Objective To investigate the effect of subanesthetic dose of esketamine on emergence agitation(EA)in patients undergoing laparoscopic hernia repair. Methods Seventy-two male patients who treated with lapa-roscopic hernia repair under general anesthesia were randomly divided into AS group(subanesthetic esketamine)and control group. In the AS group,0.2 mg/kg of esketamine was administered intravenously 30 minutes before the surgery ended, while the control group was given an equal volume of normal saline.Upon surgery completion, patients were transferred to PACU with endotracheal tube retained, and the time to extubation was recorded.Hemo-dynamic parameters were measured immediately after extubation and at 10 min,30 min,1 h,and 4 h thereafter.Patients' pain and sedation levels were assessed at the above time points using RASS and VAS,respectively. The incidence of EA was evaluated using the Confusion Assessment Method for the Intensive Care Unit(CAM-ICU).Observation duration in the PACU and recovery outcomes within 24 hours postoperatively were assessed via the QoR-40 and adverse events were recorded. Results Compared to those in the control group, patients in the AS group had higher HR and MAP at 10 min post-extubation,and the changes in HR and MAP over time were more stable(P<0.05).The RASS and VAS scores in the AS group were significantly lower than those in the control group at the time of extubation and all subsequent time points(P<0.05),both groups showed temporal changes in RASS and VAS scores(P<0.05),but the change process in the AS group was more stable(P<0.05).Postoperative extubation time,PACU observation duration, and adverse event rates(delirium, respiratory depression, nausea and vomiting)did not differ significantly between the two groups(P>0.05),while recovery quality was markedly better in the AS group(P<0.05).Conclusion Subanesthetic esketamine effectively alleviates pain and the incidence of EA,supports hemodynamic stability during PACU stay, and enhances recovery quality in patients undergoing laparo-scopic hernia repair, demonstrating clinical value.

In order to understand the characteristics of avian leukosis virus(ALV)infection and the polymorphisms of receptor genes Tva,Tvb and NHE1 sequences in different Qingyuan Ma chick-en breeding farms, the isolation and identification of exogenous ALV virus, receptor gene amplifi-cation and sequencing analysis were carried out in five Qingyuan Ma chicken breeding farms. The results of virus isolation and subtype identification showed that the positive rate of exogenous ALV virus isolation in farms A,B and C was less than 5%,among which there was ALV-J infection alone in farm A,mixed infection with ALV-J and ALV-K in farm B,ALV-K infection a-lone in farm C,and no exogenous ALV infection was detected in farm D and E.Polymorphism anal-ysis of recipient genes showed that there were different frequencies of Tvar3,Tvar4 and Tvbr3 re-sistance alleles in A-E farms, and the distribution frequency of Tvar3 was 0.2-0.6,the distribution frequency of Tvar4 was 0.3-0.7,and the distribution frequency of Tvbr3 was 0.1-0.7.In addition, there were Tvar5 resistance alleles in both B and D farms, with a distribution frequency of 0.2.A to-tal of 18 SNP mutations occurred in the NHE1 receptor gene sequence, and further analysis showed that positions 1 279,1 361,1 369,1 406,1 442,and 1 912 were non-synonymous mutations, which could cause changes in amino acids. The study suggested that there were differences in the exogenous ALV and its subtypes among the five Qingyuan Ma chicken breeders, and each farm should have a more targeted and unique purification strategy. The distribution of Tva and Tvb re-sistance alleles at different frequencies, along with the occurrence of 6 non-synonymous mutations in the NHE1 gene, indicate that Qingyuan Ma chicken have the potential of genetic resistance breeding.

Objective To compare the main pharmacodynamic actions between Siji Sanhuang Capsules (SSC) and Siji Sanhuang Pills (SSP). Methods Antipyretic, anti-inflammatory and antibacterial effects of SSC and its effect on the intestinal movement in normal mice and constipation mice were observed. Results SSC could relieve fever in rats induced by the fresh beer yeast and 2,4-dinitrophenol, significantly inhibit the auricular swelling induced by xylol and the increased capillary permeability caused by histamine in mice, and promote the intestinal movement in normal mice and constipation mice. In-vitro antibacterial experiments showed that SSC had an obvious bacteriostatic effect on Staphylococcus aureus, typhoid bacillus, Bacillus coli, Shigella flexneri, pseudomonas aeruginosa and Bacillus proteus. The effects were in a dose-dependent manner and the effects of SSC were stronger than those of SSP. Conclusion SSC exerts a Strong antipyretic, anti-bacterial, anti-inflammatory and carthartic effect.
