1.Regulation of Renal Interstitial Fibrosis-related Pathways by Traditional Chinese Medicine: A Review

Renal interstitial fibrosis (RIF) is a common pathological change process from the development of various chronic nephropathies to the end stage, and it is an important histological manifestation of renal function decline. At present, no effective anti-fibrosis drugs have been found in clinical practice. In recent years, with the continuous development of traditional Chinese medicine (TCM) pharmacology, molecular biology, system biology, and network pharmacology, the research on regulating RIF with TCM monomer, single TCM, TCM compound, Chinese patent medicine, and TCM injection is deepening. Among them, Jianpi Yishen recipe, Shendi Bushen capsules, Jianzhong Bushen Xiaozheng decoction, Liuwei Dihuangtang, and Lycium barbarum polysaccharides can regulate transforming growth factor-β/small mother against decapentaplegic (TGF-β1/Smads), Wnt/β-catenin, and neurogenic locus notch homolog protein (Notch) signaling pathways. Wulingsan, Zhenwutang, pachymic acid ZA, pachymic acid ZC, and pachymic acid ZD, which mainly induce diuresis, can regulate the Wnt/β-catenin signaling pathway. Hirudin, curcumin, and Fuzheng Huayu recipe, which mainly promote blood circulation, can inhibit inflammation-related pathways such as p-nuclear transcription factor-κB (NF-κB), Toll-like receptor 4/p-nuclear transcription factor-κB (TLR4/NF-κB), and Janus kinase 2/signal transducer and activator of transcription 3 (JAK/STAT), so as to achieve anti-inflammatory and anti-oxidation effects and alleviate the progression of RIF. Shenshuai Xiezhuo decoction, Shenkang injection, and Shenshuai recipe, which are mainly used for invigorating Qi, removing blood stasis, and removing turbidity, can inhibit transdifferentiation of pericytes-myofibroblasts through vascular endothelial growth factor receptor (VEGFR) signaling pathway. At present, there are many studies on the regulation of the RIF signaling pathway by TCM, but there is a lack of a systematic summary. In this study, by combing the signaling pathway of TCM in the treatment of RIF, the effective target of TCM treatment is screened, and its possible mechanism is found, which provides new ideas for clinical treatment and new drug research and development.

A major obstacle in type 2 diabetes mellitus (T2DM) is sleep fragmentation (SF), which negatively affects testicular function. However, the underlying mechanisms remain to be elucidated. In this study, we demonstrate that SF induces testicular damage through a mechanism involving lipid metabolism, specifically mediated by melatonin (MEL) receptor 1a (MT1). T2DM mice with SF intervention displayed several deleterious phenotypes such as apoptosis, deregulated lipid metabolism, and impaired testicular function. Unexpectedly, sleep recovery (SR) for 2 consecutive weeks could not completely abrogate SF's detrimental effects on lipid deposition and testicular function. Interestingly, MEL and MT1 agonist 2-iodomelatonin (2IM) effectively improved lipid homeostasis, highlighting MEL/2IM as a promising therapeutic drug for SF-trigged testicular damage. Mechanistically, MEL and 2IM activated FGFR1 and sequentially restrained the crosstalk and physical interaction between TAB1 and TAK1, which ultimately suppressed the phosphorylation of TAK1 to block lipid deposition and cell apoptosis caused by SF. The ameliorating effect of MEL/2IM was overtly nullified in Fgfr1 knockout (Fgfr1-KO +/- ) diabetic mice. Meanwhile, testicular-specific overexpression of Tak1 abolished the protective effect of FGF1mut on diabetic mouse testis. Our findings offer valuable insights into the molecular mechanisms underlying the testicular pathogenesis associated with SF and propose a novel therapeutic approach for addressing male infertility in T2DM.
3.CT measurement of blood perfusion in hepatocellular carcinoma: from basic principle, measurement methods to clinical application

Hepatocellular carcinoma (HCC) is one of the common and fatal malignant tumors worldwide, and the burden of HCC is particularly severe in China. Physiologically, the blood supply to healthy liver is mainly from the portal vein, supplemented by the hepatic artery. While in the development of HCC, the main source of blood supply to HCC is changed from the portal vein to the hepatic artery. The characteristics of HCC vascularization are important for imaging, surgery, interventional therapy, targeted therapy, etc. Even in the future, with the development of radiation therapy technology, such as proton and heavy ion therapy and artificial intelligence technology, the dynamic changes in HCC blood perfusion can be used as a new biomarker of tumor activity to provide accurate information on the intensity modulation of radiotherapy, so that accurate measurements of HCC blood perfusion is of great significance in guiding the diagnosis and treatment of HCC. The technologies for measurement of HCC blood perfusion have developed from invasive techniques, such as inert gas scavenging, electromagnetic flowmeter, and radionuclide-labeled erythrocyte elution in the middle of the last century to the present non-invasive techniques of CT. With the development of CT imaging technology in the last 30 years, the CT-based imaging technology can assess the status of organ and tissue perfusion relatively easily and accurately. In this paper, the various CT measurement techniques of blood perfusion in HCC were categorized into three types: semi-quantitative technique, relative quantitative technique, and absolute quantitative technique. Their basic principle, scanning methods, and clinical applications were discussed to provide a reference for the diagnosis and treatment of HCC.
4.A case of recurrent convulsions caused by acute sulfuryl fluoride gas poisoning

Sulfuryl fluoride is a kind of pesticide with strong permeability, convenient use at low temperature, non-corrosive and other characteristics, which can kill food pests and has strong lethality to termites. In acute sulfuryl fluoride poisoning, patients can see recurrent convulsions, epileptic electroencephalogram abnormalities such as matrix spikes or high amplitude spikes. In this paper, a patient with sulfuryl fluoride poisoning with convulsion-based mental system symptoms was reported, and after clinical treatment with dexamethasone and phenobarbital sodium, the patient was cured and discharged.

6.Prognostic value of thrombomodulin in patients with septic shock

Objective To investigate the prognostic value of plasma thrombomodulin(TM)in patients with septic shock.Methods A retrospective analysis was conducted on the clinical data of 180 patients with septic shock admitted to the intensive care unit of the 908th Hospital from May 2018 to November 2022.The patients were divided into survival group(106 cases)and death group(74 ca-ses)based on the 30-day follow-up outcomes.Propensity score matching(PSM)was used to match 57 surviving patients with 57 de-ceased patients in a 1∶1 ratio,based on confounding factors such as age,gender,underlying diseases,primary infection site,laborato-ry results and disease severity scores.TM and other coagulation molecular markers were compared between the two groups,and logistic regression,receiver operating characteristic(ROC)curve,survival and correlation analyses were performed.Results After PSM,the TM levels in the death group(18.3[13.2,22.3]TU/mL)were significantly higher than those in the survival group(13.7[9.0,18.3]TU/mL)(P<0.05).Multivariate logistic regression analysis showed that TM was an independent risk factor for 30-day mortality in the patients with septic shock(OR=1.137,95%CI:1.023-1.262,P<0.005).ROC curve analysis revealed that the areas under the curve(AUCs)for predicting 30-day mortality were 0.665,0.627 and 0.600 for TM,Acute Physiology and Chronic Health EvaluationⅡ(APACHE Ⅱ)and Sequential Organ Failure Assessment(SOFA)scores,respectively.Kaplan-Meier survival analysis stratified by the optimal TM cut-off value(17.9 TU/mL)showed that the 30-day survival rate of the TM<17.9 TU/mL group was 1.56 times that of the TM≥17.9 TU/mL group(Log-Rank test,P<0.000 1).Spearman correlation analysis demonstrated that TM levels were positively correlated with APACHE Ⅱ(r=0.10,P<0.005)and SOFA scores(r=0.35,P<0.005).Conclusion Plasma TM has showed a good predictive value for assessing the prognosis of patients with septic shock and may serve as a potential biomarker for determining the prognosis of septic shock.

8.The clinical application value of semiconductor laser combined with flat-panel CT in guiding foramen ovale puncture

Objective To explore the clinical application value of semiconductor laser combined with flat-panel CT guidance in foramen ovale puncture for trigeminal neuralgia(TN).Methods A total of 86 patients with primary TN were selected and randomly divided into control group(n=43)and observation group(n=43).The control group used empirical manual puncture,and the observation group used semiconductor laser combined with flat-panel CT-guided radiofrequency ablation.The first puncture success rate,puncture success time,total operation time,and intraoperative flat-panel CT scan times were carefully evaluated.Results The puncture success time,total operation time,and intraoperative flat-panel CT scan times in the observation group were significantly lower than those in the control group(P<0.05).In addition,the first puncture success rate and surgical efficiency 12 months after surgery were significantly improved(P<0.05).The incidence of facial congestion in the observation group was significantly lower than that in the control group(P<0.05),but there was no statistically significant difference in the incidence of chewing weakness,cerebrospinal fluid leakage,and intracranial infection between the two groups(P>0.05).Conclusion Minimally invasive foramen ovale puncture under the guidance of semiconductor laser combined with flat-panel CT for the treatment of TN has the advantages of simple operation,accurate positioning,low risk,and few complications,with important clinical application value.
9.Relationship between coagulation indicators and early stage prognosis in patients with acute respiratory distress syndrome

Objective To investigate the relationship between coagulation indicators and early prognosis in patients with acute respiratory distress syndrome (ARDS).Methods The data of ARDS patients receiving the treatment in the intensive care unit (ICU) from 2008-2019 were selected from the Critical Care Medicine Open Database (MIMIC-Ⅳ V2.0 version) jointly published by MIT,Beth Israel Deaconess Medical Center,and Philips Medical,the data were categorized according to the severity of the patients' disease and the causes of lung damage.The coagulation indexes and 28 d mortality (m28d) rates were compared among different ARDS patients.The receiver operating characteristic (ROC) curve was drawn.The area under the curve was calculated to evaluate the predictive values of the related indicators.The univariate and multivariate logistic re-gression was adopted to analyze the risk factors affecting m28d in the patients with ARDS.Results Maximum prothrombin time (PTmax) in the patients with pulmonary origin ARDS was significantly lower than that in the patients without pulmonary origin ARDS,and the difference was statistically significant (P<0.05).PLTmin,PLTmax and Sequential Organ Failure Assessment (SOFA) score had statistical difference among dif-ferent severity degrees of ARDS patients (P<0.05).Minimum international normalized ratio (INRmin),maxi-mum international normalized ratio (INRmax),minimum prothrombin time (PTmin),PTmax,maximum activated partial thromboplastin time (APTTmax) and SOFA score had statistical differences between the survival group and death group (P<0.05).AUC of INRmin,INRmax,PTmin,PTmax and APTTmax were 0.607,0.624,0.610,0.620 and 0.648 respectively.The multivariate logistic regression analysis showed that APTTmax (OR=1.011,95%CI:1.001-1.022,P=0.029) was an independent risk factor for affecting m28d in the ARDS patients.Conclu-sion Plasma PLT levels in different severities of ARDS patients have the difference and APTTmax on the first day in ICU is an independent risk factor for affecting early prognosis in ARDS patients.
10.Main characteristics and historical evolution of China's health financing transition

Objective:To comprehend the main characteristics and historical evolution of health finan-cing transition in China.Methods:Data were collected from various sources,including the Global Health Expenditure Database(GHED),China Health Statistics Yearbook,National Health Finance Annual Report,China's Total Health Expenditure Research Report,et al.Descriptive statistics and liter-ature study was conducted.Results:Since the beginning of the 21st century,most countries in the world had witnessed a transition of health financing,characterized by the expansion of health financing scale and the strengthening of public financing responsibility.Notably,China's health financing transition exhibited distinctive features.Firstly,there had been a more rapid expansion in health financing scale compared with global averages.Between 2000 and 2019,total health expenditure per capita experienced a remarkable increase of 816.6%at comparable prices,significantly surpassing average growth rates observed among other countries worldwide(102.1%).Secondly,greater efforts had been made to strengthen the responsibilities of public financing.From 2000 to 2019,there was a substantial decrease of 30.6 percentage points in the proportion of out-of-pocket health expenditure as a share of total health expenditure.This decline was significantly larger than the average reduction observed among other coun-tries worldwide(5.6 percentage points).Thirdly,there had been a significant shift in government health expenditure allocation patterns,with an increased emphasis on"demand-side subsidies"surpassing"supply-side subsidies".Within the realm of"supply-side subsidies",funding directed towards hospitals had notably increased and surpassed that allocated to primary healthcare institutions and public health institutions.Based on these distinctive characteristics,this paper expanded China's health financing tran-sition into three dimensions:Scale dimension,structure dimension and flow dimension.Using a compre-hensive analytical framework,the history of China's health financing transition was roughly divided into four stages:The planned economy stage,the economic transition stage,the post-SARS stage and the new health system reform stage.The main features and evolutionary logic associated with each stage were ana-lyzed.Conclusion:Above all,the health financing system should be enhanced in terms of vertical"em-beddedness"and horizontal"complementarity".Moreover,the significance of health financing transition in preserving hidden value and mitigating public risk should be emphasized,and there is a need for an improved two-way trade-off mechanism that balances value and risk.Additionally,the ethical principles associated with health financing transition should be considered comprehensively,while optimizing budget decision-making within the government's actual governance model.Lastly,it is crucial to recognize the overall and profound impact of modern medicine development and explore long-term strategies and path-ways for health financing transition in China.
