As the brain's resident immune cells, microglia perform crucial functions such as phagocytosis, neuronal network maintenance, and injury restoration by adopting various phenotypes. Dynamic imaging of these phenotypes is essential for accessing brain diseases and therapeutic responses. Although numerous probes are available for imaging pro-inflammatory microglia, no PET tracers have been developed specifically to visualize anti-inflammatory microglia. In this study, we present an 18F-labeled PET tracer (QTFT) that targets the P2Y12, a receptor highly expressed on anti-inflammatory microglia. [18F]QTFT exhibited high binding affinity to the P2Y12 (14.43 nmol/L) and superior blood-brain barrier permeability compared to other candidates. Micro-PET imaging in IL-4-induced neuroinflammation models showed higher [18F]QTFT uptake in lesions compared to the contralateral normal brain tissues. Importantly, this specific uptake could be blocked by QTFT or a P2Y12 antagonist. Furthermore, [18F]QTFT visualized brain lesions in mouse models of epilepsy, glioma, and aging by targeting the aberrantly expressed P2Y12 in anti-inflammatory microglia. In a pilot clinical study, [18F]QTFT successfully located epileptic foci, showing enhanced radioactive signals in a patient with epilepsy. Collectively, these studies suggest that [18F]QTFT could serve as a valuable diagnostic tool for imaging various brain disorders by targeting P2Y12 overexpressed in anti-inflammatory microglia.

Objective:To evaluate the short-term efficacy of surgical treatment for severe pulmonary stenosis(PS) in infants and young children, and to clarify the impact of different surgical timings on the efficacy of PS treatment. Methods:A retrospective analysis was conducted on 24 infants and young children who underwent surgical treatment for severe PS at the General Hospital of Northern Theater Command, PLA, from January 1, 2020, to October 1, 2024. Among them, 13 were males and 11 were females. The average weight of the patients was(9.2±3.6) kg, the average gestational age was(39.3±1.7) weeks, and the average age was(15.0±13.5) months. Preoperative clinical symptoms and signs(e.g., cyanosis, shortness of breath), transpulmonary valve pressure gradient(TPVPG), right ventricular systolic pressure(RVSP), and Em/Am were recorded. The patients were divided into two groups based on surgical timing: the infant group(under 1 year old, n=12) and the toddler group(1-3 years old, n=12).Results:There were no deaths among all patients. The postoperative ICU stay was significantly longer in the infant group compared to the toddler group( P<0.05). Compared to preoperative values, surgical treatment significantly improved TPVPG, reduced RVSP, and enhanced right ventricular diastolic function, which stabilized by 3 months postoperatively( P<0.05). Intergroup comparisons revealed that the infant group had significantly lower TPVPG at 6 months postoperatively compared to the toddler group. Additionally, right ventricular diastolic function improved significantly in the infant group postoperatively, while no significant improvement was observed in the toddler group. Younger age and reduced right ventricular diastolic function were identified as major risk factors for prolonged mechanical ventilation(>24 hours). Conclusion:Surgical treatment for PS during infancy and early childhood is safe and effective. Comprehensive preoperative evaluation is crucial, and early surgical intervention is recommended for patients with impaired right ventricular function to improve prognosis.

Objective:To summarize the best evidence of intracavitary electroencephalogram positioning in peripherally inserted central catheter (PICC) catheterization, and investigate the clinical application of the best evidence. Methods:The literature on the intracavitary electroencephalogram positioning technology in the head end positioning of PICC catheterization was searched through computers on websites or data bases such as Cochrane Library, Infusion Nursing Society, PubMed, ScienceDirect, China National Knowledge Infrastructure, WanFang Data, and VIP. The search period was from the establishment of the database to June 30, 2022. The literature evaluation standards and evidence grading system of the Joanna Briggs Institute Evidence-Based Health Care Center Database were used for literature quality evaluation and evidence grading, summarizing the best evidence. Based on the summary of evidence, the best evidence for intracavitary electrocardiogram positioning guided PICC catheterization was applied to the survey questionnaire. From November to December 2022, a snowball sampling method was used to select 119 PICC nurses for a questionnaire survey. Results:A total of 31 articles were included, consisting of 24 randomized controlled trials, two systematic reviews, four Meta-analysis, and one group standard. We summarized 20 pieces of evidence from 7 aspects, including preoperative evaluation and preparation, catheter delivery process, guidewire connection, guided intracavitary electroencephalogram, timing of catheter withdrawal, optimal position judgment, and effect evaluation. The survey results showed that more than half of the nurses were consistent with the evidence in terms of evaluation and preparation, catheter delivery process, guided intracavitary electroencephalogram, guidewire connection, and timing of catheter withdrawal. The selection of the optimal location judgment was relatively scattered and needed further research. Conclusions:This study provides evidence-based basis for the PICC catheterization positioning using intracavitary electroencephalogram. Based on clinical practice, specialized nurses select the best evidence and develop a management plan that suits individual circumstances, which can ensure maximum patient benefits and continuously improve nursing quality.

Radiation-induced brain injury (RBI) is the most serious complication of head and neck tumor after radiotherapy. The pathogenesis of RBI is complicated, and the clinical course is irreversible, while no effective treatment available. The activation of glial cells is one of the main theories of RBI, and the prevention and treatment of RBI by targeting glial cells is the focus of current research. As a post-transcriptional regulatory factor, microRNA (miRNA) has been confirmed to be involved in regulatingglial cell radiosensitivity, inflammation type transformation, autophagy, exosomatic, long non-coding RNA (lncRNA), circular RNA (circRNA) and other related pathways, thereby mediating the occurrence and development of cascade reaction of inflammatory injury and neurological function repair of central nervous system (CNS) disease. Therefore, the establishment of miRNA - glial regulatory network may provide a new strategy for the prevention and treatment of RBI.

Purpose To investigate the expression of gelsolin-like actin-capping protein (CapG) in gastric cancer and adjacent normal gastric tissues and its relationship with clinicopathological features and prognosis. Methods The immunohistochemical (IHC) assay was performed to detect the expression of CapG protein in 125 specimens of gastric cancer (study group) and 30 specimens of matched adjacent normal gastric tissues (control group). No patients had received chemotherapy or radiotherapy before surgery.Immunostaining was scored semiquantitatively by two independent observers who were blinded to the patients' outcome and other clinicopathological parameters. The correlation of the expression of CapG with clinicopathological characteristics and prognosis was analyzed. Results Cytoplasmic and nuclear CapG protein staining was detected in all gastric cancer tissues.CapG protein expression was absent or low in matched adjacent normal gastric tissues but significantly higher in gastric cancer tissues.Expression of CapG in the tumor was not significantly associated with gender, age, tumor size, tumor location (P > 0.05).However,elevated CapG expression was strongly correlated with the depth of invasion (P =0.044),lymph node metastasis (P =0.026),distant metastasis (P =0.001),and AJCC stage (P =0.012).Significant poorer overall survival was observed in the high CapG expression group compared with that of the low CapG expression group (P <0.001).Multivariate analysis showed overall survival correlated with the patients' tumor location, lymph node metastasis, distant metastasis,AJCC stage and the expression of CapG protein. Conclusion CapG is highly expressed in gastric cancer.Tumors with CapG up-regulation tend to have poorer prognosis, suggesting overexpression of CapG may contribute to the prediction of poorer prognosis and may be a potential therapeutic target for gastric cancer.
