1.Clinical characteristics analysis of 6 children with anomalous origin of coronary artery supported by extracorporeal membrane oxygenation

Objective:To summarize the clinical features of children with anomalous origin of coronary artery (AOCA) who received extracorporeal membrane oxygenation (ECMO) support.Methods:A case series study was conducted. Clinical data was collected from 6 children who were diagnosed with AOCA by coronary computed tomography angiography or digital subtraction angiography and received ECMO support in the Pediatric Intensive Care Unit of Henan Provincial People′s Hospital between January 2020 and August 2024. Descriptive analysis was performed on their clinical features, laboratory test results, point-of-care echocardiography results, imaging findings, surgical management, and outcomes.Results:Among the 6 children (3 males and 3 females), the age of onset was 12.5 (11.0, 13.0) years. All 6 patients were transported from other hospitals under ECMO support. Five patients were admitted with chief complaints of "cardiac arrest after strenuous activity, syncope after strenuous activity, or heart failure" and were initially diagnosed with fulminant myocarditis or cardiomyopathy. All 6 children had significantly elevated troponin and B-type natriuretic peptide levels upon admission. Point-of-care echocardiography revealed segmental left ventricular systolic dysfunction in all 6 children, and AOCA was detected in 2 cases based on bedside ultrasound. ECMO was successfully weaned in 5 children. All 6 cases were diagnosed with AOCA. Four children underwent surgical coronary artery correction, one received a heart transplantation, and one missed the optimal window for surgical correction. Heart transplantation was recommended for the latter, but the parents declined, and the patient was discharged. During the follow-up until August 2025, all 6 children survived.Conclusions:AOCA in children is prone to misdiagnosis as other diseases in the early stage. Timely ECMO support provides the possibility of surgery or heart transplantation for children experiencing acute ischemic and hypoxic episodes due to AOCA, improving survival rates.
2.Perioperative immune dynamics and clinical outcomes in patients undergoing on-pump cardiac surgery

Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.
3.Syndrome Patterns Distribution and Risk Factors of Mixed Hemorrhoids in Traditional Chinese Medicine: A Multicenter Real-world Study Using Large Language Models and Latent Class Analysis

ObjectiveTo develop a standardized classification model for traditional Chinese medicine (TCM) syndrome patterns of mixed hemorrhoids using multi-center real-world data, and unveil their distribution patterns and core risk factors, thereby providing evidence-based support for standardizing TCM syndrome differentiation and implementing precision interventions. MethodsA multi-center cross-sectional study was conducted, enrolling 13 283 mixed hemorrhoid patients from eight hospitals in Jiangsu Province between September 1st, 2023 and December 31st, 2024. DeepSeek-R1-Distill-Qwen-7B and LLaMA-3.3 large language models (LLM) were integrated with latent class analysis (LCA) to perform unsupervised learning and latent class modeling of TCM symptomatology. Potential risk factors were screened via univariate analysis, followed by logistic regression to identify independent risk factors for each syndrome pattern. ResultsThe model's performance indicators were stable and reliable across different clinical data types,i.e. in the outpatient records, past medical history (F1=99.7%), current medical history (F1=94.9%), and specialist examination (F1=90.7%); in inpatient records, past medical history (F1=98.2%), current medical history (F1=91.2%), specialist examination (F1=90.3%), and discharge status (F1=90.6%). Latent class mode-ling identified four core TCM syndrome patterns including spleen deficiency and qi sinking syndrome (915 cases, 6.89%), damp-heat pouring downward syndrome (10 820 cases, 81.46%), qi stagnation and blood stasis syndrome (1252 cases, 9.43%), and wind injuring intestinal collaterals syndrome (296 cases, 2.22%), with respective latent class probabilities of 0.069, 0.815, 0.094, and 0.022. Logistic regression demonstrated that gender, age, disease duration, hypertension, diabetes, hyperlipidemia, constipation, smoking history, and alcohol consumption were independent risk factors for pattern differentiation (P<0.05). The efficacy validation evaluation revealed that the cure rates for patients with spleen deficiency and qi sinking syndrome and qi stagnation and blood stasis syndrome were higher than those for patients with damp-heat pouring downward syndrome (adjusted P<0.05), with no statistically significant differences among other syndrome patterns. ConclusionDamp-heat pouring downward syndrome is the predominant syndrome in mixed hemorrhoids. Gender, age, disease duration, hypertension, diabetes, hyperlipi-demia, constipation, smoking history, and alcohol consumption are independent risk factors for the differentiation of syndrome types.
4.Longitudinal relationship between school adaptation and anxiety of primary school freshmen and maternal affectionate behaviors

Objective:
To investigate the effect of maternal affectionate behaviors on anxiety and school adaptation among primary school freshmen, so as to provide a reference to support their emotional health and positive adjustment to school.
Methods:
A total of 426 primary school freshmen from a primary school were selected from Baoding City by the method of convenience sampling and cluster random sampling, and the school adaptation, anxiety and maternal affectionate behaviors were intensively investigated every weekend from September to December 2024, a total of 15 times. In October 2024 (T1) and April 2025 (T2), two longitudinal follow up surveys were conducted, which enrolled other 259 primary school freshmen from Baoding City to investigate their school adaptation, anxiety, and maternal affectionate behaviors at two time points. The dynamic structural equation model and cross lag model were established by Mplus 8.0 software, and the autoregressive effect of variables and the lag effect between two variables in intensive follow up survey, and the cross lag effect between two variables in two longitudinal follow up surveys were analyzed respectively.
Results:
For the intensive longitudinal surveys, the mean scores for maternal affectionate behaviors, school adaptation, and anxiety were (3.46±0.47) (4.06±0.55) (2.17±0.55), respectively. For the two longitudinal follow up surveys, the T1 and T2 mean scores for maternal affectionate behaviors, school adaptation, and anxiety were (3.30±0.45) (3.98±0.57) (2.03±0.49), and ( 3.26± 0.45) (4.15±0.55) (1.98±0.47), respectively. Intensive longitudinal surveys indicated that the maternal affectionate behaviors in the previous week positively predicted school adaptation in the following week ( β =0.08, P <0.01). Maternal affectionate behaviors in the previous week positively predicted school adaptation of male students in the following week ( β =0.10, P < 0.01), but not for female students ( β =0.02, P =0.25). Maternal affectionate behaviors in the previous week negatively predicted anxiety of primary school freshmen, male and female students in the following week ( β =-0.09,-0.08,-0.07, all P <0.01). The two longitudinal follow up surveys indicated that maternal affectionate behaviors at T1 positively predicted school adaptation at T2 ( β = 0.10, P = 0.03); maternal affectionate behaviors at T1 positively predicted school adaptation of male students at T2 ( β =0.12, P =0.04).
Conclusion
Maternal affectionate behaviors promote school adaptation and emotional health in new primary school students.

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
7.Application of interprofessional cooperative simulation in teaching emergency care for shock patients for ICU undergraduate nursing students

Objective:To explore the effectiveness of interprofessional cooperative simulation in teaching emergency care for shock patients among intensive care unit (ICU) undergraduate nursing students.Methods:An interprofessional cooperative simulation-based teaching faculty team was established for ICU undergraduate nursing students, and a shock case library was developed. Using convenience sampling, 32 ICU undergraduate nursing students in 2022 were selected as the control group and received conventional simulation-based teaching, with students rotating through roles as nurses, standardized patients, doctors, and family members. In the experimental group, 34 ICU undergraduate nursing students in 2023 and 24 ICU clinical medicine interns were recruited to act as doctors for interprofessional cooperative simulation-based teaching. Each group was divided into subgroups, with each subgroup consisting of 4-5 nursing students. One group completed simulation-based training per month for a total of 8 sessions, with each session lasting 3 hours. The teaching adopted the on-site "tidal ward" in situ simulation, and the scenarios included patient history collection and health assessment, shock emergency care, nursing evaluation, and health education. The differences between the two groups of nursing students were compared in terms of ICU exit theoretical assessment score, objective structured clinical examination skill assessment score, and satisfaction with simulation-based teaching. SPSS 22.0 was used for independent samples t test and Mann-Whitney U test. Results:The experimental group achieved significantly higher scores in theoretical assessment (84.65±8.06), total score of satisfaction with simulation-based teaching (101.00±5.13), and clinical learning and multiprofessional team dimensions (47.32±3.35) compared to the control group ( P<0.001). The experimental group achieved higher scores in objective structured clinical examination skill assessment (81.40±7.22), guiding feedback and reflection (37.50±3.04), and judgmental thinking and clinical reasoning (16.00±2.03) compared to the control group, though the differences were not significant ( P=0.977, 0.668, and 0.636). Conclusions:Interprofessional cooperative simulation enhances the shock patient emergency care abilities and satisfaction with simulation-based teaching for undergraduate nursing students.
8.Abnormal T cell subsets of BTBR T+Itpr3tf autistic mice at different developmental stages

Objective To investigate the expression of T cell subsets in the spleen of BTBR T+Itpr3tf autistic mouse at 4,8,and 12 weeks of age,and to determine the optimal age for studying the relationship between immune abnormalities and autism in BTBR autistic mice.Methods It randomly selected 5~6 male BTBR mouse at 4 weeks,8 weeks,and 12 weeks of age and C57BL/6J mouse of the same gender at corresponding ages for the three-box social interaction test,the self-grooming test,and the marble-burying test;Single cell suspensions were prepared from the spleens of mouse at 8 and 12 weeks of age,and flow cytometry was used to detect 8 subsets of T cells(TH 1,TH2,TH17,TC1,TC2,TC17,TFH,and Treg).Results Compared with C57BL/6J mouse of the same age,BTBR mouse at 4 weeks,8 weeks,and 12 weeks of age showed a decrease in social time(P<0.001),an increase in grooming time(P<0.01,P<0.001),and an increase in the number of marbles buried(P<0.01,P<0.001)in BTBR mouse at 8 weeks and 12 weeks of age.As well,the expression of TH 1(P<0.001),TH2(P<0.01),TC 1(P<0.05),TC2(P<0.001),and TFH(P<0.01)cells in 8-week-old BTBR mouse were significantly increased,while the expression of Treg(P<0.001)cells were significantly decreased;The expression of TH 1(P<0.01),TH2(P<0.01),TH 17(P<0.05),TC1(P<0.01),TC2(P<0.001),TC 17(P<0.01),and TFH(P<0.001)increased in 12-week-old BTBR mouse,while the expression of Treg(P<0.05)cells decreased.At different age stages(P<0.050)the ratio of TH 1/Treg and TC 1/Treg in 8-week-old BTBR mouse were significantly higher than those in 12 week old mouse,while the TC 17/Treg ratio decreased.Conclusions BTBR mouse at different developmental stages exhibit varying degrees of abnormal increase in Teff/Treg ratio.Based on result of behavioral test,it is recommended to use 8-week-old BTBR mice for research on autism and immune abnormalities.
9.Literature Quality Evaluation of Randomized Controlled Trials on Acupuncture Treatment for Diminished Ovarian Reserve

Objective To evaluate the methodological quality and literature report quality of randomized controlled trials(RCTs)on acupuncture treatment for diminished ovarian reserve published both domestically and internationally;To provide reference for future related clinical research.Methods RCTs on acupuncture treatment for diminished ovarian reserve were retrieved from Cochrane Library,PubMed,Embase,Web of Science,CBM,CNKI,Wanfang Data and VIP from the establishment of the databases to 16th,March 2024.Literature was screened according to the inclusion criteria,and basic research features were extracted from literature.Cochrane Collaborative Network randomized controlled trial bias risk assessment tool RoB2 was used for methodological quality evaluation,and CONSORT statement and STRICTA checklist were used for literature report quality evaluation.Results Totally 19 RCTs were included.The evaluation results of RoB2 showed that 18 RCTs(94.74%)had a certain risk of bias,and 1 RCT(5.26%)had a high risk of bias.In the evaluation of the CONSORT statement,there were a total of 24 items with a research report rate of less than 50%,accounting for 64.86%of all items,and 14 items with a report rate of less than 10%,accounting for 37.84%of all items;in the evaluation of the STRICTA list,there were a total of 3 items with a research report rate of less than 50%,accounting for 17.65%of all items,and 2 items with a report rate of less than 10%,accounting for 11.76%of all items.Conclusion Currently,the overall number of RCTs for acupuncture treatment of diminished ovarian reserve is relatively small,the methodological quality of the study is generally low,and the risk of bias is high.The literature reporting rate of the CONSORT statement items is low.Researchers should further strengthen clinical trial design and methodological training,improve the level of clinical research,and strictly follow the CONSORT statement and STRICTA checklist for standardized reporting.
10.Changes in the intestinal microbiota structure of patients with colorectal adenoma

Objective:To investigate gut microbiota differences between individuals with and without colorectal adenoma(CRA)and to identify gut microbes associated with CRA.Methods:This cross-sectional study analyzed the gut microbiota of 100 patients with CRA and 68 individuals without CRA(aged 40-75 years)who underwent colonoscopies between March 2021 and March 2022 at Tianjin Nankai Hospital.Fecal samples were sequenced for the V3-V4 region of the bacterial 16S rRNA gene using the Illumina NovaSeq platform.Results:Compared to the non-CRA group,the CRA group exhibited reduced relative abundances of identified and unidentified Lachnospiraceae,with increased Faecalibacterium and Streptococcus.In the non-CRA group,the relative abundances of Coprococcus,unidentified Clostridiaceae,and Clostridium were higher.LEfSe analysis revealed significant enrichment of Gammaproteobacteria,Proteobacteria,Enterobacteriales,and Faecalibacterium in the CRA group,while the non-CRA group was enriched for Moraxellaceae,Acinetobacter,and Anaerostipes.Conclusions:These findings suggest a discernible disparity in the gut microbiota structure between CRA patients and individuals without adenoma.The enrichment of potential pathogenic taxa,such as Faecalibacterium and Streptococcus,in the CRA group suggests a possible association with adenoma development.
