1.Construction and Practice of Cough and Asthma Pharmacy Clinic

OBJECTIVE To explore the practice mode of cough and asthma pharmacy clinic in Affiliates Hangzhou First People's Hospital, Zhejiang University, School of Medicine. METHODS To set up a cough and asthma pharmacy clinic, equipped with corresponding hardware and software facilities, and establish a cough and asthma pharmaceutical care mode. From December 1, 2020 to August 31, 2022, 2 357 patients in the hospital cough and asthma pharmacy clinic were collected, and their general data were analyzed. The patients learned by themselves according to the inhalation video, and then were guided by pharmacists to evaluate the differences in the use skills of patients before and after the intervention of pharmacists. The 345 patients with chronic obstructive pulmonary disease who were treated in the cough and asthma pharmacy clinic from December 1, 2020 to July 31, 2021 were collected. They were divided into 30 d follow-up group(n=173) and 15 d follow-up group(n=172) according to the blind sampling method. The score difference of Morisky medication Compliance Scale(MMAS-8) at 1, 3, 6 and 12 months of follow-up was compared. RESULTS Among 2 357 patients, 1 213(51.46%) were males and 1 144(48.54%) were females, 1 309 cases(55.54%) aged ≥60 years old, 563 cases(23.89%) aged 40-59 years old, 402 cases(17.06%) aged 20-39 years old, 32 cases(1.36%) aged 14-20 years old, and 51 cases(2.16%) aged<14 years old. The main diseases of the patients were chronic obstructive pulmonary disease(55.38%) and asthma(20.73%). Budesonide and Formoterol Fumarate powder for inhalation(37.08%) and Budesonide, Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol(25.84%) were frequently used. On the basis of patients' video teaching, pharmacists' mastery of inhaler use skills was better than that of patients' self-learning through video. The score before intervention was 5.360±1.208 and that after intervention was 7.890±0.356. There was significant difference between the two groups(P<0.05). At a month of follow-up, the MMAS-8 score of the 15-day follow-up group was 6.310±1.079. The MMAS-8 score of the 30 d follow-up group was 5.250±0.977, there was a significant difference between the two groups(P<0.05), but there was no significant difference in the MMAS-8 score of the two groups at 3, 6, 12-month follow-up. CONCLUSION The establishment of cough and asthma pharmacy clinic promotes the integration of pharmacy and respiratory clinical discipline, and improves the medication compliance of patients.
2.Aligned Organization of Synapses and Mitochondria in Auditory Hair Cells.

Recent studies have revealed great functional and structural heterogeneity in the ribbon-type synapses at the basolateral pole of the isopotential inner hair cell (IHC). This feature is believed to be critical for audition over a wide dynamic range, but whether the spatial gradient of ribbon morphology is fine-tuned in each IHC and how the mitochondrial network is organized to meet local energy demands of synaptic transmission remain unclear. By means of three-dimensional electron microscopy and artificial intelligence-based algorithms, we demonstrated the cell-wide structural quantification of ribbons and mitochondria in mature mid-cochlear IHCs of mice. We found that adjacent IHCs in staggered pairs differ substantially in cell body shape and ribbon morphology gradient as well as mitochondrial organization. Moreover, our analysis argues for a location-specific arrangement of correlated ribbon and mitochondrial function at the basolateral IHC pole.

3.Effect of peer education based on information technology on self-management behavior of patients after anus-preserving surgery for rectal cancer

Objective? To explore the effect of peer education based on information technology on self-management behavior of rectal cancer patients after anus-preserving surgery. Methods? Using convenience sampling method, from January to October 2018, patients with rectal cancer who were hospitalized in General Surgery Department of a Class Ⅲ Grade A hospital in Harbin were selected as the research objects. According to the time of admission, 34 patients from January to May 2018 were taken as control group and 37 patients from June to October 2018 were taken as observation group. Patients in the control group received routine discharge guidance, while patients in the observation group received peer education based on information technology. The effect of intervention was evaluated by Self-management Behavior Questionnaire for Intestinal Symptoms of Rectal Cancer Patients after Anus-preserving Operation and Self-rating Anxiety Scale(SAS). Results? After intervention, the total scores of Self-management Behavior Questionnaire for Intestinal Symptoms of Rectal Cancer Patients after Anus-preserving Operation, "treatment management", "perianal skin management","seeking help from others", "self-emotion adjustment" and "social management" in the observation group were higher than those in the control group with statistically difference (P< 0.05), and there was no statistically significant difference in the dimensional scores of "dietary management" between the two groups (P> 0.05). After the intervention, the SAS score of the observation group was lower than that of the control group, and the difference was statistically significant (P < 0.05). Conclusions? Information technology-based peer education can improve the self-management behavior of patients with rectal cancer after anus-preserving operation and reduce the anxiety level of patients. It is worthy of popularization and application.
4.Acupoint electrosensitivity and regularity based on bibliometric.

OBJECTIVETo explore the acupoint electrosensitivity and regularity for assisting the clinical diagnosis and treatment so as to improve the clinical therapeutic effects.
METHODSBy means of the computer retrieval (CBMdisc, CNKI, VIP, WanFang, PubMed and EMBASE) and manual retrieval, the relevant literature on acupoint electrosensitivity was reviewed and the modern literature database of acupoint electrosensitivity was established. Using bibliometric, the relations were analyzed among the detected indexes, acupoints, meridians and disorders in the conditions of acupoint electrosensitivity, and the potential regularity was explored.
RESULTSThe general detection indexes of acupoint electrosensitivity were resistance, inertia area, volt-ampere area, electric conduction quantity, electric potential, electric current and capacitance. The electrosensitive points included meridian points, auricular points and extraordinary points. Among those points, the electrosensitivity easily occurred at the special points.
CONCLUSIONThe acupoint electrosensitivity is mainly reflected on the electrical impedance and volt-ampere characteristics. The relative specificity exists between acupoint electrosensitivity and disorders. In clinical practice, detecting acupoint electrosensitivity may assist the diagnosis and instruct acupoint selection to improve the clinical therapeutic effects.
5.Study on the rheumatoid arthritis mouse model induced by glucose-6-phosphate isomerase

Objective To establish the rheumatoid arthritis (RA) mouse model induced by glucose-6-phosphate isomerase (GPI),and explore the mechanism of GPI in RA.Methods Totally 36 DBA/1 mice were randomly divided into three groups:test group (injection GPI),positive group (injection of bovine collagen Ⅱ),and negative group (saline).The rates and changes of weight were observed.The score of the arthritis,the ankle histopathological changes and serum GPI content were detected.Results Toes swollen slightly,joint swelling,deformity and accompanied by block were appeared at 35th day in the test group.Compared to the control group,the rates and changes of weight in test group showed a significant difference (P < 0.05).The score of arthritis was showed by x ± s.Compared to the negative group,the test group and positive group were showed significant difference (P < 0.05).A lot of lower synovial lining exudate macrophages,fibroblasts,and other inflammatory cells were increased in the test group.The GPI content in the test group [(0.39 ±0.11)μg/ml] was significantly higher than the negative group [(0.10± 0.06) μg/ml,P < 0.05].Conclusions GPI could induce rheumatoid arthritis in mice.It provides the experimental basis to diagnose RA.
6.Construction and characterization of enterohemorrhagic Escherichia coli O157:H7 ppk- deleted strain.

OBJECTIVETo construct enterohemorrhagic Escherichia coli (EHEC) O157: H7 ppk gene deletion strains and study its biological characteristics.
METHODSThe gene fragment of kanamycin resistance was amplified using a pair of homologous arm primers whose 5' and 3' ends were homologous with ppk gene and kanamycin resistance gene, respectively. EHEC O157: H7 EDL933w competent strains were prepared and transformed via electroporation with the amplification products. The ppk gene was replaced by kanamycin resistance gene using pKD46-mediated Red recombination system. The recombinant strain was confirmed by PCR and sequencing, and its morphology, growth ability and adhesion were assessed using Gram staining, OD600 value and Giemsa staining.
RESULTS AND CONCLUSIONWe established a ppk-deleted EHEC O157:H7 EDL933w strain with kanamycin resistance and compared the biological characteristics of the wild-type and mutant strains, which may facilitate further study of the regulatory mechanism of ppk gene.

7.Diagnosis of fetal complete atrioventricular block with echocardiography

Objective To evaluate the application of M-mode echocardiography and Doppler echocardiography in diagnosing complete atrioventricular block (CAVB) in fetus. Methods M-mode and Doppler echocardiography were used to screen the fetuses and bradycadia was established as CAVB in 10 cases. Atrial and ventricular rhythm was measured by M-mode echocardiography. Flow of mitral valve, left ventricular in-flow and out-flow tract, venous duct was measured by Doppler echocardiography. The characteristics and prognoses of CAVB fetus were compared and analyzed. Results CAVB was established as independence in rhythm of atrium and ventricle. The rhythm of atrium could be in normal range, while the rhythm of ventricle should be slower than normal. Enlarged chambers were observed in 6 cases, cardiac dysfunction in 5 cases, and pericardial effusion in 7 cases. Tricuspid regurgitation and mitral regurgitation existed in 5 and 1 case, respectively. All of the CAVB fetus in this study underwent abortions. Conclusions Fetal echocardiography is proven to be a useful tool to diagnose CAVB, which greatly influenced the cardiac function in fetuses. Clear diagnosis as early as possible is crucial to the treatment of CAVB fetus.
