1.Expressions and significance of NLRP3, Caspase-1 and IL-1β in patients with hemorrhagic stroke

Objective:To explore the expressions and significance of nucleotide binding oligomerization domain-like receptor protein 3 (NLRP3), cysteinyl aspartate specific proteinase-1 (Caspase-1) and interleukin-1β (IL-1β) in patients with hemorrhagic stroke.Methods:One hundred and three patients with hemorrhagic stroke who visited the Department of Neurosurgery of the Second Affiliated Hospital of Zhengzhou University from January 2022 to January 2024 were selected as the hemorrhage group.Another 90 healthy individuals who underwent physical examinations were selected as the control group.The differences in the levels of NLRP3, Caspase-1 and IL-1β in peripheral blood between the two groups were compared. The National Institutes of Health neurological deficiency score (NHISS) was used to evaluate the degree of neurological deficits of patients in the hemorrhage group. All patients were followed up for 3 months. The prognosis of patients was evaluated by modified Rankin scale (mRS). The statistical analysis was performed using SPSS 20.0 software. The relationship between peripheral blood indexes and blood loss, severity of neurological impairment was analyzed by Spearman correlation analysis. The predictive value of peripheral blood indexes for prognosis was evaluated by receiver operating characteristic (ROC) curves.Results:The expression levels of NLRP3 mRNA (2.67±0.42, 1.04±0.28), Caspase-1 mRNA (1.24±0.26, 0.75±0.14) and IL-1β protein ((24.92±4.97) pg/mL, (13.39±2.35) pg/mL) in the hemorrhage group were higher than those in the control group ( t=31.243, 15.967, 20.126, all P<0.05). When comparing the levels of NLRP3 mRNA, Caspase-1 mRNA and IL-1β in the peripheral blood of patients with different amounts of bleeding, those with massive bleeding were higher than those with moderate bleeding, and those with moderate bleeding were higher than those with mild bleeding (all P<0.05).The Spearman correlation analysis showed that the levels of NLRP3 mRNA, Caspase-1 mRNA and IL-1β in peripheral blood were positively correlated with the amount of bleeding in patients with hemorrhagic stroke ( r=0.646, 0.585, 0.604, all P<0.05). The levels of NLRP3 mRNA, Caspase-1 mRNA and IL-1β in patients with severe neurological deficits were higher than those in patients with moderate and mild neurological deficits, and those in patients with moderate neurological deficits were higher than those in patients with mild neurological deficits (all P<0.05). The results of Spearman correlation analysis showed that the levels of NLRP3 mRNA, Caspase-1 mRNA and IL-1β in peripheral blood were positively correlated with the degree of neurological deficits in patients with hemorrhagic stroke ( r=0.607, 0.522, 0.546, all P<0.05). The expression levels of NLRP3 mRNA, Caspase-1 mRNA and IL-1β in peripheral blood in the poor prognosis group were higher than those in the good prognosis group (all P<0.05). The prediction results indicated that bleeding into the cerebral ventricles, large amount of bleeding, and high expression of NLRP3, Caspase-1 and IL-1β in peripheral blood were independent risk factors for poor prognosis in patients with hemorrhagic stroke (all P<0.05). The area under the ROC curve for the combined assessment of poor prognosis in hemorrhagic stroke by the levels of NLRP3 mRNA, Caspase-1 mRNA and IL-1β was 0.856, which was larger than the areas under the curves of the three indicators detected separately (0.720, 0.703, 0.715, P<0.05). Conclusion:The expressions of peripheral blood NLRP3, Caspase-1 and IL-1β are up-regulated in patients with hemorrhagic stroke, and their high expressions are related to blood loss and the severity of neurological deficit, which also indicate poor prognosis of patients.

3.Association between dietary diversity and the risk of MACE after PCI in patients with coronary heart disease

Objective:To investigate the association between dietary diversity and the risk of major adverse cardiovascular events (MACE) after percutaneous coronary intervention (PCI) in patients with coronary heart disease (CHD) .Methods:A total of 553 patients diagnosed with CHD and undergoing PCI in the Department of Cardiology at the Second Affiliated Hospital of Harbin Medical University between May and November 2023 were enrolled using a convenience sampling method. A Semi-Quantitative Food Frequency Questionnaire was used to assess patients' dietary intake after PCI, and the Dietary Diversity Score (DDS) was calculated. Patients were followed up for one year to determine the incidence of MACE.Results:History of hypertension, history of hyperlipidemia, body mass index, use of antiplatelet agents, use of diuretics, triglycerides, smoking index and DDS were identified as factors influencing the occurrence of MACE after PCI ( P<0.05) . Among these, higher dietary diversity had a protective effect against MACE. Conclusions:After PCI, patients with lower DDS experienced MACE more frequently than those with higher scores. Increased dietary diversity can effectively help prevent MACE in patients after PCI.

5.Progress of Research on Role of Interleukin-13 in Inflammatory Bowel Disease

Inflammatory bowel disease(IBD)is a chronic non-specific gastrointestinal disorder of unknown etiology and its global incidence is increasing.The traditional view holds that interleukin-13(IL-13)only plays an anti-inflammatory role,but recent studies have demonstrated that IL-13 also plays a pro-inflammatory role by disrupting the intestinal epithelial barrier and forming intestinal fibrosis and intestinal fistula through the IL-13 receptor alpha 2(IL-13Rα2)-mediated pathway.This article reviewed the progress of research on the role of IL-13 in IBD,aiming to provide a reference for optimizing the treatment of IBD.

7.Preliminary Discussion on the Influencing Factors of Traditional Chinese Medicine Syndrome under the Multidisciplinary Diagnosis and Treatment Model of Malignant Tumors

As a part of the multidisciplinary diagnosis and treatment model of malignant tumors, traditional Chinese medicine (TCM) plays a unique role in increasing efficiency, reducing toxicity, and preventing recurrence and metastasis. It has been gradually recognized that integrated TCM and western medicine should be used in diagnosis and treatment of tumors. Western medicine such as surgery, radiotherapy, chemotherapy, endocrine therapy, targeted therapy, and immunotherapy may act as the main factors that change the syndromes, as they can induce the changes of the tongue and pulse manifestations and symptoms after acting on the body. It is therefore advised to differentiate and analyze the attributes of yin-yang and cold-heat of western medicine as well as its impact on TCM syndromes, and use Chinese herbal medicinals precisely so as to increase efficiency and reduce toxicity. Moreover, it is better to grasp the syndrome evolution trend of modern medicine and predict the disease tendency, so as to improve the accuracy of syndrome differentiation and treatment in TCM under multi-disciplinary diagnosis and treatment model, and promote the maximization of the benefits of integrated TCM and western medicine in treating tumors.
8.Clinical features of five cases of congenital esophageal atresia complicated with gastric perforation

Objective:To investigate the clinical characteristics of congenital esophageal atresia with gastric perforation, and to improve pediatricians′ understanding of this disease.Methods:The clinical data of five neonates with congenital esophageal atresia and gastric perforation treated in the neonatal intensive care unit of the Affiliated Hospital of Qingdao University from 2012 to 2022 were analyzed retrospectively.Results:Among the five neonates, four were boys and one was girl.The gestational age was 28 + 5 to 37 + 6 weeks, the birth weight was 1 100~2 350 g. All of them had dyspnea and feeding difficulties after birth.Gastric perforation occurred in three cases during invasive mechanical ventilation, one case during non-invasive ventilation, and one case during nasal catheter oxygen inhalation.Emergency primary gastric repair was performed, followed by secondary esophageal anastomosis.All the patients were cured and discharged from hospital. Conclusion:Gastric perforation is a rare complication of congenital esophageal atresia, which is more common in premature infants and low birth weight infants.Mechanical ventilation may promote the occurrence of gastric perforation.If gastric perforation is complicated, repair should be performed as soon as possible, and esophageal anastomosis surgery should be performed early after stability to improve the final outcome.
9. Mechanism of Nuclear Factor Erythroid 2-related Factor 2 Against Intestinal Fibrosis in Inflammatory Bowel Disease

Inflammatory bowel disease (IBD) is characterized by recurrent non ⁃ specific intestinal inflammatory responses. Intestinal fibrosis is an important cause of IBD complicated with intestinal obstruction. Nuclear factor erythroid 2⁃ related factor 2 (Nrf2) is a transcription factor that has anti ⁃ oxidative stress response in cells. In IBD, Nrf2 and its downstream regulated antioxidant enzymes achieve protective effects against intestinal fibrosis by inhibiting the activation of nuclear factor ⁃ κB, regulating T helper cell 17/regulatory T cell balance of intestinal immunity, and inhibiting transforming growth factor⁃β1/Smads signaling pathway. In this review, the structure of Nrf2, the specific mechanism of Nrf2's effect on intestinal fibrosis in IBD, and the recent studies on the treatment of IBD through Nrf2 pathway were reviewed in an attempt to provide a new direction for the prevention and treatment of IBD.
10.Effect of Mini-Clinical Evaluation Exercise combined with direct observation of procedural skills in ophthalmology practice training in integrated teaching of sensory system diseases

Objective:To investigate the effect of Mini-Clinical Evaluation Exercise (Mini-CEX) combined with direct observation of procedural skills (DOPS) in ophthalmology practice training in the integrated teaching of sensory system diseases.Methods:The medical students in the five-year program who entered the stage of ophthalmology practice in the integrated teaching of sensory system diseases in The Second Clinical College of Chongqing Medical University, were selected as research group, and the medical students in the five-year program who received training from October 2020 to July 2021 were selected as control group. The two groups were compared in terms of department examination score, progressive evaluation of the research group, and the degree of satisfaction with practice. SPSS 23.0 was used for the t-test. Results:Compared with the control group, the research group had significantly better scores of medical history inquiry, medical record writing, and operational skills ( t=2.47, P=0.016; t=5.26, P<0.001; t=2.20, P=0.031). In progressive training, the Mini-CEX and DOPS scores in the second progressive formative assessment were better than those in the first assessment ( t=-21.06, P<0.001; t=-15.09, P<0.001). The degree of satisfaction with performance among the interns and the degree of satisfaction with interns among the examiners in the second assessment were better than those in the first assessment ( t=-5.53, P<0.001; t=-8.19, P<0.001). Conclusion:Mini-CEX combined with DOPS has a good effect in ophthalmology practice training in the integrated teaching of sensory system diseases and thus holds promise for application.
