1.Herpes zoster vaccination willingness and its influencing factors in middle-aged population in Zibo City

Objective To investigate the herpes zoster vaccination willingness in middle-aged population in Zibo City, and analyze its influencing factors. Methods A questionnaire survey was conducted among population aged 40-59 years in 24 streets of 8 districts and counties in Zibo City using stratified sampling method, and the willingness to vaccinate herpes zoster vaccine was statistically analyzed. The influencing factors of herpes zoster vaccination willingness in middle-aged population in Zibo City were analyzed. Results Among the 692 survey subjects, 654 cases (94.51%) were not vaccinated with herpes zoster vaccine, among which 170 cases (25.99%) were willing to be vaccinated. Multivariate logistic regression analysis showed that gender, age, education level, knowledge of herpes zoster, presence or absence of people with herpes zoster around them, knowledge of symptoms and sequelae of herpes zoster, knowledge of herpes zoster vaccine, knowledge of preventive effect of herpes zoster vaccine on herpes zoster, possible adverse reactions after vaccination, and acceptable effective rate of vaccine were the influencing factors of herpes zoster vaccination willingness in middle-aged population in Zibo City (P<0.05). Conclusion The willingness of herpes zoster vaccination is low in middle-aged population in Zibo City, and its influencing factors include gender, age, education level, herpes zoster cognition and vaccine cognition. It is necessary to strengthen the education of herpes zoster and vaccine among middle-aged population in Zibo City, and formulate promotion strategies based on individualized needs to enhance the vaccination willingness of people.

Objective: To analyze the serological phenotypes and molecular genetic characteristics of a proband with ABO forward and reverse typing discrepancy and their family members, and to investigate the molecular genetic mechanism underlying the Ael/AelB phenotype caused by the c. 575T>C variant on an ABO
A1.02 background. Methods: Serological testing and adsorption-elution assays were performed for blood group identification of the proband and family members. PCR-SBT, cloning sequencing, and Nanopore third-generation sequencing were used to analyze ABO gene variants and haplotypes. Phyre2 and PyMOL were used to predict the effect of the p. Ile192Thr variant on the structure of glycosyltransferase A (GTA), and plasma GTA activity was evaluated using an in vitro transferase assay. Results: Among thirteen family members, four members showed a serological phenotype consistent with the Ael subtype and one with the AelB subtype. Cloning sequencing and third-generation sequencing confirmed that five family members carried the ABO
A1.02 (c. 575T>C) variant, which co-segregated with the weak A phenotype in the family. Structural modeling suggested that p. Ile192Thr may alter the local spatial conformation and hydrogen-bond interactions of GTA. Plasma GTA activity testing showed markedly reduced GTA activity in the tested individuals. A1.02 (c. 575T>C) variant is associated with the Ael/AelB phenotype. The p. Ile192Thr substitution may weaken A antigen expression by altering GTA conformation and reducing enzymatic activity. This study provides additional evidence for the role of this variant in the Ael/AelB phenotype from the perspectives of familial inheritance, full-length haplotype analysis, and functional assessment, which is important for accurate ABO subtype identification and individualized transfusion strategies.
3.Serological and molecular biological analysis of a rare Dc- variant individual

Objective: To reveal the molecular biological mechanism of a rare Dc-variant individual using PacBio third-generation sequencing technology. Methods: ABO and Rh blood type identification, DAT, unexpected antibody screening and D antigen enhancement test were conducted by serological testing. The absorption-elution test was used to detect the e antigen. RHCE gene typing was performed by PCR-SSP, and the 1-10 exons of RHCE were sequenced by Sanger sequencing. The full-length sequences of RHCE, RHD and RHAG were detected by PacBio third-generation sequencing technology. Results: Serological findings: Blood type O, Dc-phenotype, DAT negative, unexpected antibody screening negative; enhanced D antigen expression; no detection of e antigen in the absorption-elution test. PCR-SSP genotyping indicated the presence of only the RHCE
c allele. Sanger sequencing results: Exons 5-9 of RHCE were deleted, exon 1 had a heterozygous mutation at c. 48G/C, and exon 2 had five heterozygous mutations at c. 150C/T, c. 178C/A, c. 201A/G, c. 203A/G and c. 307C/T. Third-generation sequencing results: RHCE genotype was RHCE

5.A systematic review of quality assessment tools for pediatric palliative care based on COSMIN guidelines

Objective To evaluate the methodological quality and measurement attribute quality of the evaluation tool for pediatric palliative care quality assessment tools,and to provide references for medical staff to select the best assessment tools.Methods The PubMed,Embase,Cochrane Library,Web of Science,CINAHL,Scopus,China National Knowledge Infrastructure(CNKI),Wanfang Database,VIP Database,Chinese Biomedical Literature Database,GIN,NGC,NICE,NRAO,medlive,WHO,AAHPM,WHPCA,APHN were searched from inception to March 28,2024.Data were screened and extracted independently by 2 researchers.The consensus-based standards for the selection of health measurement instruments(COSMIN)checklist and quality criteria were employed to evaluate the methodological quality and psychometric properties of the included pediatric palliative care quality assessment tools.Finally,recommendations were formulated based on these evaluations.Results A total of 13 articles were included,involving 9 pediatric palliative care quality assessment tools.Among them,the PICU-QODD,PaPEQu and QCPCI demonstrated good content validity and internal consistency,and are recommended as Grade A.The remaining assessment tools are recommended as Grade B or C.Conclusion The PICU-QODD,PaPEQu and QCPCI are recommended for use,but further validation of their psychometric properties is still needed.
6.Clinical Observation of ZHANG's Empirical Formula Directional Drug Delivery Combined with Auricular Acupressure on Postoperative Pain in Hand Trauma Patients with Qi Stagnation and Blood Stasis

[Objective]To explore the effects of ZHANG's empirical formula directional drug delivery combined with auricular acupressure in treatment for postoperative pain in hand trauma due to Qi stagnation and blood stasis.[Methods]A total of 152 postoperative patients with hand trauma with Qi stagnation and blood stasis who were hospitalized in the Department of Hand and Foot Surgery at Hangzhou Fuyang Hospital of Orthopedics of Traditional Chinese Medicine from September 2023 to January 2024 were selected.The patients were randomly divided into control group,directed group,acupressure group and combined group,with 38 cases in each group.The control group received routine nursing care;the directed group received ZHANG's empirical formula for directed transdermal medication in addition to the regular care;the acupressure group received auricular acupressure treatment in addition to the regular care;and the combined group received both ZHANG's empirical formula for directed transdermal medication and auricular acupressure treatment in addition to the regular care.The numerical rating scale(NRS)pain scores,peripheral blood circulation levels of the affected limb at different time points before and after intervention,clinical efficacy,cumulative use of analgesics within 72 hours post-surgery and the occurrence of adverse reactions were compared among the four groups.[Results]Thirty minutes after intervention,the NRS scores of both combined group and acupressure group were lower than those of the other groups(P<0.05).From 12 to 72 hours after intervention,the NRS scores of combined group remained significantly lower than those of the other groups(P<0.05).The effective rates were 80.6%in control group,85.7%in directed group,88.9%in acupressure group and 91.4%in combined group,with statistically significant differences among the four groups(P<0.05).After intervention,statistically significant differences were observed in peripheral blood circulation of the affected limbs among the four groups(P<0.05),and combined group demonstrated superior outcomes compared with the others.Additionally,significant differences were found in cumulative analgesic consumption within 72 hours of post-surgery(P<0.05),where the control group showed the highest usage while the acupressure group and combined group exhibited the lowest.No adverse reactions were observed in any enrolled patients.[Conclusion]ZHANG's empirical formula directional drug delivery combined with auricular acupressure can effectively alleviate post-operative pain due to Qi stagnation and blood stasis in hand trauma,reduce the cumulative analgesic consumption within 72 hours post-surgery,and improve peripheral blood circulation in the affected limb,making it worthy of further promotion and application in clinical practice.

8.Risk factors for dysphagia in community-dwelling older adults: a Meta-analysis

Objective:To systematically evaluate the risk factors for dysphagia among community-dwelling older adults.Methods:A comprehensive literature search was conducted in databases, including China National Knowledge Infrastructure, Wanfang Data, VIP, PubMed, Medline, Embase, CINAHL, PsycINFO, Web of Science, and the Cochrane Library, with the search ending on July 31, 2024. The quality of cross-sectional studies was assessed using the Agency for Healthcare Research and Quality (AHRQ) criteria, while cohort and case-control studies were evaluated using the Newcastle-Ottawa Scale (NOS). Meta-analysis was performed using Stata 18.Results:A total of 26 studies were included. The prevalence of dysphagia in community-dwelling older adults ranged from 7.9% to 63.7%. The following were identified as the risk factors for dysphagia ( P<0.05) : advanced age [ OR=1.17, 95% CI (1.08, 1.28) ], history of coughing or choking [ OR=7.25, 95% CI (3.66, 14.33) ], poor oral health [ OR=1.99, 95% CI (1.63, 2.43) ], impaired tongue-lip motor function [ OR=3.41, 95% CI (1.48, 7.85) ], reduced tongue pressure [ OR=1.63, 95% CI (1.16, 2.32) ], chewing dysfunction [ OR=2.18, 95% CI (1.57, 3.04) ], medication use history [ OR=1.22, 95% CI (1.02, 1.47) ], malnutrition [ OR=1.92, 95% CI (1.04, 3.56) ], history of the comorbid disease [ OR=4.18, 95% CI (2.61, 6.71) ], history of stroke [ OR=3.27, 95% CI (1.69, 6.33) ], depression [ OR=3.18, 95% CI (2.18, 4.65) ], frailty [ OR=1.43, 95% CI (1.12, 1.82) ], and male gender [ OR=2.91, 95% CI (1.70, 4.96) ] . Conclusions:Medical staff should pay close attention to the above-mentioned risk factors and develop targeted prevention strategies to reduce the incidence of dysphagia in community-dwelling older adults.
9.Serology Identification and Molecular Mechanism of ABO Blood Group with Weak Expression of Erythrocyte A Antigen Caused by ABO*AW Allele

Objective To investigate the serological and molecular biological characteristics of 4 samples and family with ABO positive and negative stereotypes caused by ABO*AW and to study its inheritance mode.Methods The ABO blood group phenotype of the samples was identified by conventional blood group serology and absorption-elution test.The seven exons and their flanking introns of the ABO gene were amplified by PCR,and the amplified products were analyzed by direct sequencing and cloning sequencing.A pedigree investigation was carried out on the relatives of the proband.Results The blood group serology of the proband 1,2,3 were AweakB,with IgM-A1 antibody.The blood group serology of the proband 4 was AweakB.Sequencing.The genotypes of the four probands were as follows:ABO*AW.25/B.01,ABO*AW.26/B.01,ABO*AW.37/B.01,ABO*AW.37/B.01.Compared with the ABO*A1.01,ABO*AW25 exhibits c.467C>T,c.829G>A and c.1061del G mutation sites.ABO*AW.26 has c.467C>T,c.527G>A and c.1061del G mutation sites.ABO*AW.37 has a c.940A>G mutation site.The pedigree investigation showed that the blood group serological results of the 4th proband's father,mother,husband and son were Aweak,B,A and A,the ABO*AW.37 allele existed in the ABO gene of the 4th proband's father and son.Conclusion The point mutation on the ABO*A1.01 can generate the ABO*AW,resulting in weak expression of a antigen on erythrocytes,and it can descend stably.
10.Study of 12 blood donors with c. 389T>C variant of ABO*A1.01 allele and weak expression of A from Xi′an area

Objective:To carry out serological and molecular tests on 12 blood donors and family members of one proband with discrepancy results for ABO serological typing.Methods:Twelve blood donors with ABO discrepancies identified by the Blood Center of Shaanxi Province from March 2015 to December 2023 and family members of one proband were selected as the study subjects. Serological blood typing was carried out to determine their blood phenotype. ABO genotype of the samples was determined by direct sequencing of amplicons of exons 1 to 7 and cloning sequencing of amplicons of exons 6 and 7. This study has been approved by the Ethics Committee of Blood Center of Shaanxi Province (Ethics No.202328). Results:Serological results showed that 5 samples were Aweak, 4 samples were Aweak with anti-A1 antibody, and 3 samples were AweakB with anti-A1. Direct sequencing and cloning sequencing results showed that all 12 samples had the haplotype ABO* A1.01/c.389T>C, and family studies showed that the allele could be stably inherited. Glycosyltransferase activity in the plasma was decreased in all samples. Conclusion:The c. 389T>C variant of the ABO* A1.01 allele can alter the encoded amino acid p.Leu130Pro, which weakens the activity of A glycosyltransferase, ultimately leading to the weak expression of A antigen.
