MARKED REDUCTION IN HALLUCINATION RATES WITH GPT-5: A POSITIVE DEVELOPMENT FOR MEDICAL AND SCIENTIFIC WRITING
We read with great interest the letter by Jamaluddin et al., which highlighted hallucination as a
key challenge in artificial intelligence (AI)-generated writing, particularly in academic and clinical
contexts.1
Since the publication of their correspondence, developments in large language models
(LLMs) have progressed rapidly, with newer systems demonstrating measurable improvements in
accuracy. In light of recent performance data for GPT-5, it is timely to revisit this discussion – not
to reiterate the risks alone but to acknowledge the notable reduction in hallucination rates and to
consider its positive implications for medical and scientific writing.

Leishmaniasis causes significant morbidity and mortality worldwide. In our country, there has been a
significant increase in the number of cases of leishmaniasis in the last decade. In our study, the effects
of Hypericum thymbrifolium, Hypericum scabrum and Eryngium creticum plant extracts were tested on
Leishmania major, Leishmania tropica and Leishmania infantum/donovani, which were clinically resistant
by not responding to Glucantime® therapy. Cytotoxicity of these extracts were evaluated by XTT method
in the human fibroblast cell line. Possible active ingredients were detected by GC-MS analysis from plant
extracts. Glucantime® resistance was detected at concentrations of 50 µg/mL and lower in 4 of the 7
strains tested. No living leishmania parasites were found in leishmania strains treated with plant extracts
at concentrations of 100 µg/mL or higher. The concentrations of plant extracts included in the study
on the WI-38 human fibroblast cell line were not cytotoxic. According to the GC-MS analysis, several
active substances with biological activities and anti-parasitic effects, such as Thiophene, Germacrene-D,
trans-Geranylgeraniol, Pyridine, and Maleimides, were identified. Based on the findings of the study, it
is believed that these identified active substances when supported by in-vivo studies, will pave the way
for future research and have the potential to be developed as anti-leishmania drugs.

Background:
This study evaluated national trends in cemented and uncemented reverse shoulder arthroplasty (RSA) for proximal humerus fractures using a comprehensive national surgical database. This study aimed to compare RSA used in the treatment of proximal humerus fractures with the literature and to determine the country's trend.
Methods:
A cross-sectional study was conducted using the health records of individuals aged ≥ 18 years who underwent RSA for proximal humerus fractures between 2016 and 2022. Patients were divided into cemented and uncemented groups, and demographic data (age, sex), duration of hospital stay, transfusions, revisions, mortality, and Charlson Comorbidity Index (CCI) scores were analyzed.
Results:
A total of 618 cemented RSA and 1,364 uncemented RSA procedures were reviewed. Patients who underwent cemented RSA were significantly older than those who had uncemented RSA (p = 0.002). Transfusion rates were higher in the cemented RSA group (p = 0.006). The frequency of revision surgery was 6.1%. Younger age and male sex were associated with revision (p < 0.001). CCI scores were higher among transfused patients than non-transfused patients (p < 0.001). The incidence of cemented RSA was 11.7% and 49% in 2016 and 2022, respectively. Differences were found among hospital types and geographical regions.
Conclusions
While cemented RSA has been gaining attention and increased application in recent years for proximal humerus fractures, uncemented RSA still predominates. The choice between these 2 methods is largely influenced by regional and hospitallevel factors. The type of RSA and high CCI scores were found to have no significant impact on the risk of surgical revision.

Breast cancer (BC) is a significant threat to female health, with both modifiable andnon-modifiable risk factors. It is essential to monitor patients regularly and to raise population awareness. Increasing research also suggests that E-selectin (SELE) may increase tumor angiogenesis and the development of cancer. This study investigated SELE single-nucleotide polymorphisms (SNPs) in the following positions: rs5367T/C, rs5368C/T, rs5362T/G,and rs5362T/C. Using polymerase chain reaction, significant differences in allele and genotype frequencies were found between BC patients and controls. Position rs5368 was associated with an increased risk of BC for the CT and TT genotypes, with odds ratios (ORs) of16.3 and 6.90 (Fisher probability = 0.0001, p = 0.005). Women with the T allele had a 19.3-fold higher incidence of BC, while allele C may be a protective allele against BC (OR, 0.05).Heterozygous genotypes at rs5367, rs5362, and rs5362 were significantly more common inBC patients, with ORs of 5.70, 4.50, and 3.80, respectively. These SNPs may be associatedwith the risk of BC, because the frequency of mutant alleles was significantly higher in patients (OR: 4.26, 3.83, and 4.30, respectively) than in controls (OR: 0.23, 0.30, and 0.20, respectively). These SNPs may be considered a common genotype in the Iraqi population, with the wild-type allele having a protective fraction and the mutant allele having an environmental fraction. The results also revealed a 2-fold increase in gene expression in BCpatients compared to controls, with a significant effect (p = 0.017). This study's findingsconfirm the importance of SELE polymorphisms in cancer risk prediction.

According to long-term projections, by 2030, the world’s population is predicted to reach 7.5 billion individuals, and there will be roughly 27 million new cancer cases diagnosed. The global burden of breast cancer (BC) is expected to rise. According to the Ministry of Health-Iraqi Cancer Registry, cancer is the second largest cause of death after cardiovascular disease. This study investigated the interleukin-18 (IL18) single-nucleotide polymorphisms (SNPs) –607C/A rs1946518 and –137G/C rs187238 using the sequence-specific amplification-polymerase chain reaction approach. Regarding the position –607C/A, there was a highly significant difference between the observed and expected frequencies in patients and controls (χ 2 = 3.16 and χ 2 = 16.5), respectively. The AA and CA genotypes were associated with significantly increased BC risk (odds ratio [OR], 3.68; p = 0.004 and OR, 2.83; p = 0.04, respectively). Women with the A allele had a 5.03-fold increased susceptibility to BC. The C allele may be a protective allele against BC (OR, 0.19). Although position –137G/C showed no significant differences in the CC genotype distribution (p = 0.18), the frequency of the CC genotype was significantly higher in patients than in controls. In contrast, patients had a significantly higher frequency of GC genotypes than controls (p = 0.04), which was associated with an increased risk of developing BC (OR, 2.63). The G allele frequency was significantly lower in patients than in controls (55.0% vs. 76.2%, respectively). This SNP may be considered a common genotype in the Iraqi population, with the wild-type G allele having a protective function (OR, 0.19) and the mutant C allele having an environmental effect (OR, 2.63).

Introduction: Pressure ulcers (PUs) are a leading health problem among spinal cord injury(SCI) patients. Therefore,
this comprehensive study was aimed at determining the association between potential factors and the occurrence
of PUs among SCI adults in community settings. Methods: A cross-sectional study was conducted among adults
with SCI aged 18 years and above from five selected locations in Kuala Lumpur, Malaysia. The study was carried
out through interviews based on validated questionnaires such as the Dietary History Questionnaire (DHQ), Spinal
Nutrition Screening Tool (SNST), Physical Activity Scale for Individuals with Physical Disabilities (PASIPD), Barthel
Index for Activities of Daily Living (ADL), and Instrumental Activities of Daily Living (IADL). Estimated weight and
height were obtained using recumbent formula. The stages of their PUs were assessed based on National Pressure
Ulcer Advisory Panel (NPUAP) grading system. Results: A total of 40 subjects participated in the study, 25% of
whom were having PUs. Their nutritional inadequacies were identified, whereby 90%, 70% and 85.5% of them
were receiving below their daily requirement of energy, carbohydrate and protein, respectively. The fat and sodium
intakes of 55% of them were above the Malaysian Recommended Nutrient Intakes(RNI). It was successfully found
that physical activity could decrease the risk of PUs(R2=0.654, OR=0.310, p<0.05). Conclusion: Overall, 25% of
the subjects had PUs that were associated with physical inactivity. The majority of the subjects did not meet their
nutrient requirements. Therefore, there is a need for increased physical activity and improved nutrient intake in order
to reduce the risk of pressure ulcers among SCI adults in communities.

Objective: To determine the prevalence of the filarial parasites,. ie.,. Brugia malayi, Brugia, Brugia pahangi(B. pahangi), Dirofilaria immitisand. Dirofilaria repens (D. repens) in domestic and stray cats. Methods: A total of 170 blood sample were collected from domestic and stray cats and examined for filarial worm parasites in two localities, Pulau Carey and Bukit Gasing, Selangor State, Malaysia. Results: The overall prevalence of infection was 23.5% (40/170; 95% CI = 17.4-30.6). Of this, 35% (14/40; 95% CI = 22.1-50.5) and 50% (20/40; 95% CI = 35.2-64.8) were positive for single B. pahangi nd D. repens, respectively. The remaining of 15% (6/40; 95% CI = 7.1-29.1) were positive for mixed B. pahangi and D. repens. In addition, 75% of the infected cats were domestic, and 25% were strays. No Brugia malayi and Dirofilaria immitis was detected. Eighty-four cats were captured at Pulau Carey, of which 35.7% (30/84) were infected. Among the cats determined to be infected, 93% (28/30; 95% CI = 78.7-98.2) were domestic, and only 6.7% (2/30; 95% CI = 19.0-21.3) were strays. Conversely, the number of infected cats was three times lower in Bukit Gasing than in Pulau Carey, and most of the cats were stray. Conclusions: B. pahangi and D. repens could be the major parasites underlying filariasis in the study area. Adequate prophylactic plans should be administrated in the cat population in study area.

The Medical Journal of Malaysia 2012;67(2):199-203
Mixed-genotypes hepatitis C virus (HCV) infections are
normally ignored in chronic hemodialysis patients. The aim
of this study is to investigate the prevalence of mixedgenotypes infections among hemodialysis patients in Pahang province, Malaysia. Reverse-transcription and polymerase chain reaction methods were performed using two different sets of primers, targeting the 5’ untranslated region and nonstructural 5B region. Target region base sequences were obtained by direct sequencing. Discrepancy in outcomes from phylogenetic analysis of both regions suggests double infections. Of 40 subjects in eight hemodialysis centres, evidence of mixed-genotypes infections was found in 5 subjects (12.5%) from three different centres. Four patients were infected with mixed genotypes 3 and 1 and one with genotypes 3 and 4. Cases of mixed HCV genotypes infection were considered high among hemodialysis patients in Pahang. However, further investigation is needed to confirm
whether they are true mixed infections or perhaps infection
with recombinant virus and also to assess the
clinicopathologic characteristics of the infection.
