1.Analysis of red blood cell transfusion reactions in China from 2018 to 2023

Objective: To analyze the demographic characteristics of patients with red blood cell transfusion reactions, the usage of red blood cell preparations, and the differences in the composition ratio of adverse reactions based on multi-center data from the Haemovigilance Network, in order to reveal the clinical characteristics of red blood cell transfusion and its underlying issues. Methods: Clinical data of patients who experienced transfusion reactions after red blood cell transfusion in the Haemovigilance Network from 2018 to 2023 were collected. The demographic characteristics of patients who experienced transfusion reactions with different types of red blood cell preparations, the utilization of these preparations, and the differences of the composition ratios of transfusion reactions were analyzed. Count data were expressed as numbers (n) or percentages (%), and comparisons between groups were performed using the Chi-square test. Results: Red blood cell transfusion reactions were more common in females (53.56%), with the majority of patients aged 50-69 years (35.54%). The Han polulation accounted for the vast majority of patients (92.77%), and patients in the hematology and obstetrics/gynecology departments had a relatively high proportion of transfusion reactions (13.26% and 14.26%, respectively). Leukocyte-reduced red blood cells and suspended red blood cells were the most common types of transfusion reactions reported among red blood cell preparations. Allergic reactions and non-hemolytic febrile reactions were the most common transfusion reactions, and there were significant differences in the composition ratios of allergic reactions (χ
=869.89, P<0.05) and non-hemolytic febrile reactions (χ
=812.75, P<0.05) across various types of red blood cell preparations. Conclusion: There are differences in the demographic characteristics and composition ratio of transfusion reactions among different red blood cell preparations. The management of red blood cell transfusion reactions should be tailored to patient characteristics and conditions, and the selection and use of blood products should be optimized to reduce or avoid the occurrence of transfusion reactions, such as considering the use of washed red blood cells for patients with a history of transfusion allergies or those prone to allergies.
2.Mechanism of retinal injury caused by hyperglycemia, hypertension and hyperlipidemia

Retinopathy mainly arises from abnormalities in retinal blood vessels and nerves. Patients may experience symptoms such as decreased vision and visual field deletion, and are often treated with methods such as drugs,surgery, and laser. Previous studies have shown that hyperglycemia, hypertension, and hyperlipidemia can all cause retinal injury, and the three produce a synergistic pathological effect(multi-factor interaction leading to multiplied injury)to accelerate retinopathy. Exploring the additional aspects of mechanism of the three on the retina and exploring potential therapeutic targets have extremely critical clinical value for the early prevention and control of retinopathy and improvement of symptoms. The article summarizes the mechanisms by which the three lead to retinopathy, provides a theoretical basis for the development of multi-target combined therapy of vascular endothelial growth factor(VEGF)inhibitor, antioxidants and anti-inflammatory drugs, which not only helps early intervention and precise treatment, but also lays a foundation for the development of new drugs and individualized treatment strategies, and is of great significance for the prevention and treatment of metabolic eye diseases.
3.Development and application of regulatory T cells in corneal transplant rejection

Corneal transplantation is an effective treatment for corneal blindness, and it is the only hope for patients with corneal blindness. Cornea has no blood vessels and no lymphatic vessels, which is called immune privilege organ, so the success rate of corneal transplantation is significantly higher than that of other organ transplantation, but the rejection reaction after corneal transplantation is still the main reason for the failure of corneal transplantation. The directional movement of immune cells to lymphoid tissue and inflammatory sites is the mainly immune response after organ transplantation. And the regulatory T cells(Treg)play a key role in immune regulation, which can induce immune tolerance by regulating and inhibiting the activation of effector T cells and reduce the rejection reaction after corneal transplantation. In addition, this review also discussed the effectiveness of applying cordyceps sinensis extract FTY720 to enhance the function of Treg. Based on this, we briefly reviewed the sources, mechanism of action and treatment of Treg after corneal transplantation, so as to provide some reference for the subsequent clinical application transformation and basic research.
4. Mechanism of Bawei Chenxiang powder in treatment of ischemie heart disease through mitophagy based on network pharmacology

Aim To explore the potential mechanism of Bawei Chenxiang powder against ischemie heart disease (IHD) through mitophagy based on network pharmacology, molecular docking and verification in vitro. Methods The targets of serum constituents of Bawei Chenxiang powder were mined by Swiss target predic-tion, and then the targets related to IHD and mitophagy were selected from Genecards, NCBI and OMIM data-bases to obtain the intersection targets of the three as the potential targets of Bawei Chenxiang powder for the treatment of IHD through mitophagy. Then the "ingre-dients-disease-potential target " network and " protein-protein interaction" (PPI) network were constructed to perform network analysis in order to screen the key ac-tive ingredients and core targets, using Autodock vina software for molecular docking operation. The targets CO function enrichment analysis and KEGG pathway enrichment analysis were analyzed by DAVID databas-es. The effeets of Bawei Chenxiang powder containing serum on celi viability, levels expressions mitophagy and key signaling pathway related protein in H9C2 cells were investigated by hypoxia-induced injury of H9c2 myocardial cells model in vitro. Results The 9 key active compounds and 8 core targets of Bawei Chenxiang powder were screened; molecular docking showed a good binding ability of key active ingredients and core targets. KEGG pathway enrichment analysis showed that the effect of Bawei Chenxiang powder on IHD through mitophagy was related to EGFR, PI3K-Akt, MAPK, FoxO signaling pathway, etc. Celi ex-periments showed that Bawei Chenxiang powder containing serum treatment could significantly improve the survival rate by hypoxia-induced injury in H9c2, the expression of LC3II and p62 were significantly down-regulated, and the expressions of p-PI3K/PI3K and p-AKT/AKT were significantly up-regulated. Conclu-sions Bawei Chenxiang powder plays an anti-IHD role by regulating mitophagy, which may be involved in AKT1, STAT3, MAPK3 and EGFR and other targets, through quercetin, Kaempferol, Naringenin and De-hydrodiisoeugenol as well as other components. Its mechanism may be related to improving PI3K-AKT pathway.
5.Study on the protective mechanism of Bawei chenxiang powder containing serum on H9c2 cells injured by oxygen-glucose deprivation

OBJECTIVE To study the protective mechanism of Bawei chenxiang powder containing serum on H9c2 cells injured by oxygen-glucose deprivation (OGD). METHODS H9c2 cells were divided into blank group, model group and Bawei chenxiang powder low-dose, medium-dose and high-dose groups (the dose of drug containing serum 2.5, 8, 12 g/kg). H9c2 cells were cultured in vitro to establish OGD model. After intervention with drug-containing serum, survival rate of cell was detected. The cell morphology was observed; the levels of lactate dehydrogenase (LDH), creatine kinase (CK), superoxide dismutase (SOD), catalase (CAT), respiratory chain complexⅠ (ComplexⅠ), glutathione peroxidase (GSH-Px) and malondialdehyde (MDA) were detected. The contents of reactive oxygen species (ROS), mitochondrial membrane potential and apoptosis were also detected. The expressions of oxidative stress-related proteins [Kelch ECH association protein 1 (Keap1), nuclear factor erythroid 2- related factor 2 (Nrf2), heme oxygenase 1 (HO-1), NADH oxidoreductase coenzyme 10 (Ndufa10), thioredoxin (Trx)] and apoptosis-related proteins [B-cell lymphoma 2 (Bcl-2), Bcl-2 associated X protein (Bax), Caspase-3 and cytochrome C (Cytc)] were detected. RESULTS Compared with blank group, the cell morphology of model group was damaged; the levels of LDH, CK and MDA were significantly increased (P<0.01), while the levels of CAT, ComplexⅠ, SOD and GSH-Px and mitochondrial membrane potential were significantly decreased (P<0.01). The content of intracellular ROS and apoptotic rate were significantly increased (P<0.01); the expressions of oxidative stress-related proteins (Keap1, Nrf2, HO-1, Ndufa10 and Trx) and pro- apoptosis proteins (Bax, Caspase-3 and Cytc) were significantly increased (P<0.05), while the expression of anti-apoptotic protein Bcl-2 was significantly decreased (P<0.05). After administration of Bawei chenxiang powder containing serum, the cell morphology improved, and most of the above indexes were significantly reversed (P<0.05 or P<0.01).CONCLUSIONS Bawei chenxiang powder containing serum E-mail:345783110@qq.com has a good protective effect on H9c2 cells damaged by OGD,the mechanism of which is related to the reduction of oxidative damage and inhibition of cell apoptosis.
6.Analysis of risk factors related to pterygium and establishment of prediction model in plateau area

AIM: To analyze the risk factors related to pterygium in plateau area and establish a prediction model.METHODS: Using the method of cluster random sampling, the long-term residents living in the plateau with an average altitude of 3 000m were selected to conduct a field survey of pterygium from June 2020 to June 2021. Single factor and multi-factor analysis were used to analyze the risk factors related to pterygium, and the R software was used to establish the prediction model.RESULTS: The actual number of people investigated in this study was 1 514, and the number of patients with pterygium was 210, the overall prevalence rate was 13.87%. The age >43 years old, plateau area residence history, sunshine time, gender, smoking history, drinking history, hypertension, diabetes and hyperlipidemia are risk factors for pterygium. Among them, the long-term sunshine was the most dangerous factor for pterygium(OR: 6.215, 95%CI: 4.008-9.636, P<0.001), followed by >43 years old(OR: 5.348, 95%CI: 2.06-13.88, P=0.001). The decision curve analysis(DCA)showed that when the Nomo score system was applied, the predicted probability of pterygium was completely consistent with the actual probability of pterygium.CONCLUSION: The risk factors of pterygium as follows, the age >43 years old, plateau area residence history, sunshine time, gender, smoking history, drinking history, hypertension, diabetes and hyperlipidemia. The Nomo scoring system prediction model can accurately predict pterygium and provide a theoretical basis for the intervention of pterygium in plateau areas.
7.Effect of auricular point sticking therapy on perioperative pain in patients with partial lung resection.

To observe the analgesic effect of auricular point sticking therapy during the perioperative stage in the patients with partial lung resection.
METHODS:
A total of 92 patients with partial lung resection were randomized into an auricular point group (31 cases, 1 case dropped off), the sham-auricular point group (30 cases) and a medication group (31 cases, 1 case dropped off). Auricular point sticking therapy relieves perioperative pain, shortens analgesic time, releases anxious and depressive emotions and reduces postoperative adverse reaction in the patients with partial lung resection. The analgesic mechanism is probably related to the increase of plasma concentration of β-endorphin.

8.New progress of gene detection in diabetic retinopathy

At present, gene detection technology has become increasingly mature and integrated with multi-disciplinary, which provides help for clinicians to diagnose, treat andprognosis of the disease. In recent years, gene detection technology in diabetic retinopathy(DR)has made some progress, mainly applied to the risk factors of diabetic retinopathy and follow-up personalized treatment plan. Therefore, we summarize and analyze the gene loci related to diabetic retinopathy that can be detected by gene detection technology.
9.Analysis of ARMD related risk factors and establishment of Nomoto prediction model in high altitude area

AIM: To determine the risk factors of age-related macular degeneration(ARMD)in high altitude areas and establish Nomoto prediction model.
METHODS:Using the method of cluster random sampling, the subjects were selected from some communities in Xining City, with an average altitude of 2 100m. The subjects were ≥40 years old, and the investigation method was field investigation. Single factor and multi factor analysis were used to determine the risk factors of ARMD, and R software was used to draw Nomoto.
RESULTS: The actual number of subjects in this survey is 2 595. Age, cataract, living time at high altitude, smoking, drinking, high blood pressure and mobile phone use are risk factors of ARMD. Old age was the most risk factor for ARMD(OR: 53.078, 95% CI: 28.405-77.183, P<0.001), followed by long-term use of mobile phones(OR: 9.142, 95% CI: 1.906-43.846, P<0.001). The DCA decision curve showed that when the Nomo score existed, the probability of predicting ARMD was almost the same as that of actual ARMD.
CONCLUSION: The risk factors of ARMD are old age, high altitude living time, cataract, smoking, drinking, high blood pressure and mobile phone using time, especially the old people who live in high altitude for a long time. Nomo scoring system can accurately predict ARMD and provide theoretical basis for clinicians to intervene ARMD in high altitude areas.
10.Lateral cervical lymph node mapping in papillary thyroid carcinoma: a prospective cohort study

Objective: To explore the value of nanoparticles (CN) in lateral cervical lymph node mapping in papillary thyroid carcinoma using carbon. Methods: Thyroid cancer patients with suspicious lymph node metastasis but without typical signs of metastatic disease from March 2016 to November 2017 in Fudan University Shanghai Cancer Center were prospectively included in the cohort. Neck dissection was performed in all patients (compartmentsⅡ-Ⅴ). Suspicious lateral lymph node metastasis was identified using pre-operative ultrasound or computed tomography. CN were used for lymph node mapping during surgery. Results: A total of 70 surgeries were performed in 67 patients, among which 57 were found to have lateral lymph node metastasis (81.4%). The median number of CN-dyed lateral lymph nodes was 6. Compartment IV had the highest number of CN-dyed positive lymph nodes as well as the highest rate of metastasis, followed by compartmentⅢ. In compartmentsⅢandⅣ, the incidence of lymph node metastasis was significantly higher in the CN-dyed group than in the CN-undyed group (P<0.001). When the final pathology of neck dissection was set as the gold standard, lateral CN-dyed lymph node biopsy was found to have a sensitivity of 86.0%; its negative predictive value was 61.9% and its overall accuracy was 88.6%. Conclusions:Injection of CN during surgery was a potential method of mapping lateral lymph nodes in papillary thyroid carcinoma. CompartmentⅢ-ⅣCN-dyed lymph node biopsy had a satisfactory sensitivity and thus, served as a reasonable range for lymph node biopsy.
