1.Effects of Xinnao Maikang Regulating PPARγ/LXRα/ABCA1 Pathway Mediating Lipid Metabolism on ApoE-/-Atherosclerosis Mice

Objective To explore the potential mechanism of Xinnao Maikang in improving ApoE-/-atherosclerosis mice based on PPARγ/LXRα/ABCA1 pathway.Methods Totally 50 ApoE-/-mice were randomly divided into model group,atorvastatin group,Xinnao Maikang high-,medium-and low-dosage groups,and fed with high-fat diet for 12 weeks to establish atherosclerosis model.Another 10 C57BL/6J mice were selected as normal group,and atorvastatin group was given atorvastatin calcium suspension by intragastric administration,Xinnao Maikang high-,medium-and low-dosage groups were given Xinnao Maikang Decoction 56,28,14 g/kg by intragastric administration,the normal group and model group were given equal volume of normal saline by intragastric administration for 8 weeks.Liver index of mice was detected,serum lipid level of mice was detected by automatic biochemical analyzer,TC and TG contents in liver tissue were detected by biochemical kit,morphology of aortic intima and liver tissue were observed by HE staining,lipid deposition in liver tissue was observed by oil red O staining,the expressions of PPARγ,LXRα,ABCA1,ABCG1 and SR-BⅠ in liver tissue were detected by Western blot.Results Compared with the normal group,the liver index in the model group significantly increased(P<0.05),the contents of serum TC,TG and LDL-C significantly increased(P<0.05),and the contents of HDL-C significantly decreased(P<0.05),the contents of TC and TG in liver tissue increased(P<0.05),the aortic wall was thickened,the number of subcutaneous foam cells increased,the arrangement of liver cells were disaffected,and a large number of fat vacuoles could be seen,the protein expressions of PPARγ,LXRα,ABCA1,ABCG1 and SR-BⅠ in liver tissue significantly decreased(P<0.01).Compared with the model group,the liver index of Xinnao Maikang high-,medium-and low-dosage groups and atorvastatin group significantly decreased(P<0.05),the serum contents of TC,TG and LDL-C significantly decreased(P<0.05),and the HDL-C contents significantly increased(P<0.05),the contents of TC and TG in liver tissue significantly decreased(P<0.05),the aortic wall thickened and the subcutaneous foam cells decreased,the liver cells were arranged neatly,and fat degeneration was reduced to varying degrees,the protein expressions of PPARγ,LXRα,ABCA1,ABCG1 and SR-BⅠ in liver tissue of Xinnao Maikang high-dosage group and atorvastatin group significantly increased(P<0.05,P<0.01).Conclusion Xinnao Maikang can decrease blood lipid level,reduce atherosclerotic plaque and reduce lipid deposition in liver of atherosclerotic mice,and its mechanism may be related to regulating PPARγ/LXRα/ABCA1 pathway,regulating lipid metabolism,promoting reverse cholesterol transport.
2.Analysis of potential profile and influencing factors in elderly diabetic patients with malnutrition risk

Objective To explore the potential categories of diet management ability in elderly patients suffering from diabetes and analyze the influencing factors of different potential categories,for the reference of clinical nurs-ing practice.Methods A convenient sampling method was used to select elderly patients suffering from diabetes with the risk of malnutrition treated in endocrinology departments in 2 tertiary hospitals in Zhenjiang City,Jiangsu Province from September 2023 to September 2024.General information questionnaire,Diabetes Diet Management Scale,Electronic Health Literacy Scale,Simple Disease Perception Scale and Perceived Social Support Scale were used for investigation.Results A total of 376 questionnaires were sent out,in which 349 were effectively collected with an effective questionnaire recovery rate of 92.8%.Totally 3 potential categories of diet management behavior were identified through the analysis of effective questionnaire,namely low self-management(24.9%),middle self-man-agement(45.9%)and high self-management(29.2%).Disease duration of diabetes,glycosylated hemoglobin level,Al-bumin level,E-health literacy,perceived family support level and disease perception level were the risk factors of diet management ability in elderly patients suffering from diabetes with the risk of malnutrition(P<0.05).Conclusion There are significant category characteristics of diet management ability in elderly patients suffering from diabetes with the risk of malnutrition.Medical staff can formulate targeted clinical intervention measures to help patients im-prove their diet management ability according to different category characteristics.
3.Endovascular therapy beyond 24 hours for middle cerebral artery atherosclerotic occlusion in acute ischemic stroke

Objective To assess the clinical and safety outcomes of endovascular therapy(EVT)administered beyond 24 h after the onset of symptoms in patients with acute ischemic stroke(AIS)resulting from middle cerebral artery atherosclerotic occlusion.Methods We enrolled consecutive AIS patients with middle cerebral artery atherosclerotic occlusion who received EVT in our hospital between January 2020 and June 2023 and divided them into two groups based on the time from AIS onset to EVT:EVT≤24 h group and EVT>24 h group.The National Institutes of Health Stroke Scale(NIHSS)scores after surgery,90-day functional independence(defined as mRS 0-2),symptomatic ICH(sICH),postoperative acute thrombosis,as well as mortality,were analyzed in the two groups of patients.Results A total of 161 patients were recruited,with 106 cases in the EVT≤24 h group and 55 cases in the EVT>24 h group.The NIHSS scores declined after EVT in both groups(P<0.01).The NIHSS scores after surgery were lower in the EVT≤24 h group(median:4.5 vs.7.0,P<0.05).90-day functional independence(45.3%vs.30.9%,95%CI:0.93-3.68),sICH(5.7%vs.9.1%,95%CI:0.18-2.06),postoperative acute thrombosis(6.6%vs.3.6%,95%CI:0.37-9.34),and mortality(3.8%vs.1.8%,95%CI:0.23-19.42)did not differ significantly between the groups(P>0.05).Conclusion Our findings support EVT feasibility in selected patients with middle cerebral artery atherosclerotic occlusion beyond 24 h.Prospective studies are warranted for confirmation.
4.Efficacy and safety of mechanical thrombectomy in acute ischemic stroke patients after vertebral-basilar artery stent implantation based on propensity score matching

Objective To observe the efficacy and safety of mechanical thrombectomy in acute ischemic stroke patients after vertebral-basilar artery stent implantation.Methods From May 2018 to October 2022,patients with acute vertebral basilar artery occlusion who underwent mechanical thrombectomy were retrospectively analyzed.The patients were divided into two groups:stent occlusion group and acute atherosclerotic stenosis occlusion(ICAS)group.The baseline characteristics of the two groups were balanced by propensity score matching method.The successful recanalization rate,90-day good outcome rate,90-day mortality rate,and symptomatic intracranial hemorrhage rate were compared.Results We enrolled 107 patients,with 14 cases in stent occlusion group and 93 cases in ICAS group.We analyzed 14 pairs by propensity score matching,including 14 cases in stent occlusion group and 27 cases in ICAS group.The successful recanalization rate and 90-day good outcome rate was lower in stent occlusion group than in ICAS group[(78.6%(11/14)vs.100%(27/27),P=0.062,28.6%(4/14)vs.44.4%(12/27),X2=0.976,P=0.323],but there was no statistical difference.The 90-day mortality rate in the stent occlusion group was significantly higher than that in the ICAS group[57.1%(8/14)vs.25.9%(7/27),x2=3.873,P=0.049].The incidence of symptomatic intracranial hemorrhage was higher in the stent occlusion group than in the ICAS group,with no statistical difference[35.7%(5/14)vs.14.8%(4/27),x2=1.289,P=0.256].Conclusion The successful recanalization rate,90-day good outcome rate,and incidence of symptomatic intracranial hemorrhage in stent occlusion group did not significantly differ from those in ICAS group,but the 90-day mortality rate was significantly higher in the former group.
5.Clinical observation of Jingling oral liquid combined with methylphenidate hydrochloride and thiopride hydrochloride in the treatment of attention deficit hyperactivity disorder co-induced by epilepsy

Objective:To analyze the clinical effect of Jingling oral liquid combined with methylphenidate hydrochloride and thiopride hydrochloride in the treatment of children with attention deficit hyperactivity disorder (ADHD) and its influence on serum dopamine transporter (DAT), interleukin-6 (IL-6) and 8 isoprostaglandin F2α[8-iso-PGF-(2α)] levels.Methods:A total of 120 children with ADHD co-induced by epilepsy diagnosed and treated in Handan Second Hospital from May 2022 to June 2023 were retrospectively selected and divided into observation group and control group according to treatment method, with 60 cases in each group. The control group was treated with methylphenidate hydrochloride tablets and tiapride hydrochloride, and the observation group was treated with Jingling oral liquid on the basis of the control group. Both groups were treated continuously for 16 weeks. The clinical efficacy and number of seizures of the two groups were compared, the clinical symptoms of the two groups were assessed by the Conner Parent Symptom Questionnaire (PSQ) and the Swanson Nolan and Pelham Version Ⅳ(SNAP-Ⅳ), and the intelligence level of the two groups was assessed by the Wechsler Intelligence Scale. The serum levels of DAT, IL-6, 8-iso-PGF-(2α), neurotrophic factors before and after treatment and the occurrence of adverse reactions were compared between the two groups.Results:After treatment, the effective rate in the observation group was higher than that in the control group: 93.33% (56/60) vs. 80.00% (48/60), there was statistical difference ( χ2 = 4.62, P<0.05). After treatment, the number of seizures in the observation group was less than that in the control group at 3, 4 to 6, 7 to 12 months: (10.78 ± 1.45)times vs.(14.18 ± 1.56) times, (4.86 ± 0.53) times vs.(8.63 ± 0.89) times, (2.64 ± 0.32) times vs. (4.11 ± 0.45) times, there were statistical differences ( P<0.05). After treatment, the scores of PSQ and SNAP-Ⅳ in the observation group were lower than those in the control group: (13.67 ± 1.48)scores vs. (15.18 ± 1.59) scores, (22.12 ± 2.35) scores vs. (25.37 ± 2.68)scores; while the scores of Wechsler intelligence scale was higher than that in the control group: (93.65 ± 9.77)scores vs.(89.42 ± 8.89) scores, there were statistical differences ( P<0.05). After treatment, the serum levels of DAT, IL-6 and 8-iso-PGF-(2α) in the observation group were lower than those in the control group: (0.46 ± 0.05) μg/L vs. (0.52 ± 0.06) μg/L, (8.26 ± 0.89) ng/L vs. (8.74 ± 0.92) ng/L, (60.38 ± 6.46) ng/L vs.(79.25 ± 8.14) ng/L, there were statistical differences ( P<0.05). After treatment, the serum levels of S100βprotein, brain-derived nerve growth factor and prolactin in the observation group were higher than those in the control group: (41.55 ± 4.28) ng/L vs. (37.26 ± 3.87) ng/L, (498.33 ± 54.26) ng/L vs.(442.15 ± 45.78) ng/L, (10.18 ± 1.14) μg/L vs. (9.69 ± 0.97) μg/L, there were statistical differences ( P<0.05).There was no significant difference in gelatinous-derived nerve growth factor level between the two groups after treatment ( P>0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Jingling oral liquid combined with methylphenidate hydrochloride and thiopride hydrochloride in the treatment of ADHD co-induced by epilepsy has good efficacy, and can significantly improve their clinical symptoms, regulate the levels of serum DAT, IL-6, 8-iso-PGF-(2α). It is safe and reliable.

Extracellular vesicles(EVs)are small vesicles secreted by cells,widely present in various body fluids,and they play important biological roles.In recent years,EVs have garnered significant attention as novel diagnostic and thera-peutic tools in multiple ocular diseases.Glaucoma,as a common cause of irreversible blindness,is primarily caused by optic nerve damage associated with pathologically elevated intraocular pressure.Emerging evidence indicates that EVs hold considerable therapeutic potential in glaucoma management,particularly in modulating aqueous humor circulation and pro-viding retinal neuroprotection.This review summarizes the latest research progress on EVs in the treatment of glaucoma.
7.Establishment and application of ultra-fast real-time PCR for Brucella detection

This study was aimed at establishing a method of ultra-fast quantitative PCR for Brucella detection.We used an exogenous recombinant plasmid as the internal reference and targeted the T4SS secretion system,an important Brucella viru-lence factor,to design specific primers and probes.The sensitivity,specificity,and repeatability of this method were evaluated,and a standard curve was constructed.The coincidence rate of detection findings with this method versus quantitative PCR was determined.This method markedly decreased the detection time to only 10 minutes.The standard curve demonstrated a good linear relationship(Y=-3.410 7x+38.357,R2=0.998 5)with a low minimum detection limit of 10 copies/μL.The method exhibited good specificity and did not specifically amplify several common clinical bacteria other than Brucella.The de-tection of three concentrations of positive plasmids yielded coefficients of variation(CVs)of 0.20%to 0.91%,thus demonstra-ting the method's excellent repeatability.Furthermore,140 clinical samples were analyzed concurrently with the fluorescence PCR method,which yielded a 100%compliance rate and consistent results.Our findings indicated that the Brucella ultra-fast quantitative PCR was ultrafast;had high sensitivity,high specificity,and good specificity;and can be used for the clinical de-tection of Brucella and emergency investigation of epidemics.Therefore,this method is valuable for the early diagnosis of Bru-cella.
8.Research progress of microfluidic chip technology in rapid detection of drug-resistant bacteria

Drug resistance refers to the reduced or lost sensitivity of microorganisms to key drugs used in clinical (routine) treatment. Antimicrobial resistance poses a significant threat to global public health and safety. The misuse and abuse of antibacterial drugs not only causes inefficient treatment of bacterial infectious diseases, but also exacerbates the evolution of bacterial antimicrobial resistance and the spread of antimicrobial-resistant bacteria. As one of the cutting-edge scientific techniques, microfluidic chip technology has shown extensive application potentials in various fields. In the rapid detection of drug-resistant bacteria, microfluidic chip technology has the advantages of fast speed, high degree of automation, and the ability to perform multiple detections. This article focuses on bacterial antimicrobial resistance, and briefly introduces the principle of microfluidic chips, systematically outlines the application of microfluidic technology in antimicrobial resistance detection, and provides a theoretical reference for the diagnosis of clinical bacterial infectious diseases.
9.The short-term efficacy of modified anterior pelvic ring internal fixator in the treatment of anterior pelvic ring injuries

Objective:To evaluate the short-term efficacy of our modified anterior pelvic ring internal fixator(INFIX) in the treatment of anterior pelvic ring injuries.Methods:A retrospective study was conducted to analyze the clinical data from the 16 patients with pelvic anterior ring injury who had been treated with our modified INFIX at Department of Orthopaedics, The First Hospital Affiliated to Bengbu Medical University from June 2020 to June 2023. There were 9 males and 7 females with an age of (49.1±14.3) years. According to the AO/OTA classification, 10 cases were of type B2 and 6 cases of type C1. Their time from injury to surgery was (7.6±2.9) days. Fixation with our modified INFIX was as follows: Three pedicle screws were inserted into the anterior inferior iliac spine on one side and into the pubic symphysis on both sides. Next, a subcutaneous tunnel was created from the anterior inferior iliac spine incision toward the pubic symphysis, and connecting rods were inserted for fixation. The surgical incision length, intraoperative blood loss, surgical time, postoperative fracture reduction quality, fracture healing time, incidence of complications during follow-up, and pelvic functional recovery at the last follow-up were recorded in this cohort.Results:In this cohort, surgical incision length was (5.8±0.4) cm, intraoperative blood loss (75.4±11.9) mL, and surgical time (66.1±8.9) min. By the end of one week after surgery, the quality of fracture reduction was evaluated according to the Matta scoring criteria as excellent in 11 cases and as good in 5 cases. All patients were followed up for (17.4±3.1) months after surgery. The fractures got united in all the 16 patients after (11.2±1.2) weeks. At the last follow-up, the pelvic function recovery was evaluated according to the Majeed scoring system as excellent in 13 cases and as good in 3 cases, yielding a Majeed score of (90.1±4.2) points. During the follow-up period, no patient developed complications such as nerve injury, wound infection, or loosening, breakage or withdrawal of internal fixation devices.Conclusion:In the treatment of anterior pelvic ring injuries, our modified INFIX has the advantages of few complications, simple operation and minimal invasion, leading to good short-term efficacy.
10.Advances in the role of disease-associated microglia in retinal ganglion cell injury in glaucoma

Glaucoma,characterized by optic nerve damage and progressive damage to retinal ganglion cells(RGCs),is the leading cause of irreversible blindness.However,the specific mechanism of RGC damage has not been fully elucida-ted.In recent years,there is increasing evidence that microglia,especially disease-associated microglia(DAM),may play an important role in glaucomatous ganglion cell injury.In this paper,we reviewed the role and mechanism of DAM in RGC damage in glaucoma,aiming to provide new insights for further research on the mechanism of RGC damage and subsequent protection of RGCs.
