1.Mortality and years of life lost of residents with viral hepatitis among in Pudong New Area of Shanghai in 2003 - 2023

Objective To analyze the characteristics of viral hepatitis mortality and life loss among residents in Pudong New Area from 2003 to 2023, and to provide a basis for related prevention and control work. Methods Viral hepatitis mortality data were obtained from the Pudong New Area mortality monitoring system. The crude mortality rate (CMR), standardized mortality rate (SMR), potential years of life lost (PYLL), average years of life lost (AYLL), and standardized potential years of life lost (SPYLL) were calculated to analyze viral hepatitis deaths. The average annual change (AAPC) and annual percentage change (APC) of the mortality rate were calculated by Joinpoint regression analysis to analyze the trend of mortality. Results The CMR and SMR of viral hepatitis among residents in Pudong New Area from 2003 to 2023 were 3.89/100000 and 1.98/100000, respectively. Both CMR and SMR of viral hepatitis showed a decreasing trend over time (CMR:APC=-5.476, t=-13.581, P<0.001; SMR:APC=- 7.624, t= -21.253, P<0.001). The CMR for males was 4.75/100000 and the SMR for males was 2.65/100000; the CMR for females was 3.04/100000 and the SMR for females was 1.32/100000, with a higher mortality rate for males than for females(ZCME=12.094,P<0.001; ZSMR=-14.718,P<0.001). Deaths were concentrated in the age groups of 45-64 years old and 65 years old and above, accounting for 91.62% of the total deaths. The PYLL of deaths due to viral hepatitis among residents in Pudong New Area from 2003 to 2023 was 26912 person-years, with a PYLLR of 0.45% and an AYLL of 8.88 years per person. Conclusion The mortality rate of viral hepatitis among the residents of Pudong New Area in 2003-2023 shows a decreasing trend over time. The mortality rate of males is higher than that of females, and the deaths of middle-aged and elderly people account for a large proportion of the total deaths. Chronic hepatitis B is the main cause of death.
2.Comparative analysis of the characteristics of imported malaria cases in Nanning City in 2024 and the same period of the previous year

Objective To investigate the epidemiological characteristics of malaria and provide a basis for developing improved prevention and control measures. Methods Data were obtained from the Chinese Disease Prevention and Control Information System. Malaria surveillance data for Nanning City from January 1,2023, to December 31,2024, were exported from the Infectious Disease Reporting Information Management Subsystem. The characteristics of the two groups of malaria cases were compared. Results A total of 103 imported malaria cases were reported in Nanning City in 2024, representing a 38.32% decrease compared with the same period of the previous year. No statistically significant difference were observed between cases reported in 2023 and 2024 in terms of average age, gender ratio, proportion of parasite species, and monthly reporting distribution;however, statistically significant differences were found in the proportion of reporting areas and current residence areas(χ2= 13.572 and 10.355, respectively; P = 0.001 and 0.035, respectively). The proportion of cases reported in Shanglin County and the proportion of cases residing in Shanglin County were both lower than those during the same period of the previous year. Conclusions The high aggregation of imported malaria cases in Nanning City has decreased. Medical institutions in areas other than Shanglin County should strengthen their vigilance against malaria.
3.Study on the mechanism of Jiawei xiaochaihu decoction inhibiting hippocampal neuronal apoptosis in epileptic mice via regulating AKT/Bcl-xL/Bax/Caspase-3 signaling axis

OBJECTIVE To explore the mechanism of Jiawei xiaochaihu decoction against hippocampal neuronal apoptosis in acute epileptic model mice. METHODS Thirty-six male KM mice were randomly divided into four groups with 9 mice in each group: normal control group (normal saline), epilepsy model group (normal saline), Jiawei xiaochaihu decoction group [7 g/(kg·d)] and carbamazepine group [positive control, 30 mg/(kg·d)]. Except for the normal control group, the acute epilepsy model was established in the other groups. After successful modeling, mice in each group were given corresponding drugs or normal saline by intragastric administration once a day for 2 consecutive weeks. After drug intervention, the ultrastructure and pathological morphology of hippocampal CA1 region were observed. The number of TUNEL-positive cells in hippocampus was detected, and the expression levels of protein kinase B (AKT), Caspase-3, B-cell lymphoma-extra large (Bcl-xL), Bcl-2-associated X protein (Bax), as well as the ratio of phosphorylated AKT(p-AKT) to AKT were determined. RESULTS Compared with the normal control group, severe ultrastructural and pathological damages were observed in the hippocampal CA1 region of epilepsy model group; the number of TUNEL-positive cells was significantly increased ( P <0.05). The expression levels of p-AKT and Bcl-xL were significantly down-regulated ( P <0.05), while the Bax and Caspase-3 were significantly up-regulated ( P <0.05),accompanied by a remarkable decrease of p-AKT/AKT ratio ( P <0.05). Compared with the epilepsy model group, hippocampal tissue ultrastructure, pathological damage, and the aforementioned indicators were significantly improved in Jiawei xiaochaihu decoction group ( P <0.05). CONCLUSIONS Jiawei xiaochaihu decoction can inhibit hippocampal neuronal apoptosis in acute epileptic mice, and its mechanism may be related to activating AKT phosphorylation, increasing p-AKT/AKT ratio, up-regulating Bcl-xL expression and down-regulating the expression of Bax and Caspase-3.

5.Establishment and preliminary evaluation of droplet digital PCR methods for detecting Schistosoma japonicum DNA in host infection

This study established a droplet digital PCR(ddPCR)EvaGreen assay and probe methods for Schistosoma japonicum detection,and evaluated their application in detecting early infections in the S.japonicum host oncomelania and mice.Primers and corresponding probes for both ddPCR methods were designed and synthesized,and plasmids containing target sequences were constructed.The sensitivity of the two methods was tested through detection of the corresponding plasmids,and infectious and mixed oncomelania genomic DNA.Their specificity was evaluated by the detection of genomic DNA of negative oncomelania,Schistosoma mansoni,Clonorchis sinensis,Spirometra mansoni,and S.japonicum(as a positive control).The ddPCR probe method was evaluated by detection of early infection of oncomelania exposed tomiracidium with various ratios and incubation times,and the early migration and distribution of cercaria or schistosomula in mouse hosts infected with 200 cercaria via abdominal skin contact.According to standard curves constructed through the detection of plasmid serial dilutions,the regression equation for the EvaGreen assay was y=-0.839 9x+7.050 9,with a correlation coefficient R2=0.988 1,and the regression equation for the probe method was y=-1.047 5x+7.255 1,with a correlation coefficient R2=0.999 8.The lowest limit of plasmid detection with the probe method was between 38.94 cp/μL and 194.74 cp/μL.Both methods successfully detected positive reactions in the genomic DNA samples of infectious oncomelania at concentrations above 0.002 ng/μL and in the genomic DNA of each group of oncomelania mixtures.No significant differences between probe methods were observed in the detection values in the control group and the genomic DNA of negative oncomelania,S.mansoni,C.sinensis,and S.mansoni.However,the detection value of genomic DNA of negative oncomelania(291 ng/μL)with the EvaGreen assay was(20.3±4.39)cp/μL,a value significantly higher than the(1.5±0.1)cp/μL observed in the control group.For detection of early infection in oncomelania,the probe method detected Schistosoma japonicum DNA after 30 s incubation at room temperature with a≥5∶1 ratio of miracidium to oncomelania;the detection value peaked after a short time(5 min),and the peak value showed a fold increase similar to the increase in the miracidium to oncomelania ratio.Detection of early stage infection in mice with the probe method revealed that the schistosomula entered the lungs on day 2 and the liver on day 4,and continually migrated within the organs with abundant blood supply(spleen,kidney,and brain)in the first 9 days;moreover,a tendency toward ectopic parasitism was observed in the heart and pancreas on day 9,and a constantly negative control level was observed in the testes.The ddPCR probe method was more accurate and specific than the EvaGreen assay in the detection of plasmids,and infectious and mixed oncomelania,and the latter showed non-specific reactions in negative oncomelania detection.In a practical application,the probe method was demonstrated to be sensitive,to effectively reflect the early infection of oncomelania,and to reveal schistosomula migration and distribution in multiple organs of infected mice.

7.Application of Ertapenem in Pediatric Emergency Antimicrobial Therapy

Objective To assess the clinical indications,efficacy,and safety of ertapenem for pediatric emergency infectious disea-ses,and to analyze factors guiding its use to inform rational medication choices.Methods A retrospective cohort study was performed on 538 pediatric patients with bacterial infections treated with ertapenem in the Emergency Department of the Capital Institute of Pediatrics from January to December 2023.Patient demographics,diagnoses,infection types,and treatment data were collected.Univariate and multivariate logistic regression analyses were used to identify risk factors for hospitalization.Results Ertapenem was broadly applied across all pediatric age groups,primarily for pneumonia(25.8%),suppurative tonsillitis(18.4%),and bronchitis(14.7%).Some patients with relatively common infections switched to ertapenem due to severe clinical presentations,poor response to initial antibiotics,or cephalosporin allergy.Key indications included cephalosporin allergy(48.4%),failure of prior antibiotics(37.2%),and moderate to severe infections(12.5%).Overall,80.9%of patients improved clinically;median time to defervescence was 2days,median time for inflammatory marker normalization was 4days,and median total treatment duration was 5days.The incidence of adverse events was 9.5%,mainly mild to moderate gastrointestinal disturbances(5.2%diarrhea,2.2%nausea/vomiting).Multivariate analysis identified pneumo-nia,lymphadenitis,urinary tract infection,and underlying diseases as independent risk factors for hospitalization.Conclusion Ertapen-em demonstrates good efficacy and a favorable safety profile in pediatric emergency infectious diseases,particularly for moderate to severe infections,cephalosporin allergy,and cases unresponsive to initial antibiotic therapy.

9.Serological and gene sequence analysis of rare Rhnull with immune antibody-Rh29

Objective:To investigate serological characteristics of Rhnull with anti-Rh29 and its rare gene sequence.Methods:Microcolumn gel method was used for ABO and RhCcDEe blood typing,accidental antibody identification and cross matching.RHD/RHCE genotype and sequencing analysis were performed by PCR-SSP method.RHAG gene was sequenced by fluorescence PCR method.Antigen or protein associated with Rh complex was detected by flow cytometry.Results:Blood group of the patient was A,Rh pheno-type was typed as group C-c-D-E-e,presented with symptoms of anemia,proportion of globular and oral erythrocytes in blood smear was 6.0%and 0.8%,respectively,RH genotype was CCDee,and immune anti-Rh29 high-frequency antigen were developed.RHAG gene sequencing revealed exons 8,9 and 10 deletion.Conclusion:Mechanism of"regulator"type Rhnull generation in this study is caused by deletion of exons 8,9 and 10 in RHAG gene.
