1.Study of mild cognitive impairment diagnosis based on MRI radiomics from the frontal and temporal lobes combined with machine learning algorithms

Objective To explore the value of MRI radiomics based on the frontal and temporal lobes combined with multiple machine learning algorithms in the diagnosis of mild cognitive impairment(MCI).Methods Patients who underwent cranial MR examination were retrospectively selected.According to the inclusion and exclusion criteria,a total of 173 subjects were finally included and randomly divided into training set and test set in a ratio of 7∶3.After delineating the regions of interest(ROI)of the frontal and temporal lobes on T2-fluid attenuated inversion recovery(FLAIR)images,radiomics features were extracted based on the Pyradiomics data package.Features were screened through inter-and intraclass correlation coefficient(ICC),independent samples t-test,and the LightGBM algorithm.Diagnostic models were constructed using support vector machine(SVM),random forest(RF),decision tree(DT),K-nearest neighbor(KNN),gradient boosting decision tree(GBDT),and extreme gradient boosting(XGBoost)combined with 10-fold cross-validation respectively.The training set was further divided into 9 training data sets and 1 validation data set through 10-fold cross-validation,and the hyperparameters were optimized through iterative cycles.The diagnostic efficacy of the model was evaluated by receiver operating characteristic(ROC)curve and area under the curve(AUC),and the DeLong test was applied to compare the differences between different models.Results The AUC of the radiomics models constructed by SVM,DT,RF,KNN,GBDT,XGBoost in the training set were 0.951,0.992,0.998,0.957,1.000,and 1.000 respectively,in the validation set were 0.890,0.843,0.934,0.878,0.930,and 0.945 respectively,and in the test set were 0.902,0.711,0.899,0.849,0.889,and 0.882 respectively.Conclusion MRI radiomics based on the frontal and temporal lobes combined with multiple machine learning algorithms can diagnose MCI,and the model constructed based on SVM shows the highest diagnostic value.

3.Identification of chemical constituents in Shenfu injection and study on anti-inflammatory activities of its poly- acetylene compounds

OBJECTIVE To study the chemical constituents in S henfu injection and the anti-inflammatory activities of its polyacetylene compounds. METHODS Shenfu injection was separated and purified by macroporous adsorption resin ,medium pressure liquid chromatography ,preparative thin layer chromatography and reversed-phase semi-preparative high-performance liquid chromatography,and the compound structure was identified according to the physicochemical properties and spectral data. RAW 264.7 cell inflammation model was used to evaluate the anti-inflammatory activities of polyacetylene compounds . The effects of active polyacetylene compounds on the expressions of cyclooxygenase- 2(COX-2)protein were evaluated by Western blot assay. RESULTS Twelves compounds were isolated and identified from Shenfu injection ,including 8 ginsenoside compounds ,i.e. ginsenoside Rg 1(1),ginsenoside Re (2),ginsenoside Rb 1(3),ginsenoside Rk 1(4),20(R)-ginsenoside Rh 1(5),20(S)-ginsenoside Rg3 (6),notoginsenoside R 1(7),panaxatriol(8);4 polyacetylene compounds ,i.e.(3R,9R,10R)-panaxytriol(9),panaxydol(10), heptadeca-1,8-dien-4,6-diyne-3,10-diol(11)and panaxynol (12). Among 4 polyacetylene compounds ,only compound 10 had anti-inflammatory activity. Compound 10 was not toxic to normal RAW 264.7 cells;when the concentration of compound 10 ranged 12.5-50.0 μmol/L,it could significantly reverse the lipopolysaccharide-induced NO content increase in cell supernatant (P<0.05 or P<0.01);when the concentration of co mpound 10 was 50.0 μmol/L,it could significantly reverse the lipopolysaccharide-induced protein expression increase of COX- 2 in cells (P<0.05). CONCLUSIONS Compounds 4,7,10-12 are identified and reported in Shenfu injection for the first time ,and panaxydol possesses a certain anti-inflammatory effect.
4.Optimization of Processing Technology of Ginseng Radix et Rhizoma Rubra by HPLC Fingerprint Combined with Orthogonal Test

OBJECTIVE:To establish an HPLC fing erprint of Ginsen g Radix et Rhizoma Rubra ,and to optimize its processing technology. METHODS :HPLC method was adopted. The determination was performed on Waters SymmetryShield TM RP18 column with mobile phase consisted of acetonitrile-water (gradient elution )at the flow rate of 1.0 mL/min. The column temperature was set at 30 ℃,and the detection wavelength was 203 nm. The sample size was 10 μL. Using ginsenoside Rb1 as reference peak ,HPLC fingerprints of 10 batches of Ginseng Radix et Rhizoma Rubra was established. The similarity of them was evaluated by using Similarity Evaluation System of TCM Chromatogram Fingerprint (2012 A edition ) to confirm common peak. With steaming temperature,time and drying method as factors ,using the content of ginsenoside and fingerprint similarity as index ,the processing technology was optimized with L 16(43)orthogonal test design and verified. Cluster analysis was conducted with SPSS 19.0 statistical software of 10 batches of Ginseng Radix et Rhizoma Rubra and 3 batches of optimal processed sample. RESULTS :There were a total of 13 common peaks in the fingerprints of 10 batches of Ginseng Radix et Rhizoma Rubra. The similarity was more than 0.920;3 common peaks were identified ,such as ginsenoside Rg 1,ginsenoside Re ,ginsenoside Rb 1. The optimal processing technology included that steamed at 100 ℃ for 150 min,dried at 60 ℃. The results of validation test show that the contents of ginsenoside Rg 1,Re and Rb 1 were 0.26%-0.29%,0.17%-0.20%,0.47%-0.54%,and the similarity between 3 batches of Ginseng Radix et Rhizome Rubra optimal processed sample and the control fingerprints was more than 0.970. The results of cluster analysis showed that 10 batches of Gimseng Radix et Rhizoma Rubra and 3 batches of optimal processed sample could be clustered into two categories;HS3-HS10 could be clustered into one category ,and 3 batches of optimal processed sample ,HS1 and HS 2 be clustered into one category. 5.Psychological status of transgender group before and after mastectomy

Objective To explore the difference of psychological state and the change of psycho logical state between mastectomy group for the purpose of changing gender identity and breast reduction surgery group for the purpose of improving physical beauty.Methods The psychological evaluation scale of three layers and twelve dimensions was used to evaluate the psychological status of 24 patients who underwent clinical breast masculine surgery and aesthetic breast reduction before and after operation.The psychological characteristics between the two groups before and after operation were compared and analyzed.Data analysis was carried out through Stata 13.0 software package.Results There were significant differences in depression factor,anxiety factor and obsessive compulsive symp tom factor in the aesthetic breast reduction group before and after operation (t=2.194,2.183,2.090;P=0.039,0.040,0.048,P<0.05).There were significant differences in depression factor,anxiety factor,obsessive-compulsive symptom factor,bullying factor,psychological self,physical self and social self before and after mastectomy (t=4.188,3.730,2.484,2.570,5.898,3.531,3.162,P=0.000,0.001,0.021,0.018,0.000,0.002,0.005,P<0.05).There were significant differ ences in depression,anxiety,psychoticism,obsessive-compulsive symptoms,psychological self,physiological self and social self between mastectomy group and aesthetic mastectomy group before operation.(t=2.629,4.081,2.097,4.246,5.419,2.600,2.545;P=0.015,0.001,0.048,0.000,0.000,0.016,0.018,P<0.05).There was significant difference in postoperative depression,anxiety and obsessive-compulsive symptoms between the mastectomy group and the aesthetic mastectomygroup (t=2.117,2.170,3.764;P=0.046,0.041,0.001,P<0.05).Conclusions Plastic surgeons should pay attention to the psychological status and personality characteristics of different beauty seekers and give psychological guidance,which is of great significance to improve the medical experience and surgical effect of the beauty seekers.
6.Efficacy and safety of pazufloxacin mesylate eye drops for bacterial conjunctivitis:a multicenter randomized, double-masked, parallel controlled clinical trial

Objective To compare the clinical efficacy and safety following the topical application of pazufloxacin mesylate eye drops with levofloxacin eye drops (LOFX) for bacterial conjunctivitis.Methods A multicenter,randomized,investigator-masked,parallel-controlled clinical trial was performed.Five hundred and twenty eyes of 520 patients with bacterial conjunctivitis were enrolled from March to October 2008 in seven ophthalmic centers in China.The patients were randomized into trial group and control group in 3 ∶ 1 ratio with the 390 eyes in the trial group and 130 eyes in the control group.Pazufloxacin mesylate eye drops was topically used 4 times per day for consecutively 7-14 days in the trial group,and levofloxacin eye drops was used in the same way in the control group.Microbiological cultures for conjunctiva sac secretions and drug sensitive test were carried out before and at the end of the administration of eye drops.The signs and symptoms were observed and scored before treatment and 0,3,7 and 14days after treatment.The adverse events following the administration of the eye drops were evaluated and compared.Results The intention to treat analysis (ITT) showed that the cure rate was 59.38% in the trial group and 60.47% in the control group,with the effective rate 88.80% and 86.05%,respectively,with an insignificant difference between the two groups (x2 =0.12,P =0.72).The clinically evaluable analysis (CE) exhibited that the cure rates were 63.48% in the trial group and 63.87% in the control group,with the effective rate 92.46% and 88.24%,whichwas not significantly different between them (x2 =0.54,P=0.46).The modified-ITT analysis (mITT) showed that the cure rates were 60.57% in the trial group and 62.07% in the control group,with the effective rate 90.32% and 88.51%,without significant difference between the two groups (P>0.05).Based on microbiologically evaluable analysis (ME),the clinical cure rates were 63.71% and 63.41% in the trial group and control group,and the effective rates were 93.44% and 90.24%,respectively.There was no significantly difference between the both groups (P>0.05).In the trial group and control group,the bacterial eradication rate was 89.42% and 90.80% based on ITT,90.11% and 92.77% based on CE,respectively.There was no significant difference in incidences of adverse events following the administration of the drug between the trial group and control group,including ocular tolerance,burning sensation,pricking and itching (P =0.34).Conclusions The effectiveness and adverse response were resemble between Pazufloxacin mesylate eye drops and LOFV following the topical application for bacterial conjunctivitis,which indicate that Pazufloxacin mesylate eye drops is effective and safe.
7.Effect of self-management on blood pressure control of non-hospital hypertension patients:a Meta-analysis

Objective To evaluate the effect of self-management on blood pressure control of non-hospital hypertension patients by conducting a Meta-analysis.Methods Data were collected from WanFang , CBM, CNKI, PubMed and Medline from January 2005 to December 2015.RevMan 5.3 was used for the Meta-analysis.Results A total of 13 studies were included in Meta-analysis.The results showed that self-management program could decrease non-hospital hypertension patients′systolic pressure and diastolic pressure ( P<0.01) and improve self-assessment of health score , but had no significant differences on energy and fatigue which were tested by Chinese version of chronic disease self-management questionnaire ( P >0.05 ). Conclusions Self-management can improve non-hospital patients′blood pressure , but it has little effect on patients′energy and fatigue .The results should be checked by more high quality randomized controlled trials using Chinese version of chronic disease self-management questionnaire .
8.Association between polymorphism of interleukin-28B rs12979860 T/C and susceptibility of hepatocellu-lar carcinoma:a Meta-analysis

Objective To explore the correlation between polymorphism of interleukin-28B(IL-28B) rs12979860 T/ C and susceptibility of hepatocellular carcinoma( HCC). Methods All eligible case-control studies published up to 2014-09-30 were identified by searching PubMed,EMBase,CNKI,CBM,VIP and WanFang databases. Two reviews independently identified the literature according to inclusion and exclusion criteria. Meta-analysis was performed using Rev Man 5. 2 and Stata 12. 0 software. Results A total of 6 stud-ies comprising 1 138 cases and 955 controls were finally included. Meta-analysis showed that IL-28B rs12979860 T/ C polymorphism was associated with the susceptibility of HCC. Compared with the genotype CC and CT + CC,genotype TT increased the risk of suffering from HCC(TT vs CC:OR = 2. 26,95% CI:1. 40-3. 64,Z = 3. 33,P = 0. 000 9;TT vs CT + CC:OR = 1. 90,95% CI:1. 23-2. 93,Z = 2. 89,P = 0. 004). In stratification analysis by ethnicity,we observed that the polymorphism of IL-28B rs12979860 T/ C was associat-ed with the susceptibility of HCC among Caucasian populations(TT vs CC:OR = 2. 06,95% CI:1. 22-3. 47, Z = 2. 70,P = 0. 007;TT vs CT + CC:OR = 1. 71,95% CI:1. 07-2. 72,Z = 2. 23,P = 0. 03). Conclusion The polymorphism of IL-28B rs12979860 T/ C is associated with the susceptibility of HCC,genotype TT may increase the susceptibility to HCC.
9.Effectiveness of nurse-led discharge planning for hospital inpatients with chronic disease: a Meta-analysis

Objective To evaluate the effectiveness of nurse-led discharge planning for hospital inpatients with chronic disease.Methods The Cochrane Library,PubMed,Medline,EMBASE,Chinese Biomedical Literature Database Database(CBM),China National Kmowledge Infrastructure (CNKI),VIP Chinese full text database (VIP) and Wanfang Data were searched for randomized controlled trials assessing nurse-directed discharge planning for hospital inpatients with chronic disease.Two reviewers independently extracted data and assessed risk of bias.Meta-analysis was conducted for the eligible studies by RevMan 5.3.4.Results Seventeen randomized controlled trials and 5 331 participants were included.Meta-analysis demonstrated that,compared to standard care,nurse-led discharge planning were effective in reducing hospital readmission [risk radio (RR)=0.73,95% confidence interval (CI) 0.64-0.84,P<0.01],duration of inpatient readmissions [mean difference (MD)=-2.48,95%CI-3.68--1.29,P<0.01],and all-cause mortality (RR=0.78,95%CI 0.62-0.97,P< 0.05).However,no reduction in the length of stay of the index admission (MD=-0.18,95%CI-0.64-0.27,P > 0.05) was demonstrated.Conclusions Nurse-led discharge planning has a positive impact on several aspects of care for hospital inpatients with chronic disease,including reducing readmission,readmission length of stay,and all-cause mortality.It is worth being popularized.
10.Research advances in Hedgehog signaling pathway in hepatocellular carcinoma

Hedgehog (Hh) signaling pathway is present in many animals and plays an important role in regulating embryonic development and differentiation. Aberrant activation of Hh signaling contributes to the pathogenesis of many malignancies. Recent studies have shown that dysregulated Hh signaling pathway participates in the tumorigenesis, tumor invasion, and metastasis of hepatocellular carcinoma (HCC). Investigation of the relationship between Hh signaling pathway and HCC will help elucidate the molecular mechanism of pathogenesis of HCC and provide a new insight into the development of novel anticancer therapy and therapeutic target.
