1.Stem cell exosomes and biomaterial-assisted exosomes in bone defect repair

BACKGROUND:A large number of studies have demonstrated that stem cell exosomes play an important role in the repair of bone defects,either directly as carriers for loading other small molecules or surface modifications,or by binding to biomaterials to promote the repair and regeneration of bone tissue.OBJECTIVE:To summarize the osteogenic mechanisms of stem cell exosomes from different sources and their research progress in bone defect repair.METHODS:Chinese search terms"stem cell,exosome,bone,biomaterial,carrier,bioceramic,polymer,metal,hydrogel,engineered exosome"were used to search CNKI.English search terms"stem cell,exosome,bone defect,biomaterial,carrier,bioceramic,ploymer,metal material,hydrogel,engineering exosome"were used to search PubMed database.According to the inclusion and exclusion criteria,77 relevant articles were finally included for summary.RESULTS AND CONCLUSION:Exosomes from stem cells of different origins can promote osteoblast proliferation and differentiation,promote angiogenesis,and regulate osteoclast activity and macrophage phenotype to promote bone formation and bone mineralization.In addition,many achievements of exosomes in the field of bone defect repair were described from two aspects:biomaterial-assisted exosomes and engineered exosomes.However,the current research on stem cell exosomes in bone tissue engineering is still insufficient,and most of these studies are limited to small animal models,while the treatment of bone defects in large animals,including humans,will be more complex,which will also become a major challenge for the treatment of bone defects.This will also be a great challenge in the dissemination of exosome therapy.

3.Clinical Study of He Ne Laser Combined with Compound Betamethasone Local Injection on the Treatment of Alopecia Areata

Objective:To observe the clinical study of He Ne laser combine with compound betamethasone local injection in the treatment of alopecia areata(alopecia areata,AA).Methods:This study was a prospective randomized controlled trial,used random number table,100 AA patients who were admitted to our hospital from March 2020 to July 2024 were divided into control group(treated with compound betamethasone local injection,n=50)and study group(treated with He Ne laser combined with compound betamethasone local injection,n=50).The clinical efficacy,conventional scale scores[Skin dermatology life quality index(DLQI),severity of alopecia tool(SALT)],serum inflammatory factors[leukotriene C4,c-reactive protein(CRP),interleukin-10(IL-10),soluble interleukin-2 receptor(sIL-2R)],immune function,and adverse reactions between two groups were compared.Results:The total clinical effective rate,IL-10,CD3+,CD4+,CD4+/CD8+in the study group were higher than those in the control group after treatment,while the DLQI score,SALT score,leukotriene C4,CRP,SIL-2R,and CD8+were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:He Ne laser combined with compound betamethasone local injection in the treatment of AA patients,can improve clinical efficacy,and symptoms,which may be related to the regulation of inflammatory factors and immune function,which is safe and reliable.

5.Mechanism of action of cholangiocyte senescence in cholestatic liver disease and retated targeted therapies

Cholestatic liver disease(CLD)is a liver condition caused by disorders in bile acid secretion and metabolism due to various reasons,and it has the common pathological features of various chronic liver diseases.In recent years,the role of cholangiocyte senescence(CS)in the pathogenesis of CLD has attracted more and more attention,and CS not only participates in the development and progression of CLD,but it is also significantly associated with the course and prognosis of the disease.Targeted clearance of senescent cholangiocytes or blocking senescence-related pathways can improve CLD.This article summarizes the role of CS in CLD,related influencing factors,and the research advances in CLD,in order to provide a theoretical reference for subsequent studies on CLD.

7.High-Throughput Detection of Multiple Classes of Antibiotics in Source Water Using a Functionalized Polyacrylonitrile Nanofiber Membrane

Objective To develop a novel solid-phase extraction(SPE)method based on a functionalized nanofiber membrane for the efficient co-extraction of structurally diverse antibiotics with markedly different physico-chemical properties from source water,and to establish a high-throughput analysis method by coupling this technique with ultra-performance liquid chromatography-tandem mass spectrometry(UPLC-MS/MS).Methods A polydopamine and zirconium(Ⅳ)fumarate metal-organic frameworks(MOF-801)co-modified polyacrylonitrile nanofiber membrane(PDA@PAN/MOF-801 NFMs)was prepared as the SPE adsorbent through hybrid electrospinning and dopamine self-polymerization.Critical SPE and UPLC-MS/MS parameters were optimized,and the method was applied to analyze antibiotic contamination in source water samples from 14 sources of centralized drinking water supply in Suzhou,China,to evaluate the practical application potential of the method.Results The PDA@PAN/MOF-801 NFMs adsorbent demonstrated efficient adsorption of 32 antibiotics from 6 classes through multiple retention mechanisms,including synergistic electrostatic interactions,hydrogen bonding,and π-π interactions.In combination with UPLC-MS/MS,the SPE method we developed enabled high-throughput detection of multiple antibiotics in source water,with limits of detection(LOD)being 0.001-0.05 ng/L and limits of quantitation(LOQ)being 0.005-500 ng/L.Spiked recoveries were 70.14%-111.50%.Intra-day relative standard deviation(RSD)was below 14.12%and the inter-day RSD was below 15.07%.The method demonstrated excellent sensitivity,accuracy,and precision.Conclusion In this study,we successfully developed an efficient analytical method based on a novel nanofiber membrane adsorbent.This approach provides a new technical reference for the high-throughput detection of multiple antibiotics in environmental waters and shows promising potential for practical applications.
8.Exploration of different echo times in short repetition time T2-fluid attenuated inversion recovery sequences and their effects on brain gray and white matter contrast

Objective To explore the use of T2-fluid attenuated inversion recovery(FLAIR)sequences with a shorter TR to significantly reduce examination time without compromising diagnostic accuracy and image quality,and to obtain satisfactory brain gray and white matter contrast images via adjusting the TE.Methods A total of 60 patients underwent brain MR were selected,and each patient was scanned using two different T2-FLAIR sequences.The scan parameters for sequence A group were TR 9 000 ms,inversion time(TI)2 500 ms,and TE 120 ms.For sequence B group,the scan parameters were TR 6 000 ms,TI 2 000 ms,and TE 120-200 ms(with 20 ms intervals),respectively,and all images data were labeled as B1 to B5.A double-blind method was used to observe the images from both groups,with a 5-point rating scale.The signal-to-noise ratio(SNR),contrast-to-noise ratio(CNR)of brain gray and white matter,and the contrast of brain gray and white matter were measured,respectively.SPSS 26.0 statistical software was used for subjective and objective evaluations of the images from both groups.Results For subjective evaluation,there were statistically significant differences in image quality with TE ranged from 120 to 200 ms in B group(H=41.4,P<0.05).In pairwise comparisons,images from B3 group(TE 160 ms)showed no statistically significant difference compared to A group in subjective evaluations by two physicians(P>0.05).For objective evaluation,A group exhibited significantly higher SNR compared to B group.Among the images in B group with TE ranged from 120 to 200 ms,SNR decreased gradually with increased TE,while CNR and contrast increased progressively(P<0.05).Specifically,there were no statistically significant differences between B3 and A groups in terms of CNR and contrast(P>0.05).Conclusion T2-FLAIR sequences with a shorter TR can significantly reduce examination time.By adjusting the TE,satisfactory brain gray and white matter contrast images can be obtained.When TE is 160 ms,the contrast of brain gray and white matter is optimal,and the SNR of the images meets the requirements for clinical diagnosis.
9.Development of classification and grading performance evaluation indicators for public health staff in district CDCs based on job competencies

ObjectiveTo explore the establishment of performance assessment indicators for the classification and grading of public health staff in district-level Centers for Disease Control and Prevention (CDCs), and to provide a basis for such evaluations. MethodsThrough literature review and group interviews, performance evaluation indicators were developed based on competency evaluation. Experts were invited to evaluate the weight of performance evaluation indicators for public health staff from different categories, with the average value used to represent the weight of each indicator. ResultsTwenty-nine experts from universities in Shanghai, municipal CDCs, and district CDCs participated, yielding an expert authority coefficient of 0.86. The performance evaluation indicators for department managers were categorized into three levels, with 4 indicators at the primary level, 16 indicators at the secondary level, and 42 indicators at the tertiary level, while those for general staff included 4 primary indicators, 15 secondary indicators, and 36 tertiary indicators. Significant differences were observed in the weight coefficients of the primary indicators (internal operations, professional work, and learning and growth) between department managers and general staff. The top three secondary indicators for department managers were department management, monitoring and prevention, and level of expertise. For mid-level and senior staff, the top three secondary indicators were monitoring and prevention, level of expertise, and research work. The top three secondary indicators for junior staff were monitoring and prevention, professional expertise, and professional attitude. No significant statistical differences were found among tertiary indicators. ConclusionThe developed performance evaluation indicators are reliable. Staff at different levels and classifications should be evaluated using different performance evaluation standards to accurately reflect individual performance and contributions.
10.Application of portable gastrointestinal endoscopy to telemedicine based on 5G network

Objective:To establish a portable gastrointestinal endoscopy telemedicine platform that enables the real-time transmission of endoscopy audio and video data to the consulting physicians through 5G communication technology, and to evaluate the effectiveness of this platform.Methods:From October 10 to October 20, 2023, 30 cases (16 cases from Sanya, Hainan Province, and 14 cases from Zhanjiang, Guangdong Province) from primary health institutions were selected for portable gastrointestinal endoscopy telemedicine consultations with a tertiary hospital in Shanghai. 5G network transmission parameters were recorded, and the lesion detection rate of the examination side and the consultation side, as well as the quality of the videos and images were compared.Results:The 5G network transmission parameters for telemedicine from Sanya and Zhanjiang to Shanghai included download network speeds of 168.3 Mbps and 160.4 Mbps, upload network speeds of 75.1 Mbps and 72.4 Mbps, and delays of 28 ms and 25 ms, respectively. There was no significant difference in the lesion detection rate [43.3% (13/30) VS 36.7% (11/30), χ2=0.792, P=0.598] between the physicians on the examination side and the consultation side. The endoscopic video quality (2.92±0.59 points VS 2.88±0.64 points, t=0.302, P=0.763) and picture quality (3.10±0.57 points VS 3.05±0.53 points, t=0.424, P=0.622) were compared by two senior endoscopists between the examination side and the consultation side, indicating no significant difference. Conclusion:The utilization of portable gastrointestinal endoscopy for telemedicine based on 5G network technology is effective in enabling telemedicine consultations and providing operational guidance. This approach enhances the procedural skills and diagnostic capabilities of physicians in primary healthcare settings, facilitating the equitable distribution of high-quality medical resources.
