1.Scutellarin regulates cAMP/PKA/CREB signaling pathway to improve cognitive dysfunction in rats with post-traumatic stress disorder model

Objective:To investigate the influence of scutellarin(Scu)on cognitive dysfunction in rats with post-traumatic stress disorder(PTSD)by regulating the expression of cAMP/PKA/cAMP response element binding protein(CREB)signaling path-way.Methods:The rat model of PTSD was established by compound stress.The rats were grouped into model group(PTSD),low,medium and high doses scutellarin groups(Scu-L,M,H,5,25,50 mg/kg)and positive control group(fluoxetine,10 mg/kg),in ad-dition,rats that were normally cultured(without compound stress)were set as the blank control group(NC).Open field experiment and Morris water maze experiment were applied to test the rats'autonomous activity ability and learning and memory ability;the histo-pathological changes in the hippocampus of rats was observed by HE staining;TUNEL staining was applied to observe the apoptosis of neurons in rat hippocampus;immunohistochemistry was used to detect the expression of apoptosis-related protein Caspase-3;the con-tents of IL-1β,IL-6 and TNF-α in hippocampus were measured by ELISA;the levels of SOD,cAMP and MDA in the hippocampus were detected by the corresponding kit;the expressions of p-PKA,PKA,p-CREB and CREB proteins were detected by Western blot.Results:Compared with NC group,rats in PTSD group showed pathological changes such as neuron apoptosis and inflammatory cell infiltration,the autonomic activity,learning and memory,SOD activity,the level of cAMP,the protein expression of p-PKA/PKA,p-CREB/CREB decreased obviously,the apoptosis rate of neurons,the expression of Caspase-3 protein,the contents of IL-1β,IL-6,TNF-α and MDA increased obviously(P<0.05);compared with PTSD group,the pathological changes such as neuron apoptosis and inflammatory cell infiltration of rats in Scu-M,H groups and fluoxetine group were obviously improved,the autonomic activity,learn-ing and memory,SOD activity,the level of cAMP,the protein expressions of p-PKA/PKA,p-CREB/CREB increased obviously,the apoptosis rate of neurons,the expression of Caspase-3 protein,the contents of IL-1β,IL-6,TNF-α and MDA decreased obviously(P<0.05).Conclusion:Scu can inhibit inflammation,oxidative stress and neuronal apoptosis,and improve cognitive dysfunction in PTSD rats by activating the expression of cAMP/PKA/CREB signaling pathway.
2.A study of differences in speech recognition in noise between patients with congenital and acquired single-sided deafness

Objective To investigate the differences in speech recognition in noise between patients with con-genital and acquired single-sided deafness.Methods Sixty-two patients with single-sided deafness were included in this study,which included 31 congenital single-sided deafness(CSSD)cases and 31 acquired single-sided deafness(ASSD)cases according to the onset of deafness.Thirty-one normal hearing(NH)subjects were also included in this study as the control group.The ability of speech recognition in noise were tested and compared among the three groups,meanwhile the differences between patients with left and right single-sided deafness were compared.Results The speech recognition threshold in noise of ASSD patients was significantly higher than that of CSSD patients,and both of them were significantly higher than that of the NH subjects.Under the 0 and-2 dB signal-to-noise ra-tio conditions,the speech recognition score was significantly lower in ASSD patients compared to CSSD patients,but only in ASSD patients it was significantly lower than that of the NH group,with no significant difference be-tween CSSD patients and the NH group.A significant difference in speech recognition thresholds was observed be-tween left and right CSSD patients.Conclusion CSSD have better speech recognition in noise than ASSD patients,suggesting better central function compensation.In addition,the side of deafness affects the speech recognition per-formance of CSSD patients.
3.Exploring the Mechanism of Zhinao Capsule in the Treatment of Mild Cognitive Impairment in Parkinson's Disease Based on Clinical Trials and Network Pharmacology

Objective By observing the clinical efficacy of Zhinao capsule in the treatment of Parkinson's disease with mild cognitive impairment(PD-MCI)patients,we also explored the potential mechanism using network pharmacology and molecular docking technology,with the aim of providing a new idea for the treatment of PD-MCI with traditional Chinese medicine.Methods Random number table method was applied to divide the control group and test group into 35 cases each.Parkinson's disease basic treatment plan was used in the control group,and Zhinao capsule was added in the test group.The observation course was determined to be 8 weeks.The Montreal Cognitive Assessment(MoCA)score,MDS-Unified Parkinson's Disease Rating Scale(MDS-UPDRS)Ⅰ,Ⅱ and Ⅲ,PD Traditional Chinese Medicine(PD-TCM),and safety-related indexes were completed before treatment and at the end of the 8th week of treatment.The network pharmacology method was used to obtain the targets related to Zhinao capsules and PD-MCI.Constructed and analyzed the"drug-component-target"network.Analysis of"drug-disease"intersecting targets and enrichment of Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathways was obtained using R language.Protein-protein interaction(PPI)networks were constructed using Cytoscape 3.9.1.Finally,molecular docking was conducted.Results The MoCA score of the test group was remarkably greater than that of the control group after treatment(P<0.05);and the TCM syndrome score were noticeably below that of the control group(P<0.05).The test group showed a notable increase in the mean value of MoCA scale scores,and a clear decrease in the MDS-UPDRS Ⅰand PD-TCM before and after treatment(P<0.05).The MDS-UPDRS Ⅱ and Ⅲ scale scores of the test and control groups decreased to different degrees after treatment,but there was no significant difference(P>0.05).Zhinao capsules for PD-MCI treatment has calycosin,quercetin,kaempferol,etc.as core active ingredients,and TNF,IL-1β,IL-6,etc.as core targets.Molecular docking results also showed better binding of the core target to the active ingredient.Zhinao capsules regulates the expression of PPAR,cGMP-PKG,Oxytocin and other signaling pathway-related genes.Conclusion The Zhinao capsules can improve the cognitive function of PD-MCI patients,and the mechanism may be related to the fact that the components such as calycosin,quercetin,and kaempferol act on the targets such as TNF,IL-1β,and IL-6,and mediate the signaling pathways such as PPAR,cGMP-PKG,and Oxytocin.

5.Epidemiological characteristics of injury deaths in local residents in Nanjing, 2009-2023

Objective:To understand the epidemiological characteristics of injury deaths in local residents in Nanjing from 2009 to 2023, and provide evidence for the development of injury prevention and control strategies.Methods:The injury mortality data in Nanjing from 2009 to 2023 were analyzed based on the death cause surveillance system. In the recorded 33 542 injury death cases, 19 906 (59.35%) were men, and 13 636 (40.65%) were women. The crude mortality rate, age-standardized mortality rate, age-specific mortality rate, cause-eliminated life expectancy (CELE), potential gains in life expectancy (PGLEs) and life loss rate, were calculated. Joinpoint 5.0 software was used to estimate the average annual percentage change (AAPC) and its 95% CI to assess temporal trends of injury deaths. Results:In Nanjing, the crude injury mortality rate showed an upward trend (AAPC=2.11%), while the age-standardized mortality rate exhibited a downward trend (AAPC=-1.27%) from 2009 to 2023. The ranking of injury deaths in all causes of death declined from the 4 th in 2009 to the 6 th in 2023. The crude and age-standardized injury mortality rates in men were consistently higher than those in women. The primary cause of injury deaths was fall (31.42%). Drowning was the primary cause of injury deaths in age group 0-14 years (35.94%), while traffic accident was the primary cause in age group 15-64 years. For residents aged ≥65 years, fall was the primary cause of injury deaths. From 2009 to 2023, the CELE (AAPC=0.61%, 95% CI: 0.34%-0.89%, P<0.05) and the PGLEs (AAPC=1.73%, 95% CI: 0.21%-3.29%, P<0.05) showed increasing trends. The PGLEs and life loss rate due to injury were consistently higher in men than in women, but the AAPC of PGLEs and life loss rate was higher in women. Conclusions:From 2009 to 2023, the age-standardized injury mortality rate decreased, but the life loss due to injury deaths showed an upward trend in Nanjing, indicating that injury still has non-negligible negative impact on life expectancy.

7.Study on the traditional Chinese medicine syndromes in 757 cases of children with hepatolenticular degeneration based on factor analysis and cluster analysis

Objective:
To explore the distribution of traditional Chinese medicine (TCM) syndromes in children with hepatolenticular degeneration (Wilson disease, WD) based on factor analysis and cluster analysis.
Methods:
From November 2018 to November 2023, general information (gender, age of admission, age of onset, course of disease, clinical staging, Western medicine clinical symptoms, and family history) and TCM four-examination informations (symptoms and signs) were retrospectively collected from 757 cases of children with WD at the First Affiliated Hospital of Anhui University of Chinese Medicine, and factor analysis and cluster analysis were used to investigate TCM syndromes in children with WD.
Results:
A total of 757 children with WD were included, of which 483 were male and 274 were female; the median age at admission was 12.58 years, the median age at onset was 8.33 years, and the median course of disease was 24.37 months; clinical typing result indicated 506 cases of hepatic type, 133 cases of brain type, 99 cases of mixed-type, and 19 cases of other type; 36.46% of the children had no clinical symptoms (elevated aminotransferases or abnormalities in copper biochemistry); a total of 177 cases had a definite family history, and 10 cases had a suspected family history. Forty-three TCM four-examination information were obtained, with the top 10 in descending order being feeling listless and weak, brown urine, slow action, inappetence, dim complexion, slurred speech, angular salivation, body weight loss, hand and foot tremors, and abdominal fullness. In children with WD, the syndrome element of disease location was primarily characterized by the liver, involving the spleen and kidney, and the syndrome elements of disease nature were characterized by dampness, heat, and yin deficiency. Based on factor analysis and cluster analysis, five TCM syndromes were derived, which were, in order, syndrome of dampness-heat accumulation (265 cases, 35.01%), syndrome of yin deficiency of the liver and kidney (202 cases, 26.68%), syndrome of liver hyperactivity with spleen deficiency (185 cases, 24.44%), syndrome of qi and blood deficiency (79 cases, 10.44%), and syndrome of yang deficiency of the spleen and kidney (26 cases, 3.43%).
Conclusion
The TCM syndromes of children with WD were primarily syndromes of dampness-heat accumulation, yin deficiency of the liver and kidney, and liver hyperactivity with spleen deficiency. The liver was the main disease location, and the disease nature was characterized by deficiency in origin and excess in superficiality, excess and deficiency mixed. These findings suggest that treating children with WD should be based on the liver while also considering the spleen and kidney.

9.Diagnosis and treatment of special type of laryngeal foreign body with granuloma in children.

Objective:This article explores the diagnosis and treatment of a special type of laryngeal foreign body with granuloma in children, providing a clinical reference for standardized diagnosis and treatment. Methods:This article retrospectively analyzes one case admitted to our department and five other cases reported in relevant literature involving a special type of laryngeal foreign body with granuloma in children. It summarizes the clinical characteristics of this disease and explores its diagnosis and treatment. Results:①A total of 6 cases were misdiagnosed and had a long course of illness. Five cases had no clear history of foreign body at the first visit. One case had foreign body history but no foreign body was found. ②The clinical symptoms were diverse, which could be hoarseness, cough, wheezing, dyspnea, or no symptoms. ③A total of 3 cases were diagnosed by laryngoscopy, and 3 cases were diagnosed by laryngoscopy and imaging examination. ④Foreign bodies that were not wrapped in granulomas, were removed first and then the granulomas were treated; foreign bodies wrapped in granulomas were removed after the removal of granulomas. Conclusion:A special type of laryngeal foreign body with granuloma may lack of accurate foreign body history, resulting in long-term retention of foreign bodies, leading to granuloma formation and misdiagnosis. Electronic laryngoscopy combined with CT examination contribute to early diagnosis. Low temperature plasma under endoscope and anesthetic laryngoscope is conducive to the exposure and precise removal of foreign bodies and avoids tracheotomy to a certain extent.

10.The efficacy of drug combination with immunotherapy in pediatric obstructive sleep apnea and allergic rhinitis after surgery.

Objective:To explore the efficacy of using drug combination and sublingual immunotherapy(SLIT) for pediatric obstructive sleep apnea(OSA) and allergic rhinitis(AR) after adenotonsillectomy, in order to provide a more optimized treatment plan after the surgery. Methods:A total of 95 pediatric OSA combined with AR were selected. According to the treatment plan, they were divided into the SLIT group(postoperative medication combined with SLIT) and the control group(postoperative medication treatment only). The comparisons were made between the two groups regarding the scores of the 18-item Quality of Life Questionnaire for Obstructive Sleep Apnea(OSA-18) and the Visual Analogue Scale(VAS) before and at 1 month, 3 months, 6 months, 1 year, and 2 years after treatment; the monthly total medication scores(TMS) from 1 month to 3 months, 4 months to 6 months, 7 months to 1 year, and 1 year to 2 years after treatment, as well as the number of acute attacks of AR in the 1st year and 2nd year after treatment; and the Lund-Kennedy scores and nasal resistance grading of nasal endoscopy before and at 1 month, 3 months, 6 months, 1 year, and 2 years after treatment. The effectiveness and safety were also analyzed. Results:After one year of treatment, the OSA-18 score, VAS score, TMS and Lund-Kennedy score in the SLIT group were significantly better than those in the control group. The nasal resistance was significantly reduced(P<0.05), and the frequency of AR attacks was significantly lower than that in the control group(P<0.05). After 2 years of treatment, the VAS score, Lund-Kennedy score and nasal resistance classification in the SLIT group tended to stabilize, while the OSA-18 score continued to decline. Conclusion:After surgery for pediatric OSA combined with AR, the use of drugs combined with SLIT can effectively alleviate AR symptoms, further improve OSA-related symptoms and quality of life, reduce drug dependence, decrease the frequency of AR attacks, and enhance the long-term efficacy of the surgery.
