1.Analysis of auditory-vestibular function and inner ear imaging features at different stages of Meniere's disease.

Objective:To explore of auditory-vestibular function and inner ear imaging features of patients with Meniere's disease(MD) at different clinical stages. Methods:The clinical data of 110 patients with unilateral MD who were admitted from January 2023 to March 2024 were collected, and all patients were staged according to the results of pure tone hearing threshold test, including 13 patients with stage Ⅰ, 18 cases with stage Ⅱ, 65 cases with stage Ⅲ, and 14 cases with stage Ⅳ. All patients were tested for vestibular function, including caloric tests, vestibular evoked myogenic potentials(VEMPs), vHIT and sensory integration tests(SOT). The sites of endolymphatic hydrops were evaluated by intravenous endotogidolinium-based MRI, twenty-seven patients completed electrocochleography. Results:①The disease course time of patients with different stages was different, and the disease course time of stage Ⅰ and Ⅱ was shorter than that of stage Ⅲ and Ⅳpatients(P<0.05). ②No statistical differences were found in clinical data or vestibular function between normal and abnormal ECochG groups(P>0.05). ③The results of caloric tests showed that the UW% values of stage Ⅲ(45.5±14.79) and stage Ⅳ (51.57±22.44) were higher than those of stageⅠ(31.2±14.9) and stage Ⅱ(33.5±13.31), there were statistically significant differences between stage Ⅰ and stage Ⅱ with stage Ⅲ and Ⅳ groups(P<0.05), the total abnormal rate of cVEMP was 62.72%, there was a statistically significant difference between stageⅠand stage Ⅲ with the stage Ⅳ group(P<0.05), the total abnormal rate of oVEMP was 71.82%, the difference between stage Ⅰ and stage Ⅳ group was statistically significant(P<0.05). The total score of SOT comprehensive balance gradually decreased with the increase of clinical stage, and there was a significant difference between the stage Ⅰ and Ⅳ groups(H=26.08, P<0.01), and there was a statistically significant difference in the rate of vestibular dysfunction of SOT between the two groups(χ²=6.7, P<0.05). ④Patients with vestibular and cochlear endolymphatic hydrops, and patients with simple cochlear or vestibular had significantly differences in disease course time, clinical stages, UW% value of caloric test, abnormal rate of cVEMP and oVEMP, total SOT balance score, the rate of vestibular abnormality(P<0.01). Among them, when the vestibular and cochlear endolymphatic hydrops are at the same time, the clinical stage of the patient is mainly stage Ⅲand Ⅳ. Conclusion:Auditory-vestibular and inner ear gadolinium-contrasted MRI examinations in clinical practice provide a supplementary reference for judging vestibular function and the type of endolymphatic hydrops.

2.Analysis of related factors and comparison of prediction models for readmission of patients with mood disorder

Objective:To analysis the influenced factors of readmission in hospital among patients with mood disorder,construct predictive models and compare the predictive performance of the models.Methods:The electron-ic medical record data of patients with mood disorder admitted to Beijing Anding Hospital from January 2010 to De-cember 2018 were retrospectively collected.Utilizing stepwise logistic regression to analyze the related factors of re-admission in patients with mood disorder.Logistic regression,support vector machine,random forest,extreme gradi-ent boosting(XGBoost)algorithms and convolutional neural networks(CNN)were used to construct a readmission prediction model and compare the predictive performances of the different models by using the accuracy,precision,recall,et.al.Results:A total of 6 234 patients with mood disorder were enrolled,24.9%(n=1 549)patients were readmitted after discharge.The stepwise logistic regression results revealed that readmissions were more likely to occur in patients with mood disorder who were female,had comorbidities,had a treatment outcome of cure,were treated with MECT and were on second-generation antipsychotics(OR=1.26,1.68,1.26,1.35,1.18).The CNN model demonstrated the highest accuracy,precision,and F1 scores,all at 0.87,with a recall of 0.86.The random forest achieved a recall of 0.86 and an AUC of 0.95.Conclusion:Readmission of patients with mood disorder is influenced by a multitude of factors.The convolutional neural networks and random forest models outperform other models in prediction.
3.Symptoms and impact on quality of life of patients with Ménière′s disease

Objective:To explore the symptoms of Ménière′s disease patients and their impact on quality of life.Methods:A cross-sectional study was conducted, consecutively enrolling patients of Ménière′s disease who visited the Otolaryngology Clinic at the Eye & ENT Hospital of Fudan University from October 2014 to December 2022. The Chinese version of the Ménière′s Disease Outcomes Questionnaire (MDOQ) and a Ménière′s disease-specific symptom checklist were utilized for assessment. SPSS 25.0 software was employed to perform multiple linear regression analysis to determine the impact of Ménière′s disease symptoms on patients′ quality of life.Results:A total of 790 patients with a definitive diagnosis of Ménière′s disease who met the inclusion and exclusion criteria were analyzed. The cohort comprised 418 males and 372 females, with a mean age of (54.8±13.1) years and a diagnosis duration ranging from 0 to 72 months, with a median of 3 months. The total score of the Chinese MDOQ was 61.0±12.0. The symptomatic presentations of the enrolled patients included hearing changes, tinnitus, aural fullness, drop attacks, vertigo episodes, visual instability, dizziness, and headache. Multiple linear regression analysis revealed that age, tinnitus, aural fullness, frequency of drop attacks, visual instability, dizziness, and headache were significant factors affecting the quality of life in Ménière′s disease patients ( F=145.50, P<0.05). Conclusions:The quality of life in Ménière′s disease patients requires improvement. Attention to the specific symptoms of Ménière′s disease and their impact on patients′ quality of life, along with targeted symptom interventions based on these findings, will be a focal point in future disease management.

5.Trajectory and influencing factors of rehabilitation exercise adherence in patients undergoing anterior cruciate ligament reconstruction

Objective:To investigate the level of rehabilitation exercise adherence in patients undergoing anterior cruciate ligament (ACL) reconstruction within 6 months post-surgery, analyze its trajectory, and provide a reference for clinical nursing interventions.Methods:Using a convenience sampling method, 150 patients who underwent arthroscopic ACL reconstruction at the First Affiliated Hospital of Zhengzhou University from June 2022 to August 2023 were selected. Rehabilitation exercise adherence was assessed monthly from postoperative 1 month (M 1) to 6 months (M 6) using the Orthopedic Functional Exercise Adherence Scale. A latent class growth model was employed to analyze adherence trajectory categories, and Logistic regression was used to identify influencing factors for different adherence trajectories. Results:A total of 144 patients completed follow-up (follow-up rate was 96.00%). Exercise adherence scores gradually declined over the 6 months post-surgery, with scores of (50.30±5.39), (45.92±3.85), (39.48±5.27), (35.83±6.19), (31.85±7.59), and (29.88±7.88) from M 1 to M 6, respectively. The difference in scores across time points was statistically significant ( F=85.467, P<0.01). Three potential categories were identified through the latent class growth model: the moderate-high level rapid decline group ( n=65, 45.14%), the moderate-low level stable group ( n=32, 22.22%), and the moderate-low level slow decline group ( n=47, 32.64%). Logistic regression analysis revealed that gender and age were significant factors influencing adherence trajectory categories ( P<0.05) . Conclusions:Dynamic declines in rehabilitation exercise adherence are common among ACL reconstruction patients, with varying degrees of decline. Gender and age may influence adherence trajectories, suggesting that targeted management strategies could be implemented in clinical practice.
6.Application of compliance-based sequential exercise in the rehabilitation of patients after anterior cruciate ligament reconstruction

Objective:To explore the effectiveness of a compliance-based sequential exercise model in the rehabilitation of patients after anterior cruciate ligament (ACL) reconstruction.Methods:A convenience sampling method was used to select 104 patients who underwent arthroscopic ACL reconstruction at The First Affiliated Hospital of Zhengzhou University from April 2022 to July 2023. Patients were randomly divided into an observation group and a control group (52 patients in each group). The control group received in-hospital and home-based sequential rehabilitation exercises, while the observation group followed a compliance-based graded sequential rehabilitation exercise program. Both groups received the intervention for 3 months. The differences in knee function scores and rehabilitation self-efficacy scale scores between the two groups before and after the intervention were compared.Results:After the intervention, both groups showed significant increases in knee function scores and rehabilitation self-efficacy scale scores compared to pre-intervention scores ( P<0.01). The observation group had higher knee function scores and rehabilitation self-efficacy scale scores than the control group, with significant differences ( P<0.01). Additionally, the knee function grading in the observation group was better than in the control group after the intervention, with significant differences ( P<0.01) . Conclusions:Compliance-based sequential rehabilitation exercises effectively promote knee function recovery in patients after ACL reconstruction and improve patients' rehabilitation self-efficacy.

10.Status quo and longitudinal development trajectory analysis of cognitive dysfunction in elderly patients with hip replacement

Objective:To investigate the current status and longitudinal development trajectory of postoperative cognitive dysfunction (POCD) in elderly patients undergoing hip replacement surgery and analyze the relationship between POCD and postoperative hip joint function recovery.Methods:Using the convenient sampling method, elderly patients who underwent hip replacement surgery in Department of Orthopedics in the First Affiliated Hospital of Zhengzhou University from January 2021 to March 2022 were selected as the research objects. The cognitive function at 1 week, 1 month, 2 months and 3 months after surgery was assessed using Mini-Mental State Examination (MMSE), and the longitudinal development trajectory of postoperative POCD was fitted using latent class growth model (LCGM). The differences between cognitive function categories and Oxford Hip Score (OHS) score at 3 months after surgery were compared by ANOVA.Results:A total of 307 elderly patients with hip replacement completed full follow-up and data collection. The incidence of POCD (MMSE<8 points) in patients at four postoperative time points was 30.29% (93/307), 29.97% (92/307), 28.66% (88/307) and 21.17% (65/307), respectively. The MMSE score was lowest at 1 week after surgery. Three potential categories of cognitive function longitudinal development tracks were identified by LCGM, including 208 cases (67.75%) with no cognitive dysfunction, 32 cases (10.42%) with improved cognitive dysfunction, and 67 cases (21.82%) with continuous cognitive dysfunction. Three months after surgery, patients with no cognitive impairment had the lowest OHS score, while patients with persistent cognitive impairment had the highest OHS score. There were statistically significant differences in OHS scores among patients with three longitudinal potential categories of cognitive function ( P<0.05) . Conclusions:The incidence of POCD in elderly patients with hip replacement varies at different postoperative times, with the most significant difference occurring at 1 week after surgery. Medical staff must dynamically evaluate the postoperative cognitive function of elderly patients and adopt targeted nursing strategies for different types of cognitive impairments, to promote the patient's recovery of hip function.
