1.Association between matrix GLA protein and influencing factors of idiopathic calcium oxalate stones and construction of a prediction model

Objective To investigate the role of matrix GLA protein(MGP)in the pathogenesis of idiopathic calcium oxalate kidney stones(ICOS),and to find potential biomarkers for early diagnosis and disease evaluation.Methods A total of 120 patients admitted to our hospital from September 2021 to September 2023 were prospectively included,of which 60 patients with ICOS were in the calculus group and 60 patients without calculus were in the control group.Serum biochemical indexes and immunohistochemical scores of the two groups were detected,urinary MGP levels were determined by ELISA,and MGP mRNA and protein expression in renal papilla tissues were detected by qPCR and Western blot.The independent risk factors of ICOS were screened by Logistic regression analysis,and the prediction model was drawn by nomogram.Results Compared with control group,urinary MGP content in calculus group was decreased[(1 805.91±244.44)pg/ml vs.(2 014.79±252.14)pg/mnl,P<0.05).Expression of MGP mRNA and MGP protein in renal papillae decreased(0.89±0.15 vs.1.00±0.00,P=0.001)and decreased(0.87±0.18 vs.1.00±0.00,P<0.05).MGP immunohistochemical scores of renal tissue were decreased[4(2-6)scores vs.6(4-8)scores,P<0.001].Multivariate analysis showed that urinary calcium(OR=1.370),urinary MGP(OR=1.127),renal papilla MGP relative expression level(OR=27.532)and renal tissue MGP immunohistochemical score(OR=1.359)were independent risk factors for ICOS.Area under ROC curve of the nomogram prediction model built based on the above factors is 0.839,indicating that the model has good differentiation ability in risk prediction.Conclusion MGP is closely related to the pathogenesis of ICOS.Urinary and renal tissue MGP levels may be potential biomarkers for early diagnosis and disease assessment of ICOS.
2.Construction process of theoretical and technological systems of laparoscopic anatomic liver resection for hepatocellular carcinoma

The development of surgery brings about the transformation of surgeons′ con-cepts, and in turn, each renewal of surgical concepts propels progress of surgical techniques. These two aspects complement each other. The treatment of hepatocellular carcinoma is a comprehensive therapy centered on surgery. With the deepening understanding of liver anatomy, the surgical methods have evolved from initial local resection to anatomical liver resection, and then to resection of the tumor-bearing portal vein territory. In recent years, with the emergence of hepatic membrane anatomy, portal plate theory, and three-dimensional visualization, the theoretical and technical systems of laparoscopic anatomical liver resection has become more and more mature. Based on own experience and literature reports, the authors systematically elaborate on the construction of theoretical and technological systems of laparoscopic anatomic liver resection for hepatocellular carcinoma, for reference by colleagues.
3.Prevalence survey of bloodborne occupational exposures among healthcare workers in a three-A eye,ear,nose and throat hospital of Shanghai

OBJECTIVE To investigate the current status of bloodborne occupational exposures among healthcare workers in a three-A specialized hospital of Shanghai so as to provide bases for formulating prevention measures for occupational exposures.METHODS A questionnaires titled'Survey of bloodborne occupational exposures among healthcare workers'were clinically distributed to Eye &ENT Hospital of Fudan University through'Wenjuanxing platform'from Feb.2024 to Apr.2024.The content of the questionnaire included four parts:the baseline data questionnaire,incidence of occupational exposures,awareness of knowledge about occupational exposures,and preventive behaviors regarding occupational exposures.RESULTS A total of 995 valid questionnaires were collect-ed in the survey,the result of the survey showed that the incidence of bloodborne occupational exposures was 10.65%.The clinicians and doctors engaging in advanced studies were at high risk of the occupational exposures especially when they were busy with work.The result of the survey indicated that the rate of missing report was 71.33%,the implementation rate of monitoring was only 48.11%after the exposures,of which the major cau-ses included the health care workers'insufficient attention to the exposure events,flaws in prevention and control of occupational exposures and some personnels'poor awareness of reporting process and requirements for monito-ring.Although the awareness rate of emergency processing exceeded 90%,the awareness rate of the knowledge a-bout prophylactic use of antibiotics and prevention of needlestick injuries,and the preparations before diagnosis and treatment,standardized operations and use of protective articles still needed to be improved.CONCLUSIONS The survey reveals that the health care workers pay less attention to the risk of infections,and the prevention of occupational exposure needs to be improved.It is of great importance to raise the awareness of prevention and en-hance the systematic training for occupational exposure.
4.Advances in animal models for studying human papillomavirus

Human papillomavirus (HPV) can cause various tumors such as cervical cancer in women. Further study of HPV infection and its pathogenic mechanism, as well as the development of vaccines and anti-tumor drugs, have been hindered due to the strong host specificity and tissue affinity of HPV, along with the challenges in in vitro culture and the lack of animal infection models. To advance the study of HPV infection and its pathogenic mechanism and to development better prevention and treatment measures, it is important to establish an animal model that closely resembles the infection and pathogenesis pathways in humans. This article reviews previous research on HPV-related animal models using mice, rabbits, macaques, and Drosophila, including infectious models, tumor models, and transgenic models, aiming to provide reference for the selection of appropriate animal models for HPV-related research.
5.Investigation of use of disposable and reuseable supplies for prevention and control of infections in Shanghai

OBJECTIVE To investigate the use of common disposable and reusable supplies for prevention and con-trol of infections in Shanghai.METHODS A survey was conducted for the use and reuse of disposable high-value consumables,disposable bronchoscopes and environmental cleaning and disinfection tools in Shanghai by using structured questionnaire.RESULTS Totally 81 medical institutions were involved in the survey,44(54.32%)of which were tertiary hospitals,and the median number of beds was 500.The orthopedic implants(70.37%),ul-trasound knife(69.14%)and endoscopic puncture instrument(66.67%)were the high-value consumables rank-ing the top 3 utilization rates.There was reuse of 18 types of high-value consumables in total among the 12 medi-cal institutions.The major causes of reuse of high-value consumables included fee less tan cost of consumables(58.33%)or being unable to included in charge items(33.33%).27.78%of the recycle high-value consumables were not treated with a dedicated disinfection and sterilization system and procedures for the disposable high-value consumables,and 33.33%did not have the report systems for related adverse reactions.49.09%of the medi-cal institutions reported to use the disposable bronchoscopes,4 of which reused them.The majority of the medi-cal institutions could carry out centralized cleaning and disinfection for the recycled floor cloths(60.81%)and cloth towels(56.76%),and 32.43%of the medical institutions reused the cloth towels by manual cleaning.The utilization rate of antiseptic wipes was 75.41%in adult intensive care unit,62.50%in neonatal intensive care unit.CONCLUSION The study reveals that there are some problems in the use of disposal and reusable supplies for prevention and control of infections,which may provide baseline data for management of the related supplies and the surveillance of disinfection of the recycled supplies so as to enhance the quality of management of hospital-asso-ciated infections.
6.Effects of Yitangkang on Brown Fat in db/db Mice by Regulating PGC1α-NRF1/2-TFAM Pathway

Objective To observe the effects of Yitangkang on brown fat thermogenesis and mitochondrial biogenesis of PGC1α-NRF1/2-TFAM pathway in db/db mice;To explore its mechanism of regulating glucose and lipid metabolism.Methods Totally 27 six-week-old db/db mice were randomly divided into model group,Yitangkang group(30 g/kg)and liraglutide group(200 μg/kg),another 9 db/m mice of the same age were set as normal group.All groups received intervention with drugs or saline for 6 weeks.The body mass and FBG were measured weekly.After intervention,oral glucose tolerance test(OGTT)was carried out,the contents of serum TC,TG,LDL-C and HDL-C were detected by biochemical analyzer,HE staining was used to observe the morphology of brown adipose tissue(BAT)in scapular region,RT-qPCR and Western blot were used to detect the expressions of UCP1,PRDM16,PGC1α related to BAT thermogenesis and NRF1,Nrf2,TFAM related to mitochondrial biogenesis.Results Compared with the normal group,the body mass,FBG,area under the curve of OGTT and serum TG,TC,LDL-C content of model group significantly increased(P<0.01),the content of HDL-C significantly decreased(P<0.01);the diameter of BAT cells in scapular region was larger,white vacuoles appeared,lipid droplets increased,and the mRNA and protein expressions of UCP1,PRDM16,PGC-1α,NRF1,NRF2 and TFAM in BAT decreased significantly(P<0.01).Compared with the model group,the body mass,FBG,area under the curve of OGTT and serum TG,TC,LDL-C contents of Yitangkang group and liraglutide group significantly decreased(P<0.01),the content of HDL-C increased(P<0.01);BAT cells were smaller in diameter,more closely arranged,more regular in shape,and more abundant in capillary,the mRNA and protein expressions of UCP1,PRDM16,PGC-1α,NRF1,NRF2 and TFAM in BAT increased significantly(P<0.01).Conclusion Yitangkang can regulate mitochondrial biogenesis through PGC1α-NRF1/2-TFAM pathway to activate brown fat in db/db mice and improve glucose and lipid metabolism in db/db mice.

Operation management is an important tool to promote the high-quality development of public hospitals in China.Since the founding of New China,based on China's economic and social development and medical and health system reform,the evolution of operation management of public hospitals in China can be divided into four stages,the system building stage(1949-1978),the liberalization and revitalization stage(1979-1996),the operation mechanism reform stage(1997-2020),and the new stage of high-quality development(2021 present).The development trend of public hospital operation management in future should deepen the public welfare-oriented public hospital operation management,explore the value-oriented medical operation management model based on high-quality development and the refined operation and management model of public hospitals,so as to promote the high-quality development of public hospitals in China.
