1.Application of laser microporous decellularized scaffolds in tissue regeneration

BACKGROUND:In the field of tissue engineering,decellularized scaffolds are widely used owing to their biocompatibility.Nonetheless,these scaffolds frequently encounter the limitation of inadequate cell infiltration due to their densely structured composition.Using laser micro-patterning technology for structural modification can improve cell adhesion and proliferation.OBJECTIVE:To summarize the research progress of laser microporous decellularized scaffolds in tissue regeneration.METHODS:Related literature was searched in PubMed and CNKI databases.The Chinese and English search terms were"decellularized scaffold,laser micro-patterning technology,laser microporous decellularized scaffold,extracellular matrix,laser drilling,tissue regeneration."References were screened according to inclusion and exclusion criteria,and 65 articles were finally included for analysis and discussion.RESULTS AND CONCLUSION:(1)Laser micro-patterning technology can selectively remove tissues to achieve scaffold modification.Laser treatment can have controllable effects on the pore structure,mechanical properties and cell permeability of the scaffold.(2)To obtain the optimal cell integration and remodeling effect,factors such as scaffold pretreatment,laser and decellularization process sequence,laser parameter setting,decellularization process and pore structure should be considered.(3)At present,the application of laser microporous decellularized scaffolds has shown that it can promote tissue repair,and it is also necessary to solve the problem of tissue thermal damage.Depending on the absorption of different wavelengths of laser light by the tissue,a reasonable selection of laser type is a feasible solution.(4)Tissue damage repair by laser microporous decellularized scaffold is expected to achieve clinical transformation,and optimizing laser technology and carrying out larger scale biosafety testing are future research directions.

3.Analysis of hepatitis B infection characteristics in HBsAg-/HBV DNA+ blood donors in Taiyuan

Objective: To analyze characteristics of hepatitis B infection in HBsAg-/HBV DNA+ blood donors in Taiyuan, so as to provide evidence for adjusting blood screening strategies. Methods: Blood samples of HBsAg-/HBV DNA+ donors were tested using enzyme-linked immunosorbent assay (ELISA), chemiluminescence assay, nucleic acid qualitative test, and nucleic acid quantitative test. Data on HBsAg-/HBV DNA+ donors in Taiyuan region from January 1, 2016 to December 31, 2024 were statistically analyzed to evaluate the detection rate, demographic characteristics, influencing factors of detection rate, nucleic acid quantitative results, and serological patterns of HBsAg-/HBV DNA+ donors. Results: From January 1, 2016 to December 31, 2024, 991 565 donor samples underwent nucleic acid testing in Taiyuan. A total of 309 HBsAg-/HBV DNA+ samples were detected, resulting in an HBsAg-/HBV DNA+ detection rate of 3.12 per 10 000. The detection rate varied significantly across different years (P<0.05). Males had a significantly higher HBsAg-/HBV DNA+ detection rate than females, first-time donors had a higher rate than repeat donors, and whole blood donors had a higher rate than apheresis donors (P<0.05). The detection rate also differed significantly among age groups (P<0.05). Logistic regression analysis showed that gender, age, donation frequency, and donation type were all influencing factors for HBsAg-/HBV DNA+ detection (all P<0.05). The predominant serological patterns among HBsAg-/HBV DNA+ donors were HBsAb+/HBcAb+ (43.69%, 135/309) or HBcAb+ alone (24.27%, 75/309). Viral load was detectable in 53.40% (165/309) of the HBsAg-/HBV DNA+ donors. Among these, 61.21% (101/165) donors had a viral load<20 IU/mL, and 94.55% (156/165) had a viral load<200 IU/mL. Donors with viral load<200 IU/mL primarily exhibited HBsAb+/HBcAb+ (41.67%, 65/156) or HBcAb+alone (36.54%, 57/156) serological patterns. Conclusion: The prevalence of HBsAg-/HBV DNA+ is low among blood donors in Taiyuan. Higher detection rates were observed in the 46-55 years age group, males, first-time donors, and whole blood donors. HBsAg-/HBV DNA+ donors exhibit specific serological patterns and generally have low viral loads, indicating a potential residual transfusion risk. It is recommended to add HBcAb testing, together with high-sensitivity nucleic acid testing technologies and donor follow-up, to ensure blood safety and guide donor reentry.
4.Incidence and influencing factors of major low anterior resection syndrome within six months after sphincter-preserving surgery for rectal cancer patients

ObjectiveTo explore the incidence of major low anterior resection syndrome (LARS) within six months after sphincter-preserving surgery for rectal cancer and to analyze its associated factors. MethodsClinical records, postoperative rehabilitation data and LARS scores were collected from 889 patients at 3 months postoperatively and from 844 patients at 6 months postoperatively. Patients were divided into a major LARS group and a non-major LARS group. Multivariable Logistic regression was used to analyze factors associated with major LARS at 3 and 6 months postoperatively, and the predictive value of the models was assessed using the Hosmer-Lemeshow goodness-of-fit test and the receiver operating characteristic (ROC). ResultsAmong patients undergoing sphincter-preserving surgery for rectal cancer, 247 patients (27.8%) had major LARS at 3 months postoperatively and 181 patients (21.4%) at 6 months postoperatively. Multivariable Logistic regression showed that Kegel exercises, preoperative radiotherapy, anastomotic leakage, a tumor distance from the anal verge of ≤5 cm, and an anastomotic distance from the anal verge of ≤5 cm were independent factors for major LARS at 3 months postoperatively (P<0.05). At 6 months postoperatively, occasional Kegel exercises during 0~3 months, Kegel exercises during 4~6 months, hydrotherapy during 4~6 months, preoperative chemotherapy, a tumor distance from the anal verge of ≤5 cm, and an anastomotic distance from the anal verge of ≤5 cm were independent factors (P<0.05). The models at 3 and 6 months postoperatively showed goodness-of-fit test P values of 0.986 and 0.517 and area under the curve (AUCs) of 0.843 and 0.870, indicating good predictive value. ConclusionThe incidence of major LARS within six months after sphincter-preserving surgery for rectal cancer is relatively high, and greater attention should be paid to patients who receive preoperative chemoradiotherapy, develop anastomotic leakage, or have tumor and anastomotic distances from the anal verge of ≤5 cm. Patients should also be advised that early postoperative Kegel exercises and hydrotherapy may reduce the incidence of major LARS.
5.Feasibility of deep learning-accelerated Monte Carlo simulation of EPID transit dose images

Objective To develop a deep learning-based denoising model for accelerating Monte Carlo(MC)simulation of electronic portal imaging device(EPID)transit dose images.Methods A total of 500 EPID fields were collected from 100 lung cancer patients undergoing 5-field intensity-modulated radiotherapy,with 400 fields randomly selected as training set,50 fields as validation set,and 50 fields as test set.EPID transit dose image datasets with low particle counts(1×107)and high particle counts(1×109)were simulated using the GPU-accelerated MC dose calculation engine ARCHER.A denoising network model named SUNet was constructed based on Swin Transformer and U-Net,and trained using low-particle-count images as input and high-particle-count images as output.Following training,SUNet model was used to denoise low-particle-count EPID images in the test set.Denoising performance was evaluated using structural similarity index(SSIM),peak signal-to-noise ratio(PSNR),and Gamma passing rates(3%/2 mm),and the computational efficiency of MC simulation combined with SUNet model was analyzed.Results Compared with the original low-particle-count images,the SUNet-denoised images showed significantly improved quality,reduced noise points,and smoother dose distribution.When benchmarked against high-particle-count images,the SUNet-denoised images achieved an average SSIM greater than 0.9,an average PSNR higher than 32 dB,and an average gamma passing rate exceeding 90%.The MC simulation combined with SUNet model required only 1.88 s to simulate a single EPID transit dose image,representing an approximate 40-fold improvement in computational efficiency as compared with high-particle-count MC simulation.Conclusion The deep learning-based denoising model substantially accelerates MC simulation of EPID transit dose images while preserving both image quality and dose accuracy,which provides possibilities for EPID-basedin vivodose verification.

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
7.Study on the improvement of cardiac and renal function in rats with cardiorenal syndrome by inhibiting fibrosis with 6-gingerol

Objective To evaluate whether 6-gingerol(6G)can inhibit fibrosis and improve the cardiac and renal function and in rats with cardiorenal syndrome.Methods In the in vitro experiments of this study,the incorporation of isotope-labeled amino acids was used to detect the intervention of 6-gingerol on normal rat kidney-49F(NRK-49F)and normal rat kidney-52E(NRK-52E)cells.68 male SD rats weighing 200~250 g were used to establish a rat model of cardiorenal syndrome by ligating the left anterior descending coronary artery and performing 5/6 nephrectomy.The rats were randomly divided into a control group,a model group,a low dose 6-gingerol group(6 mg/kg),a high dose 6-gingerol group(30 mg/kg),and a losartan potassium group(20 mg/kg).The 6-gingerol group received intraperitoneal injection of 6-gingerol,while the control group and model group received intraperito-neal injection of an equal amount of physiological saline.The losartan group received oral administration of losartan potassium for a total of 6 weeks.After successful modeling,blood samples were taken for biochemical and cardiac ultrasound examinations.After the experiment,blood,heart,and kidney samples were taken for Masson,immuno-histochemistry,and Western blot.Results 6-gingerol 20 μmol/L can reduce NRK-49F collagen synthesis and inhibit NRK-52E protein synthesis.Biochemical results showed that the serum creatinine,urea nitrogen,and brain natriuretic peptide(BNP)levels of rats in the low and high dose 6-gingerol groups and the losartan group were all reduced,with high dose 6-gingerol groups and losartan group showing the most significant decrease(P<0.05).Echocardiographic parameters showed that the 6-gingerol group and losartan potassium group improved cardiac contractile function and ventricular remodeling in rats(P<0.05).Masson staining and Western Blot showed renal collagen deposition,with reduced expression of collagen I and α-SMA(P<0.05).Immunofluorescence showed a decrease in the expression of renal collagen deposition I,α-SMA,and TGF-β1(P<0.05).Conclusion 6-Gin-gerol may improve the cardiac and renal function and renal fibrosis in rats with cardiorenal syndrome.
8.Different Tendon Reconstruction Techniques for Repairing Medial Meniscus Posterior Root Tears:A Biomechanical Study

Objective Through in vitro experiments,biomechanical data of the transtibial pullout suture(TPS),tendon reconstruction(TR),and tendon reconstruction with suture augmentation(TRS)were collected,so as to evaluate the biomechanical effectiveness of tendon reconstruction for repairing medial meniscus posterior root tear(MMPRT).Methods Eighteen porcine knee joint models were divided into TPS,TR,and TRS groups.Sutures were used to fix the meniscal root in TPS group.Tendons were passed through an incision at the meniscal root in TR group.Tendons were passed through an incision at the meniscal root and secured at tendon-meniscus contact area with additional sutures in TRS group.The sutures and tendons were pulled out through tibial tunnels and fixed at the anteromedial tibia.All groups underwent failure load tests,and ultimate failure load,displacement at failure load,load at clinical failure,stiffness,and failure modes of the samples were recorded.Results The maximum failure load in TPS group was significantly higher than that in TR group(P<0.05),but there was no significant difference between TPS group and TRS group(P>0.05).The maximum failure load in TRS group was significantly higher than that in TR group(P<0.05).The displacement under failure load in TR group and TRS group was significantly lower than that in TPS group(P<0.05),but there was no significant difference between TR group and TRS group(P>0.05).There were no significant differences in the load under clinical failure among the 3 groups(P>0.05).The stiffness of TRS group was significantly greater than that of TPS group(P<0.05),but no significant difference was observed between TR group and TPS group,as well as between TR group and TRS group(P>0.05).All failures were caused by suture or tendon cutting through the meniscus.Conclusions The tendon reconstruction techniques is superior to the TPS in terms of failure displacement and stiffness,while the TRS further enhances the stability of the repair.
9.Epidemic characteristics of human brucellosis and awareness status among occupational populations in Xuzhou City, Jiangsu Province, 2023

Objective:To investigate the epidemic characteristics of human brucellosis and the awareness of brucellosis prevention and control knowledge among occupational populations in Xuzhou City, Jiangsu Province.Methods:The data of brucellosis cases reported from January to December 2023 in the China Disease Prevention and Control Information System's Infectious Disease Surveillance System where the current residence was Xuzhou City, were collected. The epidemiological distribution characteristics across the three dimensions (time, region, and population) were analyzed, and the etiological characteristics of Brucella strains collected from sentinel hospitals in Xuzhou City during the same period were analyzed. Meanwhile, in September 2023, a survey was conducted on the awareness of brucellosis prevention and control knowledge among occupational populations in 7 counties (cities, districts) with a high incidence of brucellosis in Xuzhou City. Binary logistic regression was used to analyze the influencing factors. Results:In 2023, a total of 124 human brucellosis cases were reported in Xuzhou City, with an annual incidence rate of 1.37/100 000. Brucellosis occurred in all months of the year, with a peak incidence from May to August, accounting for 46.77% (58/124). The incidence rate of brucellosis in Feng County, Pei County, Suining County, Xinyi City and Pizhou City (1.86/100 000) was higher than that in Gulou District, Yunlong District, Quanshan District, Jiawang District and Tongshan District (0.71/100 000), with a statistically significant difference (χ 2 = 19.15, P < 0.001). Among 124 cases of brucellosis, 81 cases (65.32%) were males and 43 cases (34.68%) were females. The incidence rate of brucellosis in males (1.78/100 000) was higher than that in females (0.96/100 000), with a statistically significant difference (χ 2 = 11.08, P < 0.001). The 55 - 64 age group reported a relatively higher number of cases (45 cases, 36.29%), with an incidence rate of 3.97/100 000. The main occupation was farmers (85 cases, 68.55%). In 2023, a total of 9 strains of Brucella were collected from the sentinel hospitals in Xuzhou City, all of which were Brucella melitensis, mainly classified as biotype 3 (7/9). In 2023, a total of 984 occupational population with brucellosis were surveyed in Xuzhou City, and the overall awareness rate of brucellosis prevention and control knowledge was 3.25% (32/984). By binary logistic regression analysis, the awareness rates of brucellosis prevention and control knowledge among veterinarians and those with high school or vocational school education, college education or above were relatively high [ OR (95% CI): 21.24 (1.52 - 297.74), 13.32 (3.48 - 50.96), 18.90 (3.68 - 97.13), P < 0.05]. Conclusions:In 2023, the incidence rate of human brucellosis in Xuzhou City is at a relatively high level, with men, middle-aged and elderly people, and farmers as the main population. The awareness rate of brucellosis prevention and control knowledge among occupational population is relatively low, and it is necessary to strengthen publicity, education, and behavioral intervention.
10.Reviews in core competencies of zoonotic diseases professionals under the One Health concept

In the context of globalization,zoonosis prevention and control requires an integrated approach across human,animal,and environmental systems,including interdisciplinary and cross sectional collaboration,which impose heightened requirements for the core competencies among relevant professionals.By a systematic analysis of literature and policy documents,this study explored progress in the application of the One Health concept to the global prevention and control of zoonoses,with a focus on dissecting practi-cal achievements and empirical insights regarding policies promoting the development of core competencies among professionals in this field.By summarizing the existing core competency frameworks and their construction methods,this article also disscussed the guiding roles of existing frameworks,such as the"competencies for one health field epidemiology framework"and the"Ecohealth and One Health network competency framework"on the cultivation of professional talent.This work aimed at providing theoretical support for establishing a One Health-oriented competency framework in China,promoting multidisciplinary collaboration,offering scientific foundations for policy-making and talent cultivation,and contributing to achieving the strategic objectives of"Healthy China 2030"and the Sustainable Development Goals.
