS Parents Report Anxiety Short Form, and the Quality of Life in Children with Epilepsy-16 (QOLCE-16). Pearson correlation was used to analyze the relationships between quality of life in children with epilepsy and variables such as parental social support, PPCV, and child anxiety. AMOS 22.0 was utilized to establish a structural equation model to examine the relationships among the variables. A total of 315 questionnaires were distributed, with 295 valid questionnaires retrieved, yielding a response rate of 93.7%.Results:The scores for the 295 parents on the SSRS, CVS, PROMIS Parents Report Anxiety Short Form, and QOLCE-16 were (40.75±10.24), (8.08±4.84), (16.40±5.98), and (72.01±15.43), respectively. Parental social support was negatively correlated with PPCV and child anxiety ( P<0.01), and both PPCV and child anxiety were negatively correlated with the child's quality of life ( P<0.01), while parental social support had a positive correlation with the child's quality of life ( P<0.01). PPCV and child anxiety mediated the relationship between parental social support and the child's quality of life ( P<0.01), with the direct and indirect effects being 0.301 and 0.205 respectively. Conclusions:Parental social support can directly affect the quality of life of children with epilepsy, and also indirectly influence it through PPCV and child anxiety. Clinical nursing staff should provide more care and support for children with epilepsy and their parents, helping to deepen their understanding of the disease, reduce levels of PPCV and child anxiety, thereby enhancing the quality of life for children with epilepsy.
