1.Professor LIU Jinmin's Clinical Experience in Treating Epilepsy Based on the Method of Closing Yangming and Regaining Vital Activity

To summarize the clinical experience of Professor LIU Jinmin in treatment for epilepsy. It is believed that main pathogenesis of epilepsy is yangming failure to close and vital activity loss control, so a therapeutic approach focused on restoring the closure of yangming and regaining vital activity was proposed for the treatment of epilepsy. For excess syndrome, the treatment focuses on draining excess and descending qi, promoting purgation and restoring spirit. When yangming dryness-heat predominates, the approach involves unblock the bowels and regulating the spirit, descending qi and reducing fire, with modified Chengqi Decoction (承气汤) as prescription; when yangming phlegm-fire predominates, the treatment focuses on clearing heat and resolving phlegm, calming mind and suppressing fright, with modified Qingxin Wendan Decoction (清心温胆汤) as prescription; when yangming blood stasis predominates, the approach involves breaking up blood stasis and promoting purgation, eliminating stasis and awakening the mind, with Taoren Chengqi Decoction (桃核承气汤) as prescription. For deficiency syndrome, the treatment emphasizes tonifying deficiency and raising qi, strengthening the stomach and nourishing the spirit. When center qi deficiency and sinking of clear qi of the nutrients from food, the approach involves replenishing and uplifting qi while nourishing vital activity, with modified Liujunzi Decoction (六君子汤) as prescription; when yin deficiency and fluid consumption, the treatment focuses on nourishing stomach and tonifying yin, promoting fluid production and calming the spirit, with modified Maimendong Decoction (麦门冬汤) combined with Yiwei Decoction (益胃汤) as prescriptions. In clinical situations of deficiency-excess complex, it is essential to distinguish the primary condition from the secondary, applying both supplementing and draining methods flexibly to achieve optimal treatment.
3.Macrophage galactose-type lectin 1 limits mouse hematopoietic stem cell differentiation in context of inflammation by inhibiting NF-κB signaling pathway

AIM:To investigate the effects of macrophage galactose-type lectin 1(Mgl1)gene deletion on he-matopoietic stem/progenitor cells(HSPCs)under steady-state conditions and inflammation.METHODS:Mice were di-vided into a control group(wild-type)and an experimental group(Mgl1 gene-deleted).Flow cytometry was used to ana-lyze the proportions of various hematopoietic cell lineages in the peripheral blood and bone marrow of both groups,assess-ing the impact of Mgl1 gene deletion on steady-state hematopoiesis(n=3~4).Transplantation and colony-forming assays were utilized to evaluate the effects of Mgl1 gene deletion onthe repopulation capacity and colony-forming ability of HSPCs(n=5).The LPS-induced inflammation model was employed to examine the effects of Mgl1 gene deletion on the inflamma-tory response of HSPCs both in vitro and in vivo(n=5~8).Western blot and RT-qPCR were conducted to analyze the alter-ations in signaling pathways regulated by Mgl1 in the inflammatory response of HSPCs(n=3).RESULTS:(1)Mgl1 gene deletion had no significant effecton steady-state hematopoiesis(P>0.05).(2)Mgl1 gene deletion promoted inflam-mation-induced cell differentiation of HSPCs(P<0.01).(3)Mgl1 gene deletion accelerated the exhaustion of HSPCs un-der prolonged inflammatory conditions(P<0.01).(4)Mgl1 was found to regulate the inflammatory response of HSPCs via the NF-κB signaling pathway.CONCLUSION:Mgl1 gene deletion enhances the inflammatory response of HSPCs via the NF-κB signaling pathway.

5.Lipopolysaccharide induces aging phenotype of hematopoietic stem cells in bone marrow and spleen of young mice

AIM:This study aim to investigate the effects of lipopolysaccharide(LPS)-induced inflammation on the aging phenotype of hematopoietic stem/progenitor cells(HSPCs)in the bone marrow(BM)and spleen of mice.METHODS:(1)Young(2-month old)wild-type(WT)mice were treated with LPS to establish an actue inflammation model.The percentage of HSPCs in the BM and spleen of mice after LPS stimulation,as well as the ratio of mature cells in peripheral blood(PB)and spleen,were analyzed using flow cytometry.The proliferation of HSPCs in the BM and spleen was evaluated by examining the expression of the proliferation marker Ki67.In addition,changes in CD45 expression on HSPCs in the spleen of mice following LPS exposure were investigated by flow cytometry.(2)The percentage of HSPCs in BM and mature cells in PB and spleen of both young(2-month old)and old(24-month old)WT mice were analyzed by flow cytometry.(3)The transcriptome changes of hematopoietic stem cells(HSCs)after LPS stimulation was performed by an in silico analysis.RESULTS:(1)Mice exposed to LPS exhibited a significant increase in the percentage of HSPCs in BM and a marked elevation in the percentages of myeloid cells in PB and spleen compared to the mice in control group(P<0.05).(2)LPS exposure resulted in increased spleen weight and cell counts(P<0.05),along with a higher per-centage of HSPCs in the spleen compared to controls(P<0.05).(3)LPS stimulation promoted the proliferation of HSPCs in the BM and spleen(P<0.05).(4)The expression of CD45 was reduced on HSPCs from spleen of mice after LPS stimu-lation(P<0.01).(5)In comparison to young mice,aged mice showed an increase in spleen weight and a higher percent-age of HSPCs in the spleen(P<0.05).(6)Aged mice,in comparison to young mice,demonstrated a significantly higher percentage of HSPCs in the BM and myeloid skewing in the PB and spleen(P<0.01).(7)The silico analysis revealed up-regualtion of reactive oxygen species(ROS)and apoptosis signaling in HSPCs following LPS stimulation.CONCLU-SION:Young HSPCs stimulated by LPS exhibited an increase in cell number,a bias towards myeloid differentiation,en-hanced extramedullary hematopoiesis,and elevated levels of ROS and apoptosis,all of which collectively manifested the aging phenotype of HSPCs.
6.The Characteristics of Consonant Phonetic Errors in Children with Speech Sound Disorders at Different Ages

Objective To study whether there is a difference in the percentage consonants correct(PCC)of children with functional speech sound disorders(SSD)at different ages,and to explore the distribution in types of consonant errors at different ages.Methods A total of 172 children with ages from 4 to 7 years who were diagnosed with functional speech sound disorders and admitted to the Department of Rehabilitation of the Third Xiangya hospi-tal of Central South University participated in the study.The children were divided into three groups according to their ages:Groups 1 to 3.Groups 1,2,and 3 consisted of 97,45,and 30 children,respectively(Group 1:63 boys and 34 girls,aged 4 to<5 years;Group 2:36 boys and 9 girls,aged 5 to<6 years;and Group 3:36 boys and 9 girls,aged 6 to<7 years).Picture-naming task was used to assess children's phonology.The types of consonant error were recorded and the percentage consonants correct(PCC)and the incidence of consonant errors in different places of articulation(stopping,affrication,frication,nasalization,and lateralization)was calculated.Results ①There was no significant difference in the PCC results among the three groups(P>0.05).②There was a significant difference in omission errors(Z=6.531,P=0.038<0.05),but not for substitution and distortion errors among the three groups(P>0.05).③The omission of the lateral/l/was major among the three groups of children.④There were more stopping,affrication and frication in all three groups of children,yet no significant difference in the results of incidence of consonant errors in different places of articulation(stopping,nasalization,affrication,frica-tion and lateralization)among the three groups(P>0.05).Conclusion There was no significant difference in the PCC among children with functional speech sound disorders at different ages.There were obvious differences in the distribution of omission errors in different age groups,suggesting idiosyncrasy.Children with SSD demonstrated similar characteristics of consonant phonetic errors at different age stages,included mainly the omission of the lateral/1/and more stopping,affrication and frication.
7.Evaluation value of serum AGEs level combined with LIS score on prognosis of elderly patients with sepsis-related ALI/ARDS

Objective To study the evaluation value of lung injury score(LIS)and advanced glycation end products(AGEs)expression levels on the prognosis of elderly patients with sepsis-related acute lung injury/acute respiratory distress syndrome(ALI/ARDS).Methods A total of 98 elderly patients with sepsis-related ALI/ARDS admitted to First Branch of the First Affiliated Hospital of Chongqing Medical University from March 2019 to April 2021 were selected as the research group,and the patients were divided into two sub-groups according to their survival within 30 d after admission:the survival group(55 cases)and the death group(43 cases).Another 51 elderly patients with non-ALI/ARDS sepsis admitted to First Branch of the First Affiliated Hospital of Chongqing Medical University in the same period were selected as the control group.After admission,the clinical data of patients were recorded,and the levels of serum creatinine,troponin I,B-type brain natriuretic peptide(BNP),serum C-reactive protein(CRP)and procalcitonin(PCT)were de-tected.Enzyme-linked immunosorbent assay was used to determine the levels of AGEs in patients'serum.The LIS score was evaluated by LIS scale.With clinical factors as independent variables and prognosis as dependent variables,Logistic regression curve was used to analyze the death factors of elderly sepsis-related ALI/ARDS patients.Results AGEs levels,LIS scores,acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)scores decreased sequentially in the death group,survival group,and control group(all P<0.05).The levels of lactic acid,blood glucose,troponin I,PCT,BNP and CRP in arterial blood of patients in the death group were significantly higher than those in the survival group and the control group(P<0.05).The results showed that arterial lactate,blood glucose,troponin I,PCT,BNP,CRP,AGEs,APACHE Ⅱ score,and LIS score were all independent risk factors for mortality in elderly sepsis-related ALI/ARDS patients(P<0.05).The area under the curve(AUC)of LIS score predicting prognosis in elderly sepsis-related ALI/ARDS pa-tients was 0.857(95%CI:0.821-0.911),and AUC of serum AGEs was 0.861(95%CI:0.809-0.908).LIS score and AGEs level had certain predictive value for the prognosis of elderly sepsis-related ALI/ARDS pa-tients.Conclusion The LIS score and AGEs level of the elderly patients with sepsis-related ALI/ARDS are independent risk factors of death,which have important predictive value for prognosis.
8.CT-guided hook-wire localization of ≤10 mm pulmonary ground-glass nodules via different path ways before video-assisted thoracoscopic surgery:a comparative study

Objective To compare the clinical safety and efficacy of CT-guided hook-wire localization of≤10mm pulmonary ground-glass nodule(GGN)via different path ways before video-assisted thoracoscopic surgery(VATS).Methods The clinical data of a total of 128 patients with 10 mm pulmonary GGN,who received CT-guided hook wire localization before VATS at The Third Hospital of Mianyang of China between July 2018 and March 2023,were retrospectively analyzed.According to the puncturing localization path way mode,the patients were divided into vertical puncturing group(n=88)and non-vertical puncturing group(n=40).The number of puncturing times,the time spent for puncturing localization,the success rate of puncture,the operation time of VATS,and puncture-related complications of the two groups were recorded.Results No statistically significant differences in the gender,age,smoking history,GGN location,puncture position,nodule size,density characteristics of GGN,emphysema,and nodules-pleura distance existed between the two groups(all P>0.05).Compared with non-vertical puncturing group,in vertical puncturing group the number of puncturing times was smaller,the time spent for localization was shorter,the incidence of pneumothorax was lower,and the operation time of VATS was shorter,the differences in all the above indexes between the two groups were statistically significant(all P<0.05);and the subgroup analysis of patients whose GGN was overlapped with rib shadow obtained the same results.Binary logistic regression analysis revealed that non-vertical puncturing and the number of puncturing times were the independent risk factors for the occurrence of pneumothorax.Conclusion CT-guided hook-wire localization of≤10mm pulmonary GGN before VATS is clinically safe and effective.Under the condition when the lesion can be localized within the range of 2.0cm and the shadow overlapping of GGN with the rib and blood vessel can be effectively avoided,vertical puncturing path way mode should be preferred,which can effectively reduce the incidence of pneumothorax and shorten the operation time of VATS.
9.Efficacy of Acupuncture in Treating Post-stroke Insomnia

Objective To explore the clinical efficacy of acupuncture on post-stroke insomnia.Methods 148 outpatients and inpatients with post-stroke insomnia were recruited in the Shanghai municipal Hospital of Traditional Chinese Medicine from June 2016 to January 2018.They were randomly divided into two groups,acupuncture group(n=74)and control group(n=74).The acupuncture group was treated with GV20,GV24,GV29,HN22,HT7 and SP6 20 minutes each time,3 times per week for 4 weeks.The follow-up was after 4 weeks.The control group was treated with sham acupuncture on the same acupoints.The needles did not pierce the skin.The Insomnia Severity Index Scale(ISI),the Hospital Anxiety and Depression Scale(HADS)and the Stroke Special Quality of Life Scale(SS-QoL)were used to evaluate the patients before treatment,after 2 weeks of treatment,after 4 weeks of treatment and follow-up.The occurrence of adverse events of acupuncture was also recorded.Results ① ISI:after 2 weeks of treatment,4 weeks of treatment and follow-up,the score of treatment group was decreased compared with the control group,and the difference was statistically significant(P<0.01).Compared with baseline,the ISI score of the treatment group was decreased after 4 weeks of treatment(P<0.05).② HADS score:The difference between 4-week treatment and baseline score of the HADS-A,the difference between the treatment group and the control group was statistically significant(P<0.01);the difference between 4-week treatment and baseline,and between follow-up and baseline score of the HADS-D,the difference between the treatment group and the control group was statistically significant(P<0.01).③SS-QoL score:The difference between 2-week treatment and baseline,between 4-week treatment and baseline,and between follow-up and baseline score of the SS-QoL,the difference between the treatment group and the control group was statistically significant(P<0.01).Conclusion Acupuncture is an effective and safe therapy in improving the severity of insomnia of patients with post-stroke insomnia.Compared with sham acupuncture,acupuncture can improve the anxiety and depression status and quality of life of patients.
10.Clinical Observation on the Treatment of Cervical Spondylosis of Qi Stagnation and Blood Stasis Type by Tongdu Tiaoyang Rolling Needle Pricking-cupping Therapy

Objective To observe the clinical efficacy of Tongdu Tiaoyang rolling needle pricking-cupping(RNP-C)therapy for cervical spondylosis of qi stagnation and blood stasis type.Methods A total of 90 patients with cervical spondylosis of qi stagnation and blood stasis type were randomly divided into the rolling needle pricking-cupping(RNP-C)group and the electroacupuncture(EA)group,with 45 cases in either group.Both groups were treated with electroacupuncture at Tianzhu(BL 10),Jingjiaji(EX-B2),Houxi(SI 3),Shenmai(BL 62),Xuanzhong(GB 39),Geshu(BL 17),Hegu(LI 4)and Ashi points with continuous wave and 2 Hz of frequency for once a day,7 times as a treatment course.On the basis of electroacupuncture treatment,the rolling needle pricking-cupping(RNP-C)group was treated with bloodletting by rolling needle on the neck Du Mai and three yang channels,from Fengfu(DU 16)to Taodao(DU 13),Yuzhen(BL 9)to Dazhu(BL 11),Fengchi(GB 20)to Jianjing(GB 21),and Yifeng(SJ 17)to Tianyou(SJ 16),followed by fire cupping,for once every other day,and 3 times in all.The visual analogue scale(VAS)scores and Japanese Orthopaedic Association Scores(JOA)were observed before and after treatment,and the efficacy was evaluated.Results Compared before treatment,the scores of VAS in either group were all significantly reduced after treatment(P<0.01).The scores of VAS in the RNP-C group were significantly inferior to those in the EA group after treatment(P<0.01).Compared before treatment,the scores of JOA in either group were all significantly promoted after treatment(P<0.01).The differences of JOA scores between the RNP-C group and EA group after treatment were not significant(P>0.05).The therapeutic effecacy of the RNP-C group is superior to that of the traditional EA group(P<0.05).Conclusion Tongdu Tiaoyang rolling needle pricking-cupping(RNP-C)therapy combining with traditional electroacupuncture could improve the cervical pain symptoms of patients with cervical spondylosis of qi stagnation and blood stasis type in a short period of time,and the clinical efficacy is better than traditional electroacupuncture.
