1.Umbrella review analysis of the safety and efficacy of vonoprazan in the treatment of peptic ulcers and post-ESD ulcers

OBJECTIVE To analyze the safety and efficacy of vonoprazan (VPZ) in the treatment of peptic ulcer (PU) and post-endoscopic submucosal dissection (ESD) ulcers, providing evidence-based pharmaceutical evidence for clinical practice and medical decision-making. METHODS Retrieved from CNKI, Wanfang, VIP, CBM, PubMed, Embase, Web of Science, and the Cochrane Library, meta-analyses/systematic reviews related to VPZ in the treatment of PU and post-ESD ulcers were collected. Two researchers independently performed literature screening, data extraction, quality assessment of included studies, and evaluation of literature overlap. By employing the umbrella review analysis, a fresh meta-analysis was conducted on all relevant raw research data when a high degree of overlap was identified among the included studies. RESULTS A total of 17 meta-analyses were included, with quality ranging from high to very low; all outcome measures involved showed a very high level of overlap in the included meta-analyses (corrected covered area: 22.22%-100%). In the treatment of post-ESD ulcers, compared to proton pump inhibitor (PPI), VPZ significantly improved the ulcer healing rate at 4 weeks post-ESD [RR=1.27, 95%CI (1.03, 1.56), Z=2.21, P= 0.027] and the ulcer contraction rate post-ESD [MD=0.08, 95%CI (0.00, 0.16), Z=2.09, P=0.037], while significantly reducing the ulcer recurrence rate in patients with a history of PU [RR=0.49, 95%CI (0.32, 0.73), Z=3.49, P=0.001]; the delayed bleeding rate in the VPZ group was significantly lower than that in the lansoprazole subgroup [RR=0.47, 95%CI (0.25, 0.90), Z=2.28, P=0.02]. In the treatment of PU, the incidence of adverse events with VPZ was significantly higher than that with PPI in the duodenal ulcer subgroup [RR=1.13, 95%CI (1.02, 1.26), Z=2.38, P=0.017]. CONCLUSIONS For post-ESD ulcers, VPZ demonstrates superior therapeutic efficacy compared to PPI and can reduce ulcer recurrence rates in patients with a history of PU. However, it does not offer advantages in terms of safety for duodenal ulcer treatment.
2.Efficacy and safety of omadacycline in the treatment of macrolide-unresponsive Mycoplasma pneumoniae pneu-monia in children

OBJECTIVE To investigate the efficacy and safety of omadacycline in the treatment of macrolide-unresponsive Mycoplasma pneumoniae pneumonia (MUMPP) in children. METHODS A retrospective study was conducted on children aged 1-18 years old with MUMPP who were hospitalized in the Department of Pediatrics, the First Affiliated Hospital of Xinjiang Medical University from January 2022 to June 2025. According to the selection of secondary antibiotics after 72 h of initial treatment with macrolides, they were divided into the omadacycline group and the doxycycline group. Based on conventional treatment, children in the omadacycline group were given intravenous infusion of 2.4 mg/kg (once daily) of omadacycline tosylate, while children in the doxycycline group were given oral doxycycline hydrochloride tablets at 2 mg/kg (twice daily). The efficacy and safety were compared between the two groups of pediatric patients. Univariate analysis and multivariate Logistic regression analysis were performed on clinical efficacy, and subgroup analysis along with multiple sensitivity analyses were conducted to verify the robustness of the conclusions. RESULTS A total of 284 children with MUMPP were included in this study, with 142 in the omadacycline group and 142 in the doxycycline group. In terms of efficacy, although the hospitalization time of children in the omadacycline group was longer than that in the doxycycline group ( P <0.05), the lung lesion absorption rate and clinical efficacy were significantly higher or better than those in the doxycycline group ( P <0.05). The results of multivariate Logistic regression analysis showed that medication (OR=5.300, 95%CI: 2.526-11.123), length of hospital stay (OR=1.348, 95%CI: 1.167-1.556), and medication duration (OR=1.422, 95%CI: 1.169-1.729) were influencing factors of clinical efficacy ( P <0.05). The subgroup analysis results showed that the clinical efficacy of omadacycline was significantly better than that of doxycycline in all subgroups ( P <0.05). The results of multiple sensitivity analysis showed that the regression coefficients B of the four models (gradually adjust variables) before and after inverse probability of treatment weighting were significantly greater than 1 ( P <0.05). In terms of safety, there was no statistically significant difference in the inci dence of adverse drug reactions between the two groups of patients ( χ 2 =0.447, P =0.504). CONCLUSIONS In the case of hospitalization and prolonged medication, the efficacy of omadacycline in treating childhood MUMPP is superior to that of doxycycline, and its safety is good.
3.Latent classes of acceptance and striving among nursing students and their association with subjective well being

Objective:
To identify the latent classes of acceptance and striving and their associated factors among nursing students, and to examine the mediating role of positive acceptance and striving in the association between self transcendence and subjective well being, so as to provide evidence for targeted interventions to improve subjective well being among nursing students.
Methods:
From July to October 2025, a convenience sample of 861 nursing students was recruited from five schools in Sichuan and Chongqing, China. Participants were surveyed using the Self transcendence Scale, the Positive Acceptance and Striving Scale, and the General Well being Scale. Latent profile analysis was conducted to identify profiles of positive acceptance and striving. Logistic regression analysis was used to examine associated factors, and relative mediation analysis was performed to assess differences in the mediating effect across latent profiles.
Results:
Three latent classes of acceptance and striving were identified: the low acceptance-low striving profile (46.34%, n =399), the moderate acceptance-moderate striving profile (40.88%, n =352), and the high acceptance-high striving profile (12.78%, n =110). Multivariable Logistic regression analysis showed that, with the low acceptance-low striving profile as the reference, nursing students with higher self transcendence scores ( OR =151.85) and those who studied nursing because of personal preference ( OR =14.72) or parental expectations ( OR =11.16) were more likely to be classified into the high acceptance-high striving profile (all P <0.05). Male students ( OR =0.19), those perceiving nurses social status as average ( OR =0.35), and those receiving moderate teacher support ( OR =0.23) were less likely to be classified into the high acceptance-high striving profile (all P <0.05). Positive acceptance and striving partially mediated the association between self transcendence and subjective well being, with a total effect of β =0.97 and a mediating effect of β =0.48; the mediating effect accounted for 49.34% of the total effect. Relative mediation analysis showed that the mediating effect was statistically significant only in the low acceptance-low striving profile ( β =0.21), whereas the direct effects were stronger in the moderate acceptance-moderate striving and high acceptance-high striving profiles ( β =0.39, 1.06)(all P <0.05).
Conclusions
Positive acceptance and striving among nursing students exhibit substantial heterogeneity. Its predictive effect on subjective well being and the underlying mediating mechanism vary across latent profiles. Profile specific interventions should therefore be implemented to improve subjective well being among nursing students.
4.Effect of Xpert MTB/RIF on detection of rifampicin resistance under different bacterial loads

OBJECTIVE To analyze the reliability of Xpert MTB/RIF(referred to as Xpert)in detecting rifampicin resistance under varying bacterial loads of Mycobacterium tuberculosis(MTB)so as to reference for clinical diag-nosis and treatment.METHODS A total of 3 050 mycobacterial culture-positive test results from Jan.2016 to Dec.2023 at the First Affiliated Hospital of Xiamen University were selected.After excluding 220 non-tubercu-lous mycobacteria cases,2 830 culture-positive specimens underwent Xpert testing,with 2 412 positive and 418 negative cases.Based on MTB bacterial load,specimens were divided into four treatment groups-high,medi-um,low and very low-with 552,916,656 and 288 cases,respectively.Solid phenotypic drug susceptibility testing(DST)was performed on 1 801 MTB strains that were both culture-positive and Xpert-positive.RESULTS The positive rates of rifampicin in the high,medium,low and very-low treatment groups were 10.33%(57/552),11.14%(102/916),8.54%(56/656)and 8.68%(25/288),respectively.Using the solid phenotypic drug sus-ceptibility testing results as the reference standard,the specificity and negative predictive value of Xpert for detec-ting rifampicin resistance in all four bacterial load groups exceeded 96%,with no statistically significant differ-ences observed.No significant difference in sensitivity was noted among the groups(P=0.053),though the high bacterial load group exhibited relatively lower sensitivity at 84.62%(44/52),while the very-low bacterial load group showed the lowest sensitivity at 76.00%(19/25),which was lower than the medium bacterial load group's 94.05%(79/84).The positive predictive values(PPV)across all groups were generally low but without statisti-cally significant differences(P=0.239),with the high and very-low bacterial load groups registering PPVs of 77.19%(44/57)and 76.00%(19/25),respectively.The concordance rates of Xpert and solid phenotypic drug susceptibility testing were consistently high among all four bacterial load groups,including 94.95%(395/416),97.51%(666/683),96.75%(477/493)and 94.26%(197/209),respectively,with no significant difference(P=0.052).The Kappa values from high to very-low bacterial load groups were 0.778,0.889,0.831 and 0.727,re-spectively.Among the strains with discordant results between Xpert and phenotypic drug susceptibility testing,the rifampicin result consistency rates between Xpert and gene chip were 81.82%(9/11),100.00%(9/9),100.00%(2/2)and 25.00%(1/4),respectively for the high to very-low bacterial load groups.CONCLUSIONS When the bacterial load of MTB is very low,the Xpert assay demonstrates a higher probability of both false-posi-tive and false-negative results for rifampicin resistance,followed by high bacterial loads.The most reliable detec-tion of rifampicin resistance occurs with medium bacterial loads.
5.Anxiety as mediator between impulsive traits and symptoms of eating disorders

Objective:To explore the relationship between impulsivity traits,anxiety,and symptoms of eating disorders,with a focus on the mediating effect of anxiety between impulsivity and eating disorder symptoms.Me-thods:A total of 244 patients with eating disorders meeting the DSM-5 diagnostic criteria for anorexia nervosa(AN)and bulimia nervosa(BN)were enrolled,and the Eating Disorder Inventory-1(EDI-1),Barratt Impulsive-ness Scale(BIS-11),and the State Anxiety Inventory(SAI)were assessed.Mediation role analysis was performed by SPSS macro PROCESS program.Results:There was a significant positive correlation between the total score of BIS-11,SAI and EDI-1 in AN and BN patients(AN,r=0.56,0.63,0.72;P<0.001.BN,r=0.51,0.31,0.56;P<0.001 or P<0.01).The total score of SAI played a mediating effect between the total score of BIS-11 and the total score of EDI-1,but the total score of SAI played a partial mediating effect(effect ratio was 46.9%)in patients with AN,and the total score of SAI played a full mediating effect in patients with BN.Conclusion:Impulsive trait and anxiety may be positive predictors of eating disorder symptoms.Anxiety mediates the relationship between impul-sivity trait and eating disorder symptoms,with a partial mediating effect in patients with AN and a full mediating effect in patients with BN.
6.Identification and characterization of linear Fc-binding epitope for IgG1 on bovine FcγR Ⅱ

The aim of this study is to identify the linear Fc-binding epitope for IgG1 on bovine IgG Fc receptor Ⅱ(boFcγRⅡ)to understand the molecular basis of IgG-Fcγ interaction.The boFcγRⅡ molecules were expressed on cell surface of the boFcγR Ⅱ-transfected COS-7 cells.The extracel-lular domain of boFcγRⅡ was expressed in NS0 cells,and the boFcγRⅡ recombinant protein was purified from ascites by Ni-chelation chromatography.Peptides derived from the membrane-distal extracellular domain(EC2)of boFcγR Ⅱ were synthesized and conjugated to a carrier protein of IgG-free bovine serum albumin(BSA).Binding of bovine IgG1 to the different peptides was tested by dot-blot assay,and the IgG-binding peptide was further modified by truncation and mutation to identify the Fc-binding epitope as well as its key amino acids for Fc-binding.The inhibition effect of the Fc-binding peptide was determined by competitive ELISA and Fc-rosetting inhibition assay,re-spectively.The results showed that boFcγR Ⅱ molecules were stably expressed on surface of the transfected COS-7 cells,which showed about 90%rosetting with IgG1-RBCs.The soluble boFcγRⅡ recombinant protein specifically bound to bovine IgG1.The minimal effective peptide of 122FYQDRKSKIF131 of boFcγRⅡ was able to bind bovine IgG1 specifically,suggesting it repre-sents a linear Fc-binding epitope located in the putative C-C'loop of the EC2 domain on the recep-tor.The Ala-substitution of Phe122,Tyr123,Arg126,Lys127,Ser128,Lys129 or Phe131 within the linear epitope led to a complete loss of its IgG1-binding capability,indicating those residues are critical for IgG1-binding on boFcγRⅡ.The Fc-binding peptide inhibited bovine IgG1 binding to the soluble recombinant protein of boFcγRⅡ with IC50 of 20.05 μmol/L,and inhibited the rosette formation of bovine IgG1-sensitized RBCs on the boFcγRⅡ transfected cells with IC50 of 80.15 μmol/L.The re-sults indicate that boFcγRⅡ possesses the linear epitope for Fc-binding,and the Fc-binding pep-tide showed well capability of regulating boFcγR Ⅱ-IgG1 interaction on cell surface,thereby provi-ding a research foundation for understanding the IgG-Fcγ interaction.
7.Effect of Xpert MTB/RIF on detection of rifampicin resistance under different bacterial loads

9.Role of stem cells in scalp aging and related therapeutic strategies

Changes in hair follicle stem cells (HFSCs) can affect scalp aging and hair growth. With increasing age, HFSCs exhibit a decrease in quiescence maintenance and self-renewal capacity, as well as differentiation potential, leading to shortened hair growth cycles and even hair loss. This review summarizes recent research advances in the multifactorial interactions underlying hair loss, including the regulatory mechanisms of HFSC quiescence, the impact of aging on HFSC function, and aging of the stem cell microenvironment. Additionally, this review discusses the relationship between stem cells and hair shafts, and the mechanisms of action of stem cells in scalp aging, including alterations in signaling pathways, chromatin remodeling, and epigenetic regulation, etc. Furthermore, stem cell-based therapeutic strategies are summarized, such as the use of stem cells or their secreting exosomes, modulation of the stem cell microenvironment, and pharmacological interventions.
