1.Study on life expectancy among HIV-infected patients receiving antiretroviral therapy in Taizhou of Zhejiang Province, 2014 to 2023

Objective:To investigate the life expectancy of antiretroviral treatment (ART) HIV-infected patients and its trends in Taizhou City, Zhejiang Province from 2014 to 2023.Methods:The data were obtained from the China Information System for Disease Control and Prevention , and the study subjects were HIV-infected patients received ART in Taizhou City. An abbreviated life expectancy table was prepared based on Chiang's method to analyze the differences in life expectancy of HIV-infected patients receiving ART in Taizhou City with different characteristics in 2023 and to compare the trends in life expectancy of patients with different CD4 +T lymphocytes (CD4) counts at the time of initiation of ART from 2014 to 2023. Results:A total of 4 825 patients were enrolled in this study, with a cumulative follow-up of 276 648.56 person-years, and a case-fatality rate of 18.07 (95% CI: 16.48-19.65) /1 000 person-years. In 2023, male patients had lower life expectancy than females in all age groups, and those who were married had higher life expectancy than those who were unmarried and those who were divorced or widowed; patients who had been transmitted heterosexually had lower life expectancy than those who had been transmitted through homosexual transmission. Patients with different CD4 counts at the time of initiating ART had different life expectancies in all age groups. The life expectancy of patients with CD4 counts ≥350 cells/μl when initiating the treatment was higher than that of patients with CD4 counts <200 cells/μl in all age groups. The life expectancy of HIV-infected patients on ART at age 20 and 50 increased from 39.0 years and 19.1 years in 2014 to 46.0 years and 24.1 years in 2023, respectively, with an average annual percentage change of 2.43% (95% CI: 0.81%-4.07%) and 3.34% (95% CI: 1.17%-5.56%). The change in life expectancy was similar for patients with CD4 counts ≥350 cells/μl and 200-349 cells/μl at the time of initiating treatment in 2016-2023, and was higher than that for patients with CD4 counts <200 cells/μl. The rate of increase in life expectancy for patients at age 50 was higher than that at age 20 for all CD4 counts. Conclusions:The rising trend of life expectancy among HIV-infected patients on ART in Taizhou City is obvious. But the disparity between patients with different characteristics is obvious, especially among patients with baseline CD4 counts <200 cells/μl, suggesting the importance of expanded testing, early diagnosis and timely initiation of ART to improve the life expectancy of HIV-infected patients.

3.Spectrum-effect relationship between components and antitumor activity of Lonicerae Japonicae Flos based on orthogonal partial least squares regression

Lonicerae Japonicae Flos is a significant food and traditional Chinese medicine, known as plant antibiotics. It has rich chemical constituents and significant pharmacological effects. The antitumor activity of Lonicerae Japonicae Flos has been clarified, but the study on its spectrum-effect relationship has not been reported. The compounds responsible for its antitumor activity are still unknown. In this study, processed products of Lonicerae Japonicae Flos at different temperatures were taken as experimental materials, and SMMC-7721, A549, and MGC80-3 cells were tested. The orthogonal partial least squares regression method was used to analyze the common compounds in different processed products and the antitumor activity. The results show that processed products have a stronger inhibitory effect on A549 cells and MGC80-3 cells than SMMC-7721 cells. Compounds such as secologanic acid, isochlorogenic acid A, serotonin, and chlorogenic acid play an important role in their antitumor effects.

Hereditary hemochromatosis is a rare autosomal genetic disorder that can cause multi-organ dysfunction in the liver,pancreas,spleen,heart and pituitary gland,with diverse clinical manifestations,make the diagnosis difficult.In recent years,with the deepening of clinical understanding and the development of genetic diagnosis tools,the diagnostic rate of this disease has increased significantly.In this paper,we report a case of hereditary hemochromatosis type 3 involving multiple organs and complicated by severe heart failure,aiming to improve the clinicians'understanding of this disease and reduce the leakage and misdiagnosis.
5.Hypokalemia and hypocalcemia induced by caspofungin for injection

A 35-year-old female patient with diabetes ketoacidosis received insulin, meropenem, ganciclovir, correction of acidosis, and fluid infusion due to pulmonary infection. Because sputum culture results suggested aspergillus infection, ganciclovir was stopped and replaced by caspofungin; because the patient′s peak temperature dropped, meropenem was stopped and replaced by piperacillin sodium and tazobactam sodium. Before antibacterial treatments, the blood potassium and calcium were 3.7 mmol/L and 2.07 mmol/L, respectively, and the blood potassium and calcium were 3.5 and 2.18 mmol/L, 3.0 and 1.89 mmol/L, 2.6 and 1.96 mmol/L, and 3.0 and 1.97 mmol/L, respectively on the 3rd, 6th, 8th, and 10th days after using caspofungin and on the 2nd, 3rd, 7th, and 9th days of treatment with piperacillin sodium tazobactam sodium. Piperacillin sodium tazobactam sodium was stopped, caspofungin, potassium and calcium supplements, and other symptomatic treatments were continued. Two days later, blood potassium was 3.0 mmol/L and blood calcium was 1.96 mmol/L. Then caspofungin was stopped and replaced by voriconazole. Three days later, the blood potassium was 3.8 mmol/L and the blood calcium was 2.12 mmol/L. It was considered that the hypokalemia and hypocalcemia were possibly induced by caspofungin.

7. Construction and assessment of a three-dimensional finite element model of mandibualr second molar mesialization using customized lingual appliance and mini-implant

Objective:
To construct a three-dimensional (3D) finite element model and analyze the biomechanical characteristics during mandibular second molar mesialization using customized lingual appliances and mini-implant.
Methods:
One adult student volunteer from The First Affiliated Hospital of Zhengzhou University with lower left first molar extraction was selected. After CT scanning, Mimics, Geomagic, ANSYS were employed to develop a 3D finite element model including customized lingual brackets, stainless steel lingual arch wire, buccal buckles, lower dentition, periodontal ligaments and alveolar bone. Four different loading methods (1. the force of 1.470 N loaded at the lingual side only; 2, 3, 4. the forces of 0.490, 0.735 and 0.980 N loaded at both buccal and lingual sides, respectively.) were included. The initial displacements of the lower second molar and stress distribution in the periodontal ligaments were analyzed.
Results:
More uniform stress distributions in the periodontal membrane of mandibular left second molar were found when the mesial force were loaded at both buccal and lingual sides than the force loaded at lingual side only. More inclination of the second molar was observed when force loaded at both sides than at lingual side only. With the values of the force increased, the mesial inclination potential of the second molar, the initial movement of the second molar and the stress distribution in the periodontal membrane increased. Under the force of 0.735 N at both buccal and lingual sides, mesially inclined initial displacements of the mandibuar second molar were observed and the stress values of von Mises was in the suitable area.
Conclusions
Less second molar rotation was found when the force loaded at both buccal and lingual sides than loaded at lingual side only. However, force loaded at both sides would increase the measial inclination potential of the second molar.
8.Novel missense mutations of the FLG gene identified in two Chinese families affected with ichthyosis vulgaris.

OBJECTIVETo identify potential mutations of the FLG gene in two Chinese families affected with ichthyosis vulgaris.
METHODSAll coding exons and exon-intron boundary of the FLG gene were amplified by polymerase chain reaction (PCR) and analyzed by direct sequencing. The results were compared with those of 100 unrelated healthy controls.
RESULTSTwo novel missense mutations, c.1360A>G (p.T454A) and c.10363G>T (p.D3455Y), were detected in all affected individuals from family 1 and family 2 respectively but none of the controls.
CONCLUSIONThe c.1360A>G (p.T454A) and c.10363G>T (p.D3455Y) of the FLG gene may lead to alteration of the structure and function of the FLG protein and cause ichthyosis vulgaris in the two families.

9.Influence of attachment type on stress distribution of implant-supported removable partial dentures

Objective: To compare influences of different retention attachments on stress among supporting structures .Methods: By 3-dimensional laser scanner and reverse engineering computer aided design ( CAD) software , a basic partially edentulous digital model with mandibular premolar and molar missing was established .Implant attachment and removable partial dentures ( RPD) were added into the basic model to build three kinds of models: RPD only, RPD +implant +Locator attachment , and RPD +implant +Magfit attachment .Vertical and inclined loads were put on artificial teeth unilaterally . By means of 3-dimensional finite element analysis , the stress distribution and displacement of the main supportive structures were compared . Results: A complete 3-dimensional finite element model was established , which contained tooth structure , and periodontal structures .The displacement of the denture was smaller in Locator (9.38μm vertically, 45.48μm obliquely) and Magfit models (9.54μm vertically, 39.45 μm obliquely) compared with non-implant RPD model (95.27 μm vertically, 155.70 μm oblique-ly) .Compared with the two different attachments , cortical bone stress value was higher in Locator model ( Locator model 10.850 MPa vertically , 43.760 MPa obliquely;Magfit model 7.100 MPa vertically , 19.260 MPa obliquely).The stress value of abutment periodontal ligamentin Magfit model (0.420 MPa vertically) was lower than that in Locator model (0.520 MPa vertically).Conclusion:The existence of implant could reduce maximum von Mises value of each supportive structure when Kennedy Ⅰpartially edentulous mandible was restored .Comparing the structure of Magfit and Locator attachment , the contact of Magfit attachment was rigid , while Locator was resilient .Locator attachment could improve stability of the denture dramatical -ly.Locator had stronger effect on defending horizontal movement of the denture .
10.The influence of pre-injury stressful life events on the life quality and mood symptoms following cerebral concussion

Objective To test the reliability and validity of Chinese version of Stressful Life Events Questionnaire and the influence of preinjury stressful life events on the life quality and mood symptoms of the patents following cerebral concussion.Methods 285 patients with cerebral concussion were assessed by Stressful Life Events Questionnaire(SLESQ),PTSD Checklist civilian version(PCL-C),Thirty-Six-Item Short-Form Health Survey (SF-36),Hamilton Depression Rating Scale(HDRS),Hamilton Anxiety Scale(HAMA) at baseline.The patients had been reassessed with the HDRS,HAMA,SF-36 at 3 months after cerebral concussion.The hierarchical regression analyses were used to determine the influence of Stressful Life Events for the life quality and mood symptoms of the patient with cerebral concussion.Results The incidence of stressful life events was 42.8%.The multiple regression analysis showed the experience of stressful life events were the influence factor of scores on the SF-36,HDRS,HAMA after adjusted by the age,gender,education,income,social support,the scores of anxiety and depression before injury and the R 2 value of SLESQ to SF-36 physical health factor,mental health factor,HDRS,HAMA was 0.064,0.111,0.145,0.028,respectively.Conclusion The history of stressful events is the influence factor of the patient with cerebral concussion to have poor life quality and mood disorder.
