1.Clinical efficacy of endoscopic endonasal dorsum sellae and posterior clinoid process resection combined with pituitary hemi-transposition for pan-sellar area lesions

Objective To explore the clinical efficacy and safety of endoscopic endonasal dorsum sellae and posterior clinoid process resection combined with pituitary hemi-transposition in the treatment of pan-sellar area lesions. Methods Clinical data from 57 patients with pan sellar diseases who underwent endoscopic transnasal resection of the posterior clinoid process combined with lateral pituitary displacement surgery in the neurosurgery department of Zhongshan Hospital, Fudan University from January 2019 to January 2025 were collected, and their clinical surgical characteristics and prognosis were analyed. Results A total of 57 patients with pan-sellar area lesions were included in this study, comprising 27 craniopharyngiomas, 7 giant suprasellar pituitary adenomas, 7 chordomas, 6 chondrosarcomas, 5 diaphragma sellae/dorsum sellae meningiomas, and 5 rare lesions. All procedures were completed successfully, and gross total or near-total resection was achieved in all patients. No new neurovascular complications occurred intraoperatively. The follow-up duration ranged from 3 to 24 months. New postoperative anterior pituitary insufficiency occurred in 3 patients (5.3%), and new posterior pituitary insufficiency requiring long-term hormone replacement therapy occurred in 4 patients (7.0%). No additional pituitary dysfunction was observed in the remaining patients. Cerebrospinal fluid rhinorrhea occurred in 1 patient (1.8%) and was successfully repaired. Intracranial infection occurred in 3 patients (5.3%) and resolved after medical treatment. Conclusion Endoscopic resection of the posterior clinoid process combined with lateral displacement of the pituitary gland can effectively expand the surgical space, while repositioning the pituitary gland can reduce the extent of skull base defects. In addition, the risk of pituitary dysfunction caused by lateral displacement of the pituitary gland is relatively low.
2.Clinical efficacy of artificial intelligence-based pelvic floor optimization training system using human body motion analysis technique

Objective To apply an artificial intelligence (AI) -based pelvic floor optimization training system using human motion analysis technique in patients undergoing pelvic floor rehabilitation, so as to assist clinicians in providing guidance, improving clinical efficiency, and enhancing the efficacy of patients' independent training. Methods A total of 60 patients with overactive bladder (OAB) complicated with urgent urinary incontinence (UUI) treated in our hospital during Sep. 2024 and Dec. 2025 were enrolled and randomly divided into the routine guidance group (n=30) and AI guidance group (n=30). The routine guidance group received conventional pelvic floor optimization training under clinicians' instruction, while the AI guidance group underwent training with the AI-based pelvic floor optimization system using human motion analysis. After 6 weeks of intervention, outcomes were compared between the two groups, including the international consultation on incontinence questionnaire-short form (ICI-Q-SF), incontinence quality of life scale (I-QoL), overactive bladder symptom score (OABSS), patient's perception of bladder condition (PPBC) score, pelvic floor muscle function (myoelectric values in the preresting, post-resting, type Ⅰ muscle, type Ⅱ muscle, and endurance test phases), urine output, average flow rate (Qavg), and maximum flow rate (Qmax). Results After 6 weeks of treatment, the ICI-Q-SF, I-QoL, OABSS, PPBC score, type I muscle myoelectric value, Qavg and Qmax were significantly improved in both groups compared with baseline (P<0.05). After 6 weeks of intervention, the I-QoL score was significantly higher in the AI guidance group than in the routine guidance group[81.82 (79.55, 84.38) vs.77.84 (75.00, 79.55) ], with significant difference (P<0.05). Compared with the routine guidance group, the AI guidance group showed significant improvements in myoelectric values in the pre-resting, post-resting, type I muscle and endurance testing phases (P<0.05). Conclusion The AI-based pelvic floor optimization training system can effectively assist clinicians in guiding patients to complete pelvic floor optimization training, improve rehabilitation efficiency and efficacy of independent exercise, and reduce dependence on specialized clinicians, which exhibits favorable safety.
3.Association of adverse childhood experiences with psychopathology, sleep, and cognitive function in children: the mediating role of putamen volume

BackgroundAdverse childhood experiences (ACEs) are strong predictors of psychopathology across the lifespan, exerting extensive adverse effects on physical and mental health during early development. While ACEs increase the risk of childhood psychopathology, sleep quality and impair cognitive function, the early neurobiological mechanisms mediating ACE-induced cognitive impairment remain unclear. ObjectiveTo explore the association between ACEs exposure and the risk of psychopathology, sleep disorders, and cognitive function, and to examine the mediating role of the putamen volume in the relationship between ACEs and cognitive function from a neurodevelopment perspective. MethodsData were obtained from the Adolescent Brain Cognitive Development (ABCD) study, comprising 10 572 children aged 9 to 10. Participants were categorized into five groups based on the cumulative score of ACEs reported by their parents and the children themselves: no-exposure group (0 types of ACEs), the one-exposure group, the two-exposure group, the three-exposure group, and the high-risk exposure group (≥ 4 types of ACEs). Logistic regression analysis was employed to evaluate the association between ACEs and psychopathological symptoms [T score of the Child Behavior Checklist (CBCL) ≥ 65]. Multiple linear regression analysis was ultilized to analyze the correlations between ACEs and the scores of the Sleep Disturbance Scale for Children (SDSC), the NIH Toolbox Cognition Battery (NIHTB-CB), as well as the multiple structures in the cerebral cortex and subcortex. The Bootstrap method was applied to test the mediating effect of the putamen volume between ACEs and neurocognitive function. ResultsAfter adjusting for confounders, ACEs cumulative score was correlated with children's psychopathological symptoms in a dose-response manner. Compared with the no-exposed group, the high-risk exposure group had a 2.08-fold (OR=2.080, 95% CI: 1.401–3.088) increased risk of internalizing problems and a 3.13-fold (OR=3.132, 95% CI: 1.703–5.760) increased risk of externalizing problems, and the risk of conduct problems was 4.68 times that of the no-exposed group (OR=4.681, 95% CI: 2.528–8.669). ACEs cumulative score positively predicted the total score of SDSC (β=0.080, 95% CI: 0.059–0.102), with relatively stronger predictive effects on sleep-wake transition disorder (β=0.086, 95% CI: 0.063–0.109) and sleep onset and maintenance disorder (β=0.066, 95% CI: 0.043–0.089). In terms of neurocognition and brain structure, ACEs cumulative score negatively predicted the overall cognitive composite score (β=-0.025, 95% CI: -0.050–-0.001), the crystallized cognitive composite score (β=-0.035, 95% CI: -0.060–-0.010), the volume of the left putamen nucleus (β=-0.025, 95% CI: -0.046– 0.003), and the volume of the right putamen nucleus (β=-0.023, 95% CI: -0.044– 0.003). The total putamen volume played a partial mediating role between ACEs and the overall cognitive composite score, the effect value was -0.002 (95% CI: -0.004–-0.001), accounting for 7.49% of the total effect. ConclusionACEs exposure may increase the risk of psychopathological and sleep disturbances in children and may impair neurocognitive function. Reduced putamen volume may serve a potential neurobiological pathway underlying the impact of ACEs on childhood cognition. Osteoarthritis (OA) and stroke are common clinical diseases that reduce patients' quality of life and place a burden on families and society. Ruyi Zhenbaowan, a classic prescription in Tibetan medicine, have the functions of clearing heat, awakening the brain and opening orifices, relaxing tendons and promoting meridian circulation, and eliminating yellow water. Clinically, they are used to treat osteoarthritis, post-stroke sequelae, neuropathic pain, and other related conditions. Modern pharmacological studies have demonstrated their anti-inflammatory, analgesic, and nerve-repairing effects. However, current research remains insufficient regarding the appropriate indications, timing, and efficacy of this medicine in treating relevant diseases. To enhance clinicians' understanding of this medicine and promote its standardized and rational clinical use, a panel of national experts, including clinical specialists, Tibetan medicine practitioners, pharmacologists, and methodologists, formulated this consensus based on clinical experience and evidence-based practice. The Cochrane systematic review framework, the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system, and the nominal group method were employed to generate seven graded recommendations and 19 consensus-based suggestions. These recommendations clearly define the key points in the clinical application of Ruyi Zhenbaowan, including therapeutic indications, dosage and administration, treatment duration, and medication safety. The consensus specifically addresses the clinical efficacy, appropriate timing of administration, dosage strategies, treatment cycles, and combination medication strategies for treating osteoarthritis and stroke and provides an overview of safety considerations. The aim is to provide standardized guidance for hospitals and healthcare institutions nationwide to ensure the rational application of Ruyi Zhenbaowan in the treatment of osteoarthritis and stroke, reduce medication-related risks, and further leverage its clinical advantages. This consensus has been approved and issued by the China Association of Chinese Medicine, with the standard number GS/CACM 369-2024.
5.Establishment and Implementation of Post-marketing Benefit-risk Assessment Strategy for Traditional Chinese Medicine

The field of post-marketing benefit-risk assessment for traditional Chinese medicine(TCM) is still in its nascent stage, lacking a universally accepted and cohesive evaluation framework and standards. This study presented a strategy developed for the benefit-risk assessment of post-marketing of TCM, and explored the critical techniques and specific implementation steps involved in the assessment process. Initially, appropriate qualitative assessment frameworks and quantitative analysis models were selected for the integrated qualitative and quantitative benefit-risk assessment. Subsequently, key technologies were outlined, including the establishment of a benefit-risk indicator system, the assignment of indicator weights, and the definition of criteria attributes. Furthermore, the implementation steps were elaborated, which involved defining decision-making issues, data collection, evaluation methodologies, variability factors, and sensitivity analysis. Finally, a case study of the benefit-risk assessment of a TCM injection for hepatitis B treatment was conducted to validate the feasibility of the proposed strategy. The objective of this research was to provide theoretical support and practical references for the development of a comprehensive post-marketing benefit-risk assessment system for TCM.
6.Correlation of life events with depression, anxiety and somatic symptoms in graduate students: a study based on network analysis

BackgroundGraduate students frequently face life events, many of which may adversely affect their mental well-being. However, the interaction between life events and the development of depression, anxiety, and somatic symptoms remains unclear. ObjectiveTo explore the relationship between life events and the development of depressive, anxiety and somatic symptoms in graduate students, thereby informing prevention strategies for these conditions. MethodsA sample of 6 722 newly enrolled graduate students at a comprehensive university in Southwest China from September to November 2018 was selected. The assessment was conducted using the Adolescent Self-rating Life Events Checklist (ASLEC), the 7-item Generalized Anxiety Disorder scale-7 item (GAD-7), the Patient Health Questionnaire Depression Scale-9 item (PHQ-9), and the Patient Health Questionnaire-15 (PHQ-15). Network analysis was implemented by using the bootnet and qgraph packages in the R software (version 4.2.3), with centrality indices calculated to identify core and bridge symptoms within the network. ResultsThe study encompassed a total of 6 171 graduate students, representing 91.80% of the target population. The prevalence rates of anxiety, depressive, and somatic symptoms among graduate students were 12.59% (777/6 171), 16.63% (1 026/6 171), and 27.66% (1 707/6 171), respectively. Network analysis revealed that 'academic stress' was the core symptom with the highest strength and expected influence (both values=1.207), while 'feeling down, depressed, or hopeless' was the bridge symptom with the highest bridge strength and bridge expected influence (both values=0.454). There was no significant difference in global network strength and edge weight between women and men (P>0.05). ConclusionAcademic stress, emerging as the core symptom, assumes a dominant position within the symptom network and exhibits strong interactions with other negative affective states. There was no gender difference in the network structure.
7.Protective effect of secreted secretomes derived from three-dimensional cultured mesenchymal stem cells on mouse hippocampal neurons with seawater immersion and trauma

Objective To investigate the protective effects of secretomes released by three-dimensional cultured mesenchymal stem cells(MSCs)on neurons subjected to seawater immersion(SW)and stretch injury(SI),and to provide new insights into neuronal repair following SW combined with traumatic brain injury(TBI).Methods MSCs were cultured using the hanging drop method,and the conditioned medium(CM)containing MSCs secretomes was collected.A cellular model combining SW with SI was established using mouse hippocampal neuronal cells(HT22 cells).HT22 cells were randomly assigned to five groups:control,SI,SI+SW,SI+CM,and SI+SW+CM groups.Cell viability was assessed using the CCK-8 assay,apoptosis rate was measured by flow cytometry,cell migration ability was evaluated by scratch assay,and the expression levels of apoptosis-related proteins Bcl-2 and Bcl-2-associated protein(Bax),and ferroptosis-related proteins long-chain acyl-CoA synthetase 4(ACSL4)and cyclooxygenase-2(COX-2)were detected by Western blotting.Results Immersion in 15%seawater for 12 h significantly decreased HT22 cell viability(P<0.05).The CCK-8 assay indicated that cell viability in both the SI and SI+SW groups was significantly lower than that in control group after 12 h of treatment(P<0.05).Treatment with CM containing MSCs secretomes significantly increased cell viability in SI+CM group compared to SI group(P<0.0001),and in SI+SW+CM group compared to SI+SW group(P<0.001).Flow cytometry results revealed that the apoptosis rate in SI and SI+SW groups was significantly higher than that in control group(P<0.05 or P<0.001),while in SI+CM group was lower than that in SI group(P<0.05),and in SI+SW+CM group was lower than that in SI+SW group(P<0.05).Western blotting showed that compared to control group,SI and SI+SW groups exhibited reduced Bcl-2 expression level(P<0.01 or P<0.0001)and increased expression levels of Bax,ACSL4,and COX-2(P<0.01 or P<0.0001).Compared to SI group,the SI+CM group displayed increased Bcl-2 expression level(P<0.05)and decreased expression levels of Bax,ACSL4,and COX-2(P<0.05).Compared to SI+SW group,SI+SW+CM group exhibited increased Bcl-2 expression level(P<0.01)and decreased expression levels of Bax,ACSL4,and COX-2(P<0.01 or P<0.001).Scratch assay results demonstrated that at both 12 h and 24 h,the cell migration rate in SI and SI+SW groups was significantly lower than that in control group(P<0.01 or P<0.0001),while the migration rate in SI+CM group was significantly higher than that in SI group(P<0.0001 or P<0.01),and the migration rate in SI+SW+CM group was significantly higher than that in SI+SW group(P<0.0001).Conclusion Secretomes derived from MSCs cultured using the hanging drop method can alleviate neuronal damage caused by SW and TBI,potentially offering a therapeutic approach for SW combined with TBI.
8.The influence of balloon pulmonary angioplasty for chronic thromboembolic pulmonary hypertension on renal functions

Objective To assess the incidence of contrast-induced nephropathy(CIN)in patients with chronic thromboembolic pulmonary hypertension(CTEPH)after receiving balloon pulmonary angioplasty(BPA),and to evaluate the effect of the contrast agents on renal function.Methods A total of 143 CTEPH patients,who received BPA at the China-Japan Friendship Hospital of China from December 2018 to May 2022,were enrolled in this study.The clinical data,hemodynamic indicators,and serum creatinine(SC)concentrations within one week before and 48-72 h after BPA were collected.The estimated glomerular filtration rate(eGFR)was calculated according to the Modification of Diet in Renal Disease(MDRD)formula.The SC concentration and eGFR changes before and after each BPA procedure were compared.The incidence of CIN and its risk factors were evaluated,and the changes in hemodynamics,SC and eGFR after the initial and last time of BPA treatment were analyzed.Results A total of 192 BPA procedures were performed in 115 CTEPH patients,including 88 BPA procedures in males and 103 BPA procedures in females.The mean amount of contrast agent used for each BPA was(145.58±47.26)mL.After BPA,12 patients developed 13 times of CIN,with an incidence of 6.8%.There was no significant differences(P>0.05)in the baseline characteristics and SC concentration before BPA between CIN patients and non-CIN patients.In terms of the hemodynamic indexes,the mixed venous oxygen saturation(SvO2)in CIN patients was significantly lower than that in non-CIN patients(58.58%±10.38%vs.66.15%±8.02%,P=0.002),and no statistically significant differences(P>0.05)in the other hemodynamic indexes existed between CIN group and non-CIN group.No statistically significant differences in SC concentration and eGFR existed before and after each BPA procedure.In patients who had received several BPA procedures,significant improvements in the SC[(78.09±18.760)μmol/L vs.(82.26±21.37)μmol/L,P<0.001]and eGFR[(86.08±21.22)mL/(min·1.73 m2)vs.(82.07±22.05)mL/(min·1.73 m2),P=0.007]was achieved when compared with their baseline values.Conclusion CTEPH patients may develop CIN after receiving BPA treatment.After receiving several BPA treatments the patient's clinical symptoms and hemodynamics can be improved,and the patient's renal function is also significantly improved.
9.Mechanistic study of metformin-mediated modulation of cellular senescence and radiosensitivity in pancreatic cancer

Objective:
To study the effect of metformin sensitizing pancreatic cancer cells with radiotherapy, with a focus on elucidating the underlying mechanisms of radiotherapy resistance. In particular, the role of the PERK/P-eIF2/ATF4 signaling pathway in mediating these effects was preliminarily explored.
Methods :
Pancreatic cancer cell lines(PANC-1 and PANC-2) were categorized into control, radiotherapy, and drug treatment groups. Following the respective treatments, cell proliferation inhibition was assessed using the CCK-8 assay, colony formation assays, and cell death staining. Senescence was quantified by β-galactosidase(SA-β-Gal) staining. The expression of cell cycle regulators(P21, P16, γ-H2AX), apoptosis markers(Bax, Bcl-2, Cleaved caspase-3), and pathway-related proteins(PERK, P-eIF2, ATF4) was evaluated by Western blot and immunofluorescence. To further investigate the role of the PERK/P-eIF2/ATF4 axis in metformin-mediated modulation of pancreatic cancer cell senescence and radiosensitization, selective inhibitors(GSK2606414) and agonists(MK-28) of PERK were employed.
Results :
Radiotherapy markedly upregulated senescence-associated markers(P21, P16, γ-H2AX, and β-galactosidase activity) in pancreatic cancer cells. Senescent cells exhibited enhanced proliferative activity and increased tumor volume both in vitro and in vivo. Metformin mitigated radiotherapy-induced senescence by reducing the expression of senescence markers and significantly suppressing the clonogenic and proliferative capacity of treated cells. Mechanistically, radiotherapy activated the PERK signaling pathway, leading to increased expression of PERK, P-eIF2, and ATF4, thereby driving cellular senescence. Pharmacological inhibition of PERK reduced β-galactosidase activity, while PERK activation further promoted the expression of senescence-associated proteins—an effect that was reversed by metformin.
Conclusion
Metformin inhibits the activation of the PERK/P-eIF2/ATF4 signaling pathway in pancreatic cancer cells following radiotherapy, thereby delaying cellular senescence and reducing the associated radiotherapy resistance of senescent cells. This modulation contributes to the sensitization of pancreatic cancer cells to radiotherapy.
10.Construction and application of a decision support education program on hospice care for family members of patients with advanced cancer

Objective To construct a decision support education program for the family members of patients with advanced cancer and to investigate its application effects,so as to improve understanding and acceptance of hospice care for family members of advanced cancer patients.Methods Using the Ottawa Decision Support Framework as a theoretical guide,the program was initially drafted based on a literature review,qualitative interview and expert consultation.From September 2023 to January 2024,a convenience sampling method was used to select patients' families in a tertiary-level hospital in Tianjin as the research subjects,and they were randomly divided into an experimental group and a control group.The experimental group received the decision support education program in addition to routine care,while the control group received routine care.Family members' knowledge about hospice,the scores on the Death Attitude Profile Scale,and their willingness to choose hospice care were compared before and after the interventions.Results The program finally included 4 first-level items,15 second-level items,and 59 third-level items.During the program implementation phase,4 cases withdrew from the study,resulting in 46 cases in the experimental group and 47 cases in the control group.After intervention,the experimental group had higher scores on hospice knowledge and positive attitude towards death than the control group,while scores on negative attitude towards death were lower(P<0.05);their willingness to choose hospice care for themselves and for the patients was higher than that of the control group(P<0.05).Conclusion The hospice care decision support education program is scientific,feasible and practical,which can improve the knowledge of hospice care of the family members,improve their attitude towards death,and ultimately improve their willingness to choose hospice care.
