1.Research and innovative exploration of integrated traditional Chinese and Western medicine in preventing and treating gastric " inflammatory cancer transformation"

The development of intestinal-type gastric cancer follows a progression from non-atrophic gastritis — atrophic gastritis — intestinal metaplasia — epithelial dysplasia — gastric cancer,known as the " inflammation-cancer transformation. " Leveraging the advantages of integrated traditional Chinese and Western medicine,while collaborating to inhibit this transformation,remains a research focus and challenge. The incidence of gastric cancer in China has declined with the continuous elucidation of the mechanisms behind inflammation-cancer transformation,innovations in early screening and diagnosis techniques in Western medicine,the establishment of risk stratification and treatment systems,and effective interventions through traditional Chinese medicine. Leveraging the theoretical and practical advantages of the " preventing disease before it occurs" philosophy of traditional Chinese medicine,particularly the concept of " preventing transformation in existing diseases," is essential. This approach emphasizes shifting the focus of prevention and treatment to earlier stages of disease progression. Developing more effective and reliable strategies and innovative drugs that block the dynamic progression of the " inflammation-cancer transformation" in gastric cancer remains a key goal. This article reviews the latest progress in both basic and clinical research in this field,the issues related to high-level clinical research in traditional Chinese medicine,the construction of integrated diagnosis and treatment pathways combining Chinese and Western medicine,the establishment of efficacy evaluation standards,and the elucidation of the integration mechanisms of the complex system of traditional Chinese medicine. It also explores research directions and solutions within the context of multidisciplinary collaboration to provide insights and references to block inflammation-cancer transformation and to construct a gastric cancer prevention and control system with Chinese characteristics,thereby further enhancing the level of gastric cancer prevention and control.
2.Expression and significance of tumor necrosis factor alpha,nuclear factor kappaB and ionized calcium binding adaptor molecule-1 in the hippocampus of mice with aortic dissection

BACKGROUND:Hippocampal injury caused by aortic coarctation has been poorly studied,and combined detection of tumor necrosis factor α,nuclear factor κB and ionized calcium binding adaptor molecule-1 expression in aortic dissection has not been reported.OBJECTIVE:To observe histomorphologic changes in the hippocampus of a mouse model of aortic dissection and investigate the expression and significance of tumor necrosis factor alpha,nuclear factor kappaB and ionized calcium binding adaptor molecule-1 in the hippocampus of aortic dissection mice.METHODS:Sixteen healthy 3-week-old male C57BL/6 mice were randomly divided into two groups:control group and aortic dissection group,with eight mice in each group.In the aortic dissection group,mice were given β-aminopropionitrile monofumarate as drinking water for 4 weeks,and the angiotensin Ⅱ microinfiltration pump was then implanted to establish an animal model of aortic dissection.Mice in the control group were given normal diet and water.After the model was established,the maximum diameter of the ascending aorta was measured,hematoxylin-eosin staining and EVG staining were performed to evaluate the model formation rate,and the levels of inflammatory factors tumor necrosis factor α and interleukin 6 in serum were detected by enzyme-linked immunosorbent assay.The hippocampus was dissected and stained with hematoxylin-eosin to observe the pathological changes of the hippocampus in brain sections.The protein expression of tumor necrosis factor α,nuclear factor κB and ionized calcium binding adaptor molecule-1 was detected by western blot analysis.RESULTS AND CONCLUSION:(1)Compared with the control group,the maximum diameter of the ascending aorta in the aortic dissection group was significantly enlarged.(2)Hematoxylin-eosin staining of the aorta showed obvious thickening of the middle aorta and destruction and disorder of the aortic wall structure in mice.Neurons in the CA1 and CA3 regions of mice were sparsely arranged,reduced in size,and showed pyknosis with deeply stained nuclei.(3)Serum levels of inflammatory factors tumor necrosis factor α and interleukin 6 were increased in the aortic dissection group compared with the control group(P<0.01).(4)The expression levels of tumor necrosis factor α,nuclear factor κB,phosphorylated nuclear factor κB,and ionized calcium binding adaptor molecule-1 in the hippocampus were increased in the aortic dissection group compared with the control group(P<0.05).To conclude,microglial activation and increased expression of tumor necrosis factor α and nuclear factor κB may be involved in hippocampal neuron injury in aortic dissection mice.

4.Value of dual-energy CT quantitative parameters combined with clinical features in diagnosis of stages T2 and T3 colorectal cancer

Objective To investigate the diagnostic value of our regression model based on quantitative parameters of dual-energy CT and clinical features for stages T2 and T3 colorectal cancer.Methods A cross-section study was performed on 91 patients with colorectal cancer confirmed by postoperative pathology in our hospital from January 2022 to November 2023.All of them underwent dual-energy CT examination.According to the pathological T staging criteria of Chinese Colorectal Cancer Diagnosis and Treatment Standard(2020 Edition),they were divided into T2 group(n=43)and T3 group(n=48).Univariate analysis was used to compare the differences in quantitative CT parameters and clinical features between the 2 groups,and the obtained significant variables were employed to construct diagnosis models by univariate or multivariate logistic regression analysis.The area under receiver operating characteristic curve(AUC)of the CT parametric model and the model combined with clinical features was compared to evaluate the efficacy of diagnosing T2 and T3 stages.Results Univariate analysis showed that carcinoembryonic antigen(CEA),N stage,tumor location,tumor longest diameter(LD),CT value of virtual noncontrast(CT-VNC),fat fraction,electron density(Rho)and dual energy index(DEI)were significantly different between the T2 and T3 groups(P<0.05).Multivariate logistic regression analysis found that N stage,tumor location,LD,fat fraction and DEI were independent risk factors for the diagnosis of stage T3.The AUC value of the model of above CT parameters in diagnosing stage T3 colorectal cancer was 0.671(95%CI:0.558~0.783),and the AUC value of the combined model of above CT parameters and clinical features was 0.886(95%CI:0.815~0.957),and statistical difference was observed in the AUC value between the combined model and the CT parametric model(P<0.01).Conclusion The regression model constructed with dual-energy CT quantitative parameters combined with clinical features has high value in the preoperative diagnosis of stages T2 and T3 colorectal cancer before surgery.
5.Analysis of prognostic factors for chondrosarcoma of the larynx

OBJECTIVE To investigate the prognostic factors of chondrosarcoma of the larynx,deeply analyze its clinical data,and provide a theoretical basis for better treatment of chondrosarcoma of the larynx.METHODS A retrospective analysis was conducted on the complete clinical data of patients with primary chondrosarcoma of the larynx admitted to the Department of Otolaryngology Head and Neck Surgery,Beijing Tongren Hospital,Capital Medical University from January 2010 to December 2024.RESULTS A total of 15 patients were included,including 11 males and 4 females,with a gender ratio of 11∶4.The average age of onset was 57.3 years,and the average clinical symptom duration was 12.2 months.The tumors were mainly located in the cricoid cartilage in 11 patients,in the arytenoid cartilage in 2 patients,and in the thyroid cartilage in 2 patients.Tumor grading showed that 7 patients were grade I and 8 were grade II.Four patients underwent transoral laser minimally invasive surgery,2 patients underwent partial laryngectomy+tracheotomy,and 9 patients underwent total laryngectomy/cervical lymph node dissection+tracheostomy.The 5-year overall survival rate was 85.7%,the 5-year disease-specific survival rate was 100%,and the 5-year local-regional control rate was 90.9%.Gender,tumor location,tumor grade,Ki-67,tumor size,and whether larynx preservation surgery was performed did not affect the local-regional control rate or disease-specific survival rate.CONCLUSION Laryngeal chondrosarcoma generally has a longer disease history and is difficult to detect.The pathological type is mostly well-differentiated.Regional or distant metastasis is rare,and the long-term survival rate is good.Surgical resection is the preferred treatment option.On the basis of not reducing the tumor control rate,surgery that prioritizes preserving laryngeal function should be given priority,while comprehensive treatment is generally not recommended.
6.Laccase-like Nanozyme Prepared with Coordination Strategy and Their Analytical Applications

Laccase is a type of polyphenol oxidase that can catalyze the oxidation of various substances,including phenols,aromatic amines,and catecholamines.It has been widely utilized in pollutant degradation and analytical applications.However,the high cost of preparation of natural laccase and its susceptibility to environmental factors,which can lead to denaturation and inactivation,limit its practical applications.Nanozymes,which are nanomaterials that exhibit enzyme-like properties,offer advantages such as easy preparation,adjustable activity,and exceptional stability.Currently,many types of nanozymes have been developed.Inspired by the coordination of Cu2+with amino acids in the active site of natural laccase,researchers have employed coordination synthetic strategies to prepare laccase-like nanozymes.The metal nodes in these laccase-like nanozymes include copper,manganese,and cerium,while the ligands involve a variety of chemicals like nucleotides,amino acids,polypeptides,and aromatic acids.By manipulating factors such as the metal-to-ligand ratio,reducing capacity of ligands,buffer solutions,chloride ions,bromine ions,the catalytic activity of laccase-like nanozymes can be finely tuned.In this paper,laccase-like nanozymes developed through coordination strategies were categorized and summarized,along with review of their analytical applications in detection of phenolic compounds,disease biomarkers,antibiotics,pesticides,sulfur-containing pollutants,and time-temperature indicators.Furthermore,the challenges currently faced in the research of laccase-like nanozymes and future research directions were discussed.
7.Study of Reference Materials for Quantitative Analysis of Gene Copy Numbers of Lentiviral Vectors

Lentiviral vectors(LVs)are key gene delivery tools for integrating target genes into the host genome,but they may also pose risks of insertional mutagenesis.The vector copy number(VCN)in cells is critical for determining the safety of gene modification.However,the reliability and accuracy of its quantification process are influenced by multiple factors.Developing cell reference materials with specific vector copy numbers represents a viable approach to enhance the reliability and consistency of measurement results,enabling quality control of the quantification process and traceability of outcomes.However,the preparation of such reference materials faces challenges in cell sample design,preparation protocols,and advanced quantification techniques.In this study,T lymphocyte cell line Jurkat-based reference materials with LV gene copy numbers of 1 and 2 copy/cell were developed.A high-precision duplex digital polymerase chain reaction(dPCR)method was established to quantify the LV gene and endogenous genes simultaneously.Additionally,the results of dPCR were cross-validated through next-generation sequencing and flow cytometric analysis.Ultimately,confocal microscopy characterization results showed that the developed cell reference materials had intact morphology.The quantification result of VCN-1 was(1.07±0.11)copy/cell,and that of VCN-2 was(2.09±0.21)copy/cell.These cell reference materials demonstrated compliance with stability and homogeneity requirements,and could be applied for quality control throughout the VCN measurement workflow and metrological traceability,improving the accuracy,comparability,and validity of copy number measurements.
8.Study on the application value of sCD14-ST combined with sTREM-1 and blood routine in the diagnosis of bacterial bloodstream infections

Objective To evaluate the application value of soluble leukocyte differentiation antigen 14-sub-type(sCD14-ST),soluble triggering receptor expressed on myeloid cells-1(sTREM-1)and blood routine in the diagnosis of bacterial bloodstream infections,and to provide reference for clinical diagnosis and treatment.Methods A total of 148 patients who received medical treatment and underwent physical examinations at the General Hospital of Central Theater Command and Maternal and Child Health Hospital of Hubei Province from January 2022 to December 2023 were selected as the research subjects.Among them,48 patients with positive blood bacterial cultures were classified as the bloodstream infection group.Fifty patients with negative blood culture but positive bacterial culture results in sputum,urine,stool,purulent secretions and other sam-ples were taken as the local infection group,and 50 healthy individuals who underwent physical examinations were taken as the control group.The levels of serum sCD14-ST and sTREM-1 in each group were detected by enzyme-linked immunosorbent assay.The receiver operating characteristic(ROC)curve was drawn to analyze the efficacy of indicators such as sCD14-ST,sTREM-1 and blood routine in diagnosing bacterial bloodstream infections.Results Compared with the control group,the levels of white blood cells(WBC),neutrophils(N),monocytes,neutrophil/lymphocyte ratio(NLR),monocyte/lymphocyte ratio,platelet/lymphocyte ratio,sTREM-1 and sCD14-ST in the bloodstream infection group and the local infection group were significantly in-creased,while the level of lymphocytes was significantly decreased.The difference was statistically significant(P<0.05).The results of ROC curve analysis showed that the area under the curve(AUC)of WBC,N and NLR in diagnosing bacterial bloodstream infections was>0.6,indicating good diagnostic efficacy for bacterial bloodstream infections.The results of ROC curve analysis showed that the AUC of sCD14-ST in diagnosing bacterial bloodstream infections was 0.748(95%CI:0.664-0.831),and the cut-off value was 0.39 ng/mL.The AUC of sTREM-1 in diagnosing bacterial bloodstream infections was 0.670(95%CI:0.578-0.761),and the cut-off value was 25.18 pg/mL.The AUC of WBC+sCD14-ST,sTREM-1+sCD14-ST,WBC+sTREM-1+sCD14-ST,WBC+N+sTREM-1+sCD14-ST,and WBC+N+NLR+sTREM-1+sCD14-ST were 0.720,0.747,0.756,0.760,0.806 respectively.sCD14-ST was negatively correlated with PLT(r=-0.214,P<0.05).Conclusion WBC,N,NLR,sTREM-1 and sCD14-ST have certain diagnostic values for evaluating bacterial bloodstream infections.
9.Research progress on combined reconstruction of anterior cruciate ligament and anterolateral ligament

Anterior cruciate ligament (ACL) reconstruction yields favorable outcomes for restoring anteroposterior stability of the knee joint in patients with ACL tears. However, some patients still exhibit positive knee pivot shift after ACL reconstruction. Currently, combined ACL and anterolateral ligament(ALL) reconstruction has been accepted by most scholars. This surgical technique can restore the rotational stability of the knee joint in patients postoperatively and facilitate the early besumption of sports training after surgery. This article reviews the anatomical research, isometry-like point research, and reconstruction indications of the anterolateral ligament, as well as the surgical approaches, graft tunnel convergence, and reconstruction outcomes of combined anterior cruciate ligament and anterolateral ligament reconstruction.
10.A brief discussion on TCM diagnosis and treatment of myelodysplastic syndrome based on the Theory of " Sui Qi Suo De"

The theory of " Sui Qi Suo De" originates from Zhang Zhongjing's Jin Gui Yao Lue and has been further developed by later generations of practitioners, offering significant guidance for clinical practice. Myelodysplastic syndromes (MDS) are common malignant disorders of the hematopoietic system, characterized by high heterogeneity and progressive mutational changes. In Traditional Chinese Medicine (TCM), MDS falls under the category of "marrow toxin exhaustion". This article applies the theory of " Sui Qi Suo De" in TCM to analyze the pathophysiological changes during different stages of MDS. Specifically, it explores the precursor stage (focusing on health maintenance and prevention before illness, addressing the " Suo De" of "gradual decline of vital qi"), the low-risk stage (strengthening the spleen and kidneys, clearing toxic pathogens, addressing the " Suo De" of "weakened vital qi invaded by pathogens"), and the medium-to-high-risk stage (detoxifying and reinforcing the body, harmonizing physical and mental health, addressing the " Suo De" of "dominant pathogens and declining vital qi"). The goal is to provide new directions and theoretical insights for the TCM treatment of MDS.
