1.Establishment and evaluation of a rat model of phlegm-heat and Fu-organ excess syndrome following ischemic stroke

BACKGROUND:Traditional Chinese medicine has rich experience and unique advantages in the empirical treatment of phlegm-heat and Fu-organs excess syndrome of ischemic stroke.In order to further explore the therapeutic targets and mechanisms of traditional Chinese medicine for this disease,it is crucial to establish a stable and reliable animal model of phlegm-heat and Fu-organs excess syndrome combined with empirical symptoms of ischemic stroke. OBJECTIVE:To explore the establishment method and evaluation system of the rat model of ischemic stroke with phlegm-heat and Fu-organ excess syndrome. METHODS:Sixty male Sprague-Dawley rats were randomly divided into four groups:blank control group(n=12),ischemic stroke group(n=18),disease+syndrome group(n=18),phlegm-heat and Fu-organ excess syndrome group(n=12),all of which were given high-fat diet for 25 days.On the 26th day,the rats in the blank control group and ischemic stroke group were intragastrically given normal saline and high fat diet,while those in the other two groups were intragastrically given autologous feces suspension and high fat diet for 3 continuous days.After gavage,ischemic stroke models were established using the suture method in the ischemic stroke group and disease+syndrome group.The changes in diet,water intake,body mass,body temperature,fecal traits,nasal secretions,sputum in the throat,and tongue image were recorded.Neurological deficits,tongue image,blood lipid levels,morphological changes of brain tissue and carotid artery,and the serum levels of motilin and somatostatin were detected. RESULTS AND CONCLUSION:Compared with the control group,the rats in the disease+syndrome group had shortness of breath,listlessness,irritability,bradykinesia,a large number of secretions around the nose,audible and heavy sputum in the throat,decreased diet and water intake,increased body mass,body temperature,and slingual vein score,decreased fecal pellet count,Bristol score and fecal moisture content,increased serum total cholesterol,triglyceride,low-density lipoprotein and somatostatin levels,decreased motilin level,increased neurological deficit score,significant pathological changes of the carotid artery,and significant morphological changes of the brain tissue.The ischemic stroke group only showed pathological changes of ischemic brain tissue,without the characteristics of phlegm-heat and Fu-organ excess syndrome.The phlegm-heat and Fu-organ excess syndrome group could present with the typical characteristics of traditional Chinese medicine syndromes,without the pathological changes of brain tissue with ischemic stroke.To conclude,the compound modeling method of high-fat induction combined with suture method and autologous feces gavage can establish an animal model of ischemic stroke with phlegm-heat and Fu-organ excess syndrome.
2.Severity Assessment Parameters and Diagnostic Technologies of Obstructive Sleep Apnea

Obstructive sleep apnea (OSA) is an increasingly widespread sleep-breathing disordered disease, and is an independent risk factor for many high-risk chronic diseases such as hypertension, coronary heart disease, stroke, arrhythmias and diabetes, which is potentially fatal. The key to the prevention and treatment of OSA is early diagnosis and treatment, so the assessment and diagnostic technologies of OSA have become a research hotspot. This paper reviews the research progresses of severity assessment parameters and diagnostic technologies of OSA, and discusses their future development trends. In terms of severity assessment parameters of OSA, apnea hypopnea index (AHI), as the gold standard, together with the percentage of duration of apnea hypopnea (AH%), lowest oxygen saturation (LSpO2), heart rate variability (HRV), oxygen desaturation index (ODI) and the emerging biomarkers, constitute a multi-dimensional evaluation system. Specifically, the AHI, which measures the frequency of sleep respiratory events per hour, does not fully reflect the patients’ overall sleep quality or the extent of their daytime functional impairments. To address this limitation, the AH%, which measures the proportion of the entire sleep cycle affected by apneas and hypopneas, deepens our understanding of the impact on sleep quality. The LSpO2 plays a critical role in highlighting the potential severe hypoxic episodes during sleep, while the HRV offers a different perspective by analyzing the fluctuations in heart rate thereby revealing the activity of the autonomic nervous system. The ODI provides a direct and objective measure of patients’ nocturnal oxygenation stability by calculating the number of desaturation events per hour, and the biomarkers offers novel insights into the diagnosis and management of OSA, and fosters the development of more precise and tailored OSA therapeutic strategies. In terms of diagnostic techniques of OSA, the standardized questionnaire and Epworth sleepiness scale (ESS) is a simple and effective method for preliminary screening of OSA, and the polysomnography (PSG) which is based on recording multiple physiological signals stands for gold standard, but it has limitations of complex operations, high costs and inconvenience. As a convenient alternative, the home sleep apnea testing (HSAT) allows patients to monitor their sleep with simplified equipment in the comfort of their own homes, and the cardiopulmonary coupling (CPC) offers a minimal version that simply analyzes the electrocardiogram (ECG) signals. As an emerging diagnostic technology of OSA, machine learning (ML) and artificial intelligence (AI) adeptly pinpoint respiratory incidents and expose delicate physiological changes, thus casting new light on the diagnostic approach to OSA. In addition, imaging examination utilizes detailed visual representations of the airway’s structure and assists in recognizing structural abnormalities that may result in obstructed airways, while sound monitoring technology records and analyzes snoring and breathing sounds to detect the condition subtly, and thus further expands our medical diagnostic toolkit. As for the future development directions, it can be predicted that interdisciplinary integrated researches, the construction of personalized diagnosis and treatment models, and the popularization of high-tech in clinical applications will become the development trends in the field of OSA evaluation and diagnosis.
3.Effects of climate change on Lyme disease ecosystems

Lyme disease is a newly emerging zoonotic infectious disease that poses a major threat to both human and animal health.Among the many factors influencing the distribution and prevalence of Lyme disease,climate change can alter pathogens and influence the richness and abundance of host animals and vectors.These changes create appropriate conditions for disease outbreaks and epidem-ics,thereby increasing risks to human health.Herein,we review research on the effects of climate change on Lyme disease ecosys-tems,to provide a reference for predicting the occurrence and prevalence of Lyme disease.

High mobility group box protein 1(HMGB1)is one of the most widely expressed protein member in the HMGs family,which is well known for its involvement in the body inflammatory response.Previous researches have found that it plays a significant role in cell migration,immune identification and neuroprotection.Spinal cord injury is a disease that causes severe damage to the nervous system,and neural circuits are disrupted after a spinal cord injury,which leads to many conditions including ischemia and hypoxia,inflammatory responses,demyelinating lesions,and glial scar formation that are detrimental to nerve regeneration and repair,making it one of the most difficult diseases to treat in the modern spinal surgery field.HMGB1 is upregulated after spinal cord injury,thereby regulating neuroinflam-matory responses,and participating in the neuronal apoptosis,promoting neuronal regeneration,and inducing neural stem cell differentiation and migration,which plays an important role in the process of neural function recovery.This paper summarizes the structure and function of HMGB1,as well as its role in spinal cord injury,in order to provide direction for founding therapeutic target for neurological function recovery after spinal cord injury.
5.Promotion status of professional titles of healthcare-associated infection management full-time professionals in Anhui Province

Objective To analyze the current promotion status of professional titles of healthcare-associated infec-tion(HAI)management full-time professionals in medical institutions in Anhui Province.Methods A cross-sec-tional survey was designed.An electronic questionnaire survey was conducted among HAI management full-time professionals from all levels of medical institutions in 16 cities in Anhui Province by Anhui Province HAI Manage-ment Quality Control Center,and the data were analyzed.Results A total of 815 HAI management full-time pro-fessionals participated in the survey.78.16%of the professionals were with nursing background;25.89%had mas-ter's degree and higher;55.95%held an intermediate-level professional titles;49.45%engaged in HAI manage-ment for less than 5 years;most professionals with advanced title were from tertiary medical institutions.There were significant differences in the pursuing route of professional title promotion among HAI management full-time professionals with different professional backgrounds:91.94%of clinical medical professionals chose the medical promotion route;96.55%of nursing professionals chose the nursing promotion route;75.41%of public health pro-fessionals chose the medical promotion route;84.61%of laboratory professionals and 50.00%of pharmacy profe-ssionals chose the medical technician promotion route.43.31%of HAI management full-time professionals were generally satisfied with the current status of professional title promotion,only 5.65%being very satisfied.The main cause why HAI management full-time professionals were dissatisfied with professional title promotion was"current professional title promotion evaluation standards",accounting for 45.60%.There was a statistically signi-ficant difference in the evaluation on the difficulty in promoting to the next level of professional title among full-time professionals with different professional titles(P<0.001).The higher the professional title,the more difficult it was to be promoted to the next level.48.22%of HAI management full-time professionals thought the difficulties in pro-moting their professional titles was due to"the lack of professional and technical titles in HAI management",fo-llowed by"low promotion rates"(10.43%)and"weak competitiveness in promotion based on their original posi-tions"(9.69%).Conclusion Professional title promotion route for HAI management full-time professionals in An-hui Province depends on their original professional fields,and promotion is generally difficult,especially to senior professional titles.It is urgent to establish an independent technical professional title series for HAI management as well as a matched evaluation and appointment mechanism to stabilize the talent team.
6.Practice exploration of cultural integration driven by cultural and creative work in multi-campus hos-pitals from the perspective of cultural confidence

To address the challenge of cultural integration in multi-campus public hospitals,the Cancer Center of Sun Yat-Sen University has embarked on a practical exploration using cultural and creative work as a vehicle,guided by cultural confi-dence.The center has created a cultural and creative matrix for its 60th anniversary,systematically innovating the cultural and creative work mechanism,vertically activating historical and cultural resources,and linking both inside and outside the hospital campuses.Additionally,a"full-cycle"cultural and creative system has been designed for the Huangpu campus,covering multi-ple scenarios and horizontally strengthening cross-campus identity.Following the principle of"appreciating the beauty of diversity while cherishing our own,"the Cancer Center of Sun Yat-Sen University aims to transform cultural soft power into a driving force for high-quality development and enhance cultural confidence through party building leadership.The"Cancer Center of Sun Yat-Sen University approach"of promoting cultural integration in multi-campus hospitals through cultural and creative work provides an empirical example for transforming cultural soft power into a driving force for high-quality development and enhancing cultural confidence in multi-campus public hospitals.The key lessons lie in deeply tapping into historical and cultural resources,emphasi-zing identity,building a collaborative innovation mechanism,and realizing the organic integration of cultural integration and high-quality hospital development.

Hypothermia is a clinical syndrome characterized by core body temperature<35℃,caused by significant heat loss from body surface in cold environment.As a systemic cold injury,it can be lethal if treatment is delayed.Emergent diagnosis and treatment of hypothermia are expected to improve the prognosis of patients.In 2005,the U.S.Army Research Institute of Environmental Medicine(USARIEM)issued guidelines for the prevention and management of cold injuries,but there has been no corresponding standard in China.Therefore,Emergency Branch of Chinese Medical Rescue Association,Emergency Medical Equipment Society of China Association of Medical Equipment,Integrated Rehabilitation Medical Branch of Chinese Medical Rescue Association,and Pre-Hospital Emergency Care Working Committee of Chinese Aging Well Association jointly developed the Chinese Expert Consensus on Emergent Treatment of Hypothermia(2025 edition).The consensus covers the pathophysiology,etiology and epidemiology,diagnosis and severity grading,prehospital treatment,and in-hospital treatment of hypothermia,including 15 recommendations in total,aiming to provide guidance for the relevant clinical rescue work.
8.Impact of KRAS,NRAS,and BRAF gene mutations on the efficacy of neoadjuvant therapy and postoperative hepatic metastasis occurrence in patients with stage Ⅱ-Ⅲ mid-low rectal cancer

Objective:To investigate the effect of KRAS,NRAS and BRAF gene mutations on the efficacy of pre-operative short-course radiotherapy combined with chemotherapy and postoperative liver metastasis in patients with stage Ⅱ~Ⅲ mid-low rectal cancer.Methods:The clinical data of 149 patients with stage Ⅱ~Ⅲ low rectal cancer admitted to Dongnan Hospital of Xiamen University from January 2017 to June 2020 were retrospectively analyzed.All patients received neoadjuvant therapy with preoperative short-course radiotherapy combined with chemotherapy and radical surgery,and were followed up until June 30,2023.The mutations of KRAS,NRAS and BRAF genes were de-tected by pathological tissue before surgery.The effect of neoadjuvant therapy was evaluated according to tumor re-gression grade(TRG).The risk factors of postoperative liver metastasis were analyzed by Logistic multivariate analysis.Results:There were 44 cases of KRAS,10 cases of NRAS and 12 cases of BRAF gene mutation in 149 patients with stage Ⅱ~Ⅲ low rectal cancer,and the mutation rates were 29.53%,6.71%and 8.05%.The incidence of positive vas-cular invasion in patients with KRAS gene mutation was higher than negative(P<0.05).In NRAS mutation patients,the incidence of positive lymph node metastasis was higher than negative(P<0.05),and the incidence of maximum tumor diameter≥5 cm was higher than that of maximum tumor diameter<5 cm(P<0.05).The clinical stage of BRAF muta-tion was higher in stage Ⅲ than in stage Ⅱ(P<0.05),and the incidence of positive lymph node metastasis was higher than negative(P<0.05).During the follow-up period,liver metastasis occurred in 42 patients,and the liver metastasis rate was 28.18%.The KRAS,NRAS and BRAF gene mutations in the effective group were 25.81%,2.15%and 3.23%,lower than those in the ineffective group(35.71%,14.29%and 17.07%,P<0.05).Multiple factors found clinical stage Ⅲ(OR=10.620,95%CI:2.645~22.575),lymph node metastasis was positive(OR=8.774,95%CI:1.878~19.645),neoadju-vant therapy failed(OR=3.373,95%CI:1.014~11.218),KRAS gene mutation(OR=6.245,95%CI:1.876~20.789),BRAF gene mutation(OR=9.497,95%CI:1.754~19.335)were independent risk factors for postoperative liver metastasis of stage Ⅱ~Ⅲ mid-low rectal cancer.Conclusion:Mutations in the KRAS and BRAF genes of middle are associated with poorer efficacy of neoadjuvant therapy in patients with stage Ⅱ~Ⅲ mid-low rectal cancer and are also indepen-dent risk factors for postoperative liver metastasis.

The scientific research and innovation capabilities of medical students are intrinsically linked to the sustained and high-quality development of national healthcare initiatives.Cultivating outstanding medi-cal students with independent scientific capabilities and innovative consciousness is a critical component in the education and training of high-level medical professionals.Our investigation revealed that within the imperfections of the cultivating model,some faculty and students at medical schools have an insufficient understanding of scientific research and innovation and lack motivation for engaging in such activities,which hinder the progression of scientific research activities.Consequently,we initiated a teaching practice and exploratory study on the"tutorial system"aimed at fostering medical students'scientific research and innovation abilities.Based on the principle of"research informing teaching,teaching and research advan-cing together,"this study implements a"tutorial system"coordinated by tutors,supplemented by graduate and undergraduate student mentors,to cultivate innovative thinking,stimulate interest in scientific re-search,and enhance practical and research skills among medical students.Through collaborative efforts within"scientific research innovation teams,"various educational methods—including preliminary re-search,in-class and extracurricular activities,intra-group and inter-group interactions,and theoretical and practical applications—are employed to improve and strengthen the cultivation of medical students'scientif-ic research and innovation abilities.This study aims to provide valuable references for optimizing medical education management systems and enhancing the quality of medical student training.
10.Innovative exploration of multi-campus cultural integration through healing space development

Under the multi-campus development model of public hospitals,cultural integration and innovation face nu-merous challenges due to spatial constraints,personnel structure differences,and functional positioning across campuses.System-atic cultural development is required to bridge these gaps and enhance the sense of belonging among both patients and staff in new campuses.This case study focuses on the Huangpu Campus of Sun Yat-sen University Cancer Center,launched in 2021,explo-ring a dual-dimensional approach to multi-campus cultural integration through"healing spaces."The practical strategies include:①Direct Space Construction:Establishing patient-centered healing environments through service hubs to optimize clinical experi-ences and humanistic care;(②Inidirect Environment Development:Leveraging staff service centers to foster the integration of the hospital's"collaborative,fulfilling,and striving culture,"thereby strengthening employee engagement.Post-implementation,significant improvements were observed in both patient and staff satisfaction metrics.The initiative has yielded a replicable cultur-al management framework for multi-campus public hospitals,offering valuable insights for cultural innovation in similar healthcare settings.
