1.Diagnosis and treatment of pediatric sinusitis based on "the transmission of heat from gallbladder and lung" idea in Huangdi Neijing

Sinusitis is a prevalent nasal disease in children, characterized by chronic and difficult-to-treat symptoms. Its onset is related to nasal stagnation, gallbladder and lung dysfunctions. This article explores the root cause based on Huangdi Neijing by considering the physiological and pathological characteristics of children. The core pathogenesis of pediatric sinusitis is the transmission of heat from the gallbladder and lung to the brain and nose, disrupting normal nasal function. Wind and heat pathogens often persist, accumulate, and transform into turbid qi, which are common triggers of the disease. Evil qi retention and yin depletion are internal factors that cause the prolonged and unhealed condition of the disease. This article emphasizes individualized treatment approaches based on disease duration and the severity of pathogenic factors. If external pathogens remain uncleared, treatment should focus on dispelling wind, clearing heat, dispersing with pungent medicinals, and dredging nasal orifices. If internal fire is exuberant, clearing lung qi, inhibiting hyperactive liver yang, and clearing exuberant fire should be used to relieve stagnation. In chronic cases with residual pathogens and liver-kidney yin deficiency, nourishing yin, clearing fire, and moistening the nasal orifices are essential. When exuberant heat has subsided, but the symptom of a persistent runny nose continues, leading to the loss of healthy qi and damage to the lung and spleen, treatments that tonify the spleen, benefit the lung, and reinforce healthy qi should be adopted to relieve stagnation. These treatments aim to restore the balance of the body′s vital qi by addressing both the lingering symptoms and the underlying weakness of the lung and spleen. The diagnosis and treatment of pediatric sinusitis based on the theory of "the transmission of heat from gallbladder and lung" can help reduce the recurrence of sinusitis and alleviate symptoms, with the aim of broadening the approach of traditional Chinese medicine in treating this condition.
2.Mechanism of Liangxue Tuizi Formula in the treatment of Henoch-Sch?nlein purpura rats via reactive oxygen species-mediated activation of NLRP3 inflammasome

Objective To study the effect of Liangxue Tuizi Formula(LXTZF)on reactive oxygen species(ROS)-mediated NOD-like receptor thermal protein domain associated protein 3(NLRP3)inflammasome activation in Henoch-Sch?nlein purpura(HSP)rats,and to explore its possible mechanism in the treatment of HSP.Methods Twenty-four rats were divided randomly into four groups:control,model,LXTZF,and compound glycyrrhizin(CG)groups.Except for the control group,a model of HSP was established in the other groups by heat drugs combined with egg albumin.After successful modeling,rats in the LXTZF group were given LXTZF solution 7.47 g/kg,rats in the CG group were given CG solution 13.5 mg/kg by gavage,and rats in the control and model groups were given normal saline solution by gavage once a day for 4 weeks.Samples were collected 8 hours after the last gavage.Skin histopathology changes were observed by hematoxylin and eosin(HE)staining.Serum interleukin(IL)-18 and IL-1βlevels were detected by enzyme-linked immunosorbent assay(ELISA).Changes in ROS levels in the skin were detected by immunofluorescence.Apoptosis-associated speckle-like protein(ASC),NLRP3,cysteinyl aspartate-specific protease-1(Caspase-1)mRNA and protein expression levels in rat skin were detected by real-time quantitative polymerase chain reaction(RT-PCR)and immunohistochemistry and Western blot,respectively.Results The skin pathology in the model group showed obvious inflammatory cell infiltration compared with the control group.Serum IL-18 and IL-1β levels were significantly increased(P<0.05),skin ROS levels were significantly increased(P<0.05),and skin ASC,NLRP3,Caspase-1 mRNA and protein expression levels were significantly increased(P<0.05).Inflammatory cell infiltration in the skin tissues of rats was alleviated in the LXTZF and CG groups compared with the model group,while serum levels of IL-18 and IL-1β were significantly decreased(P<0.05).ROS levels in the skin were significantly decreased(P<0.05),and mRNA and protein levels of ASC,NLRP3,and Caspase-1 in the skin were significantly decreased(P<0.05).Conclusions The mechanism of LXTZF in HSP may be related to the inhibition of ROS-mediated NLRP3 inflammasome activation.
3.Based on Metabolomics,the Mechanism of Qufengxiaoschonlein Decoction in the Treatment of Henoch-Schonlein Purpura was lnvestigated

Objective To study the regulatory effect of Qufeng Xiaodian Formula on serum differential metabolites of allergic purpura,and provide scientific basis for the diagnosis and treatment of allergic purpura in traditional Chinese medicine.Methods Sixty rats were randomly divided into a blank control group(referred to as the blank group),a model group,a compound glycyrrhizin group,a low-dose Qu Feng Xiao Dian Fang group,a medium dose Qu Feng Xiao Dian Fang group,and a high-dose Qu Feng Xiao Dian Fang group according to a random number table method,with 10 rats in each group.The model group was constructed by combining dried ginger,pepper,and long pepper with ovalbumin to create an allergic purpura rat model.After successful modeling,each treatment group was intervened with corresponding drugs for 4 weeks.After 4 weeks,serum was collected and non targeted metabolomics screening of serum differential metabolites was performed using ultra-high performance liquid chromatography quadrupole time-of-flight tandem mass spectrometer(UPLC-QTOF/MS).Subsequently,data extraction and multivariate statistical analysis will be conducted to identify potential metabolic pathways.Results Compared with the control group,there were 91 possible differential metabolites in the serum of the model group rats,corresponding to 20 metabolic pathways;Compared with the model group,there were a total of 43 possible differential metabolites in the serum of rats in the wind dispelling and disease eliminating group,corresponding to 15 metabolic pathways.Among them,there are a total of 12 metabolic pathways.Inflammatory metabolites such as arachidonic acid and ceramide can damage vascular endothelium.Ten biomarkers,including arachidonic acid and ceramide,were significantly abnormal in the serum of the model group rats compared to the normal group.The Qufeng Xiaodian formula can significantly reverse these metabolites and significantly enrich them in arachidonic acid metabolism and sphingolipid metabolism pathways.Conclusion Qufeng Xiaodian Formula has a certain regulatory effect on metabolites such as arachidonic acid and ceramide that affect vascular endothelial injury.

6.Experience in Treating Children Chronic Cough based on the Theory of Healthy Qi Deficiency and Pathogenic Qi Retention

The main pathogenesis of chronic cough in children is the disorder of ascending and descending of qi movement caused by healthy qi deficiency and pathogenic qi retention. The deficiency of lung, spleen, and kidney is the root of the disease, and the retention of phlegm-fluid, food accumulation, and fire from constraint is the branch pathogenesis of the disease. In the treatment, we should reinforce and tonify healthy qi, dispel pathogen and regulate qi, with Yupingfeng Powder (玉屏风散) as the basic prescription. For lung qi deficiency syndrome, modified Yupingfeng Powder could be used for supplementing lung to consolidate the exterior; for lung and spleen qi deficiency syndrome, modified Yupingfeng Powder plus Shenling Baizhu Powder (参苓白术散) could be used for supplementing lung and fortifying the spleen, treating with both supplementation and transformation; for lung kidney qi deficiency syndrome, modified Yupingfeng Powder combined with Suzi Jiangqi Decoction (苏子降气汤) could be used for supplementing lung and replenishing kidneys, absorbing qi to the root. All the above prescriptions could combine the method of dispelling phlegm, promoting digestion and guiding out food stagnation, soothing the liver and draining fire to remove the solid pathogens, in order to treat the root and branch simultaneously, and the cough will stop if the ascending and descending of qi movement recover as usual.

8.Magnetic resonance imaging features of glioma with Brg/Brm associated factor 53a expression.

To explore the magnetic resonance imaging (MRI) characteristics of glioma with Brg/Brm-associated factor 53a (BAF53a) expression.
Methods: A total of 121 patients with glioma was divided into a BAF53a high expression group (n=79) and a low expression group (n=42) according to the results of immunohistochemistry. Then the MRI characteristics, including lesion location, number, boundary, maximum diameter, peripheral edema, midline structure shift, homogeneity, cystic necrosis, hemorrhage, strengthening degree, ependymal strengthening, pia mater enhancement, deep white matter invasion and lesion across the midline (total 14 items), were analyzed.
Results: The results showed that there were significance difference in lesion border, lesion edema, enhancement of the lesion, and deep white matter invasion between the 2 groups (all P<0.05).
Conclusion: The MRI characteristics, such as lesion border, lesion edema degree, enhancement degree of the lesion and deep white matter invasion, might be associated with BAF53a expression in gliomas.

9.Discussion on the Treatment of Children with Asthma in Remission Stage from Healthy Qi Deficiency and Pathogenic Factor Excess

Objective] The treatment of pediatric asthma in remission period is summarized from the positive and negative aspects, in order to provide new ideas and methods for clinical treatment. [Methods] By consulting the related literatures on the traditional Chinese medicine treatment of children with asthma in remission phase, the paper mainly summarized the different physicians' understanding of pathogenesis and approaches to the treatment of pediatric asthma in remission stage. [Results]The treatment of physicians for pediatric asthma in remission phase can be divided into asthenia healthy qi and sthenia pathogenic factor syndrome.The treatment of healthy qi deficiency mainly includes treating lung,spleen,kidney seperately;curing lung-spleen,lung- kidney, spleen-kidney ;treating three viscera meanwhile.The therapy of pathogenic excess contains treating wind,phlegm,stasis respectively;curing phlegm-stasis. [Conclusion]The treatment of traditional Chinese medicine for pediatric asthma has a unique advantage in improving children's physique, reducing the frequency of asthma attacks, and improve children's life quality which has significant clinical effect.
