1.Clinical study on transoral combined with submental approach endoscopic total thyroidectomy in the treatment of thyroid carcinoma

Objective To evaluate the therapeutic efficacy of endoscopic total thyroidectomy performed via a combined transoral and submental approach in patients with thyroid cancer Methods This study enrolled 227 patients diagnosed with papillary thyroid carcinoma(PTC)who underwent total thyroidectomy combined with unilateral central lymph node dissection at the Department of Thyroid and Hernia Surgery,the First Affiliated Hospital of Gannan Medical University,between October 2019 and March 2024.Of these patients,68 were treated using the fully endoscopic combined transoral and submental approach(endoscopic group),whereas 159 underwent the conventional open anterior cervical approach(open group).A retrospective analysis was performed to evaluate and compare the therapeutic outcomes between the two groups.Results Surgical procedures were successfully completed in both groups without complications,and no additional incisions were required in the endoscopic group.Compared with the open surgery group,the endoscopic group had a longer operative duration,reduced intraoperative blood loss,and greater total postoperative drainage volume.Furthermore,on the first postoperative day,patients in the endoscopic surgery group exhibited lower levels of serum calcium and intact parathyroid hormone(iPTH).The incidence of temporary parathyroid dysfunction was significantly higher in the endoscopic group(51.47%)than in the open surgery group(30.82%),and this difference was statistically significant(P<0.05).No significant differences were found between the two groups regarding preoperative serum iPTH levels,preoperative serum calcium levels,number of dissected lymph nodes,incidence of temporary recurrent laryngeal nerve injury,postoperative hemorrhage,incision infection,or submental sensory loss(P>0.05).Conclusions Compared with traditional open surgery,total thyroidectomy via TOETSMVA under full endoscopy demonstrates comparable efficacy and favorable long-term safety.However,it is associated with a higher risk of transient hypoparathyroidism in the short term,highlighting the necessity for enhanced intraoperative protection of the parathyroid glands.

3.Oral gavage of Lactococcus lactis expressing urate oxidase regulates serum uric acid level in mice.

Urate oxidase (Uox) plays a pivotal role in uric acid (UA) degradation, and it has been applied in controlling serum UA level in clinical treatment of hyperuricemia (HUA). However, because Uox is a heterogenous protein to the human body, the immune rejections typically occur after intravenous administration, which greatly hampers the application of Uox-based agents. In this study, we used Lactococcus lactis NZ9000, a food-grade bacterium, as a host to express exogenous Uox genes, to generate the Uox-expressing engineered strains to treat HUA. Aspergillus flavus-derived Uox (aUox) and the "resurrected" human-derived Uox (hUox) were cloned into vector and expressed in NZ9000, to generate engineered strains, respectively. The engineered NZ9000 strains were confirmed to express Uox and showed UA-lowering activity in a time-dependent manner in vitro. Next, in an HUA mice model established by oral gavage of yeast paste, the UA levels were increased by 85.4% and 106.2% at day 7 and day 14. By contrast, in mice fed with NZ9000-aUox, the UA levels were increased by 39.5% and 48.3% while in mice fed with NZ9000-hUox were increased by 57.0% and 82.9%, suggesting a UA-lowering activity of both engineered strains. Furthermore, compared with allopurinol, the first-line agent for HUA treatment, mice fed with NZ9000-aUox exhibited comparable liver safety but better kidney safety than allopurinol, indicating that the use of engineered NZ9000 strains not only alleviated kidney injury caused by HUA, but could also avoided the risk of kidney injury elicited by using allopurinol. Collectively, our study offers an effective and safe therapeutic approach for HUA long-term treatment and controlling.

4.Clinical Effect Addition and Subtraction Therapy of Si Junzitang Combined with Simotang to Outlet Obstructive Constipation After Stapled Trans-anal Rectal Resection

Objective:To observe clinical effect of addition and subtraction therapy of Si Junzitang combined with Simotang to outlet obstructive constipation (OOC) after stapled trans-anal rectal resection (STARR). Method:One hundred and twenty-four patients were randomly divided into control group (62 cases) and observation group (62 cases) by random number table. Patients in control group got Qirong Ruichang oral liquid, 20 mL/time, 3 times/day. After operation, patients in observation group got addition and subtraction therapy of Si Junzitang combined with Simotang, 1 dose/day. And courses of treatment in two groups were 4 weeks, and 8 weeks' follow-up was recorded. Before the operation and at the second and fourth week after treatment, and the eighth week of follow-up, scores of main symptoms of constipation and Longo ODS were graded. Before the operation and at the fourth week after treatment, levels of superoxide dismutase (SOD), malondialdehyde (MDA), constipation patients quality of life self-assessment scale (PAC-QOL), anorectal pressure, anal resting pressure (ARP), maximum anal systolic pressure (MSP), rectal defecation pressure (RSP), FSV, CRS and MTV were recorded. And incidence, recurrence, normal defecation, satisfaction at the fourth week after the operation and safety were evaluated. 5.Study on the Protective Effect and Mechanism of JXHG Decoction on Alcohol Hepatic Injury in Mice

Objective: To investigate the protective effect and mechanism of JXHG Decoction on alcoholic liver injury.Methods: Each group of mice were fed for 30 days.Then 50% alcohol used to establish hepatic injury in mice by intragastric administration.16 hours later, the levels of TNF-α, AST, ALT in serum and MDA, GSH, TG, Caspase-3, Caspase-8 in liver were measured accordingly.Results: JXHG Decoction could reduce the levels of TNF-α, AST, ALT, MDA, TG, Caspase-3, Caspase-8, NF-κB and increase GSH, while alleviated tissue necrosis and aelipose degeration of hepar.Conclusion: JXHG Decoction has favorable anti-effects on alcoholic liver injury.Its possible mechanisms are reducing the inflammatory reaction by depressing the expression of TNF-alpha and NF-kappa B in liver cells, and alleviating the hepatic cell apoptosis induced by Caspase-8 and Caspase-3.
6.Efficacy and safety of Choudongning capsule (CDN)in children with Tourette's syndrome of spleen deficiency and phlegm accumulation.

To evaluate the efficacy and safety of Choudongning (CDN)capsule in children with Tourette's syndrome of spleen deficiency and phlegm accumulation through a randomized double-blind three-arm controlled phase Ⅲ study in 588 patients from 8 hospitals. The included patients were randomly divided into test group, positive control group and placebo group at the ratio of 3∶1∶1. Patients in the test group orally took CDN capsules and simulated Tiapridal tablets; the patients in positive control group took Tiapridal tablets and simulated CDN capsules; whereas the patients in placebo group orally took the simulated agents of the above two drugs. The treatment course was 6 weeks for three groups. The global grading rates, YGTSS scores and its factor scores, the degree of social function damage, as well as traditional Chinese medicine syndrome efficacy were evaluated as the outcome measures on efficacy. The AEs/ADRs, vital signs and laboratory testing were observed as outcome measures on safety. The total effective rate of YGTSS was 75.92% in the test group, 72.65% in the positive control group, and 37.29% in the placebo group. Non inferiority test stands between the test group and the positive control group, and they were superior to placebo group in efficacy with statistical difference. Significant difference had also been found among the 3 groups in YGTSS tics score, motor tics score, vocal tics, degree of social function damage and traditional Chinese medicine syndrome efficacy. During the study, there were 5 (1.42%)ADRs in the test group, 10 (8.55%)in the positive control group and 3 (2.54%)in the placebo group. The incidence of ADRs in the test group was lower than that in the positive control group, with statistical difference. It is clear to say that CDN capsule can effectively treat the Tourette's syndrome of spleen deficiency and phlegm accumulation. Its efficacy is not inferior to the commonly used Tiapridal tablets, with even less adverse reactions, so it has clinical application value.
