Objective To investigate the value of early maternal serological indicators in predic-ting the delivery of full-term small-for-gestational-age(SGA)infants.Methods The clinical data of pregnant women who delivered at Beijing Tongzhou District Maternal and Child Health Hospital from August 2023 to August 2024 were analyzed.A total of 180 pregnant women who delivered full-term SGA infants were included in case group,and 180 pregnant women who delivered full-term appropri-ate-for-gestational-age(AGA)infants during the same period were included in control group.The levels of serum pregnancy-associated plasma protein A(PAPP-A),placenta growth factor(PLGF),and soluble fms-like tyrosine kinase-1(sFlt-1)in the first trimester(11 to 13 weeks+6 days of ges-tation)of the two groups were compared.Receiver operating characteristic(ROC)curves were used to analyze the predictive value of early maternal serological indicators for the delivery of full-term SGA infants.Results The levels of serum PAPP-A and PLGF in the first trimester of the case group were lower than those in the control group,while the level of sFlt-1 was higher(P＜0.05).The area un-der the ROC curve(AUC)for the combined detection of serum PAPP-A,PLGF,and sFlt-1 levels in the first trimester in predicting the delivery of full-term SGA infants was 0.823,which was higher than the AUCs for detection of each indicator(0.738,0.720,0.724)(P＜0.05).Conclusion The detection of early maternal PAPP-A,PLGF,and sFlt-1 levels can reflect the fetal growth status in utero.The combined detection of these three indicators has a higher predictive value for the delivery of full-term SGA infants by pregnant women,which is helpful for clinicians to take intervention measures in advance.

Objective: To understand the pollution characteristics and assess the pollution health risks of heavy metals in atmospheric PM_2.5 in Lanzhou. Methods: According to the regional characteristics of air pollution and industrial distribution characteristics in Lanzhou, atmospheric PM_2.5 was sampled monthly in Chengguan and Xigu Districts from January, 2015 to December, 2016. Detected the concentration of PM_2.5 and 12 kinds of elements (Sb, Al, As, Be, Cd, Cr, Hg, Pb, Mn, Ni, Se and Tl) by weighing method and inductively coupled plasma mass spectrometry. Enrichment factor and geo-accumulation index were used to describe the pollution characteristics, while health risk assessment was conducted using the recommended United States Environmental Protection Agency (USA EPA) model. The health risks of non-carcinogens were evaluated by non-cancer hazard quotient (HQ), the non-carcinogenic risk was considered to be negligible when HQ<1, HQ>1 meant a health risk. With a single contaminant cancer Risk value to evaluate the health risks of carcinogens, when the Risk value between 10^-6 to10^-4 as an acceptable level. Results: The daily average concentrations of PM_2.5 was 83.0 μg/m³, 77.0 μg/m³ in Chengguan and Xigu Districts, respectively, during the sampling periods, and the concentration of PM_2.5 in winter/spring was higher than summer/fall in both districts. The concentration of Al in PM_2.5 was the highest and other elements in descending order: Pb, Mn, As, Sb/Cd, Tl in both districts. Enrichment factor results showed that Al and Mn were mainly affected by natural factors, the rest of five elements were all typical man-made pollution elements and according to geo-accumulation index pollution level of Cd was the strongest in the winter. The results of health risk assessment showed that Mn had the highest non-cancer risks (HQ>1) and affected the health of the children seriously. HQ reached up to 2.44 and 1.79 in Chengguan and Xigu Districts, respectively. Pb, As, Sb, Cd had slight health impact (HQ<1), could be negligible. The cancer risks range of As, Cr were 6.33×10^-6 to 6.46×10^-5 between the acceptable level of risk (10^-6 to 10^-4), which indicated that As and Cd had potential cancer-risks. Conclusions The pollution level of atmospheric PM_2.5 and the heavy metals in it was still grim;the non-cancer risks caused by multiple metals on children deserved attention. Although the cancer risks of As and Cd were between the acceptable level of risk, the potential cancer risk still shall not be ignored.

Objective To explore the conditions, safety and efifcacy of exchange transfusion for extremely severe hyper-bilirubinemia in term newborns requiring mechanical ventilation. Methods Ten full-term newborns of extremely severe hyper-bilirubinemia requiring mechanical ventilation were selected from January 2010 to March 2013 in the department of neonatology. After stable vital sign was achieved by the use of conventional life support, peripheral arterial and venous synchronous exchange transfusion was performed. The bilirubin, platelets, blood calcium, blood coagulation and blood gas were monitored before and after exchange transfusion. The effects and adverse events of exchange transfusion in newborns on mechanical ventilation were observed. Results There was no death in the study, and the replacement rate of total bilirubin was 50.5%. The main adverse event was thrombocytopenia (80%). There was a signiifcant difference in platelet counting before and after exchange transfusion (P<0.05). There was no signiifcant difference in blood coagulation, pH/HCO3-of the blood gas, prothrombin time (PT) and acti-vated partial thromboplastin time (APTT) before and after exchange transfusion. Conclusions It is relatively safe to implement an exchange transfusion in the full-term newborns with severe hyperbilirubinemia in newborns requiring mechanical ventilation, but the use of conventional life support is prerequisite because it stabilizes vital signs. The relatively ideal replacement rate of bili-rubin and the low incidence of adverse reactions are expected. However long-term prognosis of neural system is still unknown.

Objective To explore the neuroprotection of Pabing Formula I Granules for Parkinson's disease (PD) rats. Methods PD rats were induced with injection of 6-hydroxy dopamine twice stereotaxically into the left striatum. 40 rats modeled successfully were divided into model group (n=12), high dose group (n=14) and low dose group (n=14), and the other 8 rats were as normal group. The high dose group and low dose group received Pabing Formula I Granules 18 g/kg and 9 g/kg respectively, while the model group and normal group were given distilled water at the same volume, once a day for 32 d. Then, they were assessed with rotation test. The activities of superoxide dismutase (SOD), the levels of glutathione (GSH) and malonaldehyde (MDA) of their left striatum were measured, and the expression of P-Akt (ser 473), Bcl-2, and Bax in substantia nigra were detected with immunohistochemistry. Results The rotation released was significant different among the model, high dose and the low dose groups after treatment (P<0.01), with the activities of SOD (P<0.01) and the content of GSH (P<0.01) increasing and the content of MDA (P<0.01) descreasing. There was significant difference in the content of GSH (P<0.01) and MDA (P<0.01) between the high and the low dose groups. There was significant difference in expression of P-Akt (ser 473), Bcl-2 and Bax among the model, high dose and the low dose groups, as well as the Bcl-2/Bad ratio (P<0.05). Conclusion Pabing Formula I Granules plays neuroprotective effect through enhancing antioxidation and eliminating free radicals ability and Akt signaling pathway, which appears as dose-effect relationship.

Objective To analyze optimal amount of drinking water,ways of drinking water,timelimit of drinking water on the effect of bowel preparation for intravenous pyelography (IVP) examination after oral administration of 50% magnesium sulfate 60 ml. Methods 200 patients who gave their consentfrom June 2009 to February 2010 drank 50% magnesium sulfate 60 ml for IVP check were divided into four groups. Group A: a large amount of graded oral administration of warm water 1200 ml within 1h; Group B: a small amount of oral administration of warm water 1200 ml within 1h;Group C: a large amount of graded oral administration of warm water 2000 ml within 1h; Group D: a medium amount of graded oral administration of warm water 1200 ml within 2 h. And then the bowel preparation effect of the four gro ups were analyzed. Results Through the comparison, group A was much better than the other groups on the aspects of cathartic satisfaction, the effective degree of reading films and flatulence. Conclusions To take a large amount of graded oral administration of warm water 1200 ml within 1h can improve cathartic satisfaction,the effective degree of reading films and flatulence.

Objective To observe the effect of Shujinkeli on stroke patients with spastic hemiplegia. Methods The double blind test was applied in this study. 77 stroke patients with spastic hemiplegia were randomly divided into the group A (n=38) and the group B (n=39). Under the guidance of the criterion and routine treatment, patients of the group A were treated with Shujinkeli A and patients of the group B were treated with Shujinkeli B. After treatment, curative effect of patients was evaluated. Results The group A was the treatment group, and the group B was the control group. After treatment, the treatment group was superior to the control group in the improvement of muscular tension, Fugel-Meyer score and Barthel index. Conclusion The Shujinkeli can make the high muscular tension lower, which will accelerate the disabled limbs' recovery and improve patients' motor function and self-care ability of daily life.
