1.Observation on analgesic efficacy of ultrasound-guided high fascia iliac compartment block for tourniquet-related pain following total knee arthroplasty.

OBJECTIVE:
To evaluate the analgesic efficacy of ultrasound-guided high fascia iliaca compartment block (HFICB) in managing tourniquet-related pain following total knee arthroplasty (TKA).
METHODS:
A prospective randomized controlled trial was conducted involving 84 patients with severe knee osteoarthritis or rheumatoid arthritis who underwent unilateral TKA between March 2024 and December 2024. Patients were randomly assigned to two groups ( n=42) using a random number table. In the trial group, ultrasound-guided HFICB was performed preoperatively, with 0.2% ropivacaine injected into the fascia iliaca compartment. No intervention was administered in the control group. Baseline characteristics, including gender, age, surgical side, body mass index, and preoperative visual analogue scale (VAS) scores at rest and during movement, showed no significant difference between the two groups ( P>0.05). In both groups, a tourniquet was applied after osteotomy and before pulsed lavage, and removed after the closure of the first layer of the joint capsule. Postoperative assessments were conducted at 6, 12, 24, and 48 hours, including VAS scores at the tourniquet site (at rest and during movement), Bromage motor block scores, Ramsay sedation scores, and Bruggrmann comfort scale (BCS) scores to evaluate patient comfort. Additionally, the average tramadol consumption and incidence of nausea and vomiting within 48 hours postoperatively were recorded and compared.
RESULTS:
In the trial group and control group, VAS scores during movement at the tourniquet site significantly improved at all postoperative time points compared to preoperative levels ( P<0.05). VAS scores at rest increased transiently at 6 hours after operation in both groups, and then gradually decreased to the preoperative level. Except that there was no significant difference at 48 hours after operation in the trial group ( P>0.05), there were significant differences at other time points of two groups compared to preoperative score ( P<0.05). Except for VAS score at rest at 6 hours, VAS score during movement at 48 hours, and BCS comfort score at 48 hours ( P>0.05), the trial group showed significantly better outcomes than the control group in terms of VAS score at rest, VAS score during movement, Ramsay sedation scores, and BCS comfort scores at all other time points ( P<0.05). No significant difference was found in Bromage motor block scores between the groups ( P>0.05). Tramadol was used in 3 patients in the trial group and 7 patients in the control group within 48 hours after operation, the dosage was (133.30±14.19) mg and (172.40±22.29) mg, showing significant difference ( P<0.05). Nausea and vomiting occurred in 4 patients (9.5%) in the trial group and 3 patients (7.1%) in the control group, with no significant difference in incidence between groups ( P>0.05).
CONCLUSION
Ultrasound-guided HFICB provides effective analgesia for tourniquet-related pain following TKA, facilitates early postoperative functional recovery of the knee joint, and may serve as a valuable clinical option for postoperative pain management in TKA patients.

2.Research on medication rules of Chinese medicine in treating chronic kidney disease with spleen and kidney deficiency based on the mutual admiration of spleen and kidney

Objective Based on the theory of mutual admiration of spleen and kidney,this study intends to explore the medication patterns of traditional Chinese medicine for the treatment of spleen-kidney deficiency-type chronic kidney disease(CKD)by using data mining methods and to provide reference for the clinical treatment of spleen-kidney deficiency-type CKD.Methods The literatures included in China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,and VIP databases were used as data sources.The literature related to traditional Chinese medicine treatment of spleen-kidney deficiency-type CKD was analyzed by Excel 2021,IBM SPSS Modeler 18.0,IBM SPSS Statistics 27,and systematic clustering analysis and finally visualized by Cytoscape 3.7.2,RStudio.Results A total of 90 prescriptions were included,involving 146 flavors of drugs.The top 5 high-frequency drugs were Huangqi,Fuling,Baizhu,Dahuang,and Danshen.The medicinal properties are mainly mild and warm;The medicinal taste is characterized by sweetness,bitterness,and pungent;The main meridians of drugs are spleen,liver,and kidney meridians.Association rule analysis demonstrated that the commonly used couplet drugs were Huangqi-Fuling and Huangqi-Baizhu;Commonly used corner drugs included Huangqi-Baizhu-Fuling and Huangqi-Dahuang-Fuling.Cluster analysis found that the clustering effect of spleen-kidney deficiency-type CKD treatment drugs in five categories was better.Conclusion The medication rules of traditional Chinese medicine in the treatment of spleen-kidney deficiency-type CKD are preliminarily clarified,which provides a basis for clinical medication and new prescription development.
3.Exploration on medication rules of national patents of traditional Chinese medicine compound for prevention and treatment of urinary tract infection based on R language data mining

Objective To analyze the medication rules of traditional Chinese medicine compound patents for the prevention and treatment of urinary tract infection(UTI)through data mining technology.Methods The patents of traditional Chinese medicine compounds for the prevention and treatment of UTI in patent announcement module of China National Intellectual Property Administration website as data sources.The statistical analysis platforms of Excel 2021,IBM SPSS Modeler 18.0 and IBM SPSS Statistics 27.0 were used for frequency of use,medicinal properties,association rule analysis,and systematic clustering analysis and finally visualized by Cytoscape 3.7.2,RStudio.Results Through screening,a total of 179 compound patents met the inclusion criteria,involving 466 kinds of Chinese materia medica,and the top 5 high-frequency drugs were Pugongying,Huangbai,Gancao,Jinyinhua,Bianxu.The medicinal properties are mainly cold and mild;The medicinal taste is characterized by sweetness,bitterness,and bitterness;The main meridians of drugs are liver,lung and kidney meridians.Common couplet medicines included Qumai-Bianxu,Gancao-Bianxu and the three herb drug combinations included Cheqianzi-Qumai-Bianxu,Gancao-Qumai-Bianxu.A total of 5 high-frequency combinations of traditional Chinese medicine were obtained by cluster analysis.Conclusion This study preliminarily reveals the compatibility and medication rules of traditional Chinese medicine in the treatment of UTI,which provides data support for the optimization of clinical syndrome differentiation and treatment system and the development of new prescriptions.
4.Network Pharmacology Analysis of the Mechanism of Modified Xiaoxianxiong Decoction in"Treating Different Diseases with the Same Method"for Type 2 Diabetes and Obstructive Sleep Apnea

Objective To explore the mechanism of Modified Xiaoxianxiong Decoction(MXD)in"treating different diseases with the same method"for type 2 diabetes mellitus(T2DM)and obstructive sleep apnea(OSA)using network pharmacology and molecular docking.Methods Active components and related targets of the seven herbs in MXD(composed of Coptidis Rhizoma,Pinelliae Rhizoma,Trichosanthis Fructus,Bupleuri Radix,Salviae Miltiorrhizae Radix et Rhizoma,Paeoniae Radix Rubra,Astragali Radix)were retrieved and screened from the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP).Swiss Target Prediction was used for target prediction of active components,followed by gene standardization using the UniProt database.Disease-related targets of T2DM and OSA were obtained from DisGeNET,OMIM,and GeneCards databases.A Venn diagram was used to identify overlapping targets between disease-related targets and active component-related targets,yielding potential therapeutic targets of MXD for both T2DM and OSA.The STRING database and Cytoscape 3.10.2 were employed to construct a protein-protein interaction(PPI)network and a"drug-component-target"network,respectively.Core active components and core targets were identified through network topology analysis.GO functional and KEGG pathway enrichment analyses were performed on potential targets using Metascape.Molecular docking validation was conducted using AutoDock to assess binding affinity between core targets and compounds.Results A total of 157 active components of MXD acted on 1 043 protein targets.After comparison with 1 798 OSA-related targets and 3 466 T2DM-related targets,253 potential therapeutic targets were identified.GO enrichment analysis revealed that these targets were primarily involved in biological processes such as upregulation of phosphorus metabolism,regulation of cell migration,circulatory system processes,and hormone level regulation.KEGG pathway analysis identified key pathways including Rap1,AGE-RAGE,cAMP,and Alzheimer's disease.PPI and"drug-component-target"network topology analysis screened 39 core targets(e.g.,IL-6,TNF,Akt1,IL-1β,EGFR)and core active components(e.g.,salvianolic acid B,tanshinone IIA,baicalin,curcumin).Molecular docking confirmed stable binding between these components and their corresponding target proteins.Conclusion The common pharmacodynamic basis of MXD in"treating different diseases with the same method"for T2DM and OSA includes active components such as salvianolic acid B,baicalin,and curcumin.The shared mechanism may involve key targets(e.g.,IL-6,TNF,Akt1)and modulation of signaling pathways such as Rap1,AGE-RAGE,and cAMP.
5.Clinical efficacy of single-port laparoscopy combined with GnRH agonist in the treatment of deep infiltrating endometriosis and its influence on prognosis

Objective To explore the clinical efficacy of single-port laparoscopy combined with gonado-tropin-releasing hormone agonist(GnRH agonist)in the treatment of deep infiltrating endometriosis and its influence on serum circATRNL1 levels,and analyze its predictive value for patient prognosis.Methods 220 patients with deep infiltrating endometriosis from June 2021 to October 2023 were enrolled as study subjects and divided into an experimental group(n=110)and a control group(n=110)according to different surgical methods.The experimental group received single-port laparoscopy surgery combined with GnRH agonist treatment,while the control group only received single-port laparoscopy surgery.The changes in circATRNL1 expression level before and after treatment between the two groups were compared,the short-term efficacy and long-term prognosis of the two groups were evaluated.Spearman correlation analysis was conducted to explore the relationship between circATRNL1 expression levels after treatment and long-term prognosis.All patients were regrouped based on whether they relapsed during follow-up into a relapse group(n=42)and a non-relapse group(n=178).The circATRNL1 levels before and after treatment were compared between the two groups,and the predictive value of circATRNL1 expression levels after treatment for long-term relapse after treatment in patients with endometriosis was analyzed using a receiver operating characteristic curve(ROC curve).Results After treatment,the serum circATRNL1 levels in the experimental group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).The postoperative pain VAS score of the experimental group was significantly lower than that of the control group,and the quality of life was significantly better than that of the control group,the recurrence rate was significantly lower than that of the control group,and the differences were statistically significant(P<0.05).The pregnancy rate of the experimental group was higher than that of the control group,and the postoperative dysmenorrhea rate was lower than that of the control group,but the differences were not statistically significant(P>0.05).Spearman correlation analysis showed that circATRNL1 expression levels after treatment was positively correlated with relapse among long-term prognosis indicators(r=0.290,P<0.05),but had no significant correlation with pregnancy rate(r=0.060,P>0.05).Compared to patients in the non-relapse group,the relapse group had significantly higher serum circATRNL1 expression levels after treatment,the difference was statistically significant(P<0.05).ROC curve analysis showed that the area under the curve(AUC)for serum circATRNL1 expression levels in predicting long-term postoperative relapse in patients with endometriosis after treatment was 0.746,with a sensitivity of 64.29%and a specificity of 82.02%,indicating good predictive value.Conclusion The treatment regimen combining single-port laparoscopy with GnRH agonist can significantly reduce patients'pain levels,improve their quality of life,and decrease the recurrence rate.Furthermore,this combined therapy also notably decreases serum circATRNL1 levels.The serum circATRNL1 expression levels have good predictive value for the long-term recurrence of deep-infiltrating endometriosis patients post-surgery.
6.Advances in untargeted metabolomics research on osteoporosis

Osteoporosis is a systemic disease characterized by imbalanced bone metabolism and destruction of bone microstructure, with reduced bone density, decreased bone quality, and significantly increased risk of fracture as its hallmarks. At present, osteoporosis is primarily diagnosed through bone density measurement. However, this method has low sensitivity and is challenging for the early diagnosis of osteoporosis. We analyzed osteoporosis-related metabolomics studies based on blood, urine, and fecal samples, as well as the application of multi-omics approaches in elucidating its pathogenesis. Evidence suggests that metabolomics can detect metabolic alterations prior to measurable changes in bone mineral density, offering promising avenues for early osteoporosis detection. Blood-based metabolomics studies indicate that amino acid metabolism dysregulation is a key feature of osteoporosis. Specifically, glycine, glutamine, lysine, and hydroxyproline exhibit negative correlations with bone mineral density, whereas tryptophan, branched-chain amino acids, and arginine show positive associations. Lipid metabolism disturbances are characterized by increased levels of phosphatidylcholine, phosphatidylethanolamine, and triglycerides, alongside decreased levels of sphingomyelin and carnitine. Fecal metabolomics studies highlight the significance of the "gut-bone axis" in osteoporosis, where gut microbiota dysbiosis influences bone metabolism through modulation of arginine and proline metabolism and aminoacyl-tRNA biosynthesis pathways. Multi-omics approaches integrate metabolomics, genomics, proteomics, and other omics data to provide a more comprehensive understanding of osteoporosis' molecular mechanisms, enabling the identification of key biomarkers and therapeutic targets. Metabolomics holds considerable potential for early diagnosis, while multi-omics integration offers novel insights into the complex pathophysiological mechanisms underlying osteoporosis. As detection technologies and analytical methods continue to advance, omics-based strategies are expected to play a pivotal role in the development of precision medicine for osteoporosis.
7.Relationship between ultrasonic changes of quadriceps femoris and acquired weakness of ICU in children

Objective:To explore the relationship between the changes of quadriceps femoris and ICU acquired weakness(ICU-AW)in children.Method:A prospective collection of pediatric patients treated in the intensive care unit of Shenzhen children's hospital from October 2022 to March 2023.The thickness and cross-sectional area of rectus femoris and the thickness of vastus intermedius were measured by ultrasound on the 1st,4th and 7th day after ICU.The first day served as the baseline to calculate the atrophy rate for each muscle.On the 7th day,the Medical Re-search committee muscle strength score(MRC score)and critical physical function test score(PFIT-s score)were performed.According to the MRC score<48,patients were divided into ICU-AW group and non-ICU-AW group to analyze the relationship between ICU-AW and the changes of quadriceps femoris.Result:A total of 21 critically ill children were included,including 11 males and 10 females.The incidence of ICU-AW in children was 41.2%(4 cases were not evaluated because of delirium).Within 7 days after en-tering ICU,the atrophy rate of cross-sectional area of rectus femoris was the highest,reaching-8.00%±15.10%.Both groups showed the greatest rate of change in the vastus intermedius thickness over 7 days,with the ICU-AW group mainly showing an increase in thickness(19.51%±16.51%over 4 days,14.14%±28.13%over 7 days),while the non-ICU-AW group primarily showed atrophy(-16.07%±17.46%over 4 days,-15.55%±30.04%over 7 days).The ICU-AW group had an average increase in rectus femoris thickness of 4.51%±8.38%within 7 days of ICU admission,while the non-ICU-AW group had an average atrophy rate of-8.19%±11.79%.There was no correlation between muscular atrophy rate and PFIT-s score(all P>0.05),but there was a moderate negative correlation between rectus femoris thickness atrophy rate and femoral intermediate muscle thickness atrophy rate and MRC score within 4 days,respectively(ritual color 0.541 million 0.657 million 0.004),and moderate negative correlation between MRC score and thickness atrophy rate of rectus femoris and vastus medius within 7 days.There was no significant correlation between the atrophy rate of cross-section-al area of rectus femoris and MRC score.Conclusion:Compared with the non-ICU-AW group,the muscle thickness in the ICU-AW group is mainly thickened rather than atrophied,which can be identified by ultrasound on the 4th day after ICU.The specific diagnostic threshold is worthy of further study.

10.Transcranial and peripheral magnetic stimulation act synergistically to relieve post-stroke shoulder pain

Objective:To document any effect of combining transcranial magnetic stimulation (rTMS) with peripheral magnetic stimulation (rPMS) to relieve shoulder pain in hemiplegic stroke survivors.Methods:Sixty hemiplegic stroke survivors with shoulder pain were randomly divided into a Central Group ( n=20), a Peripheral Group ( n=20), and a Combined Group ( n=20). In addition to routine pharmacological intervention and rehabilitation training, the Central Group received 10Hz rTMS over the M1 area of the affected hemisphere, the Peripheral Group received 20Hz rPMS at Erb′s point of the affected limb, while the Combined Group received rTMS followed by rPMS. Before as well as after 2 and 4 weeks of treatment, all were assessed using a numerical pain intensity rating scale (NRS), for passive range of motion (PROM) of the shoulder joint, and using the Fugl-Meyer upper extremity assessment (FMA). Results:Before the treatment there were no significant differences among the 3 groups. After 2 weeks there was significant improvement in the average shoulder PROM and FMA scores, and a significant decrease in the average NRS ratings. After 4 weeks there was continued significant improvement. After 2 and 4 weeks, the Combined Group gave significantly lower NRS ratings than the others and had greater average shoulder PROM improvement. The average FMA score of the Combined Group (36.10±13.32) after two weeks was significantly better than those of the Central and Peripheral groups. However, no significant differences were found between the Central and Peripheral Groups in the other measurements.Conclusions:Both rTMS and rPMS can relieve shoulder pain and promote the recovery of motor function, with superior synergistic effects observed in their combined application.
