1.Evaluation of changes in human salivary amylase enzyme activity

Background:
Saliva as a non-invasive biological sample can be a game-changer in early disease detection and health risk assessment.
Objective:
To examine the association between participants' dietary patterns and the activity of salivary amylase, along with serum amylase levels in humans.
Materials and methods:
This study was conducted at the research laboratory of the Department of Biochemistry, School of Biomedicine, MNUMS. A total of 30 students aged 19–22 years participated in the study. Saliva samples were collected three times at one-week intervals, and one blood sample was collected from each participant, alongside a dietary questionnaire. The activity of the amylase enzyme in both saliva and serum samples was determined using the iodine-starch method.
Results:
When evaluating the amylase enzyme activity based on participants' carbohydrate intake, the result was p > 0.05, indicating no statistically significant difference. Similarly, statistical analysis of the use of mouthwash and vitamin supplements also showed p > 0.05, which means these variables had no statistically significant effect on amylase activity. The correlation between salivary and serum amylase activity was found to be r = 0.365, indicating a weak positive correlation, but the difference was not statistically significant.
Conclusion
The intake of carbohydrates, vitamins, and mouthwash does not significantly affect the activity level of the salivary amylase enzyme, according to research findings. However, external factors such as stress and air pollution have been shown to exert a measurable influence on its activity. A comparative analysis of enzyme levels in saliva and blood using amylase as a representative marker revealed similar activity levels in both fluids. This suggests that saliva may serve as a viable non invasive sample for detecting various biomarkers and diagnosing diseases. The results underscore the potential of salivary components, particularly amylase, as valuable indicators in diagnostic applications.
2.Impact of storage conditions on the antioxidant capacity of food plants

Background:
Due to Mongolia’s harsh climate and seasonal limitations in fresh food supply, fruits, vegetables, and medicinal plants are often consumed in preserved forms. However, the preservation methods and storage conditions may significantly alter their antioxidant activity, which is crucial for mitigating oxidative stress and preventing chronic diseases.
Objective:
This study aimed to evaluate the antioxidant activity of 19 commonly consumed vegetables, berries, and dried medicinal plants under different storage conditions including fresh, cold storage (cellar), and frozen (-20°C).
Methods:
Samples were extracted in 80% methanol and tested using the DPPH radical scavenging assay. Absorbance was measured at 517 nm using a UV-Vis spectrophotometer. IC⁻⁻ values were calculated to compare antioxidant potency. Statistical differences were assessed using paired and unpaired t-tests with SPSS v27 (p<0.05)
Results:
Cold storage significantly reduced antioxidant activity in root vegetables, with IC⁻⁻ values increasing by 2.4 to 13.5 times (p<0.01), indicating diminished radical scavenging potencial. In contrast, frozen samples showed minimal change (p>0.05). Dried medicinal plants such as Rosa canina and Thymus serpyllum maintained strong activity, with IC⁻⁻<50 μg/mL.
Conclusion
Cellar storage leads to a notable decline in antioxidant capacity of common vegetables, while freezing is a more effective method for preservation. Dried medicinal herbs remain potent sources of antioxidants and may be recommended for year-round use in Mongolian diets.
3.Association between lower back pain and some inflammatory biomarkers among heavy machinery operators in open-pit mining

Background:
Musculoskeletal disorders account for 23.1-47.1% of occupational diseases in several countries. Studies
have shown that operators of heavy machinery, including tractors and dump trucks, are twice as likely to experience lower back pain compared to workers not exposed to whole-body vibration. Furthermore, research has indicated that acute
exposure to vibration can cause vasoconstriction and vascular inflammation. However, limited research has explored the
relationship between lower back pain and specific biomarkers, highlighting the need for this study.
Aim:
This study aimed to compare lower back pain prevalence and muscle inflammation biomarkers among heavy machinery operators.
Materials and Methods:
A purposive sampling method was used to recruit 15 male participants aged 25-35 years who
had worked as heavy machinery drivers for no more than three years. Inclusion criteria were: no alcohol consumption
within 24 hours prior to sampling, body mass index (BMI) between 18.5-28.9 kg/m², no prior diagnosis of musculoskeletal disorders, and absence of infectious or non-infectious diseases during the study period. Blood samples were analyzed
for Interleukin-6 (IL-6) and Tumor Necrosis Factor-alpha (TNF-α) levels using enzyme-linked immunosorbent assay
(ELISA).
Results:
The mean BMI of participants was 25.89±3.23 kg/m². Over half (53.3%, n=8) exceeded the exposure limit
for whole-body vibration. Low back pain was reported by 13 participants (86.7%) over the past six months and by 12
participants (80%) over the past seven days. TNF-α levels did not differ significantly between groups based on low back
pain status or vibration exposure. However, IL-6 levels showed a significant increase 24 hours after whole-body vibration
exposure (p=0.027).
Conclusion
Lower back pain was highly prevalent among participants exposed to whole-body vibration. Furthermore,
IL-6 levels were elevated among participants reporting lower back pain, regardless of vibration exposure levels.
4.The Correlation Between Prognostic Indicators of Chronic Liver Diseases and Certain Blood Test Parameters

Background:
Hepatocellular carcinoma (HCC) is a primary liver cancer originating from liver cells, classified as a chronic
liver disease. This cancer ranks third in the world in terms of mortality rate. The MELD (Model for End-Stage Liver
Disease) and Child-Pugh scoring systems are utilized to assess the prognosis of chronic liver diseases. Based on studies
suggesting that certain blood test indicators, particularly red cell distribution width (RDW), could be used to predict the
prognosis of liver cancer and other cancers, as well as serve as diagnostic markers, this topic was chosen to evaluate the
clinical significance of RDW in hepatocellular carcinoma.
Aim:
The aim is to study some blood test indicators and compare them with the MELD score and Child-Pugh score systems
in order to determine the prognosis of chronic liver diseases.
Materials and Methods:
A retrospective, single-center, cross-sectional study was conducted at Mongolia-Japan Hospital.
Among 322 patients diagnosed with HCC, 24 patients were selected for the case group, and 37 patients with liver cirrhosis
were included in the control group.
Results:
According to the research criteria, 61 patients were selected and divided into 3 groups, and statistical analysis
was performed. In the detailed blood test, platelet count and WBC count showed statistically significant differences
among the 3 groups (p< 0.024). In the biochemical tests, C-reactive protein (CRP) was p< 0.018, total bilirubin p< 0.001,
and the mean albumin level p< 0.015, all showing statistically significant differences among the 3 groups. A statistically
significant inverse correlation was observed between RDW-CV and the clinical MELD score (r=-0.356).
Conclusion
Platelet count, RDW, CRP, total bilirubin, and average albumin levels are significantly different across the
studied groups. RDW-CV shows a moderate inverse correlation with MELD scores, suggesting its potential as a prognostic
marker in chronic liver diseases. Further research with larger sample sizes is recommended to confirm these findings.

Background:
The synthesis of multifunctional nanoparticles by integrating the bioactive properties of the ethanol extract of Urtica dioica L. a medicinal plant widely distributed in Mongolia, with zinc oxide nanoparticles (ZnO-NP) serves as the foundation of the present study. The aim is to produce nanoparticles with synergistic antioxidant, anti-inflammatory, antibacterial, and anticancer activities.
Objective:
To synthesise dual-action nanoparticles (NPs) by integrating ZnO with the extract of Urtica dioica L. and to characterise their properties.
Materials and Methods:
The Control group, as ZnO-NPs, and the study group, as medicinal plant ethanol extraction loaded nanoparticles (UD-ZnO-NPs), were synthesised using green synthesis techniques. The morphology and particle size were characterised by Scanning Electron Microscopy (SEM), chemical bonding was analysed using Fourier Transform Infrared Spectroscopy (FTIR), and crystalline structure was examined by X-ray Diffraction (XRD). Hemolytic activity assays were conducted to assess cytotoxicity.
Results:
The Control and study group’s morphology and size distribution were uniform and spherical. The average particle size of the study group (UD-ZnO NPs) was 63 nm, while the control group (ZnO-NPs) was 77 nm. FTIR analysis showed that the basic chemical bonds in both types of nanoparticles were similar; however, additional peaks corresponding to the bioactive compounds from the Urtica dioica extract were detected in the UD ZnO-NPs. XRD analysis revealed that both types of ZnO-NPs investigated the same crystalline structure, consistent with the standard reference data (JCPDS No. 36 1451). Hemolysis assays showed that at concentrations ranging from 0.5 to 2.5 mg/ mL, the hemolytic activity was below 5%, indicating low cytotoxicity.
Conclusion
ZnO-NPs with and without Urtica dioica extract were successfully synthesized via a green method, yielding spherical, uniformly dispersed particles ranging from 63 to 77 nm in size. While the structural and crystalline characteristics of the NPs remained consistent, the presence of bioactive compounds was confirmed in the UD-ZnO-NPs. Hemolytic assays indicated dose-dependent cytotoxicity, highlighting the importance of concentration in biomedical applications.

Background:
Identifying reliable biomarkers for early detection, risk stratification, and prognosis of CVD in the context of CKD is, therefore, of critical importance. Cystatin C has emerged as a potential biomarker capable of reflecting both cardiac injury and renal impairment, particularly in patients with arterial hypertension. This study aimed to evaluate the association between serum cystatin C levels, left ventricular hypertrophy, and chronic kidney disease in individuals with hypertension.
Objective:
To assess serum cystatin C concentrations in patients with left ventricular hypertrophy and chronic kidney disease secondary to arterial hypertension.
Materials and Methods:
A case-control analytical study was conducted, enrolling 44 patients aged 45 years or older with both left ventricular hypertrophy and chronic kidney disease due to arterial hypertension alongside a control group of apparently healthy individuals. Serum cystatin C levels were measured using immunoturbidimetric assay. Statistical analysis was performed using SPSS version 25.0. Group comparisons were made using independent-sample t-tests, while multivariate regression and receiver operating characteristic (ROC) analyses were employed to explore associations and the predictive value of cystatin C.
Results:
The mean serum cystatin C concentration in the case group was 1.6±0.1 mg/L, significantly higher than in the control group (0.88±0.03 mg/L, p<0.05). Similarly, the estimated glomerular filtration rate (eGFR) was markedly reduced in the case group (44.88±6.8 mL/min/1.73 m²) compared to the controls (92.88±3.4 mL/ min/1.73 m², p<0.05). In the case group, a statistically significant inverse correlation was observed between serum cystatin C levels and glomerular filtration rate (GFR), with a regression coefficient of β=−0.028 (p<0.006).
Conclusion
The elevated serum cystatin C levels (1.6±0.1 mg/L) and decreased eGFR (38.99±12.7 mL/min/1.73 m²) observed in the case group suggest that cystatin C may serve as a potential biomarker for the early diagnosis of left ventricular hypertrophy due to arterial hypertension and chronic kidney disease, as well as for predicting related complications.

Background:
Regularly participate international High-level in sports athletes national and competitions and engage in intense training, developing endurance and resilience. Measuring blood lactate levels is crucial for improving an athlete’s performance, assessing sports performance, and enhancing the effectiveness of future training.
Aim:
To study the relationship between lactate levels in the blood plasma and lactate dehydrogenase enzyme activity in Mongolian National Team athletes.
Materials and Methods:
The study involved 51 athletes from the Mongolian National Team. Anaerobic capacity was assessed using a Monark 894E Ergomedic Peak Bike, designed to apply exercise load. Blood serum lactate level and lactate dehydrogenase enzyme activity were determined using a Biobase BK-280 fully automated biochemical analyzer. Heart rate, peripheral blood oxygen levels, and oxygen saturation were measured using a pulse oximeter.
Results:
The average age of the participants was 24.04 ± 4.15 years, with an average height of 168 ± 8.78 cm and an average weight of 71.01 ± 7.69 kg. The average BMI was 24.82 ± 4.12 kg/m². Pre exercise lactate levels averaged 3.84 ± 0.75 mmol/L, while post-exercise lactate levels averaged 9.67±3.52 mmol/L. The average heart rate before exercise was 66.04±8.9 bpm, while post-exercise heart rate was 123.6±16.06 bpm. The average VO₂ max was 95.18±2.48.
Conclusion
The lactate levels before and after exercise among the athletes participating in the study showed significant differences in the age groups 20-29 (p<0.0001). When comparing lactate levels before and after exercise by sport, statistically significant increases were observed in freestyle wrestling and judo athletes (p<0.0001)
8.Prevalence of Micronutrient Deficiencies Among Mongolian Pregnant Women

Background:
Vulnerable populations at risk of micronutrient deficiencies include children and women,
especially pregnant women, lactating mothers, infants and young children. According to
WHO guidelines, a population with a micronutrient (vitamin and mineral) deficiency of more
than 20% is considered a public health problem.
Although Mongolia has made significant progress in improving the nutritional status of the
population, especially children and women, the 2024 VI survey found that micronutrient
deficiencies are high among pregnant women.
The MMS (Multi-ingredient Micronutrient Supplement) distributed to pregnant women by
soum and household health centers contains all the essential vitamins and minerals for
pregnant women, but coverage has remained inadequate for more than 10 years.
Goal:
To establish the prevalence of micronutrients deficiency in pregnant women
Materials and Methods:
Totally 2796 pregnant women included in cross-sectional study from urban and rural
area. Survey procedures consisted of interview, anthropometric measurements, clinical
examinations, and the collection of biological (blood and urine) samples for pregnant
women. Micronutrient deficiencies were assessed by tests for hemoglobin, serum, ferritin,
soluble transferrin receptor (sTfR), 25-hydroxyvitamin D {25(ОН)D}, spot urine samples for
determination of urinary iodine concentration.
Statistical processing :
The data from the survey questionnaires were entered into a database created in the
“EpiData 3.1” software environment. The data created and verified in the EpiData software
environment were converted to the “SPSS–25” program, and statistical analysis was also performed using the SPSS-25 program. The results table was prepared using Microsoft
Word 2021 and Microsoft Excel 2021 programs.
Research ethics:
The research methodology was discussed at the meeting of the Medical Ethics Review
Committee under the Ministry of Health on 2023.03.27 (23/017), and permission to conduct
the study was obtained
Result :
The prevalence of anemia, as measured by hemoglobin, was 22.3% and the iron deficiency
anemia (IDA), as measured by adjusted serum ferritin or soluble transferrin receptor (sTfR)
was (39.8%/9.0%). Prevalence of iron deficiency as measured by serum ferritin was also
highest in Western 41.9% and lowest in Khangai region 40.2% (P<0.01). The prevalence of
vitamin D deficiency among pregnant women was high at 92.5%, and 7.5% of all pregnant
women had adequate vitamin D status. The median concentration of 76.4µg/l indicates
inadequate iodine status in pregnant women, as the desired range for adequate iodine
nutrition in pregnancy 150-249 µg/l.
Conclusions
1. 22.3 percent of pregnant women are anemic, and the prevalence of anemia has not
decreased from 2017 levels and remains common.
2. The prevalence of iron deficiency (as measured by ferritin levels) was 39.8 μg/L and the
soluble transferrin receptor level was 9.0 μg/L, which is higher than previous studies.
3. The prevalence of vitamin D deficiency among pregnant women remains common, with
76.6% having severe or moderate deficiency, and 15.9% having mild deficiency.
9.Qualitative studies on raw materials for Hepaclin-4 prescription

Background:
In 2021, 5981 of cancer new cases was registered in Mongolian population. Among those cases, liver cancer was commonly registered with a prevalence of 32.7%. Studies on anticancer agents with no-adverse effects and good-preventive efficacy against cancer have been attracted more attention from the researchers in the field of pharmaceutical sciences. Scutellaria baicalensis Georgi, Saussurrea amara.L, Chiazospermum erectum Berh, and Carthamus tinctorius.L are well recognized as effective agent against liver diseases. Using these raw materials, researchers have been invented a traditional prescription and named as Hepaclin-4. In this study, we aimed to investigate the qualitative study of raw materials and some biologically active sub- stances in the compounds.
Purpose:
To study the qualitative study of raw materials for Hepaclin-4 prescription
Materials and methods:
Some qualitative properties of raw materials for Hepaclin-4 prescription, including appearance, minerals, some organic compounds, total ash, water-soluble substances and fungi, were investigated according to Mongolian pharmacopeia and total flavonoid was detected by thin layer chromatography.
Results:
No changes were observed on the appearance of raw materials, and minerals and organic compounds weren’t detected in the prescription. No contamination with fungi and insects were identified. The moist in the raw materials were 5.9 to 8.1%, total ash was 4.7 to 13.3% and the water-soluble substances were detected 33.8 to 42.9%. Number of aerobic bacteria, fungi and E.coli, Salmonella species were detected in normal range, indicating that the prescription was matched with the requirement of pharmacopeia. According to the thin layer chromatography study of the raw materials, a yellow spot on the chromatogram were identified and same as quercetin (Rf=0.9-0.98) and rutin ((Rf=0.18-0.23)) as standard compounds, which indicated that the spot which indicated that the spot was flavonoids in the prescription.
Conclusions
These results showed that the appearance, moist, minerals, organic compound, water-soluble substances, ash and biologically active substances of the raw materials for Hepaclin-4 prescription was corresponded with the requirements of pharmacopeia, and flavonoid was detected in raw materials of Hepaclin-4.
10.A new diagnostic biomarker in early detection of Hepatocellular Carcinoma

Background and Aims:
Hepatocellular carcinoma (HCC) is a common cause of cancer related death
in Mongolia. Early diagnosis is the very important management to increase successful treatment
and survival rate. Glypican-3 (GPC3) protein is highly expressed in hepatocellular carcinoma (HCC)
tissue and in serum of HCC patients. Recent studies have been conducted and suggested as a
diagnostic biomarker for detecting HCC in the early stage. Therefore, we investigated the diagnostic
value of the serum GPC3 level and compared it to the alpha-fetoprotein (AFP) level as a diagnostic
biomarker of HCC.
Methods:
We enrolled a total of 90 participants and divided into 3 groups with HCC (30), with liver
cirrhosis (LC/30) and healthy (30) as the control group (30). GPC3 and AFP serum (sGPC-3, sAFP)
levels were measured using commercially available enzyme-linked immunosorbent assay kits. The
diagnostic accuracy was analyzed using the receiver operating characteristics (ROC) curve and
estimated sensitivity and specificity of each biomarker.
Results:
sGPC3 was significantly elevated in the HCC group as compared to liver cirrhosis and
healthy subjects (658±138.2 pg/ml, 378±25.5 pg/ml, 356.3±29 pg/ml) respectively. sGPC-3 sensitivity
was 96.6% and specificity was 100%. The area under the ROC curve (AUC) for GPC3 was 0.999
(0.996- 1.0).
In comparison, the mean of AFP was significantly higher in HCC (16.9±11.7 ng/ml) than in LC (6.7±7.6
ng/ml) and in healthy subject (3.3±2.1 ng/ml) and AFP sensitivity was 43,3 %, specificity was 95 %
with an AUC of 0.808 (0.696- 0.921).
The combination of GPC-3 with AFP achieved the highest sensitivity (97.1%) and specificity (97%).
Conclusion
Serum GPC3 has a higher sensitivity than AFP for the early diagnosis of HCC.
Combination of two markers showed greatest diagnostic accuracy.
