1.Research on the Collaborative Evolution of County Medical Communities Based on the SFIC Model

The key to strengthen the construction of county medical communities in China is multidimensional synergy.Based on the SFIC model,it deeply analyzes the collaborative dilemmas of county medical community in China from four dimensions:initial conditions,catalytic leadership,institutional design and collaborative process.Specifically,it is manifested as the differentiation of values among the main entities within the county medical community,the uneven catalytic leadership of each levels and entities,vertical and horizontal institutional fragmentation and weak coupling among stakeholders to collaborate.Based on typical regional experiences,for the current dilemma of collaboration in county medical community,it proposes aoptimized paths for the collaborative evolution of tightly integrated county medical community in China.
2.Potential value of 18F-DPA-714 PET/MR in the diagnosis of autoimmune encephalitis

Objective:To evaluate the potential value of 18×10 3 translocator protein (TSPO) radioligand ( N, N-diethy1-2-(2-(4-(2- 18F-fluoroethoxy) phenyl)-5, 7-dimethylpyrazolo[1, 5-A]pyrimidin-3-yl)acetamide, 18F-DPA-714) PET compared with conventional MR in the detection of autoimmune encephalitis (AE), the correlation with clinical symptoms, and the monitoring of immunotherapy efficacy in patients with AE. Methods:From December 2021 to June 2024, 45 AE patients (17 males, 28 females, age (38.3±17.0) years) diagnosed at Ruijin Hospital, Shanghai Jiao Tong University School of Medicine and 10 healthy volunteers (7 males, 3 females, age (28.7±5.1) years) were enrolled in this prospective study. All participants underwent baseline 18F-DPA-714 PET/MR scans, and 23 of these AE patients underwent further follow-up 18F-DPA-714 PET/MR scans. 18F-DPA-714 PET positivity was defined as having an uptake intensity threshold higher than the mean SUV ratio (SUVR)+ 2 s of the corresponding brain region in healthy controls. MR positivity was defined as abnormal hyperintensity in a specific brain region or multiple brain regions on the T 2 fluid attenuated inversion recovery (FLAIR). The positive detection rates of 18F-DPA-714 PET and MR was analyzed using McNemar χ2 test, and the differences in the uptake intensity (SUVR) of 18F-DPA-714 between symptomatic and non-symptomatic groups, and between remission and non-remission groups after immunotherapy were compared using independent-sample t test or Wilcoxon rank sum test. Spearman rank correlation analysis was used to analyze the correlation between the changing rate of SUVR and the changing of the modified Rankin Scale (mRS) score before and after treatment. Results:The positive detecting rate of 18F-DPA-714 PET for AE was significantly higher than that of MR (73.3%(33/45) vs 35.6%(16/45); χ2=11.56, P=0.001). The cerebellar SUVR of ataxia patients was significantly higher than that of asymptomatic patients (1.22(1.06, 1.33) vs 1.08(0.99, 1.20); Z=-2.14, P=0.034). Follow-up imaging showed that the SUVR of patients in the remission group after immunotherapy was significantly lower than that in the non-remission group ((-15.19±10.17)% vs (14.26±13.36)%; t=5.81, P<0.001). There was a significant correlation between the changing rate of SUVR and the changing of the mRS score before and after treatment ( rs=0.65, P<0.001). Conclusion:Compared with conventional MR, 18F-DPA-714 PET has a higher positive detecting rate for AE, and has the potential to reflect the clinical symptoms of AE and monitor the efficacy of immunotherapy.

5.Reflection on general and innovative education of palliative care in China

With the process of China’s aging population intensifying, palliative care, as an important guarantee for improving the quality of life of terminally ill patients, is receiving more and more social attention, and the demand is constantly increasing. Palliative care needs versatile professionals, and general education can enhance people’s awareness and understanding of it, enabling more people to understand, accept, and participate in palliative care. With the advancement of knowledge and technology in palliative care, the traditional cramming education models are no longer able to meet the actual needs. Therefore, there is an urgent need to innovate palliative care education strategies. By analyzing the current problems in the general education of palliative care in China, this paper proposed thoughts and suggestions for general and innovative education of palliative care in several aspects, such as establishing general and innovative education systems and evaluation systems of palliative care, diversifying educational contents and methods, strengthening medical staffs training, promoting diversified student groups, and strengthening the popularization of palliative care knowledge among the public.
6.On the Formation Logic and World Significance of A Community of Common Health for Mankind

The concept of a community of common health for mankind profoundly expresses China’s important proposition of promoting the health and well-being of people in various countries and jointly maintaining global public health security, which has a distinct formation logic. The idea of a community of common health for mankind is rooted in the "real community" theory of Marxist, reflecting its value orientation in the field of global health, and highlighting the new era’s inherent requirements of "adhering to the people first". It is an organic unity of theoretical logic, value logic, and practical logic. The construction of a community of common health for mankind gathers broad consensus, highlights the distinct theme of world peace and development, responds to international concerns, and provides Chinese proposals and contributes Chinese strength for governing global public health, practicing multilateralism, and promoting the construction of new international relations.
7.Exploration of the interactive mechanism between graduates quality and teaching reform in Chinese medicine colleges

Through the theoretical analysis of teaching reform and quality management in tradi-tional Chinese medicine col-leges, this paper puts forward the necessity of improving the quality management and optimizing the reform of education and teaching. Meanwhile, this paper further expounds the comprehen-sive, guiding, operable, qualitative and quantitative principles of teaching reform and quality management in traditional Chinese medicine colleges. Based on the above analysis, the paper puts forward and prac-tices the reform countermeasures to improve the interactive mechanism between the quality of graduates and the reform of teaching in traditional Chinese medicine colleges. The countermeasures include establishing an effective quality tracking feedback mechanism for graduates, actively carrying out the monitoring and evalu-ation of teaching quality, and constructing the measurement and evaluation system of teaching quality of traditional Chinese medicine education. This paper combines the needs of the current society for the culti-vation of talents of traditional Chinese medicine and the training quality standards, so as to promote the development of traditional Chinese medicine.
8.Study on the Quality Control Method for Ganoderma Lucidum Using Quantitative Analysis of Multi-components by Single Marker

Objective:To establish an analysis method for determining the contents of 7 components including ganoderic acid C2, ganoderic acid B,ganoderic acid G,ganoderic acid A,lucidenic acid A,ganoderma D and ganoderic acid F in ganoderma by QAMS. Methods:An HPLC method was used. The column was WatersX-bridge C18(250 mm × 4.6 mm, 5 μm), the mobile phase was acetonitrile(A)-1% acetic acid(B) (28:72) with gradient elution(0-30 min:A-28% to 38%;30-45 min:A-38% to 55%) at a flow rate of 1.0 ml·min-1,the column temperature was 30℃, and the detection wavelength was 254 nm. Ganoderic acid A was used as the internal reference, the correction factors of ganoderic acid C2, ganoderic acid B, ganoderic acid G, lucidenic acid A,ganoderma D and ganoderic acid F were calculated,and the contents of the 7 components were calculated by an external standard method to verify the feasibility and applicability of QAMS. Results:The relative correction factors of multiple assessments were reproducible,and the relative deviation of the contents of 6 components in 16 batches of Ganoderma lucidum was less than 2% when compared with that of the external standard method. Conclusion:QAMS is accurate and reliable in the quality evaluation of Ganoderma lucidum.

10.Effect of Increasing Diffusion Gradient Direction Number on Diffusion Tensor Imaging Fiber Tracking in the Human Brain.

OBJECTIVE: To assess the effects of varying the number of diffusion gradient directions (NDGDs) on diffusion tensor fiber tracking (FT) in human brain white matter using tract characteristics. MATERIALS AND METHODS: Twelve normal volunteers underwent diffusion tensor imaging (DTI) scanning with NDGDs of 6, 11, 15, 21, and 31 orientations. Three fiber tract groups, including the splenium of the corpus callosum (CC), the entire CC, and the full brain tract, were reconstructed by deterministic DTI-FT. Tract architecture was first qualitatively evaluated by visual observation. Six quantitative tract characteristics, including the number of fibers (NF), average length (AL), fractional anisotropy (FA), relative anisotropy (RA), mean diffusivity (MD), and volume ratio (VR) were measured for the splenium of the CC at the tract branch level, for the entire CC at tract level, and for the full brain tract at the whole brain level. Visual results and those of NF, AL, FA, RA, MD, and VR were compared among the five different NDGDs. RESULTS: The DTI-FT with NDGD of 11, 15, 21, and 31 orientations gave better tracking results compared with NDGD of 6 after the visual evaluation. NF, FA, RA, MD, and VR values with NDGD of six were significantly greater (smallest p = 0.001 to largest p = 0.042) than those with four other NDGDs (11, 15, 21, or 31 orientations), whereas AL measured with NDGD of six was significantly smaller (smallest p = 0.001 to largest p = 0.041) than with four other NDGDs (11, 15, 21, or 31 orientations). No significant differences were observed in the results among the four NDGD groups of 11, 15, 21, and 31 directions (smallest p = 0.059 to largest p = 1.000). CONCLUSION: The main fiber tracts were detected with NDGD of six orientations; however, the use of larger NDGD (> or = 11 orientations) could provide improved tract characteristics at the expense of longer scanning time.
