BACKGROUND:Recent studies have found that Nrf2/ARE signaling pathway activators have the characteristics of low toxicity and control, and have a protective effect against radiation tissue damage.OBJECTIVE:To investigate whether nitrooleic acid can protect submandibular gland epithelial cells from radiation injury by regulating the Nrf2/ARE signaling pathway.METHODS:Rat submandibular gland epithelial cells were cultured in vitro and CCK-8 assay was used to screen the optimal concentration and time of nitrooleic acid administration.Submandibular gland epithelial cells were divided into non-radiation group, radiation control group, nitrooleic acid group, nitrooleic acid+ML385(Nrf2/ARE signaling pathway specific inhibitor)group, and ML385 group.Submandibular gland cells were pretreated with nitrooleic acid and ML385 for 24 hous according to the experimental groups, and then irradiated with 5 Gy radiation to establish the models. At 48 hours after irradiation,CCK-8 assay was used to detect the cell proliferation rate.Real-time quantitative PCR was used to detect the expression of Nrf2,HO-1,and NQO1 mRNA in the cells.Real-time quantitative PCR and enzyme-linked immunosorbent assay were used to detect the cell secretion function and the expression of inflammatory factors.DCFH-DA fluorescent probe kit was used to detect the level of intracellular reactive oxygen species.RESULTS AND CONCLUSION:(1)Compared with the radiation control group, the proliferation rate of submandibular gland epithelial cells and the expression levels of secretion function related factors aquaporin 5 and α-amylase in the nitrooleic acid group of rats increased(P<0.05),and the expression levels of Nrf2,HO-1,and NQO1 mRNA increased(P<0.05),while the expression levels of inflammatory factors interleukin-1β,interleukin-6,and tumor necrosis factor-αdecreased(P<0.05),and reactive oxygen species generation reduced(P<0.01).(2)Compared with the nitrooleic acid group, the addition of nitrooleic acid and ML385 group resulted in a decrease in cell proliferation rate and expression levels of secretion function related factors aquaporin 5 and α-amylase(P<0.05),and mRNA expressions of Nrf2,HO-1,and NQO1 were all decreased(P<0.05),while the expression levels of inflammatory factors interleukin-1β,interleukin-6,and tumor necrosis factor-α increased(P<0.05),and generation of reactive oxygen species increased(P<0.05).(3)Results indicated that in the radiation environment, nitrooleic acid has a certain protective effect on the proliferation ability and secretion function of rat submandibular gland epithelial cells, reduces the expression of inflammatory factors, lowers intracellular reactive oxygen species levels, and alleviates the damage of rat submandibular gland epithelial cells caused by radiation. This function may be related to the activation of Nrf2/ARE signaling pathway.

Objective:To investigate the therapeutic value of visual endoscopic retrograde appendicitis therapy (vERAT) in pediatric patients with acute suppurative appendicitis (ASA).Methods:This was a retrospective cohort study. A total of 55 ASA patients who underwent vERAT at the Pediatric Department of the Tangdu Hospital of Air Force Medical University between November 2023 and January 2025 were selected and divided into groups based on the presence or absence of fecaliths: fecalith group and non-fecalith group. The baseline characteristics, initial treatment success rates, treatment costs, hospital stay duration, procedure time, and recurrence rates between two groups were compared. Mann-Whitney U test and χ2 test were used to evaluate group differences. Results:A total of 55 ASA patients were enrolled, including 38 males and 17 females, with the age of 11.2 (9.2, 13.1) years. Based on the presence of fecaliths, patients were divided into two groups: fecalith group (32 cases) and non-fecalith group (23 cases). No statistically significant differences were observed between the two groups in terms of age, gender, duration of abdominal pain, white blood cell count, neutrophil percentage, diameter of appendix, thickness of appendix clinical symptoms or signs (all P>0.05). The initial treatment success rates were 91% (29/32) in fecalith group and 96% (22/23) in non-fecalith group, with no statistically significant difference ( P=0.632). However, significant differences were noted in stent placement ( χ2=5.85, P=0.026) and procedure time ( Z=4.75, P<0.001). The follow-up duration time was 6.0 (2.0, 12.0) and 7.0 (2.0, 8.5) months for the fecalith and non-fecalith groups, respectively, with no significant difference ( Z=0.05, P=0.962). The recurrence rates were 14% (4/29) in fecalith group and 5% (1/22) in non-fecalith group, with no statistically significant difference ( P=0.375). Conclusions:vERAT can safely and effectively treat pediatric ASA, regardless of the presence or absence of fecaliths. It can provide a new treatment option for ASA.

Objective:Exploring the efficacy of indocyanine green(ICG)near-infrared fluorescence(NIRF)imaging technology in robotic-assisted laparoscopic partial nephrectomy for children with duplicated kidneys. Methods:A retrospective analysis was conducted on the medical records of 7 children with duplicated kidneys admitted to Guizhou Provincial People's Hospital from December 2023 to September 2024,including 3 males and 4 females. The patients with a median age of 36(2,132)months. All cases of duplicated kidneys were located at the renal pole, with 5 on the left side and 2 on the right side. All 7 patients had a history of worsening hydronephrosis, with 4 having a history of urinary tract infection and 1 accompanied by urinary incontinence(urine leakage outside the normal voiding period). Preoperative diagnosis was confirmed via enhanced CT of the urinary tract, diuretic renal dynamic imaging(SPECT),and voiding cystourethrography. One case had an ectopic ureteral opening. One case had recurrent urinary tract infections following ureteral cyst fenestration surgery, and five cases had terminal ureteral obstruction. All seven cases showed that the functional capacity of the duplicated upper kidney was <10%. All 7 cases underwent robot-assisted laparoscopic repeated nephrectomy of the upper kidney using ICG-NIRF technology. Intraoperative ICG fluorescence imaging was performed in three stages:a catheter was inserted through the lower kidney ureter, and after the first injection of ICG,green fluorescence was observed in the lower kidney ureter. Under fluorescence guidance, the upper and lower ureters were anatomically dissected; The second ICG injection via intravenous administration revealed the vessels supplying the upper kidney, and the blood supply to the upper kidney was blocked; After the third ICG injection via intravenous administration, the normal lower kidney showed fluorescence imaging, while the ischemic upper kidney did not. The upper kidney was resected along the anatomical boundary between the upper and lower kidneys. Results:All 7 surgeries were successfully completed, with an average surgical time of(155.1 ± 22.7)min;Intraoperative blood loss was(12.5 ± 8.7)ml;postoperative hospital stay was(5.2 ± 1.7)days. Postoperative follow-up duration was(9.2 ± 3.5)months,with preoperative concomitant urinary tract infection and post-micturition dribbling symptoms resolved. No cases of ureteral stump effusion or urinary leakage were observed postoperatively. At the 3-month postoperative follow-up SPECT examination,no loss of lower kidney function was detected. Conclusions:The application of ICG-NIRF imaging technology in robotic-assisted laparoscopic heminephrectomy for children with duplicated kidneys is easy to perform and safe. Its main advantage is the ability to clearly identify normal ureters, blood vessels of non-functional kidneys, and the boundary between the upper and lower halves of the kidney, thereby guiding the optimization of surgical strategies.

OBJECTIVES:
The risk factors and role of mother‒child gut microbiota in pediatric inflammatory bowel disease (PIBD) remain unclear. We aimed to explore the clinical risk factors associated with PIBD, analyze the characteristics of gut microbiota of children and their mothers, and examine the correlation of the microbial composition in mother‒child pairs.
METHODS:
We conducted a case-control study including children with PIBD and their mothers as the case group, as well as healthy children and their mothers as the control group. Questionnaires were used to collect information such as family illness history and maternal and early-life events. Fecal samples were collected from the children and mothers for microbiota 16S ribosomal RNA (rRNA) sequencing to analyze the composition and its potential association with PIBD.
RESULTS:
A total of 54 pairs of cases and 122 pairs of controls were recruited. A family history of autoimmune disease and antibiotic use during pregnancy were associated with an increased risk of PIBD, and a higher education level of the father was associated with a decreased risk of PIBD. Children with PIBD and mothers exhibited different gut microbiota compared to healthy children and mothers. Similarities were observed in the gut microbiota of mothers and children in the same groups. Some bacterial biomarkers of mothers discovered in this study had the power to predict PIBD in their offspring.
CONCLUSIONS
PIBD is influenced by maternal risk factors and has unique gut microbiota characteristics. The mother‒child gut microbiota is closely related, suggesting the transmission and influence of the gut microbiota between mothers and children. This study highlights the potential pathogenesis of PIBD and provides a basis for developing targeted interventions.
