Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection. Septic shock is the primary cause of mortality in sepsis, with its core pathophysiological mechanism being severe ischemia and hypoxia in critical units—composed of microcirculation and the mitochondria of functional cells—resulting from disruptions in blood flow and oxygen flow following a dysregulated host response. Due to the systemically convergent yet clinically heterogeneous nature of the host response, current understanding and management strategies for hemodynamics remain inconsistent, often leading to inadequate resuscitation or overtreatment. [摘 要] 目的:探究纺锤体蛋白1(SPIN1)促进胃腺癌细胞迁移与侵袭的分子机制。方法: 通过TCGA数据库数据分析胃腺癌组织中SPIN1 mRNA表达与上皮间质转化(EMT)评分、血管生成评分间的相关性。收集2018年8月至2021年11月期间山东第一医科大学附属省立医院手术切除的52例胃腺癌患者的癌组织制成组织芯片,每例均包含胃腺癌组织、对应癌旁组织及淋巴结转移灶,通过免疫组织化学法检测胃腺癌组织中SPIN1与STAT3的蛋白表达水平及相关性。通过Transwell实验研究干扰SPIN1对胃腺癌细胞侵袭与迁移的影响。使用GEPIA2网站分析SPIN1基因与Janus-激酶/信号转导和转录激活因子(JAK/STAT)通路相关因子在胃腺癌中的表达相关性。通过qPCR法、WB法检测干扰SPIN1后JAK/STAT通路相关mRNA和蛋白的表达变化。结果: TCGA数据库数据分析结果显示,SPIN1表达与EMT评分和血管生成评分呈正相关(均P < 0.05)。SPIN1与STAT3在胃腺癌组织和淋巴结转移灶中表达升高(均P < 0.05),在癌旁胃黏膜组织中阴性表达。SPIN1与STAT3的表达显著正相关(P < 0.05)。干扰SPIN1后胃腺癌细胞的迁移、侵袭能力明显降低(P < 0.05或P < 0.01)。GEPIA2网站分析结果显示,SPIN1基因与JAK1、JAK2、STAT1、STAT2及STAT3表达均呈显著正相关(均P < 0.05)。干扰SPIN1后JAK2、STAT3的mRNA水平下降,而JAK1、STAT1、STAT2的mRNA水平变化不明显。WB法实验结果表明,干扰SPIN1后JAK2、STAT3、p-JAK2及p-STAT3的蛋白表达均显著降低(均P < 0.01),过表达SPIN1后JAK2、STAT3、p-JAK2及p-STAT3的蛋白表达均显著升高(均P < 0.01)。结论: SPIN1可通过参与JAK2/STAT3信号通路促进胃腺癌细胞迁移与侵袭。
3.Comorbidity Mechanism Between Ulcerative Colitis and Atrial Fibrillation Based on "Gut Microbiota-gut-heart" Axis

The gut microbiota is regarded as the "eighth organ" of the human body and plays a critical regulatory role in the occurrence and progression of various diseases. Ulcerative colitis (UC) is a chronic inflammatory bowel disease with a complex etiology and a tendency toward recurrent episodes. In recent years, studies have shown that gut microbiota dysbiosis plays a key role in its pathological processes. Meanwhile, an increasing number of studies have demonstrated that imbalances in the gut microbiota and abnormalities in its metabolites are closely associated with the development of atrial fibrillation (AF). Although UC and AF belong to diseases of the digestive system and cardiovascular system, respectively, both exhibit systemic inflammatory characteristics and are often accompanied by gut microbiota dysregulation and abnormal metabolic products. However, systematic investigations into the mechanisms by which gut microbiota-derived metabolites act in these two diseases remain limited. Based on this, the present study adopts literature review and theoretical analysis methods, taking the "gut microbiota-gut-heart" axis as the entry point, to systematically summarize the signaling networks of three key classes of metabolites, i.e., short-chain fatty acids (SCFAs), bile acids (BAs), and trimethylamine N-oxide (TMAO), in the comorbidity mechanism of UC and AF. The findings indicate that these metabolites may activate key inflammatory pathways, such as NF-κB and NLRP3, thereby synergistically mediating intestinal barrier dysfunction and systemic inflammation and constructing a potential comorbidity network. On this basis, potential intervention strategies for the treatment of UC-AF comorbidity, including probiotic intervention and fecal microbiota transplantation, are further discussed. This study aims to provide new theoretical evidence and research perspectives for prevention and treatment strategies of cross-system diseases.
4.Prediction of postoperative pulmonary complications in video-assisted thoracic surgery for lung cancer based on cardiopulmonary exercise testing and machine learning

Objective To develop a predictive model for postoperative pulmonary complications (PPC) following video-assisted thoracic surgery (VATS) in lung cancer patients by integrating cardiopulmonary exercise testing (CPET) parameters and machine learning techniques. Methods A retrospective analysis was conducted on patients with early-stage non-small cell lung cancer who underwent CPET and VATS at Guangdong Provincial People’s Hospital between October 2021 and July 2023. Patients were divided into a PPC group and a non-PPC group. The least absolute shrinkage and selection operator (LASSO) regression was used to select important features associated with PPC. Six machine learning algorithms were utilized to construct prediction models, including logistic regression, support vector machine, k-nearest neighbors, random forest, gradient boosting machine, and extreme gradient boosting. The optimal model was interpreted using SHapley Additive exPlanations (SHAP). Results A total of 325 patients were included, with an average age of 60.36 years, and 55.1% were male. Significant differences were observed between the PPC and non-PPC groups in age, diabetes, coronary heart disease, surgical approach, forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), FVC% predicted, peak oxygen uptake (peak VO2), anaerobic threshold (AT), and ventilatory equivalent for carbon dioxide slope (VE/VCO2 slope) (P<0.05). In the predictive model constructed by selecting 7 key features using LASSO regression, the random forest model demonstrated the best overall performance across various metrics, with an area under the receiver operating curve of 0.930, an F1 score of 0.836, and a Brier score of 0.133 in the training set. It also exhibited good predictive ability and calibration in the test set. SHAP analysis ranked feature importance as follows: peak VO2, VE/VCO2 slope, age, FEV1, smoking history, diabetes, and surgical approach. Conclusion Integrating CPET parameters, the random forest model can effectively identify high-risk patients for PPC and has the potential for clinical application.
5.Hypoxia-induced USP22 affects the malignant biological behavior of esophageal squamous cell carcinoma by regulating HIF-1α

Objective To investigate the effect of ubiquitin specific peptidase 22 (USP22) on the occurrence and development of esophageal squamous cell carcinoma (ESCC) under hypoxic conditions, and its regulatory relationship with hypoxia-inducible factor-1α (HIF-1α). Methods Western blotting and quantitative polymerase chain reaction were used to detect the differences in USP22 protein and mRNA expression between normal esophageal epithelial cells HEEC and ESCC cell lines KYSE30, KYSE150, EC9706, and TE-1 under normoxic (5% CO2, 20% O2, 75% N2) and hypoxic (5% CO2, 1% O2, 94% N2) conditions. By transfecting USP22 plasmid or siUSP22, ESCC cells were divided into a normoxia control group, a normoxia+USP22 group, a normoxia+siUSP22 group, a hypoxia control group, a hypoxia+USP22 group, and a hypoxia+siUSP22 group. The proliferation and migration abilities of cells in each group were detected. The expression of USP22 and HIF-1α under hypoxic conditions after up-regulating or down-regulating USP22 was detected, and their regulatory relationship was verified. The interaction between USP22 and HIF-1α was verified by co-immunoprecipitation (Co-IP) technique. Results Compared with HEEC cells, the expression of USP22 in ESCC cells was significantly increased (P<0.05). Up-regulation of USP22 expression promoted the proliferation and migration of ESCC cells, while silencing USP22 inhibited the proliferation and migration of ESCC cells (P<0.05). Under hypoxic conditions, the expression of USP22 and HIF-1α increased, and with the up-regulation of USP22 expression, the expression of HIF-1α also significantly increased (P<0.05). Co-IP experiment confirmed the binding between USP22 and HIF-1α. Conclusion Up-regulation of USP22 expression promotes the proliferation and migration of ESCC cells. Hypoxia microenvironment can induce the increase of USP22 expression in ESCC. USP22 may participate in the regulation of the occurrence and development of ESCC by directly binding to HIF-1α.

MethodsA convenience sampling method was used to select 291 patients who underwent TKA in Qilu Hospital of Shandong University from May to July, 2023. Pain was assessed using Numerical Rating Scale, kinesiophobia with Chinese short version of the Tampa Scale for Kinesiophobia, social participation with Impact on Participation and Autonomy Questionnaire, and knee function with Hospital for Special Surgery Knee Score. A partial correlation network among pain, kinesiophobia, social participation and knee function was constructed using Graphical Least Absolute Shrinkage and Selection Operator. Key variables were identified through node centrality and bridge centrality analysis. Network Comparison Tests (NCT) were used to analyze network differences among subgroups based on different socio-structural characteristics. ResultsIn the network model, the nodes with the highest strength centrality were indoor participation, activity behavior and activity pain. Bridge centrality analysis indicated that activity pain, knee function, indoor participation and activity cognition were key bridge nodes. NCT revealed no significant differences in overall network structure or global strength among subgroups based on residence, education level or payment method (P > 0.05). However, significant differences in edge weights were found for specific edges such as activity cognition-activity behavior and knee function-indoor participation (P < 0.05). ConclusionThere is a network of interactions among pain, kinesiophobia, social participation and knee function in patients after TKA, with nodes such as indoor participation and activity pain playing key roles in the rehabilitation process. Although the overall rehabilitation network is similar across different socio-structural groups, variations exist in specific relational pathways among patients from rural areas, those with lower education levels, and those with out-of-pocket payment. This suggests that clinical rehabilitation interventions should focus on these core nodes and implement targeted support strategies for socio-structurally disadvantaged groups to promote rehabilitation equity.

8.Application of balloon-occluded retrograde transvenous obliteration in treatment of liver cirrhosis complications

Gastric variceal rupture and bleeding and hepatic encephalopathy are common and life-threatening complications in decompensated cirrhosis. As a minimally invasive interventional technique, balloon-occluded retrograde transvenous obliteration (BRTO) has made significant progress in the clinical management of gastric varices and hepatic encephalopathy in recent years. This article systematically reviews the technical principles, indications (e.g., isolated gastric varices and refractory hepatic encephalopathy), clinical efficacy (an acute hemostasis rate of 85% — 95%, a 1-year rebleeding rate of <15%, and an improvement rate of 60% — 80% for hepatic encephalopathy), and safety (including complications such as renal impairment and elevated portal vein pressure) of BRTO. Meanwhile, this article discusses the advantages and disadvantages of BRTO and conventional treatment modalities (e.g., transjugular intrahepatic portosystemic shunt and endoscopic treatment) and reviews the latest technological improvements in recent years, such as coil-assisted retrograde transvenous obliteration and plug-assisted retrograde transvenous obliteration. Future research should focus on the precision of patient selection (e.g., stratification based on hemodynamic parameters), the optimization of embolic materials (e.g., application of new biodegradable embolic agents), and the development of individualized treatment regimens, so as to improve efficacy and reduce the risk of complications.
9.Clinical comprehensive evaluation of four Chinese patent medicines in the treatment of hyperlipidemia

OBJECTIVE To evaluate the clinical comprehensive value of four Chinese patent medicines (Xuezhikang, Zhibitai, Zhibituo, Jiangzhiling) in the treatment of hyperlipidemia, and provide a reference for rational clinical drug use. METHODS A clinical comprehensive evaluation index system was established in accordance with the Evidence and Value: Impact on Decision-Making (EVIDEM) framework and Technical Guideline for Clinical Comprehensive Evaluation of Cardiovascular Drugs (2022 edition, trial implementation). CNKI, Wanfang data, VIP, PubMed, ScienceDirect, Embase and official websites were retrieved to collect the literature such as drug instructions, guidelines and consensus statements, and systematic reviews/meta-analyses for the four Chinese patent medicines. A comprehensive evaluation was conducted from seven dimensions: effectiveness, safety, economy, suitability, accessibility, innovation and characteristics of traditional Chinese medicine. RESULTS This evaluation index system included 7 first-level indicators, 15 second-level indicators and 30 third-level indicators. Xuezhikang achieved the highest comprehensive evaluation score of 81.4 points, and was classified as class Ⅰ recommendation. Zhibitai with 76.0 points and Zhibituo with 60.9 points were both classified as class Ⅱ recommendation. Jiangzhiling with 48.8 points was classified as class Ⅳ recommendation. CONCLUSIONS Xuezhikang demonstrates the optimal clinical comprehensive value for treating hyperlipidemia. Zhibitai exhibits certain advantages in terms of safety and characteristics of traditional Chinese medicine; Zhibituo shows a moderate performance in all aspects; Jiangzhiling has a relatively low score. Appropriate medicines can be selected clinically according to actual conditions and patients’ characteristics.
