1.Impact of vestibular dysfunction on cognitive function

Objective To investigate the impact of vestibular dysfunction on various domains of cognitive function, providing a basis for developing comprehensive vestibular-cognitive intervention strategies. Methods A total of 33 patients with confirmed unilateral vestibular dysfunction treated at Eye & ENT Hospital, Fudan University between June 2024 and December 2024. Vestibular function was assessed using vestibular evoked myogenic potential (VEMP), caloric testing, video head impulse test (vHIT), and sensory organization test (SOT). Cognitive function was evaluated using mini-mental state examination (MMSE), Montreal cognitive assessment (MoCA), Stroop color-word test, trail making test (TMT), and auditory verbal learning test (AVLT). Subjective symptoms were assessed using dizziness handicap inventory (DHI). Results In the vestibular function assessment of patients, abnormalities in caloric testing, utricle VEMP and saccule VEMP results were most common, with rates of 87.9%, 57.6%, and 66.7%, respectively; SOT abnormality primarily characterized by impaired vestibular function (21.2%). Spearman correlation analysis showed age, years of education, hearing ability, and emotional state were associated with overall or specific domains of cognitive function in patients. Greater vestibular dysfunction severity was associated with longer TMT-A time (r=0.443,P=0.010), most severe damage of short-term (r=-0.405,P=0.019) and long-term delayed recalls (r=-0.537,P=0.001). Patients with 31-60 of DHI scores showed longer TMT-A time than patients with 0-30 of DHI scores (P=0.033). Conclusions Patients with vestibular dysfunction exhibit significant impairment in low-frequency semicircular canal and utricle function, which affects attention allocation, information processing speed, and memory performance in cognitive tasks.
2.Characteristics of unintentional injury among college students and its association with sexual orientation and gender identity

Objective:
To analyze the occurrence of unintentional injuries among college students and their association with sexual orientation and gender identity, so as to provide a targeted scientific basis for injury prevention measures and intervention strategies.
Methods:
From October 24 to November 18, 2023, a sample of 1 629 college students from two general universities in Beijing was selected using convenience sampling method. A questionnaire survey was conducted to collect information on the gender identity, sexual orientation and occurrence of unintentional injuries among college students in the past year. The Symptom Checklist-90 (SCL-90), Pittsburgh Sleep Quality Index (PSQI), Childhood Trauma Questionnaire (CTQ), and Delaware Bullying Victimization Scale-Student (DBVS-S) were used to assess mental health, sleep quality, childhood trauma, and dysfunctional impulsivity status. Analyses of sexual orientation and gender identity were conducted. The t-test and Chi square test were used for intergroup comparison,and multivariate Logistic regression analysis was used to examine risk factors for unintentional injuries among college students of different gender identities.
Results:
The incidence rate of unintentional injuries among college students was 16.94%, with boys (17.08%) being higher than girls (16.90%). Compared with those who did not experience unintentional injuries (5.28± 3.60 , 118.68±41.38), college students who experienced unintentional injuries had poorer sleep quality and mental health status ( 6.38 ±3.93, 135.59±50.96)( t =-3.92, -4.26); the differences in the incidence of unintentional injury among college students with non suicidal self injury, interpersonal violence, childhood trauma, and different sexual orientations and gender identities were all statistically significant ( χ 2=28.75, 75.18, 9.83, 16.20, 4.13) (all P <0.05). Multivariate Logistic regression analysis showed that after adjusting for age, gender and body mass index, non heterosexual orientation increased the risk of unintentional injuries ( OR=1.61, 95%CI =1.09-2.38), whereas existing non suicidal self injury behaviors ( OR=2.10, 95%CI =1.02-4.37) and poorer mental health status ( OR=1.54, 95%CI =1.05-2.27) increased the risk of unintentional injuries among non heterosexual college students (all P <0.05).
Conclusions
The incidence rate of unintentional injuries among college students is relatively high, with non heterosexual groups having increased risk of unintentional injuries. Mental health status and non suicidal self injury behaviors are important factors related to unintentional injuries among non heterosexual college students.
3.Application of compliance-based sequential exercise in the rehabilitation of patients after anterior cruciate ligament reconstruction

Objective:To explore the effectiveness of a compliance-based sequential exercise model in the rehabilitation of patients after anterior cruciate ligament (ACL) reconstruction.Methods:A convenience sampling method was used to select 104 patients who underwent arthroscopic ACL reconstruction at The First Affiliated Hospital of Zhengzhou University from April 2022 to July 2023. Patients were randomly divided into an observation group and a control group (52 patients in each group). The control group received in-hospital and home-based sequential rehabilitation exercises, while the observation group followed a compliance-based graded sequential rehabilitation exercise program. Both groups received the intervention for 3 months. The differences in knee function scores and rehabilitation self-efficacy scale scores between the two groups before and after the intervention were compared.Results:After the intervention, both groups showed significant increases in knee function scores and rehabilitation self-efficacy scale scores compared to pre-intervention scores ( P<0.01). The observation group had higher knee function scores and rehabilitation self-efficacy scale scores than the control group, with significant differences ( P<0.01). Additionally, the knee function grading in the observation group was better than in the control group after the intervention, with significant differences ( P<0.01) . Conclusions:Compliance-based sequential rehabilitation exercises effectively promote knee function recovery in patients after ACL reconstruction and improve patients' rehabilitation self-efficacy.

5.Effect of transcutaneous auricular vagus nerve stimulation on tourniquet-induced hypertension in patients undergoing anterior cruciate ligament reconstruction

Objective:To evaluate the effect of transcutaneous auricular vagus nerve stimulation (taVNS) on tourniquet-induced hypertension (TIH) in the patients undergoing anterior cruciate ligament reconstruction.Methods:Seventy-four patients of either sex, aged 18-60 yr, of American Society of Anesthesiologists Physical Status classification I or II, with body mass index of 18-30 kg/m 2, undergoing elective anterior cruciate ligament reconstruction under general anesthesia combined with preoperative femoral nerve block, were divided into 2 groups ( n=37 each) using a random number table method: sham stimulation group (group SS) and group taVNS. Group SS received stimulation on the ear lobe and the tail of the helix of the left ear. Group taVNS received stimulation on the cymba concha and the earlobe of the left ear. Both groups received stimulation from 1 h before induction of anesthesia until the end of the procedure (frequency of 30 Hz, pulse width of 300 μs, and amplitude of the strongest current that could be tolerated by the patient in the absence of pain). The tourniquet inflation pressure was 280 mmHg, with an inflation time of 60-90 min. Systolic blood pressure, diastolic blood pressure and heart rate were recorded before tourniquet inflation to assess the development of intraoperative TIH. The consumption of intraoperative propofol, remifentanil, nitroglycerin, esmolol, norepinephrine and atropine was recorded, and the occurrence of postoperative nausea and vomiting, skin itching and headache and dizziness was also recorded. Results:Compared with group SS, the incidence of TIH and the number of patients used nitroglycerin were significantly reduced ( P<0.05), and no significant changes were found in the other parameters in group taVNS ( P>0.05). Conclusions:taVNS can decrease the occurrence of TIH in the patients undergoing anterior cruciate ligament reconstruction.
6.Application of multidisciplinary combined rehabilitation in patients with sarcopenia after total hip replacement

Objective:To explore the application effect of multidisciplinary combined rehabilitation in patients with sarcopenia after total hip arthroplasty (THA) .Methods:A total of 114 patients with sarcopenia who underwent THA treatment in the Department of Joint Surgery of the First Affiliated Hospital of Zhengzhou University from May to December 2020 were selected as the research objects by the convenient sampling method. The patients were divided into the control group and the observation group by the random number table method, with 57 cases in each group. The control group received routine rehabilitation nursing, while the observation group received multidisciplinary combined rehabilitation intervention. Muscle related indexes were compared between the two groups before and after intervention. The short-form Fugl-Meyer motor function score and Harris score were used to compare the motor function of the affected limb and the recovery of hip joint function in the two groups after intervention.Results:After intervention, the whole body muscle mass, limb skeletal muscle mass, affected limb skeletal muscle mass and limb skeletal muscle mass index of patients in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . The Fugl-Meyer motor function of the affected limb in the observation group was better than that in the control group, and the difference was statistically significant ( P<0.01) . The total score of simplified Fugl-Meyer motor function score and Harris score of the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.01) . Conclusions:Multidisciplinary combined rehabilitation can effectively prevent postoperative muscle atrophy and muscle mass loss in THA patients, which is beneficial to the recovery of the motor function of the affected limb and hip joint function.
7.A survey on the public 's discrimination rate of typical rumors during the coronavirus disease 2019 epidemic.

OBJECTIVES:
During the epidemic of coronavirus disease-2019 (COVID-19), the wide spread of rumors caused significant public hazards. This study aims to understand the situation of discrimination for typical COVID-19 rumors by the public and related factors.
METHODS:
An anonymous online survey was carried out using Questionnaire Star. The contents included participants' gender, age, education level, the COVID-19 information sources, and the judgmental questions about 14 representative COVID-19 rumors. The discrimination rate and 95% confidence interval of 14 rumors were estimated, and the association of discrimination rate with gender, age, and education level was analyzed by binary logistic regression.
RESULTS:
A total of 2 087 valid questionnaires were collected. The participants were mainly female (62.7%) and below 35 years old (63.4%); the education level was predominantly college/bachelor's degree (47.3%) and master's degree or above (39.1%); the participants, who accessed to COVID-19 information included internet media, accounted for 91%. The participants with different gender, age, and education level had significant differences in the distribution of COVID-19 information sources (all P<0.01). The participants' discrimination rate for 14 rumors ranged from 67.4% to 98.6%, with 4 rumors less than 80%. Women's discrimination rate of 9 rumors was significantly higher than men's (all P<0.05). There was no significant difference in the discrimination rate of rumors among the different age groups (all P>0.05), but the differences in the discrimination rate of other rumors among the different age groups varied according to the rumor. Compared to those with high school or less education levels, the discrimination rates were also higher in the respondents with high education levels (P<0.05).
CONCLUSIONS
A few publics are still unable to identify typical rumors during the COVID-19 epidemic. There are associations among genders, age, and the education levels with the discrimination of some rumors. The government authorities should strengthen the true information regarding COVID-19, and therefore enhance the public's ability to identify rumors.

8.Effect of medical alliance combined with video recording guidance on the out-of-hospital rehabilitation of elderly patients undergoing hip replacement

Objective:To explore the effect of medical alliance combined with video recording guidance on the out-of-hospital rehabilitation of elderly patients undergoing hip replacement.Methods:Using the convenient sampling method, a total of 58 aged patients who were hospitalized in the Department of Orthopedics and underwent hip replacement in the First Affiliated Hospital of Zhengzhou University from March 2018 to June 2019 were selected as research objects. They were randomly divided into the observation group and the control group, with 29 cases in each group. The control group was given conventional out-of-hospital rehabilitation management while the observation group was given medical alliance combined with video recording guidance. The serum albumin, Harris score, total body skeletal muscle mass, skeletal muscle mass of the affected limb, incidence of deep venous thrombosis (DVT) and incidence of artificial hip dislocation were compared between the two groups before and after the intervention.Results:After intervention, the serum albumin of the observation group was (36.73±1.95) g/L, the Harris score was (89.29±5.25) , the total body skeletal muscle mass was (17.91±1.18) kg and the skeletal muscle mass of the affected limb was (5.38±0.79) kg, which were all higher than those of the control group, and the differences were statistically significant ( t=3.524, 7.231, 4.584, 4.252; P<0.05) . After intervention, the incidence of DVT in the observation group was 3.45% (1/29) , and the difference was not statistically significant when compared with 13.79% (4/29) in the control group (χ 2=1.970, P>0.05) . After the intervention, the incidence of artificial hip dislocation in the observation group was 0 (0/29) , which was lower than 20.69% (6/29) in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:Medical consortium combined with video guidance can effectively improve the out-of-hospital rehabilitation effect of elderly patients undergoing hip replacement surgery and reduce the occurrence of complications. Medical alliance combined with video recording guidance can effectively improve the out-of-hospital rehabilitation effect of elderly patients with artificial hip replacement and reduce the incidence of complications.
9.DNA assembly technologies: a review.

DNA assembly is the core technology of synthetic biology. With the development of synthetic biology, researchers have developed different DNA assembly technologies that rely on DNA polymerase or DNA ligase, and also have developed some non-enzyme-dependent DNA assembly techniques to facilitate the automation of DNA assembly. The assembly of large fragments of DNA from a few hundred kb to Mb is mostly dependent on microbial recombination. In this paper, the three types of DNA assembly technologies, including enzyme-dependent, non-enzymatic and in vivo homologous recombination, are reviewed.

10.Diagnostic value of flow cytometry in postoperative infection after renal transplantation

Objective To assess the value of flow cytometry in the diagnosis of postoperative infection following renal transplantation. Methods According to postoperative imaging findings and laboratory examination outcomes, 51 recipients undergoing the first renal transplantation were divided into the bacteria (n=33), fungus (n=9) and BK virus (n=9) groups. Twenty-eight recipients with stable conditions after renal transplantation were assigned into the stable group. Flow cytometry was adopted to detect the percentage and absolute counting of lymphocyte subpopulation in the peripheral blood of recipients in each group. Renal function, percentage and absolute counting of lymphocyte subpopulation in the peripheral blood were statistically compared among different groups. Receiver operating characteristic (ROC) curve was drawn to evaluate the diagnostic value of the percentage and absolute counting of lymphocyte subpopulation in infectious diseases after renal transplantation. Results Compared with the stable group, the serum creatinine (Scr) and blood urea nitrogen (BUN) levels in the bacteria, fungus and BK virus groups were significantly up-regulated, respectively (P=0.035, 0.007, 0.024; 0.037, 0.006, 0.032). Compared with the stable group, the percentage of CD16+CD56+natural killer (NK) cells was significantly declined in the bacterial (P=0.036) and fungus groups (P=0.015), and the proportion of CD4+/CD8+T cells was dramatically decreased in the fungus group (P=0.004). Compared with the bacterial group, the percentage of CD3+CD8+T cells was significantly elevated (P=0.013 and 0.008), the proportion of CD3+CD4+T cells was considerably declined (P=0.003 and 0.010), and the percentage of CD4+/CD8+T cells was significantly declined (P=0.003 and 0.005) in the fungus and BK virus groups. Compared with the stable group, the quantity of CD3+T cells, CD3+CD8+T cells and CD16+CD56+NK cells was significantly declined in the bacterial, fungus and BK virus groups, respectively (P=0.025, 0.002, 0.003; 0.015, 0.005, 0.006; 0.001, 0.001, 0.031). In addition, the quantity of CD3+CD4+T cells was considerably decreased in the fungus and BK virus groups (P=0.001, 0.003). The quantity of CD19+B cells was significantly reduced in the BK virus group (P=0.019). Compared with the bacterial group, the quantity of CD3+CD4+T cells was considerably lower in the fungus group (P=0.023). ROC curve analysis revealed that the quantity of CD3+CD4+T cells [area under curve(AUC)=0.8492] and CD16+CD56+NK cells (AUC=0.8889) yielded relatively high accuracy in the diagnosis of fungal infection. The quantity of CD3+T cells (AUC=0.8472), CD3+CD4+T cells (AUC=0.8452) and CD19+B cells (AUC=0.8115) yielded relatively high accuracy in the diagnosis of BK virus infection. Conclusions Flow cytometry detection of the lymphocyte subpopulation in peripheral blood can evaluate the immune function of patients. Absolute counting of lymphocyte subpopulation can directly assess the degree of immunity. These two combined parameters provide guiding significance for the diagnosis and differential diagnosis of infectious diseases in recipients after renal transplantation.
