1.Application value of constructing a prediction model based on nutritional status combined with clinical data for infectious complications after radical gastrectomy for gastric cancer

Objective To explore the application value of predictive model based on nutritional status combined with clinical data in infectious complications after radical gastrectomy for gastric canc-er.Methods The clinical data on 394 gastric cancer patients who underwent radical gastrectomy at Suzhou Ninth Hospital Affiliated to Soochow University from January 2017 to July 2024 were retrospec-tively collected.According to whether the patients developed infection after surgery,they were divided into infection group(n=73)and non-infection group(n=321)based on whether they developed postoperative infections or not.The least absolute shrinkage and selection operator(LASSO)method was used to obtain the optimal related features associated with infectious complications after radical gas-trectomy for gastric cancer.Multivariate Logistic regression analysis was conducted to screen for risk factors of infectious complications after radical gastrectomy.R software was used to randomly select 70%of data as training set(for establishing the nomogram model)and 30%of data as test set.The area under the curve(AUC)of the receiver operating characteristic(ROC)curve,Bootstrap sampling validation,and decision curve analysis(DCA)were used to assess the value of the predictive model.Results of the 394 gastric cancer patients,73 developed postoperative infectious complications,in-cluding 53.42%with lung infections,17.81%with intra-abdominal infections,16.44%with uri-nary tract infections,and 12.33%with wound infections.Age,diabetes mellitus,preoperative nu-tritional risk,high preoperative PIV,combined organ resection,and prolonged operation time were identified as risk factors for infectious complications after radical gastrectomy(P<0.05).In the training set,the AUC of the nomogram model was 0.898,and the AUC validated in the test set was 0.891.Bootstrap sampling validation with 1,000 iterations showed that the average errors between the predicted probabilities and actual values in the training and test sets were 0.019 and 0.024,re-spectively.DCA results indicated that the model had clinical utility.Conclusion Infectious com-plications after gastrectomy for gastric cancer are common,and the predictive model based on nutri-tional assessment combined with clinical data has high value in predicting the risk of infectious com-plications after gastrectomy.

Objective To investigate the expression of ferritinophagy-related gene ELAV-like RNA binding protein 1(ELAVL1)in multiple myeloma(MM)and elucidate its diagnostic and prognostic value for MM.Methods First,we analyzed ELAVL1 expression level in healthy controls and MM patients using data from the GEO and TCGA databases.Subsequently,bone marrow specimens were collected from 28 newly diagnosed MM patients and 20 healthy controls,and qRT-PCR was employed to validate ELAVL1 expression.The diagnostic and prognostic potential of ELAVL1 was assessed using ROC curve analysis and Kaplan-Meier survival curves.Additionally,univariate and multivariate COX regression analyses were performed to identify independent risk factors for MM prognosis.Finally,KEGG and GO enrichment analyses were performed using the DAVID online platform.Results The level of ELAVL1 expression was significantly higher in newly diagnosed MM patients and refractory/relapsed MM patients than in the healthy controls(P<0.001).Moreover,ELAVL1 expression was positively correlated with the International Staging System(ISS)stage of MM(P<0.01).Furthermore,qRT-PCR validation confirmed that ELAVL1 expression was elevated in the 28 newly diagnosed MM patients compared to the 20 healthy controls(P<0.001).ROC curve analysis demonstrated that ELAVL1 could effectively differentiate between newly diagnosed MM patients,healthy controls,and MGUS patients(P<0.001 and P=0.000 2,respectively).Survival analysis revealed that high ELAVL1 expression was associated with shorter progression-free survival(P=0.0141)and overall survival(P=0.008 0).Univariate and multivariate COX regression analyses identified high ELAVL1 expression as an independent risk factor for poor MM prognosis(P=0.005 0).KEGG analysis suggested that ELAVL1 might be involved in the Hippo and MAPK signaling pathways.Conclusion High ELAVL1 expression in MM may serve as a biomarker for diagnosis and poor prognosis.ELAVL1 may promote MM initiation and progression via the Hippo and MAPK signaling pathways.

Objective To investigate the regular spot check implementation of central sterile supply department(CSSD) at hospitals of all grades across China and analyze its influencing factors to provide the basis for further improvement of regular cleaning quality inspection. Methods Cross-sectional study was conducted form February to July of 2017,and 132 hospitals all over the country were selected to investigate with the questionnaire the current status of CSSD management model, staffing and regular cleaning quality inspection. Results The ratio of the nursing staff and the number of beds was 1.15:100,the ratio of staff and the number of beds was 2. 00:100. 81. 82% (n =108) of the hospitals adopted the centralized management model. The qualification rate for regular spot check execution was 87.12% (n=115). The results of multiple logistic regression showed that the centralized management model was more conducive to the regular spot check than the non-centralized management model (OR=4.71,95% CI:1.05-21.08). Conclusions The centralized management model proves positive for the implementation of regular spot checks;and CSSDs should pay more attention to the regular spot check of the cleaning quality.
7.Design of the nurse occupation well-being scale and evaluation of its reliability and validity

Objective To prepare an occupation well-being scale for nurses, and test its reliability and validity. Methods After literature review and semi-structured interviews, the item pool of the occupation well-being scale for nurses was created based on theory of structure of the subjective well-being. After group discussion, and evaluation by the specialists, a pilot scale was obtained. A total of 350 clinical nurses were selected by using stratified sampling method, and their occupation well-being were surveyed by the scale. The data were used to assess the reliability and validity of the scale. Results The occupation well-being scale for nurses had 3 dimensions, including 8 exploring factors. The values of Cronbach Alpha and the split half reliability coefficient of the total scale, endogenous exogenous satisfaction, satisfaction of nurses occupation happiness were all larger than 0.9, which meaned the internal consistency was good. In the emotion dimension, the value of Cronbach Alpha and the split half reliability coefficient were less than 0.7, which meant the internal consistency was not so good. The reliability coefficient of repeated measurement of the whole scale and each dimension were greater than 0.7, suggesting the external consistency of the scale was good. The content validity index of 3 dimensions were all larger than 0.9,and the cumulative ex-planatory variables were all larger than 55%. Conclusion The occupation well-being scale for nurses proposed in this study has good reliability and validity, and can be used to evaluate the occupation well-being of the nurses.
8.Relationship of young nurses’ job satisfaction and subjective well-being

Objective To investigate the correlations between young nurses ’ job satisfaction and subjective well-being.Methods We selected 135 young nurses as the samples, used the general well-being scale, MMSS satisfaction scale to do the questionnaire survey and analyzed the correlations between them.Results The overall score of young nurses’ job satisfaction was (2.83 ±0.42) at the low level.Job satisfaction was positive correlated with subjective well-being ( P<0.01).Young nurses’ job satisfaction was positive correlated with subjective well-being (r =0.445,P <0.01).The influencing factors of subjective well-being included four aspects:work had been praised and recognized, the relationship with coworkers, the work of control and chance to participate in decision-making and scheduling satisfaction (R2 =0.981,F=1 677.18,P<0.01).Conclusions We should create a nursing career development opportunities for young nurses, and enhance spiritual incentive, managers’ understanding and support, respect and trust, team cooperation, self actualization and so on.Thus we can effectively promote their job satisfaction of nursing, so as to improve the subjective well-being.
9.Effect of human tissue kallikrein gene therapy on insulin resistance and nephropathy in type 2 diabetic rats

Objective To evaluate the therapeutic effect of recombinant adeno-associated viral vector (rAAV) expressing human tissue kallikrein gene (rAAV-HK) on insulin resistance and renal complications in tyDe2 diabetic rats. Methods Male Wistar rats were injected low dose streptozotocin and fed with high fat and sucrose diets to form type 2 diabetic model. rAAV mediated HK gene (HK group) or LacZ gene (LacZ group) were introduced to the diabetic rats, and their systolic blood pressure, fasting blood glucose and insulin, serum creatinine, urine creatinine, urine osmolarity and urine microalbumin were measured. The homeostasis model assessment of insulin resistance (HOMA-IR), urinary albumin excretion rate (UAER) and creatinine clearance rate (Ccr) were calculated. The expression of PI3-kinase p11o catalytic subunit (p110) and Akt phosphorylation on Thr-308 were detected by Western blot. The morphology of kidney wag observed. Results Delivery of rAAVHK resulted in a reduction in blood pressure at 2 weeks and the hypotensive effect lasted for the duration of the study. The HOMA-IR was significantly lower in HK group than LacZ group (4.76±0.33 vs 8.36±0.48, P<0.01) at the end of the study, fasting insulin level was reduced [(8.19±2.45 vs 13.85±3.76)mIU/L. P<0.01], but there was no significant change in fasting blood glucose [(13.09±3.01 vs 13.58±2.88)mmol/L].The phosphorylation of p11o and Akt Thr-308 were significantly decreased in skeletal muscle and liver in LacZ group and were almost corrected by HK gene therapy. The UAER and Ccr were significantly lower and urinary osmolarity were higher in HK-treated rats compared with LaeZ rats. Histological assessment indicated that the renal complication was relieved by HK gene delivery. Conclusion The rAAV-mediated HK gene delivery efficiently attenuated insulin resistance partly through PI3K/Akt pathway and diabetic nephropathy in type 2diabetic rats.

Objective To study the effects of extract 5F from Pteris semipinnata L. on the apoptosis of skin fibroblasts in patients with systemic sc leroderma (SSc) and the possible mechanism of its inhibition on the proliferatio n of skin fibroblasts. Methods The cultured SSc skin fibroblasts were used as an experimental model, and the apoptosis of SSc skin fibroblasts before and afte r the extract 5F stimulation was detected by TUNEL assay and fluorescent microsc opy. Results The apoptosis of SSc skin fibroblasts was induced by the extract 5F, and the apoptosis was enhanced along with the increase of the concentration and incubation time of this compound within definite limits. Conclusions These results indicate that the extract 5F can induce the apoptosis of SSc skin fibro blasts, and significantly inhibit the proliferation and collagen synthesis in th ese cells.
