OBJECTIVE:
To evaluate the analgesic efficacy of ultrasound-guided high fascia iliaca compartment block (HFICB) in managing tourniquet-related pain following total knee arthroplasty (TKA).
METHODS:
A prospective randomized controlled trial was conducted involving 84 patients with severe knee osteoarthritis or rheumatoid arthritis who underwent unilateral TKA between March 2024 and December 2024. Patients were randomly assigned to two groups ( n=42) using a random number table. In the trial group, ultrasound-guided HFICB was performed preoperatively, with 0.2% ropivacaine injected into the fascia iliaca compartment. No intervention was administered in the control group. Baseline characteristics, including gender, age, surgical side, body mass index, and preoperative visual analogue scale (VAS) scores at rest and during movement, showed no significant difference between the two groups ( P>0.05). In both groups, a tourniquet was applied after osteotomy and before pulsed lavage, and removed after the closure of the first layer of the joint capsule. Postoperative assessments were conducted at 6, 12, 24, and 48 hours, including VAS scores at the tourniquet site (at rest and during movement), Bromage motor block scores, Ramsay sedation scores, and Bruggrmann comfort scale (BCS) scores to evaluate patient comfort. Additionally, the average tramadol consumption and incidence of nausea and vomiting within 48 hours postoperatively were recorded and compared.
RESULTS:
In the trial group and control group, VAS scores during movement at the tourniquet site significantly improved at all postoperative time points compared to preoperative levels ( P<0.05). VAS scores at rest increased transiently at 6 hours after operation in both groups, and then gradually decreased to the preoperative level. Except that there was no significant difference at 48 hours after operation in the trial group ( P>0.05), there were significant differences at other time points of two groups compared to preoperative score ( P<0.05). Except for VAS score at rest at 6 hours, VAS score during movement at 48 hours, and BCS comfort score at 48 hours ( P>0.05), the trial group showed significantly better outcomes than the control group in terms of VAS score at rest, VAS score during movement, Ramsay sedation scores, and BCS comfort scores at all other time points ( P<0.05). No significant difference was found in Bromage motor block scores between the groups ( P>0.05). Tramadol was used in 3 patients in the trial group and 7 patients in the control group within 48 hours after operation, the dosage was (133.30±14.19) mg and (172.40±22.29) mg, showing significant difference ( P<0.05). Nausea and vomiting occurred in 4 patients (9.5%) in the trial group and 3 patients (7.1%) in the control group, with no significant difference in incidence between groups ( P>0.05).
CONCLUSION
Ultrasound-guided HFICB provides effective analgesia for tourniquet-related pain following TKA, facilitates early postoperative functional recovery of the knee joint, and may serve as a valuable clinical option for postoperative pain management in TKA patients.

Neuroscience Bulletin 2025;41(7):1181-1197
High mobility group box 1 (HMGB1), when released extracellularly, plays a pivotal role in the development of spinal cord synapses and exacerbates autoimmune diseases within the central nervous system. In experimental autoimmune encephalomyelitis (EAE), a condition that models multiple sclerosis, the levels of extracellular HMGB1 and interleukin-33 (IL-33) have been found to be inversely correlated. However, the mechanism by which IL-33 deficiency enhances HMGB1 release during EAE remains elusive. Our study elucidates a potential signaling pathway whereby the absence of IL-33 leads to increased binding of P300/CBP-associated factor with HMGB1 in the nuclei of astrocytes, upregulating HMGB1 acetylation and promoting its release from astrocyte nuclei in the spinal cord of EAE mice. Conversely, the addition of IL-33 counteracts the TNF-α-induced increase in HMGB1 and acetylated HMGB1 levels in primary astrocytes. These findings underscore the potential of IL-33-associated signaling pathways as a therapeutic target for EAE treatment.

Objective: To evaluate the long-term effectiveness of an intelligent blood transfusion system in intraoperative blood management by comparing its performance with clinicians' decisions. Methods: A retrospective analysis of 26 760 surgical cases (2017-2024) was conducted, comparing pre- and post-implementation (2017-2019 vs 2020-2024) metrics, including transfusion prediction accuracy, rationality of blood use, and clinical outcomes. The system, powered by XGBoost, integrated patient demographics, laboratory results, and surgical data to predict red blood cell transfusion needs. Results: The intelligent blood transfusion systems achieved an accuracy of 80.62% in predicting transfusion necessity, significantly outperforming clinicians (24.83%, P<0.001). Its blood-use rationality rate was 83.92% vs 18.02% for clinicians (P<0.001). Post-implementation, major surgeries (grades Ⅲ-Ⅳ) increased while the requested blood units decreased. High physician compliance (>75%) correlated with 88.18% rationality. Conclusion: The intelligent blood transfusion system significantly improves the accuracy of transfusion decision-making, reduces excessive red blood cell use, optimizes perioperative transfusion management, and enhances the utilization of blood medical resources.

Laboratory Animal and Comparative Medicine 2024;44(6):695-699
ObjectiveTo provide a reference for the diagnosis of amyloidosis in experimental animals through the pathological diagnosis of systemic amyloidosis in a case of a New Zealand white rabbit. MethodsIn a 6-month repeated ocular toxicity study, an abnormal finding was noted during the routine gross anatomical examination of one New Zealand white rabbit. Its organs were prepared as paraffin sections and stained with hematoxylin-eosin (HE) staining and Congo red staining. The histopathological features were observed under optical and polarized light microscopy. ResultsGross anatomical examination of the animal revealed an enlarged spleen and changes in the color and texture of the lung. HE staining showed that the splenic tissue structure was destroyed, the white pulp of the spleen was surrounded by dense amyloid deposition in the form of nodular rings, along with pressure atrophy of the white pulp. Amyloid deposits were also observed in the submandibular lymph nodes, mesenteric lymph nodes, ileum, sacculus rotundus, vermiform appendix, jejunum, cecum, and rectum. Congo red staining showed that the amyloid deposition in the affected organs appeared salmon-pink, and exhibited characteristic apple green birefringence under polarized light microscopy. Conclusion The histo-pathological features of the New Zealand white rabbit are consistent with the diagnostic characteristics of systemic amyloidosis.
6.The sinicization and reliability and validity test of the Modified Version of Motivation Scale for Stroke Rehabilitation based on cognitive interview
Chinese Journal of Modern Nursing 2024;30(8):1067-1072
Objective:To sinicize the Modified Version of Motivation Scale for Stroke Rehabilitation (MORE) based on cognitive interviewing, and test its reliability and validity. Methods:The initial Chinese version of the MORE was developed based on the Brislin model and expert analysis method. From April to May 2021, purposive sampling was used to select 22 stroke patients from Linquan People's Hospital as the research subject for three rounds of semi-structured interviews. The Chinese version of the MORE was revised based on interview results to form the final version. From June 2021 to April 2023, convenience sampling was used to select 200 stroke patients from Linquan People's Hospital for investigation, to test the reliability and validity of the scale. Results:After three rounds of semi-structured interviews, a Chinese version of the MORE consisting of 17 items was formed. The content validity index at the item level was 0.913 to 1.000, and the content validity index at the scale level was 0.921. Three common factors were extracted using the maximum variance orthogonal rotation, with a cumulative variance contribution rate of 77.318%. The Cronbach's α coefficient of the Chinese version of the MORE was 0.937, the half coefficient was 0.898, and the test-retest reliability was 0.911.Conclusions:The understanding of the Chinese version of the MORE by stroke patients is comprehensively understood through cognitive interviewing. The reliability and validity test finds that the Chinese version of the MORE is an effective and reliable scale for evaluating the rehabilitation motivation of stroke patients, providing a reference basis for analyzing the influencing factors of rehabilitation motivation of stroke patients and formulating prevention and intervention measures.

Journal of Chinese Physician 2024;26(4):503-508
Objective:To explore the influencing factors of esophageal and gastric varices (EGV) in patients with liver cirrhosis, and to construct a column chart prediction model for EGV occurrence. Methods:A cross-sectional study was conducted to select patients ( n=127) who underwent gastroscopy at Minhang Hospital, Fudan University from January 2022 to April 2023 due to liver cirrhosis. Patients were divided into EGV group ( n=74) and non EGV group ( n=53) based on the presence of EGV. Clinical and auxiliary examination data of the two groups were compared. Univariate and multivariate logistic regression analysis were used to explore the independent influencing factors of EGV occurrence, and a column chart prediction model was further constructed to evaluate its predictive efficacy. Results:Multivariate logistic regression analysis showed that splenomegaly ( OR=28.38, 95% CI: 6.30-127.89, P<0.001), schistosomiasis infection ( OR=0.05, 95% CI: 0.01-0.28, P=0.001), and fatty liver ( OR=0.09, 95% CI: 0.02-0.33, P<0.001) were independent influencing factors for the occurrence of EGV in patients with liver cirrhosis. Based on this model, a column chart was constructed to predict the probability of EGV occurrence in patients with liver cirrhosis. The column chart model showed good consistency and discrimination (area under the curve was 0.927, 95% CI: 0.883-0.972), and was internally validated using the Bootstrap method, showing an average absolute error of 0.029. Conclusions:Spleen enlargement, schistosomiasis infection, and fatty liver are independent influencing factors for the occurrence of EGV in patients with liver cirrhosis. The Nomogram column chart prediction model for the occurrence of EGV in patients with liver cirrhosis has good predictive performance and can provide scientific basis for non-invasive risk screening and prevention strategies for EGV in liver cirrhosis.
