1.Research Progress of Autonomic Nerve Regulation in the Treatment of Myocardial Infarction.

The autonomic nervous system imbalance caused by the overactivation of the sympathetic nerve and the weakened activity of the parasympathetic nerve is closely related to the occurrence and development of myocardial infarction.Autonomic nerve regulation is a new therapeutic approach aiming at inhibiting sympathetic activity and increasing parasympathetic activity.It encompasses magnetic nerve stimulation,optogenetic neuromodulation,and microinjection of botulinum toxin,which could promote the rebalance of the autonomic nervous system,thereby curbing the deterioration of the cardiac function and reducing the occurrence of ventricular arrhythmias after myocardial infarction.This paper reviews the anatomical basis,mechanisms of action,and research advances in intervention strategies of the autonomic nervous system in myocardial infarction.

2.Literature case analysis of drug-induced liver injury induced by GLP-1 receptor agonists

OBJECTIVE To investigate the clinical characteristics of drug-induced liver injury (DILI) induced by glucagon- like peptide-1 receptor agonists (GLP-1RAs), and to provide a reference for safe clinical medication. METHODS Using search terms such as “GLP-1”“GLP-1RAs”“semaglutide” “drug-induced liver injury”, relevant studies from PubMed, Embase, the Cochrane Library, CNKI, Wanfang Data and VIP were retrieved. Descriptive analysis was performed on cases of DILI induced by GLP-1RAs. RESULTS A total of 11 studies, comprising 11 patients, were included. Among them, 4 were male (36.4%) and 7 were female (63.6%). Patient ages ranged from 17 to 64 years; 5 patients (45.5%) were between 50 and 65 years old. Six patients were treated for diabetes, and five for weight loss. Ten patients had underlying diseases. The shortest time to the onset of DILI was 5 days after medication, while the longest was approximately 180 days. The DILIs induced by GLP-1RAs were mainly hepatocellular injury type (6 cases); severity levels included severe (3 cases), moderate (6 cases), and mild (2 cases). Gastrointestinal symptoms and jaundice were the most common clinical manifestations. The association between DILI and GLP- 1RAs was assessed as “probable” in 10 cases and “possible” in 1 case. All 11 patients improved after drug discontinuation and (or) corresponding treatment. CONCLUSIONS DILI induced by GLP-1RAs is relatively concentrated in patients aged 50-65, with a higher incidence in females. The risk may be further increased in patients with underlying diseases. Clinical use of these agents should enhance pharmaceutical care, including identification of high-risk populations and patient education (especially symptom recognition). When relevant symptoms appear, the drug should be discontinued immediately, with liver-protective therapy initiated when necessary, to ensure patient safety of drug use.
3.Develop and validate an early risk prediction model for hip fracture among the elderly in community

Objective To investigate the incidence of hip fracture among the elderly in communities,explore related influencing factors,and develop and validate a risk prediction model.Methods A stratified sampling method was used to collect sociodemographic data,lifestyles and risk factors in hip fracture between January 2023 and January 2024 among the elderly residents in communities in Deyang.With random splitting,479 elderly people(68.00%)were assigned to the model training set,and 221(32.00%)to the model validation set.In the model training set,the participants were divided into a fracture group and a non-fracture group based on hip fracture or not.Data from both groups were compared,and R software(version 4.3.1)was employed to develop and validate the risk prediction model.Results A total of 700 elderly residents in communities were included,62 of them had hip fracture within one year yielding a cumulative incidence rate of 8.86%.The risk prediction model identified six predictors:frequent consumption of preserved foods,daily exercise time,daily sunlight exposure,osteoporosis,times of fall within a year,and with≥20 pieces of natural teeth.In the training set,the model achieved an AUC of 0.945(95%CI:0.908-0.982),with a sensitivity of 88.89%and a specificity of 89.40%.The calibration curve demonstrated a good agreement between predicted and actual values,indicating a strong calibration.Decision curve analysis(DCA)showed a positive net benefit.In the validation set,the AUC was 0.892(95%CI:0.784-0.999),with a sensitivity of 82.35%and a specificity of 93.63%,confirming a good model fit and predictive performance.The calibration curve exhibited a strong consistency,and DCA indicated a positive net benefit.Conclusion The developed risk prediction model for hip fracture in elderly community residents demonstrates a strong predictive value.It provides a practical reference for community workers and healthcare professionals to screen and assess the risk of hip fracture among the elderly residents in communities.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

7.Discussion on clinical application of Dazhui acupoint based on data mining

Objective:To discuss dominant symptoms and compatibility rules of Dazhui(GV14) based on data mining.Methods:Literature related to Dazhui (GV14) was retrieved from CNKI, Wangfang, VIP, China Biomedical Literature Database (CBM) and Pubmed databases from January 1, 2012 to August 15, 2022, and the main symptoms of Dazhi (GV14) and the compatibility of acupoints were summarized. Gephi 0.9.5 software was used for complex network analysis to compare the treatment for dominant symptoms with single acupoint of Dazhi (GV14) and the compatibility of the acupoint. SPSS Modeler 18.0 software was used to analyze the association rules of acupoint combination based on Apriori algorithm. The clustering analysis of high frequency acupoints was carried out by SPSS Statistics 26.0 software.Results:A total of 722 articles were included, involving 732 prescriptions. The dominant symptoms of single acupoint were cervical spondylosis, acne, and cold; the treatment for dominant symptoms with compatibility included 14 types, such as cervical spondylosis, allergic rhinitis, ischemic stroke sequelae. The meridian compatibility was dominated by bladder meridian, and the frequency of yang meridians was higher than yin meridians. The compatibility of specific acupoints such as Xiahe acupoint, Beishu acupoint and Bahui acupoint were the main acupoints, and the high frequency acupoints were 33 acupoints such as Feishu (BL13), Baihui (GV20), Fengchi (GB20) and Zusanli (ST36), obtaining 4 series and 8 types of compatible combinations of Dazhui (GV14).Conclusions:Dazhui (GV14) is widely used in the treatment of internal diseases, such as respiratory diseases, nervous system diseases and vertebral artery type of cervical spondylosis. It tends to be flexibly used with multiple compatibility and clustering combination of specific acupoints.
8.Clinical characteristics and prognosis analysis of patients with IgG4-related hepatobiliary-pancreatic disease

Objective:To explore and analyze the clinical features of patients with immunoglobulin (Ig)G4-related hepatobiliary-pancreatic disease and the independent factors affecting the prognosis of IgG4-related sclerosing cholangitis (IgG4-SC).Methods:The clinical data of 179 adult cases diagnosed with IgG4-related hepato-pancreato-biliary disease in the Division of Gastroenterology and Hepatology, Renji Hospital, Shanghai Jiao Tong University School of Medicine from January 2011 to December 2022 were retrospectively analyzed. Patients were divided into three groups: isolated IgG4-SC, IgG4-SC/type 1 autoimmune pancreatitis(type 1 AIP), and isolated AIP according to the clinical manifestations. Demographic characteristics, baseline biochemical immunological indexes, and imaging manifestations were analyzed. The treatment response rate and survival rate were compared. The COX proportional hazards model was used to analyze the independent factors related to prognosis.Results:The mean age of diagnosis of patients with IgG4-related hepatobiliary-pancreatic disease was 60.3±12.0 years. Males accounted for 74.9%, and the median follow-up time was 38 months. The 1-year clinical response rate of patients with isolated IgG4-SC was lower than that of IgG4-SC/AIP (67.9% vs. 91.7%, P=0.019), and the primary endpoint-free 5-year survival rate was significantly reduced (64.9% vs. 95.9%, P<0.001). COX regression analysis showed that having cirrhosis before treatment ( HR=6.708, P=0.004) and poor response after half a year of treatment ( HR=11.488, P=0.002) were independent risk factors associated with the occurrence of adverse events in hepatobiliary diseases among patients with IgG4-SC. Conclusions:The clinical response rate and survival rate of patients with isolated IgG4-SC are lower than those of patients with IgG4-SC/AIP. Patients with IgG4-SC who do not respond well at six months of treatment and who have progressed to cirrhosis before treatment are at significantly increased risk of adverse events.
9.Correlation between serum lncRNA CDKN2B-AS1,miR-184 levels and the occurrence of coronary artery restenosis after interventional treatment in patients with acute coronary syndrome

Objective To analyze the correlation between serum levels of long non-coding RNA cyclin-de-pendent kinase inhibitor 2B antisense RNA 1(lncRNA CDKN2B-AS1),and microRNA-184(miR-184)in pa-tients with acute coronary syndrome(ACS)and the occurrence of coronary artery restenosis(RS)after percu-taneous coronary intervention(PCI)treatment.Methods A total of 288 ACS patients who underwent PCI treatment in the hospital from February 2020 to March 2023 were selected.According to the results of follow-up angiography at 6 months after surgery,they were separated into a RS group of 96 cases and a non RS group of 192 cases.The quantitative real-time PCR(qRT-PCR)method was applied to detect the relative expression levels of serum lncRNA CDKN2B-AS1 and miR-184.Pearson correlation was applied to analyze the correlation between serum lncRNA CDKN2B-AS1 and miR-184.The factors affecting the occurrence of RS in ACS pa-tients after PCI were analyzed using multivariate Logistic regression analysis.Receiver operating characteristic(ROC)curve was applied to analyze the evaluation value of serum lncRNA CDKN2B-AS1 and miR-184 for the occurrence of RS in ACS patients after PCI.Results Compared with the non RS group,the serum lncRNA CDKN2B-AS1 level in the RS group was obviously increased,while the miR-184 level was obviously reduced(P<0.05).There were statistically significant differences in the levels of high-sensitivity C-reactive protein(hs-CRP),interleukin-18(IL-18),total bilirubin(TBIL),and cardiac troponin Ⅰ(cTnⅠ)between the two groups(P<0.05).Pearson correlation analysis showed that there was a obvious negative correlation(r=-0.427,P<0.05)between the expression levels of serum lncRNA CDKN2B-AS1 and miR-184 in ACS pa-tients.Multivariate Logistic regression analysis results showed that lncRNA CDKN2B-AS1,hs-CRP,IL-18,and cTnⅠ were risk factors affecting the occurrence of RS in ACS patients after PCI,while miR-184 and TBIL were protective factors affecting the occurrence of RS in ACS patients after PCI(P<0.05).ROC curve re-sults showed that the area under the curve(AUC)of serum lncRNA CDKN2B-AS1,miR-184,and their com-bination in evaluating the occurrence of RS in ACS patients after PCI was 0.787,0.844,and 0.929,respective-ly,and the AUC of the combined evaluation the occurrence of RS in ACS patients after PCI was obviously higher than those of serum lncRNA CDKN2B-AS1 and miR-184 alone(Zcombination-lncRNACDKN2B-AS1=4.490,Zcombination-miR-184=3.429,all P<0.05).Conclusion The serum lncRNA CDKN2B-AS1 level in ACS patients is obviously elevated,while miR-184 level is obviously reduced,which is correlated with the occurrence of RS in ACS patients after PCI.Both are factors that affect the occurrence of RS in ACS patients after PCI,and the combination of the two has better evaluation effect on the occurrence of RS in ACS patients after PCI.
10.Current situation and prospect of surgical treatment of perihilar cholangiocarcinoma

Perihilar cholangiocarcinoma(PHC)is a common malignancy of biliary tract,for which surgery is the most effective treatment.However,its prognosis is not satisfactory even after surgical resection.In recent years,there have been some new advances in the surgical treatment of PHC.In this paper,we reviewed the existing literatures,demonstrated the current situation of preoperative biliary drainage,liver hyperplasia,hepatic resection,liver transplantation and minimally invasive surgery in the treatment of PHC,and prospected the future research direction.
