Objective:To explore in depth the real experiences of preoperative prehabilitation among colorectal cancer patients undergoing stoma surgery, in order to provide a reference for the development of preoperative prehabilitation protocols. Methods:This study was a descriptive qualitative research. Using purposive sampling, 18 colorectal cancer ostomy patients who were hospitalized in the Department of Anorectal Surgery of the Third Affiliated Hospital of Anhui Medical University from March to August 2024 were selected for semi-structured in-depth interviews. Thematic analysis was conducted using content analysis methods. Results:The experiences of colorectal cancer ostomy patients with preoperative prehabilitation were summarized into four major themes: cognition and attitudes toward prehabilitation (willingness to actively participate, expectations for prehabilitation outcomes, psychological barriers), positive effects of prehabilitation (promoting postoperative recovery, improving adverse outcomes, enhancing stoma adaptation), challenges of prehabilitation (poor physical condition, negative emotions, inadequate preparation time) and suggestions and needs regarding prehabilitation (availability of information resources, refinement of individualized programs, need for psychological support) .Conclusions:Colorectal cancer ostomy patients generally hold a positive attitude toward preoperative prehabilitation, which helps accelerate postoperative recovery. Medical staff should enhance patient compliance and engagement by constructing an information support and shared decision-making model. Meanwhile, a tripartite psychological support system involving hospitals, communities, and families should be established, and a personalized and tiered prehabilitation program should be developed to ensure the safe and effective implementation of prehabilitation protocols, thereby improving the overall effectiveness of prehabilitation.

Objective:To investigate the risk factors of depression in patients with lacunar cerebral infarction 3 months after discharge, and build a risk prediction model. Methods:The clinical data of 237 patients with lacunar cerebral infarction diagnosed and treated in the Second Affiliated Hospital of Wannan Medical College from January 1, 2021 to January 1, 2022 were retrospectively analyzed. Demographic data, laboratory indicators and risk factors of cardiovascular disease were collected when the patients were admitted to hospital. The patient health questionnaire-9 (PHQ-9) score was used to assess the risk of post-stroke depression (PSD) 3 months after discharge. Univariate and multivariate logistic regression analyses were used to identify independent risk factors of depression. A predictive model was constructed based on the above risk factors. The predictive efficacy was evaluated by receiver operating characteristic (ROC) curve, and the clinical applicability of the predictive model was measured by calibration curve and decision curve.Results:Fifty-four patients with lacunar infarction (22.80%) showed symptoms of depression at 3 months after discharge. Multivariate logistic regression showed that women ( OR=2.952, 95% CI: 1.403-6.213, P=0.004), cystatin C ( OR=2.964, 95% CI: 1.222-7.191, P=0.016), apolipoprotein B ( OR=4.836, 95% CI: 1.533-15.258, P=0.007) were the independent risk factors for PSD in patients with lacunar infarction. Activity of daily living (ADL) score ( OR=0.939, 95% CI: 0.915-0.964, P<0.001) was a protective factor for PSD in patients with lacunar infarction. The area under the curve (AUC) of the prediction model was 0.779 (95% CI: 0.703-0.855, P<0.001), which was better than other predictors. Hosmer-Lemeshow H test showed that the model was well calibrated ( P=0.588). In addition, the prediction model of decision curve analysis had a good clinical applicability. Conclusions:The predictive model based on gender, laboratory indicators and daily life ability score can evaluate the risk of PSD in patients with lacunar infarction 3 months after discharge. It is good predictive efficacy and clinical applicability which worth clinical promotion and application.

Dopamine D3 receptor (D3R) is implicated in multiple psychotic symptoms. Increasing the D3R selectivity over dopamine D2 receptor (D2R) would facilitate the antipsychotic treatments. Herein, novel carbazole and tetrahydro-carboline derivatives were reported as D3R selective ligands. Through a structure-based virtual screen, ZLG-25 (D3R Ki = 685 nmol/L; D2R Ki > 10,000 nmol/L) was identified as a novel D3R selective bitopic ligand with a carbazole scaffold. Scaffolds hopping led to the discovery of novel D3R-selective analogs with tetrahydro-β-carboline or tetrahydro-γ-carboline core. Further functional studies showed that most derivatives acted as hD3R-selective antagonists. Several lead compounds could dose-dependently inhibit the MK-801-induced hyperactivity. Additional investigation revealed that 23j and 36b could decrease the apomorphine-induced climbing without cataleptic reaction. Furthermore, 36b demonstrated unusual antidepressant-like activity in the forced swimming tests and the tail suspension tests, and alleviated the MK-801-induced disruption of novel object recognition in mice. Additionally, preliminary studies confirmed the favorable PK/PD profiles, no weight gain and limited serum prolactin levels in mice. These results revealed that 36b provided potential opportunities to new antipsychotic drugs with the multiple antipsychotic-like properties.

Objective:
To investigate the epidemiological characteristics of newly reported HIV/AIDS cases at ages of 50 years and older in Zhengzhou City from 2017 to 2021, so as to provide the evidence for formulating the AIDS control measures among the elderly.
Methods:
Epidemiological data of newly reported HIV/AIDS cases at ages of 50 years and older in Zhengzhou City from 2017 to 2021 were collected through the Zhengzhou Municipal Intelligent Public Health Management System. The epidemiological characteristics including population distribution, transmission route and route of detection were analyzed using a descriptive epidemiological method.
Results:
A total of 533 newly reported HIV/AIDS cases at ages of 50 years and older were diagnosed in Zhengzhou City from 2017 to 2021, accounting for 25.94% of all HIV/AIDS cases. There were 400 male cases and 133 female cases, with a male-to-female ratio of 3.01∶1, 288 cases at ages of 50 to 59 years (54.03%), 467 cases with an educational level of junior high school and below (87.62%), 391 cases with household registered residence in Zhengzhou City (73.36%), and 333 farmers (62.48%). Sexual contact was the main route of transmission (524 cases, 98.31%), and HIV/AIDS cases were predominantly detected by medical institutions (305 cases, 57.22%), followed by counseling and testing (167 cases, 31.33%). The proportion of HIV/AIDS cases diagnosed by medical institutions appeared a tendency towards a decline in Zhengzhou City from 2017 to 2021 (χ2trend=10.953, P=0.001), while the proportion of HIV/AIDS cases diagnosed by counseling and testing showed a tendency towards a rise (χ2trend=5.438, P=0.020).
Conclusions
The newly reported HIV/AIDS cases at ages of 50 years and older were predominantly local male farmers in Zhengzhou City from 2017 to 2021, with sexual contact as the main transmission route and medical institutions as the predominant route of detection. The proportion of newly reported HIV/AIDS cases appeared a tendency towards a rise.

Objective:
To investigate the current basic situation of ultrasound machines of ultrasound departments in Shanghai medical institutions, and to provide the basis for making management policy in order to promote medical quality.
Methods:
Questionnaire surveys about ultrasound machines and service ability including producing countries of ultrasound machines, the number of ultrasound machines, using years of ultrasound machines, yearly workload and the yearly number of patients that each ultrasound machine accepted were made in 2013 and 2018, respectively. Statistical results were compared between the two surveys.
Results:
Compared with 2013, the share of imported ultrasound machines declined in 2018 (82% vs 91%). Compared with 2013, the number of ultrasound machines in Shanghai medical institutions had increased by 31% in 2018 (2 123 vs 1 617). The occupancy rate of ultrasound machines in tertiary hospitals was the highest (tertiary hospitals 40%, secondary hospitals 25%, primary grade hospitals 20%, and private hospitals 15%). Compared with 2013, the proportion of ultrasound machines that have been used for more than 10 years increased (9% vs 4%), the yearly workload of ultrasound examination had increased by 46% (19.82 million person-time vs 13.59 million person-time). Tertiary hospitals bored the highest proportion of the workload (tertiary hospitals 51%, secondary hospitals 35%, primary grade hospitals 4%, and private hospitals 10%). Currently, the number of ultrasound machines per 10, 000 people in Shanghai was 1.14. The yearly number of patients that each ultrasound machine accepted had increased by 11% (9300 person-time vs 8400 person-time in 2018).
Conclusions
The scale of ultrasound departments in Shanghai medical institutions has been developing. Brand localization of ultrasound machines is improving. However, the renewal ability of ultrasound machines still needs to be improved. The workload of ultrasound department is getting heavier. Hierarchical diagnosis and treatment is unbalanced.
