Objective:To develop an ultrasound multi-view fusion recognition model and evaluate its clinical value in diagnosing fetal conotruncal defects (CTD).Methods:This prospective study collected cardiac ultrasound images from fetuses at 20-32 weeks of gestation undergoing prenatal ultrasound at Dongguan Maternal and Child Health Hospital between September 2022 and May 2024. The case group comprised fetuses diagnosed with CTD, while controls with normal cardiac structures were collected at a 1∶2 ratio. Both groups were divided into modeling training and validation sets at a 3∶1 ratio. One optimal standard image each from the four-chamber view, left ventricular outflow tract view, right ventricular outflow tract view, and three vessels and trachea view was included per fetus. A deep learning-based multi-view fusion recognition model was developed to differentiate normal conotruncal anatomy from CTD. Model performance was validated against post-abortion pathology or postnatal echocardiography results. SAS software was used for statistical analysis to calculate the sensitivity and specificity of three fusion models (based on positivity in any two, three, or four views, and were designated as Fusion Model 1, Fusion Model 2, and Fusion Model 3, respectively), with the optimal model determined by the maximum Youden index. Senior, intermediate, and junior prenatal sonologists independently diagnosed cases in the validation set under blinding conditions. Their diagnostic results were compared with those of the optimal model. Paired Chi-square test (Cochran's Q test) was employed to compare the differences between the diagnostic accuracy rates of sonologists at different experience levels and the sensitivity of the optimal model, thereby analyzing the auxiliary diagnostic value of the multi-view fusion recognition model. Results:The study included 88 CTD cases, excluding six cases (non-CTD diagnosed by post-abortion pathology or postnatal echocardiography or poor image quality), divided into 60 training and 22 validation cases (12 tetralogy of Fallot, four double outlet right ventricle, three transposition of great arteries, three persistent truncus arteriosus). The control group included 176 cases, excluding 15 cases (other cardiac abnormalities confirmed postnatally or poor image quality after re-evaluation), divided into 120 training and 41 validation cases. The sensitivities of Fusion Model 1, Fusion Model 2, and Fusion Mudel 3 were 0.86, 0.64, and 0.27, while their specificities were 0.76, 0.95, and 1.00, respectively. Fusion Model 1 demonstrated the highest Youden index (0.62) and was selected as optimal. Its diagnostic sensitivity showed no significant difference from senior sonologists [86% vs. 91% (20/22), Bonferroni-corrected P>0.999], but was significantly higher than intermediate [55% (12/22), Bonferroni-corrected P=0.049] and junior sonologists [32% (7/22), Bonferroni-corrected P=0.003]. Conclusion:The deep learning multi-view fusion model achieved diagnostic performance comparable to senior sonologists, demonstrating potential value in assisting CTD diagnosis, training less experienced sonologists, and supporting research and education.

Objective To establish the genetic typing detection technique of matrix-assisted laser de-sorption ionization-time or flight mass spectrometry(MALDI-TOF MS),and to apply it to investigate the polymorphism of the human platelet antigen(HPA)-29-35w low-frequency gene in blood donor population from Guangxi area.Methods The RS number of 7 target gene mutation sites in dbSNP and 21 primers were designed by using Assay Design Suite(ADS)of MassARRAY platform online primer design tool.Seven over-expression vectors inserting into HPA-29-35bb mutant sequence were constructed.Twenty-nine blood donors were randomly selected among the blood donors team in Guangxi area.The samples and vectors conducted the iPLEX Pro multiple genotyping analysis and mass spectrometric detection.Meanwhile,the above samples were sequenced and the sequencing results conducted the comparison validation with the mass spectrometric detec-tion results,then the MALDI-TOF MS genotyping detection technology was established.Then adopting this technology conducted the platelet antigen HPA-29-35W genotyping detection and polymorphism analysis in the samples from 588 blood donors in Guangxi area.Results The MALDI-TOF MS HPA genotyping detec-tion results were consistent with the sequencing results.The HPA-29-35w genotyping results showed that theHPA-29-35w genotype of 588 blood donors in Guangxi area was aa homozygote.Conclusion The genotype detection method of MALDI-TOF MS for HPA-29-35w is successfully established and applied to the screening of HPA-29-35w gene among the blood donor population in Guangxi area.

Through the screening of blood donors, detection of pathogen antigen and antibody, full coverage of nucleic acid detection, the risk of infectious blood transfusion has been reduced to a very low level. Especially, pathogen inactivation technology (PRT) has played an irreplaceable role in ensuring blood safety. The best way to inactivate pathogens is not only to effectively remove the target pathogens in the blood, but also to maintain the activity of active ingredients in the blood to the maximum extent, and it doesn′t affect the effect of blood therapy. In this paper, the development of pathogen inactivation technology is summarized, and the influence of pathogen inactivation treatment on the quality of blood components is discussed. It provides references for improving or developing new processing technology.

Objective? To explore the effects of family participation in the respiratory rehabilitation of the patients with chronic obstructive pulmonary disease (COPD) in terms of their treatment compliance and outcomes. Methods? Totally 80 COPD patients admitted in The First Affiliated Hospital of Guangzhou Medical University from March 2016 to August 2017 were selected and divided into the control group (n=40) and the observation group (n=40) according to the random number table. Patients in the control group received conventional respiratory rehabilitation intervention, while the patients had their family members participated in the rehabilitation process in the observation group. Treatment compliance and effects were compared between the two groups. Results? The compliance of the observation group totaled 97.50%, while that of the control group totaled 87.50% (χ2=7.207, P＜ 0.05). After 3 months' intervention, the walking distance within 6 min of the observation group was (278.72±45.89) m; the PEF pred% was (93.46±13.47)%; and FEV1% was (87.42±14.71)%, better than those of the control group (t=4.127,7.156,3.859;P＜ 0.05). Conclusions? Family participation in respiratory rehabilitation process can improve the compliance of COPD patients significantly, thus improve the treatment effects, which is worth promoting in clinical practice.

Objective:To investigate the status of the vestibular function of the patients with chronic positional symptoms after peripheral acute vestibular syndrome (AVS) and the curative effect of the vestibular rehabilitation therapy (VRT). Method:Using caloric test (CT), head shaking nystagmus test (HST), cervical vestibular evoked myogenic potentials as well as ocular vestibular evoked myogenic potentials to estimate the function of semicircular canal and otolith organs. The patients with normal VEMPs are divided as Group A. Otherwise are as Group B. Both groups are treated with VRT. The curative effect is estimated by vestibular symptom index (VSI) and Berg balance scale (BBS). Result:Thirty-three of 37 patients (86.5%) had an abnormal result of CT and HST, with 23 of these patients (65.7%) had an abnormal of both test. Twenty-two patients (59.5%) were in Group A and 15 (40.5%) in Group B. Before the therapy, Group B had a higher score of the balance and dizziness symptoms of VSI (P<0.05), and Group A had a higher score of the BBS (P<0.05). After the therapy, the VSI scores of both groups dropped and scores of the BBS raised. Conclusion:Patients with chronic positional symptoms after peripheral AVS have dynamic vestibular lesions to different extents. Those with otolith organs lesions tend to have a worse function of balance. Nevertheless, patients have a better off after VRT.

Objective To investigate the distribution situation of ABO and Rh blood groups among Zhuang population in Nanning area to guide the scientific voluntary blood donor recruitment and rational storage of blood in this area.Methods 2 052 blood samples from Zhuang population in Nanning area were performed the identification of ABO and Rh blood groups.Whether the gene distributions conforming to the Hardy-Weinberg equilibrium principle was assessed by using the Chi-square test.The data was compared with the distribution characteristics in other areas or other minorities.Results The distribution characteristics of ABO blood groups in Nanning area were O > B> A > AB.Blood group O was highest (46.78%)and blood group AB was lowest (4.34%).The gene frequencies of A,B and O were 0.131 9,0.181 5 and 0.686 6,respectively.The blood groups distribution con-formed to the Hardy-Weinberg equilibrium principle;the RhD positive proportion(RhD+ )in Zhuang population was 99.90%,while the RhD negative (RhD- )proportion was 0.10%,which was lower than that in Han population.No phenotypes of ccdE,CcdE, CCdee,CCdE,CCDEE and ccDee were observed in this study.Conclusion The distribution of the ABO and Rh blood groups among Zhuang population in Nanning still maintains their own exclusive characteristics.The extremely low proportion of RhD- will bring about serious difficulties to the patients with long term blood transfusion.

OBJECTIVE: To investigate the risk factor,type and characteristic nystagmus of the otolith abnormal migration during diagnosis and treatment for posterior semicircular canal benign paroxysmal positional vertigo (PSC-BPPV). The therapy and prevention is also discussed. METHOD: Four hundred and seventy-nine patients with PSC-BPPV were treated by Epley's canalith repositioning procedures(CRP) from March 2009 to March 2012. We observed otolith abnormal migration complicating during diagnosis and treatment. According the type of otolith abnormal migration, the additional repositioning maneuver was performed. RESULT: The rate of complication was 8. 1%(39/479), with canal conversion in 5.4%(26/479) and primarily canal reentry in 2.7%(13/479). The rate of incidence of conversion to horizontal canal conversion and anterior canal were 4. 8%(23/479)and 0. 6%(3/479) respectively. All the patient was cured in follow up. The risk factors were unappropriated head movement during or after CRP, including another Dix-Hallpike were performed immediately. CONCLUSION To prevent the complications,the pathognostic positioning sequence and angle of head rotation are commenced during CRP. Appropriate short time postural restrictions post-treatment is necessary. Careful observation of nystagrnus variation is crucial to determine the otolith abnormal migration.

Objective To investigate the roles of cardiac activity index(Tei index) on evaluation the right ventricular function after neonatal asphyxia.Methods Sixty neonatal asphyxia who included 35 cases of mild asphyxia(mild asphyxia group) and 25 cases of severe asphyxia(severe asphyxia group) and 30 cases of normal full-term newborns(control group) were selected.Echocardiographic examinations were performed on 24-48 h after birth,which included pulmonary artery systolic pressure (PASP),right ventricular ejection fraction(RVEF),tricuspid early diastolic peak(peak E) and late diastolic peak(peak A),and E/A ratio was acquired.The right ventricular cardiac activity index (RV-Tei index) was measured by Doppler spectrum.Results There was no significant difference in RVEF,E/A ratio among mild asphyxia group,severe asphyxia group and control group (P ＞ 0.05).RV-Tei index in mild asphyxia group and severe asphyxia group was increased compared with that in control group (0.489 ± 0.090,0.625 ± 0.100 vs.0.345 ± 0.120),and there was significant difference (P＜ 0.05 or ＜0.01).There was significant difference in RV-Tei index between mild asphyxia group and severe asphyxia group (P ＜ 0.05).RV-Tei index in neonatal asphyxia was positively correlated with PASP (r =0.950,P ＜ 0.05),and there was no relationship between RV-Tei index and gestational age,weight,heart rate (r =-0.068,-0.280,-0.360,P ＞0.05).Conclusions Neonatal asphyxia can lead to disorders of the right ventricular function.Tei index can evaluate early overall changes of the right ventricular function and is better than conventional ultrasound technology in neonatal asphyxia.

ObjectiveTo explore the clinical value of respiratory mechanics in neonates with neonatal respiratory distress syndrome (NRDS).MethodsAccording to prognosis,55 newborns with NRDS who had been treated with pulmonary surfactant in our hospital from Jan 2006 to Dec 2008 were divided into 2 groups:44 newborns in survival group and 11 newborns in death group.The dynamical changes of compliance of the respiratory system (Crs),respiratory index (RI) and PA-a O2 were measured during mechanical ventilation.ResultsAt 6 h after mechanic ventilation,there were significant differences in Crs,PA-a O2,OI and RI between the survival group and the death group (t=6.871,-6.450,-10.316,-8.815,P＜0.05 ).In the survival group,RI,PA-aO2 and OI decreased with the time of ventilation,and there were significant differences among the different time point ( F=215.56,147.85,384.64,P＜0.05).In thesurvival group,Crs increased with the time of ventilation,and there were significant differences among the different time point ( F=292,P＜0.05).In the death group,RI,PA-aO2 and OI increased respectively just before dying than at 6 h after mechanic ventilation (t=-5.699,-5.578,-6.924,P＜0.05 ).In the death group,Crs decreased respectively just before dying than at 6 h after mechanic ventilation (t=11.767,P＜0.05 ).Crs was negative correlated with OI,RI and PA-aO2 (r=-0.813,-0.800,-0.859,P＜0.05 ).ConclusionMonitoring in NRDS children's dynamic changes of Crs,RI,OI,PA-a O2 can help observing effect of treatment,guiding ventilator treatment,grasping the opportunity and assessing the prognosis offline in NRDS.
