1.Who succeeds in insulin deintensification? Real-world predictors and modifiable factors from primary care

Introduction:
Insulin therapy is essential in the management of type 2 diabetes mellitus (T2DM), but is often associated with treatment
burden, hypoglycemia, and potential overtreatment. Insulin deintensification is increasingly recommended for
appropriately selected patients; however, there is limited real-world evidence to guide patient selection and to identify
modifiable factors that influence successful insulin deintensification. This study aimed to identify clinical predictors,
including modifiable factors, associated with successful insulin deintensification in a primary care setting.
Methodology:
A multicentre retrospective observational study was conducted across seven government primary care clinics in the Petaling
District. Adult patients with T2DM undergoing insulin deintensification were included. Successful insulin deintensification
was defined as maintenance or improvement of hemoglobin A1c following insulin discontinuation, dose reduction, or
reduction in injection frequency. Paired outcomes were analyzed using the Wilcoxon signed-rank test. Between-group
comparisons were performed using the Mann–Whitney U test and Chi-square or Fisher’s exact test. Multivariable logistic
regression was used to identify independent predictors of successful insulin deintensification.
Results:
A total of 261 patients were included. Glycemic control remained stable following insulin deintensification (p = 0.334).
Significant reductions in body weight (−0.41 kg, p = 0.012) and total daily insulin dose (23.1% reduction, p <0.001) were
observed. Univariate analysis did not demonstrate significant differences between groups. However, multivariable logistic
regression identified SGLT-2 inhibitor use (aOR 3.23, 95% CI 1.28–8.18, p = 0.013) and regular SMBG (aOR 2.00, 95% CI
1.05–3.81, p = 0.035) as independent predictors of successful insulin deintensification.
Conclusion
Insulin deintensification can be successfully implemented without compromising glycemic control. Identified predictors,
including modifiable factors such as SGLT-2 inhibitor use and SMBG, provide clinically actionable insights to guide patient
selection and treatment optimization. These findings challenge the traditional reluctance toward insulin deintensification
and support a more evidence-based and individualized approach in routine clinical practice.

One of the most frequent complaints of individuals with hearing impairment is listening comfort in noisy environments.
In order to improve listening comforts in background noise, digital noise reduction (DNR) systems are incorporated into
hearing aids (HAs). Each hearing aid manufacturer has its proprietary algorithm for the DNR system. The amount of
attenuation (dB) provided by the DNR system can be quantified using the hearing aid analyser. However, the standard
test signals in the hearing aid analyser could not quantify the attenuation of DNR for speech mixed with noise signals.
Therefore, this study aimed to (i) develop speech-plus-noise test signals that incorporate Malay sentences and (ii)
quantify the efficacy of DNR systems in commercial hearing aids using the newly developed test signals. Six different
brands of hearing aids with identical technology but from different manufacturers were subjected to electroacoustic
testing utilising newly created Malay speech-in-noise test signals with and without DNR enabled. The total root-meansquare (RMS) gain reduction for each HA was calculated. The results show that the types of noise, the signal-to-noise
ratio and the gender of the speaker have a significant effect (p<0.05) on the amount of gain reduction in the HA output
as a result of the DNR system in each HA . In conclusion, the newly developed Malay speech-in-noise test signals can
be used to verify the efficacy of DNR system in commercial hearing aids.
3.Profiling Bartonella infection and its associated risk factors in shelter cats in Malaysia

Background:
Poor disease management and irregular vector control could predispose sheltered animals to disease such as feline Bartonella infection, a vector-borne zoonotic disease primarily caused by Bartonella henselae.
Objectives:
This study investigated the status of Bartonella infection in cats from eight (n = 8) shelters by molecular and serological approaches, profiling the CD4:CD8 ratio and the risk factors associated with Bartonella infection in shelter cats.
Methods:
Bartonella deoxyribonucleic acid (DNA) was detected through polymerase chain reaction (PCR) targeting 16S-23S rRNA internal transcribed spacer gene, followed by DNA sequencing. Bartonella IgM and IgG antibody titre, CD4 and CD8 profiles were detected using indirect immunofluorescence assay and flow cytometric analysis, respectively.
Results:
B. henselae was detected through PCR and sequencing in 1.0% (1/101) oral swab and 2.0% (1/50) cat fleas, while another 3/50 cat fleas carried B. clarridgeiae. Only 18/101 cats were seronegative against B. henselae, whereas 30.7% (31/101) cats were positive for both IgM and IgG, 8% (18/101) cats had IgM, and 33.7% (34/101) cats had IgG antibody only. None of the eight shelters sampled had Bartonella antibody-free cats. Although abnormal CD4:CD8 ratio was observed in 48/83 seropositive cats, flea infestation was the only significant risk factor observed in this study.
Conclusions
The present study provides the first comparison on the Bartonella spp. antigen, antibody status and CD4:CD8 ratio among shelter cats. The high B. henselae seropositivity among shelter cats presumably due to significant flea infestation triggers an alarm of whether the infection could go undetectable and its potential transmission to humans.
4.A comparison of medication adherence between subsidized and self-paying patients in Malaysia

Background: Limited efforts have been made to evaluate medication adherence among subsidized
and self-paying patients.
Objective: To investigate medication adherence among patients with and without medication
subsidies and to identify factors that may influence patients’ adherence to medication.
Setting: Government healthcare institutions in Kuala Lumpur, Selangor, and Negeri Sembilan and
private healthcare institutions in Selangor and Negeri Sembilan, Malaysia.
Methods: This cross-sectional study sampled patients with and without medication subsidies
(self-paying patients). Only one of the patient’s medications was re-packed into Medication Event
Monitoring Systems (MEMS) bottles, which were returned after four weeks. Adherence was
defined as the dose regimen being executed as prescribed on 80% or more of the days. The factors
that may influence patients’ adherence were modelled using binary logistic regression.
Main outcome measure: Percentage of medication adherence.
Results: A total of 97 patients, 50 subsidized and 47 self-paying, were included in the study.
Medication adherence was observed in 50% of the subsidized patients and 63.8% of the self-paying
patients (χ²=1.887, df=1, p=0.219). None of the evaluated variables had a significant influence on
patients’ medication adherence, with the exception of attending drug counselling. Patients who
attended drug counselling were found to be 3.3 times more likely to adhere to medication than
those who did not (adjusted odds ratio of 3.29, 95% CI was 1.42 to7.62, p = 0.006).
Conclusion: There is no significant difference in terms of medication adherence between
subsidized and self-paying patients. Future studies may wish to consider evaluating modifiable
risk factors in the examination of non-adherence among subsidized and self-paying patients in
Malaysia.
5.Mitochondrial 10398A>G NADH-Dehydrogenase Subunit 3 of Complex I Is Frequently Altered in Intra-Axial Brain Tumors in Malaysia.

BACKGROUND: Mitochondria are major cellular sources of reactive oxygen species (ROS) generation which can induce mitochondrial DNA damage and lead to carcinogenesis. The mitochondrial 10398A>G alteration in NADH-dehydrogenase subunit 3 (ND3) can severely impair complex I, a key component of ROS production in the mitochondrial electron transport chain. Alteration in ND3 10398A>G has been reported to be linked with diverse neurodegenerative disorders and cancers. The aim of this study was to find out the association of mitochondrial ND3 10398A>G alteration in brain tumor of Malaysian patients. METHODS: Brain tumor tissues and corresponding blood specimens were obtained from 45 patients. The ND3 10398A>G alteration at target codon 114 was detected using the PCR-RFLP analysis and later was confirmed by DNA sequencing. RESULTS: Twenty-six (57.8%) patients showed ND3 10398A>G mutation in their tumor specimens, in which 26.9% of these mutations were heterozygous mutations. ND3 10398A>G mutation was not significantly correlated with age, gender, and histological tumor grade, however was found more frequently in intra-axial than in extra-axial tumors (62.5% vs. 46.2%, p < 0.01). CONCLUSION: For the first time, we have been able to describe the occurrence of ND3 10398A>G mutations in a Malaysian brain tumor population. It can be concluded that mitochondrial ND3 10398A>G alteration is frequently present in brain tumors among Malaysian population and it shows an impact on the intra-axial tumors.
