ObjectiveTo explore the effectiveness and potential mechanism of Shugan Jianpi Granules (疏肝健脾颗粒) combined with Chushi Zhiyang Ointment (除湿止痒软膏) in the treatment of atopic dermatitis with sleep disturbance of liver constraint and spleen deficiency syndrome. MethodsSixty-six patients with atopic dermatitis combined with sleep disturbance of liver constraint and spleen deficiency syndrome were randomly divided into 33 cases each in the treatment group and the control group. Both groups were treated with Chushi Zhiyang Ointment for external use. The treatment group additionally received Shugan Jianpi Granules orally, one dose per day; and the control group received cetirizine hydrochloride tablets orally, 10mg per day, taken before bedtime. Both groups were under the treatment for 4 weeks. Clinical effectiveness was evaluated after treatment, and the SCORing Atopic Dermatitis (SCORAD) index, pruritus visual analog scale (VAS) score, Insomnia Severity Index (ISI) score, and Pittsburgh Sleep Quality Index (PSQI) score were recorded before and after treatment, and serum levels of melatonin (MLT), interleukin-4 (IL-4), interleukin-6 (IL-6), interleukin-13 (IL-13), and tumor necrosis factor-α (TNF-α) were measured. The patients were also tested for blood routine, liver function and kidney function before and after treatment, and the occurrence of adverse events was also observed. ResultsThe total effective rate in the treatment group (90.63%, 29/32) was significantly higher than that in the control group (66.67%, 20/30, P<0.05). After the interventions, the scores for SCORAD, VAS, ISI, and PSQI, as well as the levels of IL-4, IL-6, IL-13, and TNF-α, were significantly reduced in both groups. In contrast, the MLT levels in treatment group significantly elevated compared to that before treatment (P<0.05). Comparing between the two groups after treatment, the scores of SCORAD, ISI, PSQI, and VAS, as well as the levels of serum IL-4 and IL-13 in the treatment group were significantly lower than those in the control group, while the MLT levels were markedly higher than that in the control group (P<0.05 or P<0.01). There was no obvious abnormality in blood routine, liver function and kidney function in both groups when compared before and after treatment. The incidence rate of adverse reactions was 9.38% (3/32) in the treatment group and 13.33% (4/30) in the control group, and the difference between groups showed no statistical difference (P>0.05). ConclusionOn the basis of topical application of Chushi Zhiyang Ointment, the combined application of Shugan Jianpi Granules can significantly alleviate the degree of skin lesions and itching, and improve the quality of sleep in atopic dermatitis patients with sleep disturbance of liver constraint and spleen deficiency syndrome, and its mechanism may be related to regulating the immune balance, reducing the secretion of inflammatory factors, and elevating the level of MLT.
2.Pharmacodynamic Substances and Mechanisms of Da Chengqitang in Treating Stroke: A Review

Stroke is the main cause of death and disability among adults in China and is characterized by high incidence, disability, mortality, and recurrence rates. The combination of traditional Chinese and Western medicine has great potential in treating stroke and its sequelae. The classic traditional Chinese medicine prescription Da Chengqitang (DCQT) has a long history and proven efficacy in treating stroke. Clinically, DCQT is often used to treat stroke and its sequelae. However, the number and quality of clinical trials of DCQT in treating stroke need to be improved. Because of the insufficient basic research, the active ingredients and multi-target mechanism of action of DCQT remain unclear. Our research group has previously confirmed that DCQT can effectively reverse neurological damage, reduce iron deposition, and downregulate the levels of pro-inflammatory cytokines in the rat model of hemorrhagic stroke. The treatment mechanism is related to the nuclear factor erythroid 2-related factor 2 (Nrf2)-mediated signaling pathway and p38 mitogen-activated protein kinase (MAPK) signaling-mediated microglia activation. To clarify the pharmacodynamic basis and anti-stroke mechanism of DCQT, this article reviews the research progress in the treatment of stroke with DCQT in terms of clinical trials, pharmacodynamic material basis, safety evaluation, and mechanisms of absorbed components. This article summarizes 45 major phytochemical components of DCQT, 11 of which are currently confirmed absorbed components. Among them, emodin, rhein, chrysophanol, aloe-emodin, synephrine, hesperidin, naringin, magnolol, and honokiol can be used as quality markers (Q-markers) of DCQT. The mechanism of DCQT in treating stroke is complex, involving regulation of inflammatory responses, neuronal damage, oxidative stress, blood-brain barrier, brain-derived neurotrophic factor, and anti-platelet aggregation. This article helps to deeply understand the pharmacodynamic basis and mechanism of DCQT in treating stroke and provides a theoretical basis for the clinical application of DCQT in treating stroke and the development of stroke drugs.

The gut microbiota plays a pivotal role in the immunomodulatory and protumorigenic microenviron-ment of colorectal cancer(CRC).However,the effect of ginsenoside Rk3(Rk3)on CRC and gut microbiota remains unclear.Therefore,the purpose of this study is to explore the potential effect of Rk3 on CRC from the perspective of gut microbiota and immune regulation.Our results reveal that treatment with Rk3 significantly suppresses the formation of colon tumors,repairs intestinal barrier damage,and regulates the gut microbiota imbalance caused by CRC,including enrichment of probiotics such as Akkermansia muciniphila and Barnesiella intestinihominis,and clearance of pathogenic Desulfovibrio.Subsequent metabolomics data demonstrate that Rk3 can modulate the metabolism of amino acids and bile acids,particularly by upregulating glutamine,which has the potential to regulate the immune response.Furthermore,we elucidate the regulatory effects of Rk3 on chemokines and inflammatory factors associated with group 3 innate lymphoid cells(ILC3s)and T helper 17(Th17)signaling pathways,which inhibits the hyperactivation of the Janus kinase-signal transducer and activator of transcription 3(JAK-STAT3)signaling pathway.These results indicate that Rk3 modulates gut microbiota,regulates ILC3s immune response,and inhibits the JAK-STAT3 signaling pathway to suppress the development of colon tumors.More importantly,the results of fecal microbiota transplantation suggest that the inhibitory effect of Rk3 on colon tumors and its regulation of ILC3 immune responses are mediated by the gut microbiota.In summary,these findings emphasize that Rk3 can be utilized as a regulator of the gut microbiota for the prevention and treatment of CRC.
5.Effects of whole body vibration training combined with blood flow restriction on motor function and com-munity activity in elderly stroke patients with hemiplegia

Objective To investigate the impact of whole body vibration training combined with intermittent blood flow restriction on the motor function and community activity in elderly stroke patients with hemiplegia.Methods A total of 80 convalescent hemiplegic patients after stroke who were hospitalized in the Rehabilitation Medicine Department of Yichang Central People's Hospital(Xiling Campus)from October 2021 to December 2023 were included and randomly divided into control group(n=25),vibration group(n=28)and combination group(n=27)using the random number table method.All patients received conventional rehabilitation training,on the basis;the vabration group received whole body vibration training,while the combination group received whole body vibration training combined with intermittent blood flow restriction.Before and after 6 weeks of training,the balance function was evaluated with Berg balance scale(BBS),while the gait function was tested with 6-minute walking test(6MWT)and the Community Balance and Mobility scale(CB&M)was used to assess the community activity ability.The community balance and mobility(CB&M)were evaluated in the first and third month after discharge.Results There was no significant difference in BBS,CB&M scores and 6MWT walking distance between the three groups before training(P>0.05).After 6 weeks of training,the three groups showed significant increases in the BBS,CB&Mscores and the walk distance of 6MWT(P<0.001).Furthermore,compared with the control group,the combination group and the vibration group were significantly beter(P<0.05),and there was no signifi-cant difference between the combination group and the vibration group(P>0.05).At the first and third month of follw-up after discharge,the CB&M scores of the three groups were significantly higher than those of the control group(P<0.05),while there was no significant difference between the CB&M scores of the combination group and the vibration group at the first month of follow-up(P>0.05).But the CB&M scores of the combination group was higher than those of the vibration group at the third month of follow-up,and the difference was statistically signifi-cant(P<0.05).Conclusion On the basis of conventional rehabilitation,whole body vibration training combined with intermittent blood flow restriction can significantly enhance balance function,balance confidence,walking ability and mobility in early stage of community life for elderly stroke patients with hemiplegia,potentially facilitat-ing their return to community life.
6.Cost-effectiveness analysis of different cleaning frequency on reducing pathogens on hospital bed-side curtains

Objective To explore the pathogen contamination and cost-effectiveness of hospital bed-side curtains under different cleaning frequencies.Methods Fourteen departments in a hospital with relatively higher incidence but smaller fluctuation of incidence of infection from January to June 2023 were selected.Nine wards in each depart-ment were selected,and two curtains were selected from each ward.According to the cleaning frequency,wards were divided into low,medium and high frequency groups.The qualified rate of environmental hygienic detection on pathogens on the bed-side curtains and bacterial contamination on the surface of curtains after 6 months were com-parted,cost-effectiveness analysis was conducted.Results The qualified rate of environmental hygienic detection of bed-side curtains was the highest in high frequency group,at 88.10%.Bacterial counts on the upper,middle,and lower segments of the bed-side curtains in the high frequency group were all lowest(P<0.05).Within 6 months,the incidence of healthcare-associated infection(HAI)in patients in the high and the medium frequency groups(0.16%and 0.25%,respectively)were both lower than 0.85%in the low frequency group(x2=5.653,3.915,respectively,both P<0.05).Cost-effectiveness analysis showed that for every additional unit of qualified rate for the high and medium frequency groups,the costs increase by 124.51 and 44.10 Yuan,respectively.Conclusion The increase of cleaning frequency can effectively reduce bacterial contamination of bed-side curtains,decrease the risk of HAI,and improve the qualified rate of environmental hygiene of bed-side curtains in the wards.Cleaning once every 2 months can achieve better cost-effectiveness.
7.Hemodynamic analysis of aortic bileaflet mechanical heart valve under different physiologic conditions

The velocity distribution,vortex evolution,viscous shear stress distribution and Reynolds shear stress distribution downstream of the aortic bileaflet mechanical heart valve are investigated under different physiological conditions using computational fluid dynamics method.At peak systole,the blood jet is more intense in the exercise state than in the other two states,with a maximum blood flow rate of 2.1 m/s.The analysis on vortex evolution shows that the shear layer is the obvious flow feature in the 3 physiological states and that the blood interacts more strongly with the aortic sinus at peak systole.For viscous shear stress,when the valve leaflets are fully open,the leaflets impede blood flow across the valve in the valvular region.The interaction between the blood flow and the valve leaflets results in high shear stress,and the maximum viscous shear stress observed in the 3 physiologic states is less than 8 N/m2.The maximum Reynolds shear stress observed in the 3 physiologic states is 700 N/m2.The study provides a theoretical basis for surgical valve selection and postoperative rehabilitation.
8.Expression and biological significance of long non-coding RNA HOXA transcript at the distal tip in oral squamous cell carcinoma

Objective:To investigate the expression of long non-coding RNA HOXA transcript at the distal tip(HOTTIP)in oral squamous cell carcinoma(OSCC)and examine its effect on the biological function of OSCC cells.Methods:A total of 52 patients with OSCC who received surgical treatment in The First Affiliated Hospital of Zhengzhou University from January 2020 to June 2021 were collected.The expression levels of HOTTIP in OSCC and adjacent tissues,human OSCC cell lines HSC-4,SCC-9 and CAL-27,and human normal oral keratinocyte line HOK were detected by quantitative real-time polymerase chain reaction(RT-qPCR),and its correlation with clinicopathological data and overall survival(OS)of OSCC patients was analyzed.The effects of down-regulating HOTTIP or co-down-regulating HOTTIP and miR-637 on the proliferation,apoptosis,migration,invasion,and epithelial-mesenchymal transition(EMT)of CAL-27 cells were analyzed using Cell Counting Kit-8(CCK-8),flow cytometry,Transwell assays,and Western blot.The targeting relationship between HOTTIP and miR-637 was analyzed by dual luciferase reporter gene assays.Results:The expression level of HOTTIP in OSCC tissues was significantly increased com-pared with adjacent tissues(P<0.05)and was correlated with tumor size,TNM stage,degree of differentiation,lymph node metastasis,and OS(P<0.05).The expression of HOTTIP in HSC-4,SCC-9,and CAL-27 cells significantly increased compared with normal HOK cells(P<0.05).After down-regulation of HOTTIP expression,the proliferation,migration,invasion,and EMT abilities of CAL-27 cells were significantly weakened,whereas the apoptosis rate significantly increased(P<0.05).HOTTIP had a targeting relationship with miR-637;after down-regu-lating HOTTIP,the expression of miR-637 in CAL-27 cells significantly decreased(P<0.05).After the simultaneous downregulation of HOTTIP and miR-637,the proliferation,migration,invasion,and EMT abilities of CAL-27 cells were significantly enhanced,whereas the apoptosis rate significantly decreased(P<0.05).Conclusions:HOTTIP is highly expressed in OSCC tissues and cells and can target miR-637 to regulate the proliferation,apoptosis,migration,invasion,and EMT of OSCC cells.

Background Under the guidance of achieving carbon peaking and carbon neutrality goals, the demand for lithium-ion batteries has increased significantly. However, during the production, use, and maintenance of lithium-ion batteries, workers are inevitably exposed to various occupational hazards, and some chemicals are nephrotoxic. Objective To evaluate the kidney function and potential determinants among male workers in a lithium-ion battery-related enterprise in Shanghai. Methods The data of occupational health examination carried out by an occupational disease prevention and control institution for workers in a lithium-ion battery-related enterprise in Shanghai were collected. The workers participating pre-employment occupational health examination were treated as a control group, and the other group was recruited from those participating periodic health examination. Serum creatinine, urea nitrogen, uric acid, and renal ultrasound were used to assess the kidney function of workers. Kidney function was classified according to the reference range of kidney function indicators in Diagnostics (9th Edition, national planning textbook for high education in medicine). Binary logistic regression and generalized linear regression were used to identify potential determinants of abnormal values in kidney function indicators in workers. Results There were 6184 workers in the control group (pre-employment) with a mean age of (27.40±4.50) years. There were 3526 workers on the job with a mean age of (29.40±4.99) years and the median time of service was 2.00 (1.00, 3.42) years. The prevalence rates of high serum creatinine, high urea nitrogen, and high uric acid, and abnormal kidney ultrasound among the control group were 0.66%, 2.47%, 30.32%, and 10.12%, respectively; the indicators in the on-the-job workers were 0.96%, 3.35%, 38.25%, and 12.68%, respectively, significantly higher than those in the control group (P<0.05). After adjusting for worker age, length of service, smoking status, drinking status, hypertension, and hyperglycemia, the binary logistic regression models showed that regular smokers had a higher risk of high urea nitrogen than nonsmokers (OR=1.411, 95%CI: 1.011, 1.969). The risk of high uric acid was lower in older workers (OR=0.966, 95%CI: 0.953, 0.979), and higher in workers with more years of service (≤1 year, OR=1.295, 95%CI: 1.093, 1.534; >1-3 years, OR=1.747, 95%CI: 1.494, 2.042; >3 years, OR=1.866, 95%CI: 1.511, 2.304), hypertension (OR=1.400, 95%CI: 1.055, 1.859), and hyperglycemia (OR=1.565, 95%CI: 1.221, 2.006). Workers who were older (OR=1.038, 95%CI: 1.022, 1.054) and had longer working years (>1-3 years, OR=1.518, 95%CI: 1.201, 1.920), occasional smoking habits (OR=1.239, 95%CI: 1.039, 1.478), regular drinking habits (OR=1.875, 95%CI: 1.139, 3.087), and hypertension (OR=1.465, 95%CI: 1.075, 1.998) were at a higher risk of renal ultrasound abnormalities. The generalized linear models showed that length of service (>1-3 years, β=1.120, 95%CI: 0.360, 1.880; >3 years, β=1.451, 95%CI: 0.543, 2.358), smoking status (occasional, β=0.818, 95%CI: 0.156, 1.479; regular, β=0.841, 95%CI: 0.066, 1.616), and hypertension (β=2.742, 95%CI: 1.390, 4.094) were the influencing factors of serum creatinine concentration in the workers. Age (β=0.014, 95%CI: 0.009, 0.019) and length of service (>1-3 years, β=0.079, 95%CI: 0.012, 0.146) were the influencing factors of urea nitrogen. Age (β=−1.759, 95%CI: −2.288, −1.231), length of service (≤1 year, β=10.676, 95%CI: 4.035, 17.316; >1-3 years, β=26.117, 95%CI: 19.962, 32.272; >3 years, β=34.558, 95%CI: 26.116, 43.001), hypertension (β=23.162, 95%CI: 11.617, 34.707), and hyperglycemia (β=15.017, 95%CI: 4.853, 25.180) were the influencing factors of uric acid. Conclusion The prevalence of abnormal kidney function of workers in selected lithium-ion battery-related enterprise is varied by age, length of service, smoking status, drinking status, hypertension, and hyperglycemia. There may be a trend that the longer the time working in a lithium-ion battery-related enterprise, the worse the workers' kidney function. Therefore, the enterprise should pay attention to the possible reasons for their changes and take targeted interventions.
