1.A qualitative study on the development and effectiveness evaluation of a training program for a nutritional support training base built on core competencies

Objective:To evaluate the application effectiveness of a training program for a nutritional support training base from the perspective of clinical nurses, and to provide a reference for optimizing and improving the training program.Methods:In October 2022, a clinical training program for nutritional support nurses was established based on the core competencies. The clinical training program was carried out from July to September 2023. The trainees who completed the training course were invited to participate in qualitative interviews to evaluate the course. The interview content was recorded, transcribed, and encoded using NVivo 11.0 software. The data were sorted and analyzed using thematic framework analysis.Results:Semi-structured qualitative interviews were conducted with 10 clinical nurses until data saturation was achieved. Three general themes and 10 sub-themes were extracted, including overall evaluation of the training course (rich, comprehensive, and reasonable training content; professional and skilled teaching team), changes brought by the course (improvement of clinical work confidence, strengthening of peer learning, enhancement of the initiative of independent learning, and identification of the factors that hinder the development of specialty), and recommendations for the course (the diversity of teaching forms, reasonable arrangement of practical training departments, increase in operational practice opportunities, establishment of online courses).Conclusions:The students had high overall evaluation on the training course of the nutritional support base. Based on their suggestions, the course can be optimized in the future by diversifying the teaching forms, rationally arranging the practical training departments, strengthening operational practices, and constructing online courses.

3.Development of indicators and analysis of barriers for assessment and prevention of cutaneous graft-versus-host disease after haematopoietic stem cell transplantation in children

Objective·To gain a comprehensive understanding of the current clinical status of assessment and prevention of cutaneous graft-versus-host disease(GVHD)after haematopoietic stem cell transplantation in children,construct review indicators,analyze obstacles and facilitators in the process of conducting the study in light of clinical reality,and formulate strategies for change.Methods·Utilizing the JBI Evidence-Based Health Care Model as a theoretical guiding framework,the clinical problem was clarified.An evidence-based practice team was established,and a systematic literature search was conducted.The evidence was then evaluated and summarized.Additionally,review indicators were constructed,and review methods were clarified in relation to the evidence.Using the Ottawa Model of Research Use,the three dimensions of evidence change,potential adopters,and practice environment were analyzed for barriers and facilitators,and corresponding strategies were developed.Results·A total of 24 pieces of best evidence were included in the study,comprising five dimensions:skin assessment,assessment of protection,management of adverse skin reactions,medication guidance and follow-up and screening.Twenty-two review indicators were constructed on this basis,of which 14 had an implementation rate of<60%and 10 had an implementation rate of 0%;8 had an implementation rate of>60%and 6 had an implementation rate of 100.00%.The main obstacles were the lack of relevant training and the absence of a rational management mechanism.The main facilitating factors were reliable sources of evidence and a high level of cooperation from potential stakeholders.Considering the results of the analysis of the implementation rate of the indicators under review and the obstacles,alternative measures were formulated.Conclusion·There is a large gap between the evidence and clinical practice for the assessment and prevention of cutaneous GVHD after paediatric haematopoietic stem cell transplantation.Clinical change should be effectively implemented based on barriers and facilitators.

5.Clinical study of three-dimensional reconstruction combined with intraoperative ultrasound precise hepatectomy in complicated intrahepatic bile duct stones

Objective:To evaluate the efficacy of preoperative three-dimensional (3D) image reconstruction combined with intraoperative dynamic ultrasonography (IOUS) in laparoscopic precision hepatectomy for intrahepatic bile duct stones.Methods:The clinical data of 66 patients with intrahepatic bile duct stones undergoing laparoscopic hepatectomy in the First People's Hospital of Fuyang District, Hangzhou from January 2018 to January 2023 were retrospectively analyzed, including 32 males and 34 females, aged (49.6±15.2) years old. Patients were divided into the study group ( n=32), who underwent laparoscopic precision hepatectomy using 3D reconstruction combined with IOUS, and the control group ( n=34), who underwent conventional laparoscopic hepatectomy. Perioperative data including the operation time, intraopera-tive blood loss, total volume of drainage on postoperative day (POD) 3, and the rates of complications were compared between the groups. Serum levels of interleukin-6 (IL-6), C-reactive protein (CRP), liver function indices including total bilirubin (TBil) and alanine aminotransferase (ALT) were monitored preoperatively and on POD 1, 3, 5, and 7. Results:The operation time was shorter in the study group [(178±17) min vs. (189±18) min, t=2.55, P=0.010]. The intraoperative blood loss was reduced in the study group [(218±19) ml vs. (395±21) ml, P<0.001]. The incidence of total volume of drainage >300 ml on POD 3 were comparable between the groups [9.4%(3/32) vs. 14.7%(5/34), P=0.507]. There were no significant differences in preoperative serum level of TBil, ALT, CRP and IL-6 between the groups (all P>0.05). Compared to the control group, serum levels of TBil, ALT and CRP in the study group were decreased on POD 1, 3, 5 and 7, and IL-6 was decreased on POD 1 and 3 (all P<0.05). The occurrences of postoperative bile leakage [9.4% (3/32) vs. 29.4% (10/34)] and liver cutting surface fluid accumulation [12.5% (4/32) vs. 35.3% (12/34)] and the rate of stone retention [3.0% (1/32) vs. 20.6% (7/34)] were lower in the study group (all P<0.05). Conclusion:Preoperative 3D image reconstruction combined with IOUS in laparoscopic precision hepatectomy for intrahepatic bile duct stones could reduce intra-operative blood loss, hepatic inflammatory response, and postoperative complications.
6.Correlation between vascular remodeling pattern and perforator stroke after stenting in patients with symptomatic intracranial vertebrobasilar artery stenosis

Objective:To investigated the correlation between vascular remodeling pattern and perforator stroke after stenting in patients with symptomatic intracranial vertebrobasilar artery stenosis.Methods:Patients with symptomatic intracranial vertebrobasilar atherosclerotic stenosis underwent stenting and high resolution magnetic resonance imaging (HR-MRI) from January 2017 to August 2020 were enrolled retrospectively. The data of demography, vascular risk factors, plaque characteristics, operation process and postoperative complications were collected. The plaque characteristics were observed by HR-MRI, and the correlation between vascular remodeling pattern and perforator stroke after stenting was analyzed.Results:A total of 41 patients were enrolled in the analysis. Their age was 60.1±8.8 years (range, 49-77 years). There were 31 males (75.6%). Among them, 21 (51.2%) were positive remodeling, 20 (48.8%) were non-positive remodeling, and 5 (12.2%) had perforator stroke after procedure. The incidence of perforator stroke in the positive remodeling group was significantly higher than that in the non-positive remodeling group (23.8% vs. 0%; P=0.048). The positive remodeling rate of the perforator stroke group was significantly higher than that of the non-perforator stroke group (100.0% vs. 44.4%; P=0.048). Conclusions:Patients with intracranial vertebrobasilar atherosclerotic stenosis and positive vascular remodeling were more likely to have perforator stroke after stenting.
7.Application of linkage teaching in pediatric nursing skills course based on integration of theory and practice

Objective:To explore the application of linkage teaching model between hospitals and colleges based on integration of theory and practice in the course of pediatric nursing skills.Methods:Two undergraduate classes of pediatric nursing in a college in 2017 were selected as the experimental group and the control group by random sampling. The experimental group adopted the combined teaching method of colleges and hospitals based on the integration of truth and practice, while the control group adopted the traditional teaching method. The results of skill operation assessment were compared between the two groups, and the results were evaluated by the self-designed teaching effect evaluation questionnaire.Results:The results of the combined nursing skills teaching in colleges and universities were higher in the experimental group (91.24±3.01) than that in the control group (87.33±2.96), the difference was statistically significant ( t value was 5.027, P<0.05). The evaluation score of teaching effect of the experimental group was 62.61 ± 2.94, which was significantly higher than that of the control group (49.67 ± 7.79) ( t value was 7.965, P<0.05). Conclusions:The combined teaching mode of colleges and hospitals based on the integration of theory and practice is beneficial to improve students′ comprehensive quality and learning effect, further shorten the gap between college theory and clinical practice, and is an effective form of pediatric nursing education.
8.Restoration of orbicularis oculi muscle function with totally implanted artificial facial nerve

Objective:To investigate the effect of the artificial facial nerve on the restoration of orbicularis oculi muscle function for unilateral peripheral facial paralysis in rabbit.Methods:Artificial facial nerve was implanted into the rabbit with unilateral peripheral facial paralysis between January 2018 and May 2019. At different time points after operation, the affected orbicularis oculi muscles′ stimulation closure threshold and the synchronism about the motion of the two sides of orbicularis oculi muscles were monitored. T test was used with SPSS 13.0 software. Results:There was no significant difference in the closure threshold of the orbicularis oculi muscle on the 7th and 28th days after artificial facial nerve implantation ( P>0.05). The synchronism of the normal side eye closing movement triggered the affected orbicular oculi muscle movement with the artificial facial nerve was up 19/20 to 20/20 at different times. Conclusions:The artificial facial nerve system had the features of stable working condition and high synchronizing effect for stimulating movement. It could restore the closed eye function in animals with peripheral facial paralysis animals and had great clinical application prospects.
9.A qualitative study on recognition and attitudes of donated breast milk to parents of premature infants

?? [Abstract]? Objective? To discuss the premature parents' recognition and level of acceptance of donated breast milk, and to provide the reference to further developing milk banks and increasing breastfeeding rate in premature infants. Methods? Semi-structured in-depth interviews were conducted among 12 parents of premature infants who were admitted to NICU in a specialized hospital in Shanghai from August to December 2017, and the Colaizzi analysis method was used to analyze data. Results? The premature parents recognized the meaning of the breast milk bank, but the level of acceptance of donated breast milk was not high for reasons such as lack of knowledge, traditional ideologies and the popularity of formula milk. Conclusions? At present, parents of premature infants lack objective recognition of donated breast milk. Hospital and breast milk banks should carry out a series of marketing projects and educate the mass about donated breast milk, in order to improve parents' recognition and acceptance of donated breast milk.
10. Laparoscopic ultrasound assisted hepatectomy in treatment of intrahepatic and extrahepatic bile duct stones

Objective:
To study laparoscopic ultrasound assisted hepatectomy in treatment of intrahepatic and extrahepatic bile duct stones.
Methods:
The data of 52 patients with hepatolithiasis who underwent laparoscopic hepatectomy from May 2014 to January 2019 at the Fuyang District First People's Hospital in Hangzhou were analyzed retrospectively. There were 28 men and 24 women. The median age was 56 years. Operative laparoscopic ultrasonography was used to detect the location, size, distribution of stones and their relations to blood vessels. A total of 43 patients underwent anatomic hepatectomy, while 38 patients underwent choledochoscopic stone extraction. A " T" tube was used according to intraoperative conditions.
Results:
Laparoscopic surgery was successfully carried out in 50 patients, while conversion to open surgery was required in 2 patients because of adhesions and bleeding. Laparoscopic ultrasonography revealed intrahepatic calcifications in 5 patients and choledochoscopy in 2 patients. Postoperative complications included 5 patients who devleoped abdominal abscesses. The operation time was (289.0±132.0) minutes. The intraoperative blood loss was (451.0±256.0) ml. The hospitalization after operation was (12.0±3.0) days. In 52 patients, 4 patients had residual stones and the residual rate was 7.7%. All of them were completely removed by T-tube sinus 8 weeks after operation.
Conclusions
Laparoscopic ultrasound helped to detect relevant bile ducts containing stones, reduced chance of bleeding in surgery, helped to clarify location and distribution of stones, improved accuracy of diagnosis, and reduced unnecessary hepatectomy by clearly defining intrahepatic bile duct stones intraoperatively. The residual intrahepatic and intrahepatic bile duct stones rates were reduced, and the safety and accuracy of the operations were improved.
