Objective To study the five-year survival status and influencing factors of elderly patients with lung cancer complicated with chronic obstructive pulmonary disease (COPD). Methods A cohort study was conducted to follow up 450 patients with lung cancer and chronic obstructive pulmonary disease who were hospitalized in our hospital from January 2018 to December 2023. The endpoint of the follow-up was the end of a five-year period or death. The Life Tables method was used to calculate survival rates and plot survival curves. The Cox proportional hazards model was used to analyze the influencing factors of five-year survival. Results The results indicated that the overall five-year survival rate of patients was 4.89%, and it decreased year by year. Cox regression analysis showed that age, gender, family functioning, and psychological status significantly influenced patient survival rate (all P<0.05). Stratified analysis found that the smoking status, family functioning, and psychological status of male patients all had an impact on survival rate (all P<0.05), while the psychological status of female patients had a more significant impact on survival (P=0.008). Conclusion This study provides a scientific basis for comprehensive intervention of elderly lung cancer patients with COPD. It is recommended that clinical attention should be paid to psychological and family factors to improve patient prognosis.

ObjectiveBased on the theory of "spleen governing transportation and transformation"， this study investigates the efficacy of Atractylodis Macrocephalae Rhizoma processed with Aurantii Fructus Immaturus juice（AMR-AFI） in improving slow-transit constipation（STC）， as well as the synergistic regulatory mechanism involving the microbiota-metabolism axis， thereby elucidating the scientific basis of its processing theory. MethodsAnimals were randomly divided into the control group， model group， positive drug（mosapride） group（3 mg·kg^-1）， and low-， medium-， and high-dose groups of AMR-AFI（3.9， 7.8， 15.6 g·kg^-1）. Except for the control group， the remaining five groups were induced with STC using loperamide hydrochloride. Following modeling， interventions were administered. All groups received continuous administration for 15 d， during which fecal samples， colon tissue， and serum were collected. Constipation improvement was assessed by measuring fecal moisture content and small intestinal propulsion rate， histological morphology of colonic tissue was observed via hematoxylin-eosin（HE） staining， and the levels of interleukin（IL）-6， tumor necrosis factor（TNF）-α， and IL-2 in serum were detected using enzyme-linked immunosorbent assay（ELISA）. Furthermore， the microbial community structure in mouse feces was analyzed by 16S rRNA sequencing， while transcriptomic sequencing was employed to screen differentially expressed genes in colonic tissue， followed by gene ontology（GO） and Kyoto Encyclopedia of Genes and Genomes（KEGG） enrichment analyses. Finally， Spearman correlation analysis was conducted to explore the association between differential microbiota and differential genes. ResultsCompared with the control group， the intestinal propulsion rate and fecal moisture content in the model group were significantly decreased（P<0.01）， while serum levels of IL-6， TNF-α， and IL-2 were significantly elevated（P<0.01）. HE staining showed damage and shedding of colonic mucosal epithelial cells， along with a reduction in goblet cells in the model group. In comparison with the model group， all treatment groups improved the pathological state of the colonic mucosa to varying degrees and reduced serum levels of IL-6， TNF-α， and IL-2（P<0.01）. Among these， the high-dose group of AMR-AFI significantly increased the intestinal propulsion rate and fecal moisture content of rats（P<0.05， P<0.01）. Further transcriptomic analysis revealed that a total of 104 differentially expressed genes were identified from comparisons between the model group and the control group， as well as between the model group and the high-dose group of AMR-AFI. These genes were mainly enriched in pathways closely related to STC pathogenesis， such as arachidonic acid metabolism and aldosterone-regulated sodium reabsorption. 16S rRNA sequencing results indicated that AMR-AFI reversed the structural imbalance of the gut microbiota in model mice， increased species richness， downregulated the relative abundance of pro-inflammatory bacteria such as Parasutterella， and enriched beneficial and butyrate-producing bacteria， including Lachnospiraceae_NK4A136_group， Ruminococcaceae， and Lachnospiraceae. Spearman correlation analysis further showed that the beneficial bacteria enriched in the AMR-AFI group were negatively correlated with genes involved in the arachidonic acid metabolic pathway and positively correlated with genes in the aldosterone-regulated sodium reabsorption pathway. In contrast， pro-inflammatory bacteria in the model group exhibited the opposite correlation trends. ConclusionAMR-AFI can effectively exert synergistic therapeutic effects on STC by regulating intestinal microbiota， arachidonic acid-mediated inflammatory metabolism， and aldosterone-regulated water-salt balance pathways.

OBJECTIVE: To explore the genetic etiology for a pregnant woman with a history of multiple adverse pregnancies and assess the phenotype-genotype correlation of 22q11.2 microduplication syndrome in her family. METHODS: Amniotic fluid sample was taken from a pregnant woman for whom non-invasive prenatal screening indicated chromosome 22 abnormalities in the fetus. Peripheral blood samples from the woman, her brother and parents were collected for high-throughput low-depth whole genome sequencing (CNV-seq). A pedigree traceability analysis of the results was conducted in conjunction with analysis of clinical manifestation. Relevant literature (from establishment to March 2025) was systematically searched. This study was approved by the Medical Ethics Committee of Mianyang Maternal and Child Health Care Hospital (Ethics No.: Lun Shen [2024]009). RESULTS: CNV-seq revealed that the fetus had harbored a 6.02 Mb duplication at 22q11.21q11.23. Karyotyping confirmed it as 46,X?dup(22)(q11.2). Pedigree verification demonstrated that the pregnant woman, her brother and mother had all carried the same duplication. Phenotypic analysis of the affected family members showed classic features of 22q11.2 microduplication syndrome, including hypernasal speech, low nasal bridge, congenital heart disease, and cognitive impairment. A total of 44 cases with full information (including three patients from this pedigree) were included in the analysis. The penetrance of 22q11.2 duplication was approximately 29.5% (13/44), and 52.3% (23/44) of the cases had inherited the variant from a phenotypically normal parent. CONCLUSION This study has identified the genetic basis for the woman's recurrent adverse pregnancies and phenotypic abnormalities in her family members. The scoliosis identified in her younger brother has not been previously reported, thereby may enrich the clinical phenotype of this syndrome. For fetuses identified with a 22q11.2 microduplication, detailed fetal imaging is recommended, and genetic counseling should be provided to the couples.

ObjectiveTo investigate the diagnostic value of coagulation function indicators and proteinuria in preeclampsia and its severity assessment. MethodsA total of 239 pregnant women who visited from January 2022 to December 2023 were selected as research subjects. They were divided into healthy control group （n=50）， non-gestational hypertension group （n=39）， and gestational hypertension group （n=150） based on clinical diagnosis. The gestational hypertension group was further subdivided into three subgroups： simple hypertension group （n=50）， mild preeclampsia group （n=50）， and severe preeclampsia group （n=50）. The study compared differences in plasma protein S （PS）， plasma protein C （PC）， fibrin degradation products （FDP）， fibrin monomer （FM）， and proteinuria levels among different groups. The correlation between proteinuria and coagulation function indicators in patients with gestational hypertension was analyzed， and ROC curves were used to analyze the diagnostic efficacy of coagulation indicators and proteinuria for gestational hypertension and different stages of preeclampsia. ResultsThe gestational hypertension group showed lower PS and PC values compared to the non-gestational hypertension group and healthy control group， while FDP， FM values， and proteinuria levels were higher than those in the non-gestational hypertension group and healthy control group （P<0.05）. Significant differences in coagulation indicators and proteinuria levels were observed among pregnant women with different degrees of gestational hypertension （P<0.05）. The AUC values for diagnosing gestational hypertension using PS， PC，FDP， FM， and proteinuria were 0.928， 0.957， 0.968， 0.948， and 0.932 respectively （P<0.000 1）. For diagnosing simple hypertension and mild preeclampsia， the AUC values were 0.875， 0.777， 0.830， 0.679， and 0.936 respectively （P<0.01）. For diagnosing mild and severe preeclampsia， the AUC values were 0.901， 0.776， 0.780， 0.807， and 0.848 respectively （P<0.000 1）. ConclusionCoagulation function indicators and proteinuria show significant differences between healthy pregnant women and those with gestational hypertension. PS， PC， FDP， FM， and proteinuria levels vary among pregnant women with different stages of preeclampsia. The aforementioned indicators exhibit certain diagnostic efficacy for gestational hypertension and preeclampsia.

Objective To investigate the prevalence of thyroid nodules in the physical examination population in Mianyang region and analyze its association with metabolic syndrome. Methods A retrospective study was conducted on 9 978 individuals who underwent health examinations at our hospital from January 2024 to May 2025. Thyroid examinations were performed using color Doppler ultrasound to analyze the prevalence of thyroid nodules in this population. Clinical data of all subjects were collected, and logistic regression analysis was employed to assess the association between metabolic syndrome and the risk of thyroid nodule development. Results The prevalence of thyroid nodules in the physical examination population of Mianyang region was 17.98% (1 794/9 978). The logistic regression results showed that after adjusting for gender, age, BMI, occupation, consumption of non-iodized salt, staying up late, daily sleep duration, anxiety, and depression, metabolic syndrome (OR=6.593, 95% CI: 3.961-10.975) was associated with thyroid nodules (P<0.05). Conclusion The prevalence of thyroid nodules among the physical examination population in the Mianyang area is 17.98%, and metabolic syndrome remains associated with the risk of thyroid nodules after effectively controlling for confounding factors.

Objective To explore the correlation between nutritional risk screening 2002 (NRS-2002) score and risk of acute exacerbation during 1-year follow-up in patients with chronic obstructive pulmonary disease (COPD), and to analyze the predictive value of NRS-2002 score on acute exacerbation. Methods A total of 314 patients hospitalized for acute exacerbation of COPD in the hospital were included between January 2021 and December 2024. All patients reached the stable disease standard after standardized treatment, and completed the NRS-2002 nutritional risk score assessment and other clinical data collection before discharge. 1-year follow-up was performed after discharge. According to whether acute exacerbation occurred again, they were classified into 62 cases in the acute exacerbation group and 252 cases in the non-acute exacerbation group. Univariate and logistic multivariate regression analyses were performed to analyze the risk factors for acute exacerbation. The correlation between NRS-2002 score and acute exacerbation risk during 1-year follow-up was analyzed by Spearman correlation analysis. ROC curve was drawn to analyze the predictive value of NRS-2002 score on acute exacerbation. Results The body mass index (BMI) and forced expiratory volume in one second as a percentage of predicted value (FEV₁%) in the acute exacerbation group were lower while the proportion of smoking and NRS-2002 score were higher compared to the non-acute exacerbation group (P<0.05). After logistic multivariate analysis, it was found that low BMI, low FEV₁%, smoking history, and NRS-2002 score were high risk factors for acute exacerbation in patients with stable COPD within 1 year (P<0.05). Spearman correlation indicated that NRS-2002 score exhibited a positive association with acute exacerbation risk (r=0.546, P<0.05). ROC curve analysis suggested that the area under the curve (AUC), sensitivity and specificity of NRS-2002 score in predicting acute exacerbation were 0.900, 69.35% and 98.41% respectively. Conclusion NRS-2002 score is positively linked to the risk of acute exacerbation in patients with stable COPD. Early assessment of NRS-2002 score can predict the risk of acute exacerbation.

Objective To investigate the correlation of dietary lipid intake and apolipoprotein with Helicobacter pylori (Hp) infection combined with gastric mucosal lesions. Methods A retrospective analysis was conducted on 320 patients diagnosed with Hp infection during physical examinations from March 2024 to March 2025. The daily dietary lipid intake of all patients was statistically analyzed. The levels of apolipoprotein A1‌ (Apo-A1) and apolipoprotein B‌ (Apo B) were detected. Logistic regression was used to analyze the risk factors influencing Hp infection combined with gastric mucosal lesions. Results Compared with the non-gastric mucosal lesion group, patients in the gastric mucosal lesion group had higher daily dietary lipid intake, lower Apo-A1, higher Apo B, and higher proportion of family history of gastric cancer (P<0.05). Logistic regression analysis revealed that daily dietary lipid intake, low level of Apo-A1, and high level of Apo B were independent risk factors of Hp infection with gastric mucosal lesion (P<0.05). Conclusion Daily dietary lipid intake, Apo-A1, and Apo B are closely related to Hp infection with gastric mucosal lesion. For patients with high daily dietary lipid intake, decreased Apo-A1 level, increased Apo B level, and complicated Hp infection, it is necessary to further screen gastroscopy to determine whether gastric mucosal lesion occurs.

This paper aims to evaluate the pharmacodynamic effect and mechanism of Zhifuxin in the prevention and treatment of vascular dementia(VD), providing a theoretical basis for later development. Bilateral common carotid artery ligation in male Wistar rats was conducted to replicate the long-term hypoperfused VD model, and the drug was given to groups after one month. The rats were fed daily with nimodipine of 20 mg·kg~(-1), Zhifuxin of 50, 100, and 200 mg·kg~(-1), or the same volume of solvent for four weeks. 24 hours after the last dose, Morris water maze experiments were performed to detect the learning and memory abilities of rats. Hematoxylin-eosin(HE) staining was used to observe the pathological changes in the brain tissue of rats; the immunohistochemical method was used to detect the expression of muscarinic acetylcholine receptors M1 and M4 in rats and determine the content of acetyl choline(Ach), acetylcholin esterase(AchE), malondialdehyde(MDA), choline acetyl transferase(ChAT), and dimethyl arginine hydrolase 1(DDAH1) in the cerebral cortex of rats. Western blot was employed to detect protein expression of endothelial nitric oxide synthase(eNOS), caveolin-1, monoamine oxidase A(MAO-A), and monoamine oxidase B(MAO-B). RT-qPCR was utilized to detect mRNA expression of eNOS, caveolin-1, MAO-A, and MAO-B. The results showed that compared with the model group, the different doses of Zhifuxin were able to shorten the latency of VD rats in the water maze positioning navigation test, increase the number of crossing platforms in the space exploration test, and alleviate cone cell contracture in the hippocampus of VD rats. The expression of biochemical indicators related to the cholinergic system in the cerebral cortex: M1 and M4 receptors increased, as well as ChAT activity, and AchE activity significantly decreased. The protein and mRNA expression of indicators related to the eNOS/NO pathway: DDAH1 content, eNOS, and caveolin-1 increased, and that of indicators related to monoamine oxidase(MAO): MAO-A and MAO-B significantly decreased. The results show that Zhifuxin can improve cognition ability in long-term hypoperfused VD rats, and its mechanism of action may be related to its ability to modulate the cholinergic system and the eNOS/NO pathway and inhibit MAO expression.

The chemical constituents were systematically separated from the roots of Berberis polyantha by various chromatographic methods, including silica gel column chromatography, HP20 column chromatography, polyamide column chromatography, reversed-phase C_(18) column chromatography, and preparative high-performance liquid chromatography. The structures of the compounds were identified by physicochemical properties and spectroscopic techniques(1D NMR, 2D NMR, UV, MS, and CD). Four phenylpropanoids were isolated from the methanol extract of the roots of B. polyantha, and they were identified as(2R)-1-(4-hydroxy-3,5-dimethoxyphenyl)-1-propanone-O-β-D-glucopyranoside(1), methyl 4-hydroxy-3,5-dimethoxybenzoate(2),(+)-syringaresinol(3), and syringaresinol-4-O-β-D-glucopyranoside(4). Compound 1 was a new compound, and other compounds were isolated from this plant for the first time. The anti-inflammatory activity of these compounds was evaluated based on the release of nitric oxide(NO) in the culture of lipopolysaccharide(LPS)-induced RAW264.7 macrophages. At a concentration of 10 μmol·L~(-1), all the four compounds inhibited the LPS-induced release of NO in RAW264.7 cells, demonstrating potential anti-inflammatory properties.
