1.Development and reliability and validity test of the Post-Intensive Care Syndrome Scale for Children

Objective To develop an assessment scale for post-intensive care syndrome in pediatrics(PICS-p)and evaluate its reliability and validity,aiming to provide a scientifically sound and feasible tool for clinical assessment.Methods Based on the PICS-p conceptual framework,combined with literature review,semi-structured interviews,and Delphi expert consultation,a preliminary scale was developed.From June to December 2023,a survey was conducted among 330 pediatric patients who were discharged from a tertiary children's hospital in Hangzhou,followed by a two-week retest with 30 children to assess the reliability and validity of the scale.Results Finally 304 children completed the survey.The Post-Intensive Care Syndrome Scale for Children consists of 4 dimensions of physical dysfunction,cognitive dysfunction,psychological dysfunction and social dysfunction,with a total of 29 entries,with a cumulative variance contribution of 61.705%.The content validity index for individual items ranged from 0.800 to 1.000,and the content validity index for the scale as a whole was 0.98.The dimensions of the scale showed positive correlations with the Functional Status Scale(r=0.438-0.581,P<0.001).The overall Cronbach's α coefficient of the scale was 0.847;the split-half reliability was 0.868;the test-retest reliability was 0.832.Conclusion The scale demonstrates good reliability and validity,and it is suitable for assessing the severity of PICS-p in pediatric patients discharged from the PICU.
2.Fluorescence labeling for simulation of contamination with operators and their surroundings during debridement and dressing

OBJECTIVE To stimulate the debridement and dressing process under the approximately real clinical scene by fluorescence labeling and understand the contamination status of the operators' bodies and their surround-ings so as to improve the strategies.METHODS A total of 41 trainees were recruited from Enterostomal Therapist Nursing Education Program of Peking University School of Medicine and International School of Wound Thera-pists of Peking University First Hospital in 2023.The examination was designed by stimulating the debridement and dressing operations with fluorescence labeling,the orange was used to simulate the wound of the patients,and the pad towel was placed under the orange to represent the contaminated surroundings of the wound.A round le-sion area with diameter of 2 cm was uniformly marked on the surface of the orange peel by the operators,the trai-nees were asked to remove the lesion tissues and take dressings.The fluorescent powders were smeared evenly on the orange and pad towel before the examination,and no fluorescence labeling for the surroundings was guaranteed.The trainees were required to wear hats,masks,isolation gowns and gloves during the operations,and take off the gloves and trace the scope of fluorescent contamination with ultraviolet radiator after the examina-tion.RESULTS The contamination rate was 92.68%for the trainees' bodies,100.00%for their surroundings.The hands and forearms were the most severe contaminated body sites,and the contamination rates were 85.37%(35/41)and 34.15%(14/41),respectively.The downside of the pad towel was the most frequently contaminated area of the surroundings,with the proportion of 90.24%(37/41);the right side of the pad towel was the area with the contamination disseminated farthest,with the median distance of dissemination 13.50 cm.In addition,the dressing change carts of 75.61%(31/41)of the trainees were contaminated.CONCLUSIONS The operations of de-bridement and dressing change may lead to varying degrees of contamination of their bodies and surroundings.It is necessary to strengthen the hand hygiene and protective isolation during the process of dressing change and pay attention to the disinfection of the dressing change carts so as to minimize the risk of hospital-associated infec-tions.

5.Construction of ostomy nurse-led clinics in tertiary hospitals in China

Objective:To investigate the construction of ostomy nurse-led clinics in tertiary hospitals in China.Methods:Using the convenient method, managers of ostomy nurse-led clinics in tertiary hospitals in 31 provinces, autonomous regions and municipalities in China in August 2020 were selected as the survey objects. Questionnaire for Survey on Status Quo of Ostomy nurse-led clinics in ClassⅢ Grade A hospital in China was used to investigate. A total of 367 questionnaires were recovered in this survey, 313 valid questionnaires were recovered, and the effective recovery rate was 85.29%.Results:A total of 313 tertiary hospitals have set up ostomy nurse-led clinics. Among the ostomy nurse-led clinics, 86.58% (271/313) of the ostomy specialist nursing outpatient visits were independent visits by nurses. The opening period of stoma clinic was (6.27±4.08) years and the weekly opening time was (5.30±3.93) half-days. The average monthly outpatient number was 100, with a minimum of 4 and a maximum of 3 000. In terms of job management, 50.48% (158/313) of the stoma specialist nursing outpatient jobs were part-time, managers were mostly undergraduates (74.44%, 233/313) , the professional title of managers was mostly nurse in charge (48.24%, 151/313) , 38.98% (122/313) of hospitals had promotion system and 36.74% (115/313) of hospitals had performance management system. There were large differences in environmental layout, facilities and equipment, goods management and specialist management and there was no unified standard.Conclusions:There are great differences in the construction of ostomy nurse-led clinics in tertiary hospitals in China, which need to be further standardized and unified. It is suggested that relevant departments should introduce relevant policies to encourage the establishment of full-time nurse posts in ostomy nurse-led clinics, guarantee the training, promotion and performance reward of visiting nurses and fully encourage the subjective initiative of ostomy specialist nurses to promote their career development.
6.Development and reliability and validity test of post quality standard scale for wound, ostomy and continence nurses

Objective:To compile the post quality standard scale for wound, ostomy and continence nurses in China and to test the reliability and validity of the scale.Methods:The first draft of the scale was formed through literature review, semi-structured interview and Delphi expert letter consultation. Using the purposive sampling method, a total of 796 wound, ostomy and continence nurses from 31 provinces, municipalities and autonomous regions of China were selected from April to May 2019 to test the reliability and validity of the scale. A total of 796 questionnaires were sent out and 693 were effectively received, with effective recovery of 87.1%.Results:The post quality standard scale for wound, ostomy and continence nurses included 3 first-level items and 77 second-level items. Three common factors were extracted from exploratory factor analysis and the cumulative variance contribution rate was 72.466%. The correlation coefficient between the first-level items was 0.709-0.898 and the correlation coefficient between the first-level item and the total scale was 0.859-0.937. The Cronbach's α coefficient of the total amount table was 0.992 and the half-fold reliability coefficient was 0.917 ( P<0.01) . Conclusions:The post quality standard scale for wound, ostomy and continence nurses has good reliability and validity, which can be used to evaluate the post quality of wound, ostomy and continence nurses.
7.Construction of position responsibilities index system of wound, ostomy and continence nurses based on Delphi method

Objective:To construct a position responsibilities index system of wound, ostomy and continence nurses (WOCN) based on the Delphi method.Methods:Through literature review, semi-structured interviews, and investigation and research on the current working situation of WOCN, the expert inquiry questionnaire was self-designed with reference to opinions of the expert coordination group. From January to April 2019, a total of 45 senior experts including professional doctors, WOCN representatives, representatives of international Enterostomal Therapist Training Program (ETNEP) and chairmen of various provincial professional committees in China were invited to conduct 3 rounds of Delphi expert consultation, and construct the position responsibilities index system of WOCN.Results:The positive coefficients of the 3 rounds of expert consultation were respectively 100.0% (45/45) , 93.3% (42/45) and 100.0% (42/42) , and the authority coefficient of the experts was (0.92±0.16) . The final position responsibilities index system of WOCN included 5 first-level indexes such as clinical practice, education, research, coordinated management and professional promotion and 33 second-level indexes.Conclusions:The index system constructed in the study clarifies the position responsibilities of WOCN, which helps to provide a certain theoretical basis for the work, performance and classification of specialist nurses.
8.Construction of post quality standard index system for wound, ostomy and continence nurses

Objective:To establish a post quality standard index system for wound, ostomy and continence nurses (WOCN) .Methods:An expert coordination group was established in January 2019. Through literature search, semi-structured interviews, WOCN work status surveys, and concerning the opinions of the expert coordination group, the WOCN post quality standard index system was initially constructed. The convenient sampling method was adopted to conduct three rounds of Delphi expert letter consultation among 45 senior experts from all over China from January to April 2019.Results:The expert positive coefficients of three rounds of expert letter inquiries were respectively 100.0%, 93.3% and 100.0%, the expert authority coefficient was (0.92±0.16) , Kendall coordination coefficient was 0.241-0.383 ( P<0.01) and the coefficient of variation was all less than 0.25. The final WOCN post quality standard index system includes 5 first-level indicators (clinical practice ability, education ability, research ability, coordination management ability and professional promotion ability) and 79 second-level indicators. Conclusions:Clarifying WOCN post quality standards can promote the formulation of human resources decisions in various aspects, such as retraining and promotion of specialist nurses, and improve the scientific and effective management of specialist nurses.
9.Analysis of 90 hospitalized children with refractory Tourette′s syndrome

Objective:To analyze the diagnosis and treatment process, treatment methods and clinical efficacy of children with refractory Tourette′s syndrome (TS), thus providing a basis for comprehensive prevention and treatment of refractory TS.Methods:A total of 90 children with refractory TS treated in the Child Mental Health Research Center of Nanjing Brain Hospital Affiliated to Nanjing Medical University from May 2012 to July 2019 were recruited.Their baseline characteristics, diagnosis of comorbidities before and after hospitalization, combined non-drug therapy during hospitalization, the drug types used before and after admission, the dosages of main anti-tic drugs used before admission and at discharge, and the treatment outcomes of comorbidities after admission were retrospectively analyzed.The Yale global tic severity scales (YGTSS) scores and the reduction rate were used to assess the severity of tic disorder and therapeutic effect, and the clinical global impression-efficacy index (CGI-EI) scores were graded for assessing the final therapeutic efficacy.Results:Among the 90 children with refractory TS, 82 children were males and 8 children were females.There was a significant difference in the YGTSS scores at admission and discharge (25.04±12.77 vs.67.64±12.46) ( t=27.55, P<0.05). The proportion of all recruited children diagnosed with comorbidities at discharge was significantly higher than that of admission (85.56% vs.47.78%, χ2=28.90, P<0.05). Combined non-drug therapies after admission mainly included psycho-education and supportive therapy (90 cases), comprehensive behavioral intervention for tics (47 cases) and relaxation therapy (19 cases). The distribution of drugs used before and after admission was the same, and there was no significant difference in the dosages of the five major anti-tic drugs before admission and at discharge (all P>0.05). There were no significant differences in YGTSS scores and reduction rate, and CGI-EI scores of children with or without comorbidities before and after admission (all P>0.05), suggesting the similar therapeutic outcomes. Conclusions:There is no difference in efficacy between outpatient treatment and anti-tic medication of children with refractory TS, and a comprehensive hospitalized intervention can significantly improve their clinical symptoms.Diagnosis and treatment of comorbidities and combined non-drug treatments like comprehensive psychological and behavioral interventions are the key events to improve the prognosis of children with refractory TS.
10.Diagnosis and classification of high-functional autism spectrum disorder and attention deficit hyperactivity disorder by combining empathy and executive function

Objective:To explore the significance of empathy and executive function indexes in the diagnosis and classification of autism spectrum disorder (ASD) and attention deficit hyperactivity disorder (ADHD).Methods:According to DSM-Ⅴ diagnostic criteria, 33 children with ASD, 30 children with ADHD and 39 typical development (TD) children and adolescents were enrolled as the research subjects.The empathy and executive function characteristics were compared and analyzed in the subjects.Based on empathy and executive function indicators, the three groups of subjects were diagnosed and classified by machine learning method.Results:The total score of Griffith empathy measure parent ratings(GEM-PR)(ASD: (0.67±0.64), ADHD: (1.00±0.79), TD: (0.98±0.73)) and each factor score ( F=3.595-10.363, all P<0.05) and the total score of behavior rating inventory of executive function(BRIEF)(ASD: (62.79±7.45), ADHD: (59.47±8.77), TD: (49.08±7.91)) and each factor score of the three groups were different ( F=6.557-33.205, all P<0.01). Among them, the scores of empathy and executive function in ASD and ADHD groups were generally higher than those in TD children (all P<0.05). When combined with BRIEF scale and GEM scale, the classification accuracy of the three groups reached 62.75%. Conclusion:Both ASD and ADHD children have damages in empathy and executive function.Combining empathy and executive function indexes are more helpful for diagnosis and classification than single index.
