1.Relevant Mechanism of Traditional Chinese Medicine in Treatment of Hyperandrogenism in Polycystic Ovary Syndrome: A Review

Polycystic ovary syndrome (PCOS) is a common endocrine and metabolic disorder among women of reproductive age. Hyperandrogenism (HA), one of its core pathological features, is closely associated with the clinical manifestations and metabolic complications of the disease. Current western medical treatments for PCOS-HA mainly include anti-androgen therapy and ovulation induction, such as short-acting oral contraceptives like Diane-35 and Yasmin. However, long-term use of these medications may result in adverse reactions like increasing the risk of liver dysfunction and exacerbating lipid metabolism disorders, with unsatisfactory long-term efficacy when used alone. Traditional Chinese medicine offers unique advantages in the treatment of PCOS-HA due to its holistic approach and multi-target regulatory mechanisms. In the view of traditional Chinese medicine, PCOS-HA is classified under the categories such as "delayed menstruation", "amenorrhea", and "infertility", with kidney deficiency as the root, as well as liver stagnation and spleen deficiency as the manifestations. Phlegm and blood stasis are considered to be intertwined throughout the disease course. Modern studies have shown that traditional Chinese medicine is significantly effective in improving the androgen levels, restoring ovulation, and improving insulin resistance in PCOS-HA patients. Representative prescriptions, such as Erxian Tang, Jiawei Xiaoyaosan, Guizhi Fulingwan, and Cangfu Daotantang, exert therapeutic effects through various mechanisms including regulation of the hypothalamic-pituitary-ovarian axis, reduction of ovarian androgen synthase activity, improvement of insulin signaling pathways, and inhibition of inflammation and oxidative stress, which demonstrates the characteristics of comprehensive treatment with traditional Chinese medicine. Based on the perspectives of etiology and pathogenesis of traditional Chinese medicine, modern medical cognition, typical prescriptions, and action mechanisms, this paper reviewed the research progress of traditional Chinese medicine in the treatment of PCOS-HA, aiming to provide a reference for in-depth research and clinical applications in this field.

3.Research hotspots in nutrition for patients with head and neck cancer from 2014 to 2024

Objective:
To understand the current status, international cooperation, research hotspots, and development trends of nutritional studies on patients with head and neck cancer from 2014 to 2024, and to predict future research trends.
Methods:
The Web of Science Core Collection database was searched to retrieve nutritional studies on patients with head and neck cancer from January 2014 to March 2024. The type of studies were “articles,” the language was English, CiteSpace 6.1 R6 software was used to conduct the bibliometric analysis, and the results were visualized to form a scientific knowledge map.
Results:
A total of 1 528 documents were retrieved, with a linear increase in the number of annual publications. The country with the highest number of publications was the United States, and the institution with the highest number of publications was the University of Queensland, with closer collaboration between authors and institutions. The most frequently cited publication was a set of nutrition guidelines, and the highest-impact articles were mainly concerned with performing percutaneous endoscopic gastrostomy. Keyword analysis showed that quality of life, radiotherapy, and weight loss were the keywords of highest interest. The keyword cluster analysis resulted in 17 clusters, which were divided into five main categories: head and neck cancer, treatment, outcome results, intervention modalities, and rehabilitation. Body composition, enteral nutrition, and accelerated postoperative rehabilitation were persistent research hotspots. Keyword highlighting revealed that “enhanced recovery after surgery” has been the focus of research in the last two years, with “index” and “model” emerging as theme words.
Conclusion
The number of publications in the literature related to nutrition for patients with head and neck cancer has increased annually over the past 10 years. The research hotspots mainly focus on the quality of life and weight loss during radiotherapy, the content and application prospect of body composition assessment, different modes of nutritional support interventions and enteral nutritional tube feeding routes, and perioperative nutritional management in enhanced recovery after surgery. The potential clinical value of preoperative nutritional intervention under the concept of enhanced recovery and the construction of new types of nutritional index are the trends of future research.
4.Impact of temperature on varicella incidence in Jiangjin District, Chongqing

Objective To examine the effect of temperature on the risk of varicella in Jiangjin District, Chongqing, and to provide reference for formulating varicella prevention and control strategies. Methods Data on varicella cases from 2015 to 2020 were collected through the China Disease Prevention and Control Information System, while meteorological data for the same period were collected through the China Meteorological Data Sharing Service Network. A distributed lag non-linear model was employed to analyze the correlation between different lag days (lag) and temperature and the incidence of varicella, as well as their impact on sensitive population. Using the lowest risk temperature as a reference, the relative risk of extreme low temperature 8°C (P5) and extreme high temperature 31.5°C (P95) on the onset of varicella was estimated. Results Over the six-year period, Jiangjin District reported 7 547 varicella cases, predominantly among children aged 5-9, accounting for 40.64% (3 067 cases) of the total. The incidence of varicella exhibited obvious seasonality and a bimodal distribution pattern. The exposure-response curve between daily varicella cases and temperature was U-shaped. At a low temperature of 8°C with a 3-day lag, the general population exhibited the highest risk, with a relative risk (RR) of 1.43 (95% CI: 1.22-1.67). After a 7-day lag, the cumulative relative risk (CRR) escalated to 2.79 (95% CI: 1.95-3.98). Conversely, at a high temperature of 31.5°C with no lag, the peak risk yielded an RR of 1.77 (95% CI: 1.23-2.37), and a 7-day lag resulted in a CRR of 2.05 (95% CI: 1.47-2.86). The temperature had the greatest impact on the incidence of varicella in preschool children. At 8°C with a 7-day lag, the CRR of varicella incidence in preschool children was 2.34 (95% CI: 1.89-2.91), with an attributable fraction (AF) of temperature reaching 38.49% (95% CI: 34.45%-42.06%). Conclusion Exposure to both low and high temperature in Jiangjin District, Chongqing can increase the risk of varicella, and the effect is more pronounced in preschool children.
5.Exploration on the paths of medical ethics education for medical students from the perspective of youth moral cultivation

The view of youth moral cultivation clearly defines the scope of “morality” and puts forward the requirement of “cultivating morality” for the youth. This is a systematic concept aligned with the main theme of the times and is worth deeply exploring and integrating into the practice of medical ethics education for medical students. With the requirements for innovation and development in medical education, the cultivation of medical students has also been endowed with new connotations. Guided by the connotations of the view of youth moral cultivation and aligned with the objective requirements of medical students’ cultivation, this paper leveraged the core values of traditional Chinese medicine as its entry point. It also explored the realization paths of medical ethics education for medical students that reflect advantages, highlight characteristics, and maintain clear orientation through strengthening the medical ethics education of “dedicated study of medicine,” “honesty and prudence in words and deeds,” “benevolence in heart and skills,” and “doctor-patient trust and harmony,” aiming to cultivate guardians of people’s health with noble medical ethics and superb medical skills.
6.Methods for Developing Rapid and Living Evidence-based Guidelines

The aims of developing rapid and living guidelines is to keep the recommendations in the guideline up-to-date. Compared with the conventional guideline, the rapid and living guideline can make better use of the existing evidence and apply and transform the evidence in a timely manner. This paper introduces the advantages and usage of rapid and living guidelines, the development process and existing challenges, and offers some insights, in order to provide reference for domestic organizations and scholars engaged in guideline development.
7.Well-differentiated/dedifferentiated liposarcoma associated with myxoid-like morphology: a clinicopathological and molecular genetic characteristics analysis of 34 cases

Objective:To investigate the clinicopathological and molecular genetic characteristics of well-differentiated/dedifferentiated liposarcoma (WDLPS/DDLPS) with myxoid-like morphology, and to distinguish them from myxofibrosarcoma (MFS) with similar morphology.Methods:Twenty-nine cases of myxoid-like liposarcoma and 5 cases of MFS were collected from Henan Provincial People′s Hospital, Zhengzhou, China and the First Medical Center of PLA General Hospital, Beijing, China from January 2015 to March 2023. Relevant markers were detected using immunohistochemistry and fluorescence in situ hybridization (FISH). The literature was also reviewed.Results:There were 24 males and 10 females, with ages ranging from 41 to 73 years. The tumor sites included retroperitoneum ( n=17), abdomen ( n=9), lower limbs ( n=5), scrotum ( n=1), upper limb ( n=1) and axilla ( n=1). WDLPS was commonly seen as lipomatoid type (12 cases), while the dedifferentiated components of DDLPS included low-grade (13 cases) and high-grade (2 cases) morphology, with low-high grade myxofibrosarcoma, dermatofibrosarcoma protuberans, and low-grade fibrosarcoma structures. Twenty-nine liposarcomas had various proportions of myxoid-like morphology, while 16 showed various degrees of tumor necrosis. The myxoid-like component showed myxoid pleomorphic liposarcoma (MLPS)-like morphology, lobulated growth, characteristic slender, ramified capillary network,"chicken claw-like"morphology, mucus-rich stroma and lung edema-like morphology. Tumor cells were spindle and oval, with many variable vacuolar lipoblasts. MDM2 gene amplification was detected using FISH and present in all tested cases (29/29). DDIT3 break-apart mutation was not detected, but its cluster amplification was present (24/29). Among the MFS cases, one showed cluster amplification (1/5), but no cases showed break-apart or amplification of MDM2 gene. Conclusions:WDLPS/DDLPS with myxoid-like morphology is most commonly seen in the retroperitoneum and abdominal cavity and mostly harbors DDIT3 break-apart probe amplification, while this amplification is not specific to liposarcoma. For core biopsy specimens or very rare tumors in the limbs, when histology has mucinous stroma and MLPS-like morphology, misdiagnosis of MLPS or other non-lipomatous neoplasms with myxoid morphology should be avoided.
8.Discussion on pathological diagnosis of vascular diseases based on ISSVA classification

The diagnosis of vascular diseases is not only about distinguishing neoplastic or non-neoplastic lesions, but also focusing more on emphasizing the essence of the disease, namely the presence or absence of endothelial cell proliferation, and further to distinguishing true hemangioma tumors or vascular malformation. This article is based on the International Society for the Study of Vascular Anomalies (ISSVA) classification, which is widely used in clinical practice, and discusses the related pathological diagnosis issues of vascular diseases.
9.Clinicopathological and genetic characteristics of common lymphatic malformations in superficial soft tissues

Objective:To investigate the clinicopathological features, classification, and genetic characteristics of common lymphatic malformation (CLM) in superficial soft tissue.Methods:A retrospective study of 110 patients with the diagnosis of CLM at the Henan Province People′s Hospital, China from August 2019 to August 2022 was performed. The clinicopathological features, relevant immunohistochemical (IHC) staining results, and fluorescence quantitative PCR of PIK3CA mutation were analyzed, and patients were followed up.Results:Among the 110 CLM patients, there were 53 males and 57 females; 65 cases (65/110, 59.1%) were first detected when the patients were≤2 years old. The most common location was the head and neck in 41 cases (41/110, 37.3%). Clinically, 102 cases (102/110, 92.7%) were solitary, 83 cases (83/110, 75.5%) were skin-colored, 69 cases (69/110, 62.7%) had indistinct borders, and 10 cases (10/110, 9.1%) had diffuse and severe macroscopic manifestations. There were 52 macrocystic type (52/110, 47.3%), 23 microcystic type (23/110, 20.9%), and 35 combined type (35/110, 31.8%). The macrocystic CLM presented as soft, translucent masses with large cystic cavities on the cut surface, and histologically they were composed of large, irregularly dilated channels that were thicker with irregular smooth muscle and lymphocytic infiltration. Microcystic CLM showed wartlike projections or translucent blisters on the skin, with small honeycomb structures on the cut surface, and histologically consisted of round or angular dilated small lymphatic vessels with little or no smooth muscle. The combined CLM had both macrocystic and microcystic morphologies. IHC staining showed that the lymphatic endothelial cells were positive for LYVE-1, D2-40, PROX1, CD31, and VEGFR3 but negative for CD34; in the macrocystic and combined CLM vessel walls were positive for SMA. Eight of 13 CLM had PIK3CA mutation. All patients were followed up, and 24 (24/110, 21.8%) had relapses, which more frequently occurred in combined type, followed by microcystic type.Conclusions:CLM is a congenital vascular malformation composed of dilated, abnormal lymphatic channels, with PIK3CA mutation. There are significant differences in clinicopathological characteristics among the different types. Since microcystic and combined CLM are prone to recurrence, accurate pathological subtyping is necessary to guide treatment and to predict prognosis.
10.Evaluation of Clinical efficacy and Safety of Tislelizumab Combined with Anlotinib in the Treatment of Patients with Head and Heck Cancer

Objective To evaluate the safety and efficacy of small molecule targeted drug anlotinib combined with anti-PD-1 in-hibitor tislelizumab in the treatment of patients with head and neck cancer.Methods A total of 32 patients with head and neck cancer who received PADF regimen(loplatin+fluorouracil+anlotinib+tislelizumab)in the Department of Head and Neck Surgery,Beijing Cancer Hospital from January 2018 to September 2021 were retrospectively selected as the observation group,and 23 patients with head and neck cancer who received DF regimen(loplatin+fluorouracil)during the same period were selected as the control group.The safety and tolerability of the combination therapy were evaluated by the incidence of adverse events,and the clinical efficacy was evaluated by tumor response and patient survival.Results In the observation group,1 patient(3.1%)had complete response,14 patients(43.8%)had partial response,15 patients(46.9%)had stable disease,total response rate and disease control rate were 46.9%and 93.8%,respectively.Grade 3-4 adverse events occurred in 7 patients(21.9%).No unexpected adverse events or significantly in-creased toxicity were observed.Conclusion Tislelizumab combined with anlotinib has a higher response rate than traditional chemothera-py DF.
