1.Distribution of virulence genes in clinical isolates of hospital-associated and community-associated methicillin-susceptible Staphylococcus aureus from Terengganu, Malaysia

Staphylococcus aureus is a common bacterial pathogen known to cause various kinds of infections due to
its repertoire of virulence factors. This study aimed to investigate the distribution of 19 types of virulence
genes among clinical isolates of methicillin-susceptible S. aureus (MSSA) using the polymerase chain
reaction. A total of 109 MSSA isolates, i.e., 63 hospital-associated (HA) and 46 community-associated (CA)
were collected from Hospital Sultanah Nur Zahirah, the main tertiary hospital in Terengganu, Malaysia,
from July 2016 to June 2017. The most frequent virulence genes detected were hla (78.9%, n=86) and
hld (78.0%, n=85) encoding hemolysins, lukED (56.9%, n=62) encoding leukotoxin ED, followed by seb
(26.6%, n=29) and sea (24.8%, n=27) encoding enterotoxins. Among 34 (31.2%) isolates carrying six
or more virulence genes, only five were multidrug resistant (MDR) while the remaining isolates were
susceptible. Significant associations were discovered between the hld gene with CA-MSSA (p=0.016) and
the seo gene with HA-MSSA (p=0.023). However, there is no significant association between virulence
genes among the different types of infection. The clinical MSSA isolates in Terengganu showed high
prevalence and high diversity of virulence gene carriage.

Prior to COVID-19, dengue was an important public health problem in Malaysia. Due to the movement control order
imposed by the Malaysian government to curb the COVID-19 transmission, a study predicted that mosquito-borne
diseases would increase during lockdown and partial lockdown seasons. Thus, this study aims to determine the current
situation of dengue incidence during the pre-COVID-19 pandemic (2019) and during the COVID-19 pandemic (2020
and 2021). We compared the number of laboratory-confirmed cases in the pre-COVID19 year (2019) and during the
COVID-19 pandemic (2020 and 2021). In addition to that, we characterized the clinical manifestation, dengue serotype
and viremia levels of dengue patients that were admitted to the Hospital Cancelor Tuanku Muhriz. We found a significant
decrease in the number of laboratory-confirmed cases between COVID-19 pandemic and the pre-covid period
(p2020=0.064; p2021<0.001). In this study, we found DENV 4 serotype was the most common serotype in dengue
patients admitted to our hospital. There was no significant correlation between DENV serotype/viremia level with
clinical manifestation of dengue fever and dengue with warning signs. However, patients infected with DENV4 had the
highest viral load compared to patients infected with other serotypes. We also found high viremia levels were significantly
associated with the febrile phase.
3.GFAT1: A Potential Prognostic Biomarker in Colorectal Cancer

Introduction: There is an increasing demand for additional techniques to diagnose and treat cancer including CRC
or colorectal cancer effectively. Utilizing antibodies as biomarker could contribute to accurate diagnosis of cancer
due to its high specificity and sensitivity. One of the etiologies of CRC progression was proposed as the alterations
of hexosamine biosynthetic pathway which could subsequently influence the rate-limiting enzyme, glutamine-fructose-6-phosphate aminotransferase (GFAT1). These increased enzymatic activities resulted in an elevation of glucose
uptake that provides nutrients facilitating the progression of cancer cells. Therefore, we attempted to determine the
potential of GFAT1 as the biomarker for CRC by correlating its expression with clinicopathological features of the patients. Methods: A total of 132 10% formalin-fixed paraffin embedded tissue were retrieved. Immunohistochemistry
(IHC) was performed on the tissue sections and digital images were subsequently acquired. All the images were automatedly analyzed using IHC Profiler. GFAT1 immunoreactivity in colorectal tissues was calculated using an adapted
H-score formula. Clinicopathological features of the patients were statistically correlated with the status of GFAT1.
Results: Colorectal adenocarcinoma tissues had the significantly highest GFAT1 H-scores with the mean of 103.18
compared to adenoma and non-tumor tissues. There have been no significant associations between clinicopathological characteristics of the patients and the status of GFAT1 except for tumor size. Conclusion: Immunoreactivity of
GFAT1 was significantly different between non-tumorous tissues and adenocarcinoma as well as between adenoma
and adenocarcinoma tissues. GFAT1 could serve as one of the prognostic biomarkers or useful targets.

Speech and language skills are among the crucial components in determining cochlear implant habilitation outcomes.
The purpose of this study was to investigate speech and language skills of Malay cochlear implant children using a
developmental scale questionnaire and to identify the demographic factors that contribute to their performance. This
study involved 26 children recruited from the National Cochlear Implant Program under the Malaysian Ministry of
Health with chronological ages between 33 to 99 months (mean=72, SD=18.9), implant ages between 18 to 71 months
(mean=40, SD=13.5) and hearing ages between 13 to 48 months (mean=30, SD=10.5). The instrument used was The
Integrated Scale of Development-Malay version 2 (ISD-Mv2). The questionnaires were given to the parents or caregivers
followed with by a phone call interview later. Results from the study showed that only 9 subjects (35%) were able score
on the ISD-Mv2 equally or higher than their hearing age. From the six components in the ISD-Mv2, cognitive was scored
the highest with 96.79% meanwhile expressive language was the lowest with 76.21%. Pearson correlation test revealed
strong positive correlation between audition-receptive language (r=0.554, p<0.05) and cognitive–social communication/
pragmatic (r= 0.625, p<0.05). Speech and language performance of children with demographic factors did not show
significant differences. The findings suggests that majority of the Malay cochlear implant children demonstrated delayed
speech and language performance as compared to normal hearing children.
5.HOARSENESS SECONDARY TO LARYNGEAL HISTOPLASMOSIS: A CASE REPORT

Histoplasmosis usually occurs in immunocompromised patients through the dissemination of the fungus from lungs to other organs but laryngeal involvement is rare. We report a 52-year-old male chronic smoker who presented with hoarseness for 3 months was initially investigated for laryngeal carcinoma. However biopsy showed granulomatous inflammation. He was treated with anti-tuberculosis treatment for 2 months but symptoms persisted and a repeat biopsy revealed laryngeal histoplasmosis. There were no sign and symptoms of pulmonary or systemic involvement. Treatment with intravenous amphotericin B and oral itraconazole lead to complete resolution of symptoms and signs. Monitoring and early revisiting the diagnosis are important when a patient’s symptoms do not improve with the treatment. Further investigation including a repeat biopsy of lesion is vital to establish the diagnosis and lead to appropriate management.
6.High incidence of Plasmodium knowlesi malaria compared to other human malaria species in several hospitals in Malaysia

Through the regional control programme, Malaysia has been successfully reducing the incidence of Plasmodium falciparum and Plasmodium vivax infections. However, the incidence of zoonotic malaria Plasmodium knowlesi infection is increasing and now has been the major cause of malaria in Malaysia especially Malaysian Borneo. The emergence of knowlesi infection has threatened the malaria elimination programme which the government aims to reduce the overall malaria infections by 2020. Unlike other benign human Plasmodium spp., P. knowlesi can cause fatal infections. The aim of this study was to determine the incidence and distribution of five human malaria parasites including P. knowlesi in Peninsular Malaysia and Malaysian Borneo. A total of 112 blood samples were collected from seven states and district hospitals in Peninsular Malaysia and Malaysian Borneo from year 2015 to 2016. The samples were examined by microscopy and further confirmed by nested PCR assay targeting 18S rRNA gene of Plasmodium spp. Following the nested PCR assays, a total of 54 (48.2%) samples were positive for P. knowlesi infections, 12 (10.7%) cases were positive for P. vivax infections, followed by 7 (6.3%) cases of P. falciparum and 4 (3.5%) cases of P. malariae. There were 3 cases (2.7%) of mixed infections (P. knowlesi/P. vivax). However, no cases were identified as P. ovale. A total of 32 (28.6%) cases were found as negative infections. LoopMediated Isothermal Amplification Assay (LAMP) was performed to confirm inconclusive results produced by microscopy and nested PCR. P. knowlesi showed the highest prevalence in Sarawak (n= 30), Sabah (n=13), Pulau Pinang (n=5) and Pahang (n=6). PCR and LAMP was not able to detect a large number of microscopy positive samples due to DNA degradation during storage and shipping. Among all the states involved in this study, the highest prevalence of P. knowlesi infection was found in Sabah and Sarawak.
7.Occupational Sharps Injury Among Healthcare Workers In Hospital Melaka 2013 – 2015: A Cross Sectional Study

Sharps injury imposed a major threat towards safety and health among healthcare workers (HCWs). Many studies in Malaysia concentrated on prevalence of needle stick injury (NSI), however the prevalence of sharps injury among HCWs based on local guideline was still scarce in Malaysia. This information gap leads us to conduct this study. Our study aimed to determine the prevalence of sharps injury among HCWs in Hospital Melaka and to describe the factors among staff with sharps injury. This cross-sectional study was conducted in Hospital Melaka, Malaysia. 165 reported cases from 2013-2015 were reviewed and secondary data extracted. Data was analyzed using IBM SPSS version 20. Among reported sharps injury cases in this study, 65 (39.4%) were male, while 100 (60.6%) were female. The mean age was 27.41 (SD: 6.06). More than half of the reported sharps injury occurred among doctor, 113 (68.5%) specifically House Officer; 89 (53.9%) followed by paramedic, 26 (15.8%) and others, 26 (15.8%). Mostly occurred in ward, 114 (69.1%). The device which accounted for most of sharps injury cases was hypodermic needle, 67 (40.6%). Many of the sharps injury cases occurred while withdrawing needle from patient, 26 (15.9%). Prevalence of sharps injury among HCWs in this study was 0.8% in year 2013, 1.1% in year 2014 and 0.5% in year 2015. Nearly half from the total number of reported sharps injury cases among HCWs in Hospital Melaka were from Medical Department, 71 (43%). Recommended measures include usage of Safety-engineered devices (SEDs) and emphasize on standard precaution.
8.Effects of different surface treatments on the shear bond strength of veneering ceramic materials to zirconia

PURPOSE: To evaluate and compare the effect of different materials and techniques on the shear bond strength of veneering ceramic materials to zirconia. MATERIALS AND METHODS: 136 sintered zirconia cubes were prepared and randomly divided into four study groups according to corresponding methods of surface treatment and materials: GLN (grinding followed by laser scanning using Noritake Cerabien ZR), SLN (sandblasting followed by laser scanning using Noritake Cerabien ZR), GLV (grinding followed by laser scanning using VITA VM 9), and SLV (sandblasting followed by laser scanning using VITA VM 9). Spraying technique was performed to coat the core. Profilometer, SEM, XRD, EDS, universal testing machine, and stereomicroscope were used to record surface roughness Ra, surface morphology, phase transformation, elemental compositions, shear bond strength SBS values, and failure types, respectively. Specimens were investigated in unaged (not immersed in artificial saliva) and aged (stored in artificial saliva for a month) conditions to evaluate SBS values. RESULTS: Grinding and GLN as first and second surface treatments provided satisfactory Ra values in both conditions (1.05 ± 0.24 µm, 1.30 ± 0.21 µm) compared to sandblasting and other groups (P < .05). The group GLN showed the highest SBS values in both conditions (30.97 ± 3.12 MPa, 29.09 ± 4.17 MPa), while group SLV recorded the lowest (23.96 ± 3.60 MPa, 22.95 ± 3.68 Mpa) (P < .05). Sandblasting showed phase transformation from t-m. Mixed failure type was the commonest among all groups. CONCLUSION: GLN showed to be a reliable method which provided satisfactory bond strength between the veneer ceramic and zirconia. This method might preserve the integrity of fixed dental crowns.

9.Portal Hypertension of a Delayed Onset Following Liver Abscesses in a 12-Month-Old Infant: A Case Report and Review of the Literature

We report a 12-month-old female infant who had a history of neonatal sepsis with liver micro-abscesses that resolved with intravenous antibiotics during neonatal period. During her neonatal admission period, no umbilical vein catheter was inserted. Also, she did not undergo any abdominal surgeries or had a postnatal history of necrotizing enterocolitis. However, the child developed upper gastrointestinal bleeding in form of hematemesis and melena secondary to esophageal varices at the age of 12 months with an extra-hepatic portal vein obstruction with cavernous transformation and portal hypertension subsequently. The child underwent a successful endoscopic injection sclerotherapy. She is now 20-month-old and has portal hypertension but otherwise asymptomatic. We are proposing the possibility of a delayed-onset portal hypertension as a complication of liver abscess and neonatal sepsis.

10.Cytogenetic Analysis Of Buccal Cells From Farmers In Tanjung Karang And Kelantan Who Were Exposed To Pesticides

Pesticides and chemical fertilizers are widely used in agriculture to increase crop productivity among farmers.However, exposure to pesticides will give potential risk to human health. The aim of this study was to analyze thefrequency of micronucleus (MN) and binucleus (BNu) formation in buccal cells from farmers who were exposedto pesticides using the MN assay. Buccal swabs were collected from the farmers in Tanjung Karang (n = 32) andKelantan (n = 43) using wooden tongue depressor. A structured questionnaire was used to obtain demographic dataof the farmers. Cytogenetic analysis was carried out by Acridin Orange (AO) staining 0.0025% (w/v). The frequencyof MN and BNu as the biomarkers for cytogenetic damage was observed by using a fluorescence microscope.Comparison of frequency of MN and BNu is conducted in two areas namely Tanjung Karang, Selangor and Kelantanbecause of the agricultural activity and the type of pesticides used are different. Results showed that the frequencies of bothMN and BNu among farmers in Tanjung Karang were significantly higher (p < 0.05) compared to farmers in Kelantan.Meanwhile, for the socio-demographic factors (age, smoking status, working period), MN and BNu frequencies amongfarmers in Tanjung Karang were also significantly higher (p < 0.05) as compared to farmers in Kelantan. While in theaspect of pesticide exposure, the frequencies of MN and BNu showed no significant difference between the frequency ofpesticide spraying (p > 0.05) and the practices of PPE (Personal Protective Equipment) (p > 0.05). This may suggeststhat cytogenetic changes were not influenced by these factors. In addition, correlation study shows positive correlationbetween the frequency of MN with the pesticide exposure of farmers in Tanjung Karang (p > 0.05, r = 0.015) and Kelantan(p > 0.05, r = 0.0158). Besides, the frequency of BNu also has a positive correlation with the pesticide exposure amongfarmers in Tanjung Karang (p > 0.05, r = 0.036) and farmers in Kelantan (p > 0.05, r = 0.013). Hence, this present study demonstrated that exposure to pesticides increasedthe formation of MN and BNu among farmers and theprolonged use of pesticides may induce genotoxicity andDNA damage to human.
