1.Status and influencing factors of impaired dignity in elderly incontinence-related dermatitis patients

Objective:To investigate the status of dignity impairment in elderly incontinence-related dermatitis patients, and to systematically analyze the influencing factors in order to provide reference opinions for the formulation of medical strategies by clinical medical staff.Methods:Using the cross-sectional survey method, from April 15 to April 30, 2024, aged incontinence-related dermatitis patients were selected from 37 tertiary comprehensive hospitals in 10 provinces and municipalities, including Zhejiang Province, Sichuan Province, Guangdong Province, etc. by convenient sampling method. The general data questionnaire and Patient Dignity Inventory were used to investigate their current situation.Results:A total of 315 questionnaires were collected, 307 were valid, and the effective rate was 97.5%. Among 307 cases, 146 cases were males, 161 cases were females, aged (71.46 ± 9.35) years old. The incidence of impaired dignity in elderly patients with incontinence-related dermatitis was 49.5%(152/307). The average score of Patient Dignity Inventory was (45.50 ± 20.17). The results of multivariate regression analysis showed that farming ( OR = 2.405, 95% CI 1.108-5.217), individual industrial and commercial households ( OR = 5.680, 95% CI 2.504-12.888), the primary disease were respiratory diseases ( OR = 2.716, 95% CI 1.018-7.246) and nervous system diseases ( OR = 6.117, 95% CI 2.572-14.551) were risk factors for impaired dignity in elderly incontinence-related dermatitis (both P<0.05). Not living alone ( OR = 0.381, 95% CI 0.184-0.790) and self-rated family relationship harmony ( OR = 0.630, 95% CI 0.449-0.882) were protective factors for dignity impairment in elderly incontinence-related dermatitis patients (both P<0.05). Conclusions:There are many factors that affect the dignity of patients with incontinence-related dermatitis in the elderly, and medical staff should comprehensively consider a variety of factors and establish and improve a prevention and management mechanism suitable for the dignity of patients with incontinency-related dermatitis in the elderly.

Cancer incidence in China has been rising steadily,with a particularly heavy burden from several high-prevalence malignancies.Medical examination for cancer plays a critical role in the early detection of cancer,precancerous lesions,and precursor conditions,thereby facilitating timely diagnosis and intervention.Such examination also addresses the growing demand for person-alized cancer screening services among diverse population groups.The development of evidence-based,context-specific cancer screening guidelines is essential to enhance the standardization,quality,and equity of preventive screening practices across the country,ultimately improving out-comes in early cancer detection and treatment.Guided by the Department of Medical Emergency Response of the National Health Commission,the Guidelines for Medical Examination for Cancer in Health Examination Agency(2025 Edition)were developed under the leadership of the National Cancer Center.A multidisciplinary panel of experts formulated the guidelines in accordance with the principles and methodology of the World Health Organization Handbook for Guideline Deve-lopment.The guidelines provide evidence-based recommendations on key clinical domains:target cancers and populations,overall screening workflow,screening protocols,diagnostic technolo-gies,result interpretation,follow-up procedures,and quality control.The primary objective is to standardize cancer screening practices in health examination agency and strengthen China's ca-pacity for prevention and control of high-burden cancers.

4.Relationship between killer cell immunoglobulin like receptor genotype and haplotype with dry eye diseas

Objective:To investigate whether killer immunoglobulin-like receptor(KIR)genotypes and haplotype are associated with the dry eye disease(DED)in a Chinese Han population.Methods:Polymerase chain reaction with sequence-specific primers(PCR-SSP)method was used to genotype KIR genes in 106 DED patients and 220 healthy controls.Results:A total of 23 KIR geno-types were found in DED patients and healthy controls,including 10 newly discovered KIR genotypes.The genotype G and haplotype 4 were associated with respectively increased risk of DED(P=0.025,P=0.004);while the haplotype 2 appeared to have an inverse asso-ciation with the disease(P=0.016).The frequency of KIR genotype B/B in DED patients was also significantly higher than that in con-trol group(P=0.027).KIR haplotype A and B had distinctive centromeric(Cen)and telomeric(Tel)gene-content motifs,and the Cen-B/B was associated with increased risk of DED(P=0.037).Conclusion:There may be an association between KIR genotype and hap-loid type with DED in Han population.

Objective:To explore the diagnostic value of cardiac color Doppler ultrasonography for left ventricular hypertrophy(LVH)complicated left heart failure(LHF).Methods:A total of 117 LVH patients complicated LHF admitted in Ningde City Hospital Affiliated to Ningde Normal College between January 2019 and January 2023(heart failure group)and 100 healthy people(control group)who underwent general physical examination in our hospital simultaneously were selected for the study.Left ventricular ejection fraction(LVEF),left atrial diameter(LAD),left ventricular end-diastolic diameter(LVEDd),early diastolic peak flow velocity/late diastolic peak flow velocity(E/A)were measured by cardiac color Doppler ultrasound in two groups.Receiver operating charac-teristic(ROC)curve was used to analyze the diagnostic value of cardiac color Doppler ultrasound indexes for LVH complicated LHF.In addition,the 117 patients with LVH and LHF were divided into mild group(n=54)and moderate to severe group(n=63)according to cardiac function class.Spearman correlation analysis was used to an-alyze the associationof cardiac color ultrasound parameters with cardiac function class.Results:Compared with par-ticipants in control group,those in heart failure group had significant lower LVEF[(47.88±4.75)% vs.(69.81±5.64)%],and significant higher LAD[(44.03±4.88)mm vs.(27.56±2.76)mm vs.],LVEDd[(55.68±5.04)mm vs.(42.19±1.38)mm],E/A[(13.04±3.58)vs.(6.60±1.67)](P<0.001 all).ROC analysis indicated that the area under the curve(AUC)of the combination of various parameters of cardiac ultrasound for diagnosing LVH with LHF was significantly higher than those of the single tests(Combined detection:AUC=0.901,95%CI 0.853~0.937 vs LVEF:AUC=0.644,95%CI 0.577~0.708,LAD:AUC=0.703,95%CI 0.637~0.763,LVEDd:AUC=0.633,95%CI 0.565~0.697,E/A:AUC=0.748,95%CI 0.685~0.804,Z=7.062,5.764,7.292,4.864,P<0.001 all).Compared with patients in mild group,those in moderate to severe group had signifi-cant lower LVEF[(45.67±3.37)% vs.(50.47±4.86)%],and significant higher LAD[(46.31±4.42)mm vs.(41.36±3.98)mm],LVEDd[(60.09±1.75)mm vs.(50.53±1.41)mm]and E/A[(13.99±2.96)vs.(11.93±3.92)](P<0.01 all).Spearman correlation analysis indicated that LVEF was negatively correlated with cardiac function class(r=-0.474),while LAD(r=0.511),LVEDd(r=0.863),E/A(r=0.269)were positively corre-lated with cardiac function class(P<0.01 all).Conclusion:Cardiac color Doppler ultrasound could better diagnose LVH complicated LHF,and also could effectively predict the cardiac function of patients with LVH and LHF.

9.The predictive value of an intratumoral and peritumoral radiomics nomogram based on high b-value diffusion apparent diffusion coefficient maps for prostate cancer

Objective To explore the preoperative diagnostic value of a radiomics nomogram based on intratumoral and peritumoral apparent diffusion coefficient(ADC)maps for prostate cancer.Methods A retrospective collection was conducted on MRI images of 503 patients with prostate lesions confirmed by pathology.The region of interest(ROI)was delineated on the ADC maps and extended 1-5 mm outward to form the peritumoral region.Radiomics features were extracted from both intratumoral and peritumoral regions,and radiomics models were established.A combined model integrating clinical model was constructed and a nomogram was drawn.The performance of each model and nomogram were evaluated.Results The combined model achieved the highest area under the curve(AUC)in the test set(AUC=0.823)at a peritumoral distance of 3 mm.The nomogram based on the combined model showed good predictive performance and clinical utility on both decision curve analysis(DCA)and calibration curve.Conclusion The radiomics nomogram based on intratumoral and peritumoral ADC maps has the greatest diagnostic value in distinguishing benign and malignant prostate cancer at a peritumoral distance of 3 mm before surgery.
10.Economic costs of self-monitoring of gestational diabetes mellitus in Beijing Area

Objective To analyze the economic cost of self-monitoring of gestational diabetes mellitus, and provide a basis for measuring the economic burden of gestational diabetes mellitus, and to provide a reference for the formulation of intervention development and the adjustment of resource allocation. Methods The individual economic cost of self-monitoring for gestational diabetes mellitus was measured based on a decision tree model, and the total economic cost of self-monitoring for gestational diabetes mellitus in Beijing was estimated. The uncertainty of the model parameters was analyzed using one-way sensitivity analysis. Results The average individual economic cost of gestational diabetes self-monitoring was 1184 RMB, and the individual cost incurred by choosing different types of blood glucose meters ranged from 403 to 18 000 RMB. The average individual economic cost of finger-stick blood glucose monitoring was 606 RMB and the average individual economic cost of continuous glucose monitoring was 2 374 RMB. The total economic cost of gestational diabetes self-monitoring in Beijing was 23.818 0 million RMB, and the total economic cost incurred by choosing different types of blood glucose meters ranged from 0.292 5 to 9.027 9 million RMB. The proportion of the finger-stick blood glucose monitoring had the greatest impact on the robustness of the results. Conclusion Finger-stick blood glucose monitoring is still the dominant self-monitoring method and is less costly than continuous glucose monitoring. Self-monitoring of pregnant women with gestational diabetes mellitus incurs certain economic cost and causes an economic burden on society.
