1.A Case of Multidisciplinary Treatment for Inflammatory Myofibroblastic Tumor Complicated by ANCA-Associated Vasculitis

A 51-year-old male presented with nasal obstruction, followed by progressive hearing loss and blurred vision. Imaging identified space-occupying lesions in the paranasal sinuses, orbits, and paraspinal regions, while laboratory tests confirmed positive anti-proteinase 3 anti-neutrophil cytoplasmic antibody(PR3- ANCA) immunoglobulin G (IgG)and markedly elevated serum IgG4. Despite treatment with corticosteroids, immunosuppressants, and radiotherapy, the patient exhibited steroid dependency with relentless disease progression. Following multidisciplinary consultation, a diagnosis of inflammatory myofibroblastic tumor (IMT) coexisting with ANCA- associated vasculitis (AAV) was favored, though IgG4-related disease remained a critical differential. Ultimately, profound immunosuppression precipitated a severe herpesvirus infection, leading to disseminated intravascular coagulation and multiple organ dysfunction syndrome. This case underscores the rarity and diagnostic complexity of concurrent IMT and AAV, highlights the therapeutic dilemma of balancing primary disease control against fatal opportunistic infections, and emphasizes the critical role of multidisciplinary collaboration in the diagnosis and treatment of complex diseases.
2.Study on the safety and efficacy of micro-perfusion device for preserving isolated porcine limbs

Objective To evaluate the safety and efficacy of a self-developed micro-normothermic machine perfusion (NMP) system (micro-perfusion device) for preserving isolated porcine limbs. Methods Five healthy Landrace pigs were selected, and their left and right forelimbs were randomly divided into the NMP group and static cold storage (SCS) group. The NMP group was perfused with the self-developed micro-perfusion device and polymerized hemoglobin perfusate for 32 hours at normothermia, while the SCS group was preserved at 4 ℃. Hemodynamic parameters such as perfusion pressure and flow were monitored. The pH value, partial pressure of oxygen (PO2), lactic acid (Lac), creatine kinase (CK) and lactate dehydrogenase (LDH) in the perfusate were measured. Hematoxylin-eosin staining was used to assess the muscle tissue structure, terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling was employed to evaluate muscle cell apoptosis, and immunohistochemistry staining was applied to detect the expressions of tumor necrosis factor (TNF)-α and interleukin (IL)-6. A mixed-effects model was used to analyze the effects of time and treatment methods on tissue structure, cell apoptosis and inflammatory factors. Results The device could stably maintain a perfusion pressure of (69±15) mmHg and a flow rate of (117±42) mL/min. The pH value and electrolytes of the perfusate were generally stable, with PO2 maintained at a high level. Lac was maintained at 5.38(3.81, 6.45) mmol/L, while CK and LDH increased over time. After 32 hours of perfusion in the NMP group, both the myocyte spacing and apoptosis rate were better than those in the SCS group. Mixed-effects model analysis showed that there were statistically significant differences in the effects of NMP treatment and SCS treatment on myocyte spacing and apoptosis rate per unit time (both P < 0.05). There were no statistically significant differences in TNF-α and IL-6 between the two groups, and mixed-effects model analysis showed no statistically significant differences in the effects of NMP treatment and SCS treatment on TNF-α and IL-6 per unit time (both P > 0.05). Conclusions The micro-perfusion device used in this study may achieve 32-hour normothermic preservation in a porcine limb amputation model, maintain basic metabolism and ionic homeostasis, reduce muscle structural damage and cell apoptosis without inducing additional inflammatory responses. This technology is expected to significantly extend the time window for replantation of amputated limbs in disaster rescue and long-distance transportation, providing an important technical basis for clinical translation and subsequent replantation research.

BACKGROUND:H-type vessels(CD31hiEmcnhi)possess unique functionalities that offer new perspectives and entry points for comprehensively understanding the mechanisms of vascular-mediated bone metabolism regulation.This has triggered a significant paradigm shift in bone science research from a"bone-centric"approach to an"integrative bone-vascular"framework.OBJECTIVE:To perform a visual analysis based on literature data in the field of H-type vessels,aiming to identify research hotspots and emerging trends.METHODS:A systematic search was conducted in the CNKI and Web of Science databases for English and Chinese publications related to H-type vessels,covering the period from 2014 to 2024.The CiteSpace 6.2.R4 information visualization software was utilized to analyze and visualize data on publication countries,authors,institutions,keywords,and timeline views.RESULTS AND CONCLUSION:(1)A total of 59 Chinese and 185 English publications were included in the analysis.Since the introduction of the H-type vessel concept in 2014,the number of related research publications has been steadily increasing,accompanied by a significant growth in academic output.China leads the research in this domain,with major institutions including Southern Medical University,Sichuan University,and Shanghai Jiao Tong University.Keyword analysis indicated that current research hotspots primarily focus on the generation and regulatory mechanisms of H-type vessels,their roles in bone formation and remodeling,and their associations with bone metabolic diseases.Additionally,emerging keywords such as"induced membrane,""cartilage repair,"and"traditional Chinese medicine"suggest that research is progressively moving towards diversification and multidisciplinary integration.(2)H-type vessels play a crucial role in the development and progression of bone diseases,and relevant studies are pivotal for a deeper understanding of the physiological and pathological processes of bone tissue.Future research should further elucidate the specific mechanisms of H-type vessels in various disease contexts and promote the clinical translation of basic research findings.These efforts will provide innovative ideas and strategies for the prevention and treatment of bone-related diseases.

5.Evaluation of the quality of Jingangteng capsules based on UPLC fingerprinting combined with multi-component content determination

OBJECTIVE To establish the UPLC fingerprint and the method for multi-component content determination in Jingangteng capsules, and to evaluate its quality by combining chemical pattern recognition analysis. METHODS An UPLC method was established. Separation was performed on a Zorbax SB-C 18 Rapid Resolution HD column, with acetonitrile-0.1% formic acid as the mobile phase for gradient elution.Using the Similarity Evaluation System for Chromatographic Fingerprints of Traditional Chinese Medicines (2012 edition), UPLC fi ngerprints were established for 10 batches of Jingangteng capsules, and similarity was evaluated. SPSS 22.0 and SIMCA 14.1 software were used to perform hierarchial-cluster analysis and orthogonal partial least squares discriminant analysis (OPLS-DA), respectively. The same UPLC method was employed to determine the contents of chlorogenic acid, 3,5-dihydroxy-2-methylbenzoic acid-3- O -glucoside (M1), caffeic acid, astilbin, oxyresveratrol, quercitrin and resveratrol in the 10 batches of samples. RESULTS A total of 17 common peaks were identified in UPLC fingerprints of the 10 batches of samples, of which 7 were identified as chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol. The similarities of 10 batches of samples ranged from 0.820 to 0.985. The results of hierarchial-cluster analysis showed that 10 batches of samples were grouped into four categories: S1-S4 formed one group, S5 and S6 formed another, S7, S8 and S10 formed a third, and S9 formed a fourth, consistent with the OPLS-DA results; the variable importance projection values for peaks 7, 10, 2, 16 (resveratrol), 13 (oxyresveratrol), 11, 6 (caffeic acid), 5 (M1) and 15 (quercitrin) were >1. Quantitative analysis results showed that the contents of chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol were 1.650 8-4.213 7, 0.636 2-2.161 7, 0.031 0-0.086 5, 0.239 1-1.069 3, 0.211 9-1.104 0, 0.488 8-2.399 2, and 0.164 0-0.699 8 mg/g, respectively. CONCLUSIONS UPLC fingerprint and content determination methods established in this study are simple to operate, accurate, reliable and reproducible; when combined with chemical pattern recognition analysis, they can be used to evaluate the quality of Jingangteng capsules. Nine components, such as resveratrol, oxyresveratrol, caffeic acid, M1 and quercitrin, may serve as markers of quality variation.
6.Echocardiographic features of critically ill patients with concurrent infections and their predictive value for 28-day mortality and cardiac injury

Objective To explore the echocardiographic features of critically ill patients with infection, and to evaluate the predictive value of echocardiographic parameters for 28-day mortality and myocardial injury. Methods Using a single-center prospective cohort design, 120 critically ill patients with infection admitted to the Intensive Care Unit, Jinshan Hospital of Fudan University from January 2024 to January 2025 were enrolled consecutively. According to the severity of infection, patients were divided into the sepsis group (n=75) and the non-sepsis group (n=45). Cox proportional hazards models and modified Poisson regression were used to analyze risk factors of 28-day all-cause mortality and myocardial injury. Kaplan-Meier survival curve and log-rank test were used to assess prognosis differences among patients with different ultrasound parameters. ROC curve and area under the curve (AUC) were applied to evaluate the predictive performance of ultrasound parameters, and restricted cubic spline (RCS) model was used to explore nonlinear relationships. Results The E/e′ ratio was significantly higher in the sepsis group than in the non-sepsis group (P<0.001), while left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) were lower in the sepsis group (P<0.05). The Cox proportional hazards model found no association between ultrasound parameters and 28-day all-cause mortality. Modified Poisson regression showed that, after adjusting for confounding factors, elevated E/e′ was an independent risk factor of myocardial injury (RR=1.12, 95% CI 1.07–1.17, P<0.001). Survival analysis demonstrated that patients with E/e′ >10.9 had lower 28-day survival rates (P=0.02), and the AUC for E/e′ predicting myocardial injury was 0.835. RCS analysis suggested a nonlinear relationship between E/e′ and myocardial injury risk (P<0.001). Conclusions E/e′, LVEF, and LVFS have no significant predictive value for 28-day mortality risk in critically ill patients with infections. Elevated E/e′ is an independent risk factor of myocardial injury in such patients, with a nonlinear relationship.
7.Clinical Efficacy and Mechanism of Banxia Xiexin Tang-related Prescriptions in Treating Inflammation-cancer Transformation of Digestive System Based on Theory of Treating Different Diseases with Same Method: A Review

Digestive system tumors are the leading cause of cancer incidence and mortality globally, and their occurrence and development generally follow the key pathological process of inflammation-cancer transformation. Therefore, intervening in the precancerous lesion stage is an important strategy to block malignant transformation of the disease and reduce the incidence rate of tumors. Banxia Xiexin Tang-related prescriptions (including Banxia Xiexin Tang, Shengjiang Xiexin Tang, and Gancao Xiexin Tang) originated from the Shanghan Zabinglun They follow the principle of pungent dispersing and bitter descending, combination of cold and warm medicinals, and tonifying deficiency and purging excess, which aligns with the core pathogenesis of digestive system precancerous lesions characterized by cold-heat intermingling and disorder of ascending and descending. These prescriptions are widely used in the clinical treatment of various digestive system inflammatory disorders such as atrophic gastritis, ulcerative colitis, and reflux esophagitis. This review systematically summarizes the theoretical basis, clinical evidence, and therapeutic mechanisms of such prescriptions in preventing and treating the inflammation-cancer transformation process in the digestive system. Clinical studies have shown that whether used alone or in combination with modern therapies, Banxia Xiexin Tang can effectively alleviate symptoms, repair histopathological changes, and improve patients' quality of life. Basic research further reveals that their efficacy stems from multi-target systemic regulatory effects: ameliorating the chronic inflammatory microenvironment, antagonizing oxidative stress, reshaping the gut microbiota, restoring the immune balance, regulating cell proliferation and apoptosis, etc. On the basis of the convergence of traditional Chinese and Western medicine understanding of inflammation-cancer transformation, this article constructs a research framework centered on regulating the inflammatory microenvironment homeostasis to systematically elucidate the mechanism of treating different diseases with the same method (Banxia Xiexin Tang). In view of the limitations of current research in terms of evidence level, disease-syndrome combination models, and overall mechanism analysis of compound prescriptions, this article proposes future research directions of integrating high-quality clinical research, systems biology, and cutting-edge technologies, providing a new theoretical basis and translational ideas for the precise prevention and control of digestive system tumors with traditional Chinese medicine.
8.Study on antidepressant effects and mechanisms of total triterpenes from Poriae Cutis

OBJECTIVE To investigate the antidepressant effect and underlying mechanism of total triterpenes from Poriae Cutis (PCTT). METHODS UPLC-Q-TOF-MS technology was adopted to analyze the chemical constituents of PCTT. Network pharmacology was applied to screen the antidepressant active ingredients, key targets and signaling pathways of PCTT, and molecular docking was performed to verify the binding capacity between potential active ingredients and core targets. In animal experiment, mice were intragastrically administered PCTT at doses of 80, 160 and 320 mg/kg for 14 consecutive days. Lipopolysaccharide was used to establish an acute depression mouse model. Subsequently, behavioral assays were conducted. The levels of serum inflammatory factors, hippocampal neurotransmitters and brain-derived neurotrophic factor (BDNF) were detected. Western blot and qPCR assays were adopted to validate the antidepressant mechanism of PCTT. RESULTS A total of 101 chemical constituents were identified in PCTT. Network pharmacology screened out 10 potential active ingredients (including poriacosone B, poriacosone A, pachymic acid, etc.), six core targets [including tumor necrosis factor (TNF), peroxisome proliferator-activated receptor, caspase-3, etc.], as well as multiple signaling pathways including TNF and interleukin-17 (IL-17). All 10 potential active ingredients exhibited strong binding affinity to the six core targets. Animal experimental results demonstrated that 320 mg/kg PCTT markedly increased the movement distance and movement speed of depression model mice (P<0.05), and shortened immobility time in tail suspension test and forced swimming test (P<0.05). PCTT reduced serum levels of IL-6, IL-1β and TNF-α, decreased hippocampal glutamate level; it also downregulated the relative mRNA expression of TNF-α, tumor necrosis factor receptor-2 (TNFR2), phosphoinositide 3-kinase (PI3K) and protein kinase B (AKT), as well as TNFR2 protein expression and the ratios of phosphorylated-PI3K/PI3K,phosphorylated-AKT/AKT in the prefrontal cortex. Meanwhile, PCTT elevated the levels of 5- hydroxytryptamine, dopamine and BDNF in the hippocampus. CONCLUSIONS PCTT exerts antidepressant effects possibly by regulating the TNF-α/PI3K/AKT signaling pathway to downregulate the expression of pro-inflammatory factors, thereby modulating the release of neurotransmitters and BDNF.
9.Clinical observation of sacituzumab tirumotecan combined with bevacizumab in the treatment of HER2-negative breast cancer with brain metastases

OBJECTIVE To evaluate the efficacy and safety of sacituzumab tirumotecan combined with bevacizumab for the treatment of human epidermal growth factor receptor 2-negative (HER2-) breast cancer with brain metastases.METHODS Clinical data of 90 patients with HER2- breast cancer with brain metastases treated at the First Affiliated Hospital of Nanyang Medical College from December 2024 to November 2025 were retrospectively collected. According to the treatment regimens, the patients were divided into a control group ( n =55) and an experimental group ( n =35). Patients in the experimental group received sacituzumab tirumotecan combined with bevacizumab, whereas those in the control group received conventional systemic therapy, including albumin-bound paclitaxel-based regimens, capecitabine-based regimens, or gemcitabine/vinorelbine combined with platinum-based regimens. Short-term efficacy, progression-free survival (PFS), brain metastasis-related efficacy, adverse events, treatment discontinuation due to adverse events, and treatment-related deaths were compared between the two groups.RESULTS The objective response rate (ORR), disease control rate, median PFS, intracranial disease control rate, and proportion of patients with improvement in neurological symptoms in the experimental group were significantly higher than in the control group ( P <0.05). Stratified analysis showed that PFS of the experimental group was significantly longer than the control group among patients with Breast-Graded Prognostic Assessment scores ≥2.0 and <2.0 ( P <0.05). There were no statistically significant differences in the proportion of patients with glucocorticoid dose reduction/discontinuation, the incidence of adverse events of different grades, or the rate of treatment discontinuation due to adverse events between the two groups ( P >0.05). No treatment-related deaths occurred in either group.CONCLUSIONS Compared with conventional systemic therapy, sacituzumab tirumotecan combined with bevacizumab can improve the ORR and prolong PFS in patients with HER2- breast cancer with brain metastases, with an overall manageable safety profile.
10.Epidemiological characteristics of respiratory adenovirus infections among hospitalized patients under 18 years of age with pneumonia in Yancheng City of Jiangsu Province from 2023 to 2024

ObjectiveTo investigate the epidemiological characteristics of respiratory adenovirus (AdV) infection among hospitalized patients under 18 years of age with pneumonia in Yancheng City of Jiangsu Province from 2023 to 2024, and to provide a basis for the prevention and control of adenovirus infection. MethodsNasopharyngeal swab samples were collected from hospitalized patients under 18 years of age diagnosed with pneumonia at two national influenza sentinel hospitals in Yancheng City from February 2023 to June 2024. A respiratory pathogen nucleic acid detection kit was used to test for 28 pathogens, including influenza A virus, influenza B virus, respiratory syncytial virus, human rhinovirus, adenovirus, Mycoplasma pneumoniae, Chlamydia pneumoniae, human metapneumovirus, human bocavirus, enterovirus, polyomavirus K1, polyomavirus WU, parainfluenza viruses 1‒4, coronavirus 229E, HKU1, OC43, and NL63, Haemophilus influenzae, Bordetella pertussis, Group A Streptococcus, Staphylococcus aureus, Streptococcus pneumoniae, Legionella pneumophila, Klebsiella pneumoniae, and the novel coronavirus, and to analyze patients’ mixed infections. Adenovirus-positive samples were subjected to polymerase chain reaction (PCR) amplification using Hexon and Fibre as target genes, and the products were analyzed by sequencing. Whole-genome sequencing analyses were performed on positive samples with Ct values less than 22. ResultsAmong 368 hospitalized pneumonia patients under 18 years of age in Yancheng City from February 2023 to July 2024, most were male (51.63%, 190/368), with preschool-aged children (42.66%, 157/368) and primary school-aged children (40.22%, 148/368) comprising the majority. A total of 44 cases tested positive for adenovirus, yielding a positivity rate of 11.96%. Single infections, dual infections, and multiple infections accounted for 61.36% (27/44), 36.36% (16/44), and 2.27% (1/44), respectively. From November 2023, the adenovirus positivity rate rose sharply, peaking at 51.61% in December. Following PCR amplification and sequencing, valid sequences were successfully obtained for 37 strains, comprising 36 AdV-3 strains and 1 AdV-C strain. Eight adenovirus-positive samples with Ct values below 22 were inoculated into HEp-2 cell lines for virus isolation, and whole-genome sequencing analyses were performed on the five successfully isolated strains. Phylogenetic analyses revealed that the AdV-3 strains collectively belonged to the same evolutionary clade. Whole-genome sequencing results indicated that four AdV-3 strains (HAdV-B3) showed no evidence of genetic recombination, whereas one AdV-C strain (HAdV-89) might have undergone genetic recombination. ConclusionThe adenovirus positivity rate among hospitalized pneumonia patients aged under 18 years of age in Yancheng from 2023 to 2024 was slightly higher than that reported in previous studies. Adenovirus infection exhibited a pattern of high incidence in winter, with the AdV-3 type being the predominant pathogen; one AdV-C strain (HAdV-89) might have undergone genetic recombination. It is recommended to strengthen surveillance of respiratory adenovirus type AdV-3 and its variants during the winter season, promote the use of multiplex pathogen detection techniques, and explore the clinical application of an AdV-3 vaccine.
